The use of magnetic resonance imaging (MRI) has led to increased clinical and research applications using 3D segmentation and reconstructed volumetric data in musculoskeletal imaging. Lesions of the deep digital flexor tendon (DDFT) are a common pathology in horses undergoing MRI. Three-dimensional MRI reconstruction performed for volumetric tendon analysis in horses has not previously been documented. The aim of this proof-of-concept study was to evaluate the 3D segmentation of horses undergoing repeated MRI at several time points and to perform an analysis of the segmented DDFTs across time. MRI DICOM files were acquired from six horses undergoing repeated MRI examination of the foot for DDFT injury. Once segmented, volumetric tendon surface tessellation language (STL) files were created. Thickness and volumetric data were acquired for each tendon in addition to a tendon comparison across timepoints within each horse. Pearson correlation coefficients were calculated for comparison of MRI reports to computer analysis. There was a significant and positive correlation between MRI and medial record reports of clinical improvement or deterioration and computer analysis (r = 0.56, p = 0.01). The lower end range limit for tendon thickness varied between 0.16 and 1.74 mm. The upper end range limit for DDFT thickness varied between 4.6 and 23.6 mm. During tendon part comparison, changes in DDFT were reported between −3.0 and + 14.3 mm. Changes in DDFT size were non-uniform and demonstrated fluctuations throughout the tendon. The study was successful in establishing the volumetric appearance and thickness of the DDFT as it courses in the foot and tracking this over time. We encountered difficulties in accurate segmentation of the distal insertion of the DDFT as it blends with the distal phalanx. The data demonstrated that the DDFT can be segmented and volumetric studies based on size and shape can be performed using an in silico approach.

OBJECTIVE To evaluate the effects of exercise in an underwater treadmill (UWT) on forelimb biomechanics and articular histologic outcomes in horses with experimentally induced osteoarthritis of the middle carpal joint. ANIMALS 16 horses. PROCEDURES An osteochondral fragment was induced arthroscopically (day 0) in 1 middle carpal joint of each horse. Beginning on day 15, horses were assigned to exercise in a UWT or in the UWT without water (simulating controlled hand walking) at the same speed, frequency, and duration. Thoracic and pelvic limb ground reaction forces, thoracic limb kinematics, and electromyographic results for select thoracic limb muscles acting on the carpi were collected on days -7 (baseline), 14, 42, and 70. Weekly evaluations included clinical assessments of lameness, response to carpal joint flexion, and goniometric measurements of thoracic limb articulations. At study conclusion, articular cartilage and synovial membrane from the middle carpal joints was histologically examined. RESULTS Exercise in a UWT significantly reduced synovial membrane inflammation and resulted in significant clinical improvements with regard to symmetric thoracic limb loading, uniform activation patterns of select thoracic limb muscles, and return to baseline values for carpal joint flexion, compared with results for horses with simulated hand walking. CONCLUSIONS AND CLINICAL RELEVANCE Overall improvements in thoracic limb function, joint range of motion, and synovial membrane integrity indicated that exercise in a UWT was a potentially viable therapeutic option for the management of carpal joint osteoarthritis in horses.

Sagittal groove injuries of the proximal phalanx are an important cause of lameness in performance horses. The purpose of this retrospective case series study was to describe standing low-field magnetic resonance imaging (MRI) characteristics of these injuries in a group of Warmblood horses. Horses with an MRI diagnosis of sagittal groove injuries involving the proximal phalanx and that had follow-up MRI and clinical outcome information were included. Findings from clinical examinations, diagnostic tests, and other imaging modalities were recorded. All MRI studies were retrieved for re-evaluation by an experienced, board-certified veterinary radiologist. A total of 19 horses met inclusion criteria. All horses had MRI lesions consistent with unilateral or bilateral sagittal groove injuries of the proximal phalanx and abnormal mineralization of the sagittal ridge of the third metacarpal/metatarsal bone. Fifteen horses (79%) had concurrent osteoarthritis of the affected metacarpophalangeal/metatarsophalangeal joint. Eighteen horses received conservative therapy and all horses still had osseous abnormalities detected at the time of follow-up MRI. Thirteen horses (68.5%) were still lame at the time of follow-up, whereas the other six horses (31.5%) had become sound and returned to the previous level of exercise. Findings indicated that, for mature Warmblood horses, acute or chronic injuries of the sagittal groove of the proximal phalanx may have variable standing low-field MRI characteristics. Based on this sample of 19 horses, findings also indicated that the prognosis for performance soundness in horses diagnosed with sagittal groove injury of the proximal phalanx and concurrent osteoarthritis is poor.

