The article reports on a study of the efficacy of tuberculin purified protein derivative antigens through intralesional immunotherapy in the excision of warts. The result shows significant increase clearances of patients warts after being injected by tuberculin purified protein derivative and concludes also that the prevention of tuberculosis and mandatory Bacillus Calmette-Guérin (BCG) vaccination as the cause of higher rate of clearance of warts of patients in South India.

Objective To estimate the frequency of purpura and bleeding in patients on calcium channel blockers and association between calcium channel blocker therapy and petechiae due to capillary fragility induced by Hess test. Methods A facility based cross-sectional study was conducted involving 60 adult patients only on calcium channel blockers (CCBs) for cardiovascular causes for at least 3 months duration. All the patients were questioned about the spontaneous occurrence of purpura and were examined for purpura. Hess test was performed in all the subjects and the result noted. Patients also underwent investigations to rule out other causes of purpura or bleeding. Results None of the 60 hypertensive patients included in our study who were only on amlodipine, a calcium channel blocker, had any history suggestive of or examination findings suggestive of spontaneous bleeding or occurrence of purpura. Abnormal Hess test was also not seen in any of the subjects. Platelet count, bleeding time, clotting time and prothrombin time, the key investigations in cases of bleeding disorders were all normal in all the subjects. Conclusion The results of our study showed that treatment with calcium channel blockers (CCBs), particularly amlodipine, is not related to purpura or bleeding. Our study also shows that therapy with the CCB amlodipine is not associated with petechiae due to capillary fragility induced by Hess test. [ABSTRACT FROM AUTHOR]

Background: Psoriasis patients are at increased risk of developing the metabolic syndrome (MS). Proinflammatory cytokines such as tumor necrosis factor-α, interleukin-6 that are increased in the psoriatic plaques are known to contribute to features of MS such as hypertension, dyslipidemia and insulin resistance. Aims: (1) To establish the frequency of MS in patients with psoriasis. (2) To study the risk factors associated with MS in psoriasis. Materials and Methods: A hospital based comparative study was conducted involving 40 adult patients with psoriasis and 40 age- and sex-matched controls. All participants were evaluated for components of MS. Results: Both groups included 31 males and 9 females. The mean age of the cases and controls were 49.95 years and 49.35 years, respectively. Psoriasis patients with MS had a statistically significant higher mean age (56.31 ± 11.36 years) compared with those without MS (46.89 ± 11.51 years). MS was present in 13 out of 40 (32.5%) patients with psoriasis and 12 out of 40 (30%) controls; this difference was not statistically significant. Higher age and female gender correlated with the presence of MS in psoriasis patients. The presence of MS in psoriasis patients was statistically independent of psoriasis area severity index score, body surface area involvement or psoriatic arthropathy. Conclusion: Our results suggest that there is no close correlation between psoriasis and MS in South Indian patients. [ABSTRACT FROM AUTHOR]

Background HIV-infected patients with skin diseases have impaired dermatology quality of life due to severe and refractory nature of dermatoses occurring in them but literature on this aspect is sparse. Aims To assess the dermatology life quality index (DLQI) in HIV-infected patients with skin diseases and to assess the effect of antiretroviral therapy (ART) on DLQI. Material and methods A hospital based comparative study was conducted involving 40 adult HIVinfected patients with skin disease and 40 age- and sex-matched controls using DLQI questionnaire devised by Finlay et al. Results Each group had 14 males and 26 females. Of the 40 cases, 35 were on antiretroviral therapy. Most common skin diseases in HIV cases was pruritic papular eruption of HIV (18/40, 45%) followed by infections (12 cases, 30%) whereas in the controls these were eczema (13/40, 32.5%) followed by polymorphic light eruption (6 cases, 15%). The mean DLQI score in the cases and controls were 8.73±3.922 and 8.45 ±4.032, respectively (not statistically significant). The mean DLQI score in cases on ART and not-on-ART were 8.46±4.01 and 10.6±2.88 respectively (statistically not significant). Conclusion DLQI score in HIV-infected patients was not significantly different from the comparison group. ART marginally improved DLQI. [ABSTRACT FROM AUTHOR]

A letter to the editor is presented about a study on hyperpigmentary disorders in children.

Mal de Meleda is a rare autosomal recessive transgradient palmoplantar keratoderma characterized by transgradient keratoderma with associated scleroatrophy, nail changes, pseudoainhum around digits and perioral erythema, without a tendency for spontaneous resolution. Involvement of the lip by keratoderma has not been reported in the English literature. Here we present two cases of Mal de Meleda with unusual lip involvement. The first case was a 15-year-old girl, born of second-degree consanguineous marriage, who presented with transgradient palmoplantar keratoderma from 6 months of age, with lichenoid papules and plaques on the elbows and knees, conical tapering of the distal digits, flexion deformity of several fingers, digital constriction, knuckle pads and lip involvement. The second case was a 24-year-old male with transgradient palmoplantar keratoderma since birth. He also had scaly plaques on the extensors of bilateral knees and elbows, knuckle pads, pseudosclerodermatous fingers with conical tapering, digital constrictions at various places with mild flexion deformity and lip involvement. A letter to the editor about a case report of a 40-year-old male patient with recurrence of zosteriform vesicular lesions on the contralateral dermatome is presented.

The article presents a case study of a 17-year-old boy presented with an asymptomatic, well-defined, irregular, sessile, slow-growing tumor on the extensor aspect of right forearm for the past three years. It notes that the patient has been affected with pacinian neurofibroma (PN) which is a rare tumor of neural origin characterized by the formation of components resembling Pacinian corpuscles. Moreover, it claims that the condition is not associated with neurofibromatosis.

A 45-year-old man presented with a thickened and raised nail of his left fifth finger since birth. He was otherwise healthy. On examination, the nail of the left little finger was markedly thickened, hyperkeratotic, and situated at an angle of approximately 45 degrees to the long axis of the distal phalanx. There was prominent subungual hyperkeratosis. A diagnosis of congenital onychogryphosis of the little finger of idiopathic nature was considered. Visual analogy to the leaning tower of Pisa encouraged us to describe it as congenital leaning tower nail.