When evaluating suspensory ligament branch splits, there is a discrepancy between high field MRI findings and weight bearing ultrasonographic imaging characteristics. In this descriptive and retrospective case series, ultrasonographic examination was performed on suspensory ligament branches with the limbs in weight bearing and non-weight bearing positions. Suspensory ligament branch splits were defined as linear regions of decreased echogenicity when imaged with the limb in a weight bearing position that increased in size and became anechoic with the limb in a non-weight bearing position. This appearance was considered an indication of pathologic change in the branch. A total of 62 suspensory ligament branches were included in the study from 37 horses, with 14 partial splits, 11 intrasubstance splits, and 14 complete splits of which two had extension of fetlock synovial fluid and synovial membrane through the split. Recheck ultrasonographic examinations performed in eight horses up to 14 months following the initial examination demonstrated persistence of the split in two horses, partial resolution in five horses, and complete resolution in one horse. Ultrasonographic examination using the non-weight bearing approach proved valuable for increasing the lesion conspicuity as compared to the weight bearing images. Certain abnormalities, such as longitudinal fiber disruption (split) in suspensory ligament branches may only be evident when imaged non-weight bearing. The use of this technique provides a more accurate representation of lesion severity and allows for monitoring over time. A longitudinal study is necessary to determine the clinical relevance of suspensory ligament branch splits.

OBJECTIVE To describe the effects of distal limb immobilization and remobilization in the equine metacarpophalangeal joint. STUDY DESIGN Randomized, prospective experimental study. ANIMALS Eight healthy, skeletally mature horses. METHODS One forelimb of each horse was immobilized in a fiberglass cast for 8 weeks; this was followed by 12 weeks of a treadmill-based training program after the cast had been removed. Clinical examinations, radiography, computed tomography (CT), nuclear scintigraphy, MRI, and histomorphometry were used to examine the third metacarpal (MC3), proximal phalanx, proximal sesamoid bones, and associated soft tissues in each horse. The femorotibial joints are a common source of lameness in Western performance horses. The objective of this prospective study was to compare the radiography, ultrasonography, computed tomographic arthrography (CTA), and arthroscopy findings in horses with lameness localized to the femorotibial joints. Twenty-five stifles in 24 horses were included and were evaluated with all four of these diagnostic methods. Defects detected in femorotibial joint structures were compared between diagnostic methods using a McNemar's test to evaluate for disagreement. Cranial medial meniscotibial desmopathy was most detected on arthroscopy (in 14/25 cases) and was only detected on ultrasonography in three out of 11 (27.3%) arthroscopically observed cases, but was detected on CTA in nine out of 12 (75%) arthroscopically observed cases. Medial meniscal injury located on the craniolateral border was most detected on arthroscopy (n = 9) and was detected on CTA in five cases, but on ultrasonography in 0 cases. Detection of articular cartilage defects on the medial femoral condyle was most detected with arthroscopy (24/25, 96% cases) and was also detected on CTA in 12/20 (60%) cases with a significant disagreement identified between modalities (P = 0.02). Cranial and caudal cruciate ligament defects were detected on CTA in 6/22 (27.3%) and 7/19 (36.8%) cases, respectively, and with arthroscopy in 3/25 (12%) and 2/25 (8%) cases, respectively. The use of CTA detected more defects in the cruciate ligaments, proximal tibia, and ligament entheses than the other diagnostic methods, but was not reliable for detection of articular cartilage damage on the medial femoral condyle.

Summary Magnetic resonance imaging (MRI) has become a valuable tool for the diagnosis of a multitude of conditions previously grouped into the vague diagnosis of palmar heel pain based on the response to palmar digital nerve diagnostic analgesia. The use of MRI not only determines a specific pathological diagnosis of the numerous structures within the foot but also the findings of the MRI help direct treatment and rehabilitation protocols, as well as providing prognostic information. Common injuries include damage to the navicular apparatus, deep digital flexor tendon, arthropathy of the distal interphalangeal joint and desmopathy of the collateral ligaments of the distal interphalangeal joint. This article reviews common MRI abnormalities in the equine foot and how treatment can be directed by the findings. Medical and surgical treatment options, as well as shoeing and rehabilitation protocols, are also discussed in relation to MRI findings. Using MRI can help create a more individually tailored and case-specific treatment regimen, which can, in turn, promote a more positive outcome in equine cases.

Background: The chondrogenic potential of culture-expanded bone-marrow-derived mesenchymal stem cells (BMDMSCs) is well described. Numerous studies have also shown enhanced repair when BMDMSCs, scaffolds, and growth factors are placed into chondral defects. Platelets provide a rich milieu of growth factors and, along with fibrin, are readily available for clinical use. The objective of this study was to determine if the addition of BMDMSCs to an autologous platelet-enriched fibrin (APEF) scaffold enhances chondral repair compared with APEF alone. Methods: A 15-mm-diameter full-thickness chondral defect was created on the lateral trochlear ridge of both stifle joints of twelve adult horses. In each animal, one defect was randomly assigned to receive APEF+BMDMSCs and the contralateral defect received APEF alone. Repair tissues were evaluated one year later with arthroscopy, histological examination, magnetic resonance imaging (MRI), micro-computed tomography (micro-CT), and biomechanical testing. Results: The arthroscopic findings, MRI T2 map, histological scores, structural stiffness, and material stiffness were similar (p > 0.05) between the APEF and APEF+BMDMSC-treated repairs at one year. Ectopic bone was observed within the repair tissue in four of twelve APEF+BMDMSC-treated defects. Defects repaired with APEF alone had less trabecular bone edema (as seen on MRI) compared with defects repaired with APEF+BMDMSCs. Micro-CT analysis showed thinner repair tissue in defects repaired with APEF+BMDMSCs than in those treated with APEF alone (p < 0.05). Conclusions: APEF alone resulted in thicker repair tissue than was seen with APEF+BMDMSCs. The addition of BMDMSCs to APEF did not enhance cartilage repair and stimulated bone formation in some cartilage defects. Clinical Relevance: APEF supported repair of critical-size full-thickness chondral defects in horses, which was not improved by the addition of BMDMSCs. This work supports further investigation to determine whether APEF enhances cartilage repair in humans.

SummaryReasons for performing study Injection of the navicular bursa is commonly performed from the palmar aspect of the limb, which results in penetration of the deep digital flexor tendon (DDFT). Objectives To report a radiographic guided injection from the lateral aspect of the limb that avoids puncture of the DDFT and to assess synovial and soft tissue penetration by the needle. Study design Prospective clinical and cadaveric study. Methods Prospective analysis of cadaver limbs and clinical cases in which the navicular bursa was injected from the lateral aspect. Cadaver limbs were placed in a stand to simulate weight bearing and injection was performed in limbs without synovial distension or with distension of either the distal interphalangeal (DIP) joint, digital flexor tendon sheath (DFTS) or navicular bursa. In cadaver and clinical limbs, contrast was injected and the needle position assessed with radiographs. Cadaver (but not clinical) limbs were also examined using magnetic resonance imaging with the needle in situ. Results Successful navicular bursal injection was achieved in all limbs (n = 71). Background: The goal of this study was to test the ability of an injectable self-assembling peptide (KLD) hydrogel, with or without microfracture, to augment articular cartilage defect repair in an equine cartilage defect model involving strenuous exercise. Methods: Defects 15 mm in diameter were created on the medial trochlear ridge and debrided down to the subchondral bone. Four treatment groups (n = 8 each) were tested: no treatment (empty defect), only defect filling with KLD, only microfracture, and microfracture followed by filling with KLD. Horses were given strenuous exercise throughout the one-year study. Evaluations included lameness, arthroscopy, radiography, and gross, histologic, immunohistochemical, biochemical, and biomechanical analyses. Results: Overall, KLD-only treatment of defects provided improvement in clinical symptoms and improved filling compared with no treatment, and KLD-only treatment protected against radiographic changes compared with microfracture treatment. Defect treatment with only microfracture also resulted in improved clinical symptoms compared with no treatment, and microfracture treatment resulted in repair tissue containing greater amounts of aggrecan and type-II collagen compared with KLD-only treatment. Microfracture treatment also protected against synovial fibrosis compared with no treatment and KLD-only treatment. Treatment with the self-assembling KLD peptide in combination with microfracture resulted in no additional improvements over microfracture-only treatment. In general, the nature of the predominant tissue in the defects was a mix of noncartilaginous and fibrocartilage tissue, with no significant differences among the treatments. Conclusions: Treatment of defects with only KLD or with only microfracture resulted in an improvement in clinical symptoms compared with no treatment; the improvement likely resulted from different causes depending on the treatment. Whereas microfracture improved the quality of repair tissue, KLD improved the amount of filling and protected against radiographic changes. Clinical Relevance: Treatment of defects with only microfracture and with KLD only resulted in clinical improvements compared with untreated defects, despite differing with respect to the structural improvements that they induced.

SummaryReasons for performing study There is limited knowledge of the foot lesions that influence the outcome of palmar/plantar digital neurectomy (PDN). Objectives 1) To report the short- and long-term outcomes of horses that underwent PDN to alleviate chronic foot pain due to lesions diagnosed by magnetic resonance imaging (MRI) and 2) factors that may influence the outcome of PDN. Study design Multicentre retrospective study. Methods Medical records of 50 horses subjected to PDN due to chronic foot pain were reviewed. Age, breed, sex, athletic activity, duration of lameness, affected limb(s), response to anaesthesia of the palmar/plantar digital nerves, MRI findings and surgical technique were analysed together with follow-up data to identify factors that influenced the long-term outcomes. Results Forty-six of 50 horses (92%) responded positively to surgery; 40 (80%) were able to return to their previous athletic use for a median time of 20 months (range: 12–72 months). Eighteen (36%) horses developed post operative complications including residual lameness, painful neuromas, or early recurrence of lameness. Horses with pre-existing core or linear lesions of the deep digital flexor tendon (DDFT) had significantly shorter periods of lameness resolution after surgery than horses with dorsal border lesions of the DDFT or other foot lesions. Conclusions Palmar/plantar digital neurectomy can improve or resolve lameness in horses with foot pain unresponsive to medical therapy without serious post operative complications. However, horses with core or linear lesions of the DDFT should not be subjected to PDN as these horses experience residual lameness or early recurrent lameness after surgery. Magnetic resonance imaging can be used to identify these horses.

This article addresses the clinical application of magnetic resonance imaging (MRI) and computed tomography (CT) as applied to the standing equine patient. This discussion includes the logistics, advantages, disadvantages, and limitations of imaging a standing horse. In addition, a brief review is given of the physics of these modalities as applied in clinical practice, and the currently available hardware and software required by these techniques for image acquisition and artifact reduction. The appropriate selection of clinical cases for standing MRI and CT is reviewed, focusing on cases that are capable of undergoing standing surgeries following lesion diagnosis.

Tendons are formed during the second half of embryonic developmentwhen tendon precursor cells deposit narrow-diameter (∼30 nm) collagenfibrils that are parallel to the long axis of the tissue. During post nataldevelopment, the narrow fibrils are replaced by large-diameter (up to500 nm)fibrils.Theabilityoftendontotransmitforcefrommuscletobone,and to dissipate forces during locomotion, is directly attributable to thecollagen fibrils. How the fibrils are synthesised, how they are alignedparallel to the tendon long axis, and how this arrangement can bereinstated during tendon healing are poorly understood. Ultrastructuralstudies of tendon lesions show the reappearance of narrow-diametercollagen fibrils and cells with slender cytoplasmic protrusions (calledfibripositors) that normally only occur in tendon during embryonicdevelopment. Recapitulation of development is a hypothesis that isgaining increasing support from researchers of tendon disease. A betterunderstanding of the genetic, molecular and environmental cues duringembryonicdevelopmentisexpectedtoprovidebetterinsightsintohowtoimprove the rate and fidelity of tendon repair in mature horses. Tendondevelopment can conveniently be considered to have an early ‘cellular’phaseandasubsequent‘matrix’phase.Inthematrix-dominatedphaseoftendon development 3D scanning electron microscopy of mouse tendonsuggests that fibripositors of the cells are the site of new fibril formationandthemechanicalinterfacebetweenthecellandtheextracellularmatrix.It is hypothesised that fibripositors exert pulling forces on collagen fibrils,and their cellular forces require functional myosin II, which is anintracellular molecular motor that is part of the actinomyosin system. Adetailed understanding of how cells set the tensional homeostasis oftendon is expected to lead to new strategies for regulating collagen fibrilassemblyinhealthandintendinopathy.

Previous studies have proposed that standard ultrasonography may not adequately represent the pertinent anatomic characteristics of the equine proximal suspensory ligament. The purpose of the study was to compare the use of standard ultrasonography, angle contrast ultrasonography, MRI, and histology for identification of the anatomic characteristics of the normal equine suspensory ligament in the forelimb. Horses free from forelimb lameness with no palpable abnormalities in the region of the suspensory ligament were included in the study. The proximal suspensory ligaments in 20 forelimbs were examined using the standard ultrasound technique, angle contrast ultrasound technique, and MRI, followed by histologic evaluation. Total transverse (cross-sectional) area of the proximal suspensory ligament was estimated using the standard ultrasound and the angle contrast ultrasound techniques, MRI, and histologic sections for the following parameters: total area of the ligament, ligament fibers, muscle, and fat. The proximal suspensory ligament lobe size and tissue distribution were compared and subjectively graded (0-4) for asymmetry. Subjectively, angle contrast ultrasound technique improved differentiation of fibers from the remaining tissue types and allowed identification of the peripheral ligament margin. There was no significant difference in asymmetry scores between modalities. The asymmetry scores of the right and left forelimbs were significantly different with both ultrasound and MRI, based on the level of measurement. The angle contrast ultrasound technique has limitations compared to MRI. However, it provides additional diagnostic information that is not available with the standard ultrasound technique.

OBJECTIVE To evaluate the efficacy of IV administration of a product containing hyaluronan, sodium chondroitin sulfate, and N-acetyl-d-glucosamine for prevention or treatment of osteoarthritis in horses. ANIMALS 32 healthy 2- to 5-year-old horses. PROCEDURES The study involved 2 portions. To evaluate prophylactic efficacy of the test product, horses received 5 mL of the product (n = 8) or saline (0.9% NaCl) solution (8; placebo) IV every fifth day, starting on day 0 (when osteoarthritis was induced in the middle carpal joint of 1 forelimb) and ending on day 70. To evaluate treatment efficacy, horses received either the product or placebo (n = 8/treatment) on days 16, 23, 30, 37, and 44 after osteoarthritis induction. Clinical, diagnostic imaging, synovial fluid, gross anatomic, and histologic evaluations and other tests were performed. Results of each study portion were compared between treatment groups. RESULTS Limb flexion and radiographic findings were significantly worse for horses that received the test product in the prophylactic efficacy portion than for placebo-treated horses or product-treated horses in the treatment efficacy portion. In the prophylactic efficacy portion, significantly less articular cartilage erosion was identified in product-treated versus placebo-treated horses. In the treatment efficacy portion, joints of product-treated horses had a greater degree of bone edema identified via MRI than did joints of placebo-treated horses but fewer microscopic articular cartilage abnormalities. CONCLUSIONS AND CLINICAL RELEVANCE Results suggested that caution should be used when administering the evaluated product IV to horses, particularly when administering it prophylactically, as it may have no benefit or may even cause harm.

International audience; Injury to the suspensory ligament can result in significant lameness. Multiple imaging modalities may be required to fully characterize injury to the suspensory ligament. Ultrasonography of the suspensory ligament is challenging, due to the normal anatomy. This article describes imaging of the proximal suspensory ligament in the front and hind limbs of the horse.

Eight skeletally mature equine cadaver distal forelimbs were imaged using T2-weighted fast spin echo (FSE) sequences in a 1.0 T horizontal bore magnet. Each limb was parallel to the main magnetic field and with 16° angulation of the limb relative to the main magnetic field, which places one of the collateral ligaments of the distal interphalangeal joint at or near the magic angle. Each limb was imaged using an echo time (TE) of 80, 100, 120, and 140 ms. Reversal of the magic angle effect was achieved at echo time of 140 ms. However, given the alterations in tissue contrast and subjective decrease in the signal-to-noise ratio at this TE, it may be preferable to use a shorter TE for clinical imaging. A T2-weighted FSE sequence with an echo time of 120 ms maintained image quality while subjectively minimizing the magic angle effect. A sequence with long TE can be used to aid in the differentiation of pathologic change from artifactual increases in signal intensity in collateral ligaments of the equine distal interphalangeal joint, but could decrease the sensitivity for small or low contrast lesions. Multiple factors should be considered when selecting the TE for a T2-weighted FSE sequence that will be utilized in a musculoskeletal protocol including evaluation of equine feet.

Objective To describe regions of the thoracic and pelvic limb proximal interphalangeal (PIP) joints that are arthroscopically accessible and identify soft tissue structures that should be avoided during arthroscope and instrument placement. Study Design Experimental ex vivo descriptive study. Animals Horses (n = 15). Methods Cadaver limbs (n = 36) were used for anatomic modeling, magnetic resonance imaging (MRI) with MRI-compatible needles, computed tomography with contrast arthrography, and arthroscopy of the PIP joint. Two arthroscopic approaches to the dorsal joint pouch were compared. Results With arthroscopy, 62.4% of the joint perimeter could be observed from the dorsal and palmar/plantar joint pouches with no significant difference in the amount of joint observed when using the more proximal or distal approach to the dorsal joint pouch (P =.59). Conclusion The dorsal and palmar/plantar joint pouches provide adequate arthroscopic visibility for the axial portions of the articular surface of the proximal and middle phalanx. The abaxial portions of the articular surface were difficult to view because of narrowing of the joint pouches abaxially. When comparing the proximal and distal approach to the dorsal joint pouch, arthroscope insertion 1.5 cm proximal to the joint allowed the easiest manipulation of the arthroscope. Palmar/plantar portals were placed dorsal to the neurovascular bundle, proximal to the epicondyle of the middle phalanx to prevent tendon and ligament injury.

Recent advancements in the quality and availability of imaging modalities have allowed clinicians to diagnose fractures in horses with hindlimb lameness. Many imaging modalities aid in the diagnosis of pelvic fractures, including radiography, nuclear scintigraphy, computed tomography, and ultrasonography. Ultrasonography is an appropriate initial diagnostic tool when a pelvic fracture is suspected. The use of ultrasonography minimizes many of the risks and complications associated with the radiographic, scintigraphic, and computed tomographic evaluation of pelvic fractures, and is readily available to equine practitioners. This manuscript provides a detailed description of a complete transrectal and transcutaneous ultrasonographic examination of the equine pelvis. The described method has been effective in the diagnosis of pelvic fractures in a series of eight cases. Transrectal ultrasonography was found effective in revealing fractures of the ischiatic table, acetabulum, pubis, and ilium. Transcutaneous ultrasonography effectively identified fractures of the ilium, acetabular rim, femoral neck, greater trochanter, and a capital physeal fracture with a subluxated femoral head.

High- and low-field magnetic resonance (MR) imaging systems are available for clinical diagnosis of collateral desmopathy of the equine distal interphalangeal joint (DIJ). Knowledge of the normal appearance, size, shape, and signal variation of these ligaments on high- and low-field MR images is essential when assessing desmopathy detected by MR imaging. However, there are no descriptions of the normal features of DIJ collateral ligaments on images obtained with a standing low-field MR system. Low-field MR imaging characteristics of normal collateral ligaments of the DIJ of cadaver feet were corroborated with high-field MR imaging and histologic examination to exclude desmopathy. The size and shape of the collateral ligaments of the DIJ was similar among limbs; however, the signal pattern of the ligaments varied depending on the segment of the ligament being assessed and the MR sequence used. In limbs positioned within the magnet as recommended for clinical MR imaging, collateral ligaments of the DIJ have heterogeneous signal pattern with a peripheral region of increased signal intensity at the level of the middle phalanx that can be confused with a desmopathy. The MR imaging characteristics of normal collateral ligaments of the DIJ are related to their anatomy and fiber configuration. The results of this study support the presence of magic angle effect within the axial margin of the CL of the DIJ at the level of fiber divergence within the proximal to mid-portion of the ligament.

Summary Reasons for performing study: While descriptions of the visible soft tissues of the femorotibial joints exist for both arthroscopy and ultrasonography, there are few examples in the literature that discuss in detail the combined findings of these modalities. Objectives: To further elucidate the ultrasonographic and arthroscopic boundaries of the normal equine femorotibial joints and improve the understanding of the benefits and limitations of each individual modality. Methods: Simultaneous arthroscopy and ultrasonography were performed in 10 equine cadaver stifles as well as bilateral stifles on a horse that underwent nonrecovery surgery. The arthroscopic probe was visualised ultrasonographically and concurrent video and still images acquired. Results: Arthroscopy provided good visualisation of the cranial meniscal ligaments, the distal portion of the cranial cruciate ligament, proximal portion of the medial collateral ligament within the fibrous tissue of the joint capsule and a limited view of the abaxial border of meniscus. Ultrasonography allowed for almost complete visualisation of the menisci, collateral ligaments and cranial meniscal ligaments and a portion of the cranial cruciate ligament. Conclusions: By comparing the ultrasonographically and arthroscopically visible structures, this study allowed for a more complete understanding of the advantages and limitations of each modality. The ability of ultrasonography to resolve mild pathological changes should be further explored. Potential relevance: When used together, these modalities can provide a more global image of the femorotibial joints.

Magnetic resonance (MR) imaging is often performed to determine the cause of palmar heel pain. We evaluated how distension of the navicular bursa affected the MR appearance of the navicular bursa and associated structures. An MR evaluation was performed on normal cadaver limbs and cadaver limbs from horses with lameness localized to the foot. The normal navicular bursae were injected with 2, 4, or 6 ml of solution. The bursae of the feet from lame horses were injected with 4 or 6 ml, and the MR study was repeated. All bursae were examined grossly to verify the presence or absence of adhesions. Clinical patients that had initial MRI abnormalities suggesting adhesions were also evaluated. Distension of the proximal recess of the normal navicular bursa, proximal to the collateral sesamoidean ligament was achieved with 2 ml. Separation of the collateral sesamoidian ligament from the deep digital flexor tendon (DDFT) was achieved with 4 ml. The separation of the navicular bone from the DDFT and distal sesamoidian impar ligament required 6 ml. Adhesions were more clearly defined in the bursa of the two pathologic cadaver limbs following distension. MR bursography used on clinical patients allowed the determination of the presence or absence of adhesions. In these horses, this determination could not have been definitively made without this technique. MR bursography is useful in horses where the presence of adhesions cannot be clearly defined by MRI.

Ungual cartilage ossification in the forelimb is a common finding in horses. Subtle abnormalities associated with the ungual cartilages can be difficult to identify on radiographs. Magnetic resonance (MR) imaging findings of 22 horses (23 forelimbs) with a fracture of the distal phalanx and ossified ungual cartilage were characterized and graded. All horses had a forelimb fracture. Eleven involved a left forelimb (seven medial; four lateral), and 12 involved a right forelimb (five medial; seven lateral). All fractures were nonarticular, simple in configuration, and nondisplaced. The fractures were oriented in an axial proximal to abaxial distal and palmar to dorsal direction, and extended from the base of the ossified ungual cartilage into the distal phalanx. The fracture involved the fossa of the collateral ligament on the distal phalanx in 17 of 23 limbs. The palmar process and ossified ungual cartilage was abnormally mineralized in all horses. Ligaments and soft tissues adjacent to the ossified ungual cartilages were affected in all horses. The routine site of fracture in this study at the base of the ossified ungual cartilage extending into the distal phalanx suggests a biomechanical cause or focal stress point from cycling. The ligamentous structures associated with the ungual cartilages were often affected, showed altered signal intensity as well as enlargement and were thought to be contributing to the lameness. In conclusion, ossified ungual cartilages may lead to fracture of the palmar process of the distal phalanx and injury of the ungual cartilage ligaments.

Six cadaver forelimbs were imaged in two high-field magnetic resonance (MR) systems and one low-field MR system following the creation of osteochondral defects on the palmar distal aspect of the third metacarpal bone. The following sequences were performed using all three systems: proton density (PD) turbo spin echo, T2* gradient echo (GRE), T2-weighted fast spin echo, and short tau inversion recovery. In addition, 3D T1 GRE sagittal standard and motion insensitive sequences were obtained using the low-field system. PD fat saturated and 3D T1-weighted spoiled GRE images with and without fat suppression were acquired with the high-field systems. Lesions were measured and assigned a confidence score. The images obtained using high-field systems (1.0 and 1.5 T) more accurately represented the osteochondral defects when compared with low-field system (0.27 T) images. The largest difference was observed when evaluating articular cartilage defects, which were not identified on the low-field images. Sequence selection affected the appearance of the lesions. On all systems the turbo and fast spin echo sequences more accurately represented the lesion size and shape when compared with the GRE sequences. The T1 GRE sequence is the only sequence that appears to allow visualization of the articular cartilage on the low-field images, but is limited in providing adequate cartilage visualization. Confidence scores were greater on the high-field systems when compared with the low-field system.

Summary Reasons for performing study: Currently, there are limited data regarding the long-term outcome of horses with foot pain treated with corrective shoeing, rest and rehabilitation, and intrasynovial anti-inflammatory medication to target lesions detected with MRI. Objective: To report the long-term (≥12 months) outcome of horses with foot lesions following medical therapy. Hypotheses: 1) There is no association between clinical parameters considered and a poor response to therapy. 2) Horses with a deep digital flexor tendinopathy are less likely to respond to medical therapy than horses without a deep digital flexor tendinopathy. Methods: The medical records of horses with foot pain subjected to MRI examination and medical therapy (2005–2007) were evaluated retrospectively. Data collected included history, signalment, occupation, duration and severity of lameness at the time of MRI, radiological and MRI abnormalities. Number of treatments, complications and long-term response to treatment were obtained by detailed telephone questionnaires. Association between clinical and MRI findings and long-term lameness were investigated. Results: Frequent abnormal structures included the navicular bone, the deep digital flexor tendon, the navicular bursa and the distal interphalangeal joint. Thirty-four of 56 horses (60.7%) failed to return to previous level of exercise due to persistent or recurrent lameness or owners' decision to decrease the horse's athletic level; however, 11 horses (32.3%) were being used for light riding. Prognosis for horses with concurrent deep digital flexor tendon, navicular bone and navicular bursa lesions was worse than horses with individual lesions. Deep digital flexor tendinopathy was strongly associated with persistent or recurrent lameness. Conclusions: Horses with multiple foot lesions managed with conservative therapy have a guarded prognosis for long-term soundness. Deep digital flexor tendinopathies negatively influence prognosis.

The objective of the present study was to determine the efficacy of urinary bladder matrix (UBM) in collagenase-induced superficial digital flexor (SDF) tendonitis by using clinical, ultrasonographic, and histologic data. A total of eight healthy adult horses were used in this study. Bilateral forelimb SDF tendonitis was created in the horses by injecting collagenase. After 14 days, one randomly selected forelimb SDF tendon was blindly treated with UBM and the opposite tendon was treated with a control (saline). Clinical and ultrasonographic parameters including lameness, lesion size, ultrasonographic fiber pattern, and echogenicity were measured throughout the study. After 84 days, horses were euthanized and SDF tendon lesions from the two groups were compared statistically using an analysis of variance with significance set at P ≤ .05.Results showed that there were no significant differences between the treated and control tendons for any of the clinical, ultrasonographic, gross, or histologic variables. UBM does not appear to be an effective treatment for collagenase-induced SDF tendonitis. However, there may be differences in clinical tendonitis that might render the treatment more effective in the clinical setting.

Distal forelimb specimens of eight skeletally mature horses were imaged using proton density turbo spin echo, T1-weighted spoiled gradient echo, T2*-weighted gradient echo, short tau inversion recovery and T2-weighted fast spin echo sequences with the limb parallel to the main magnetic field, and with angulation of the limb relative to the main magnetic field. The magic angle effect can be identified in the collateral ligaments of the distal interphalangeal joint when imaged in a high-field magnetic resonance (MR) imaging system with a horizontally oriented main magnetic field. This effect has previously been described in the collateral ligaments of the distal interphalangeal joint in a low-field system with a vertically oriented main magnetic field. The curvature of the ligaments places the fibers at the magic angle in both horizontally and vertically orientated main magnetic fields. This effect can be identified on short time of echo sequences and impacts the signal pattern of the ligaments at the level of the middle phalanx with the limb in a neutral position and with angulation of the limb. Magic angle effect should be considered as a possible cause of an asymmetrical signal pattern, depending on the positioning of the limb and the sequences used for imaging, when evaluating the collateral ligaments of the distal interphalangeal joint on images acquired with a high-field MR imaging system that has a horizontally oriented main magnetic field.

The purpose of this study was the assessment of clinical, biochemical, and histologic effects of intraarticular administered adipose-derived stromal vascular fraction or bone marrow-derived mesenchymal stem cells for treatment of osteoarthritis. Osteoarthritis was induced arthroscopically in the middle carpal joint of all horses, the contralateral joint being sham-operated. All horses received treatment on Day 14. Eight horses received placebo treatment and eight horses received adipose-derived stromal vascular fraction in their osteoarthritis-affected joint. The final eight horses were treated the in osteoarthritis-affected joint with bone marrow-derived mesenchymal stem cells. Evaluations included clinical, radiographic, synovial fluid analysis, gross, histologic, histochemical, and biochemical evaluations. No adverse treatment-related events were observed. The model induced a significant change in all but two parameters, no significant treatment effects were demonstrated, with the exception of improvement in synovial fluid effusion PGE2 levels with bone marrow-derived mesenchymal stem cells when compared to placebo. A greater improvement was seen with bone marrow-derived mesenchymal stem cells when compared to adipose-derived stromal vascular fraction and placebo treatment. Overall, the findings of this study were not significant enough to recommend the use of stem cells for the treatment of osteoarthritis represented in this model.

Objective—To determine clinical outcome following intrathecal injection of the podotrochlear (navicular) bursa for signs of foot pain in horses evaluated via magnetic resonance imaging (MRI) and evaluate efficacy of corticosteroids administered with or without hyaluronate. Design—Retrospective case series. Animals—23 horses. Procedures—Data collected included signalment, history, intended use, duration and severity of lameness, results of diagnostic anesthesia, radiographic abnormalities, MRI abnormalities, and outcomes for return to use. Results—MRI was conducted on 23 horses with lameness localized to the foot. Thirteen horses had bilateral forelimb lameness, and 10 had unilateral forelimb lameness. Mean duration of lameness was 10.5 months. Seventeen of 23 (74%) horses had excellent outcomes and returned to intended use within 2 to 4 weeks after navicular bursa injection. Hyaluronate treatment was not associated with outcome; however, horses receiving < 10 mg of trimacinolone had significantly worse outcomes than those treated with hyaluronate. Among horses with excellent outcomes, mean duration of soundness was 7.3 months. Seven of 8 horses with erosive lesions of the flexor surface of the distal sesamoid (navicular) bone diagnosed via MRI had a poor outcome. Horses with navicular bursitis responded optimally to injection, compared with horses with other problems. Conclusions and Clinical Relevance—Results suggested that intrathecal injection of corticosteroid in horses with erosions of the flexor surface of the navicular bone associated with deep digital flexor tendon adhesions yielded a poor response. Treatment of horses with navicular bursitis via injection of the navicular bursa should be highly effective in alleviating lameness.

Objective—To assess clinical, biochemical, and histologic effects of polysulfated glycosaminoglycan (PSGAG) or sodium hyaluronan administered intra-articularly in treatment of horses with experimentally induced osteoarthritis. Animals—24 horses. Procedures—Osteoarthritis was induced arthroscopically in 1 middle carpal joint of all horses. Eight horses received hyaluronan (20 mg) and amikacin (125 mg) intra-articularly on study days 14, 21, and 28. Eight horses received PSGAG (250 mg) and amikacin (125 mg) intra-articularly on study days 14, 21, and 28. Eight control horses received 2 mL of saline (0.9% NaCl) solution and amikacin (125 mg) intra-articularly on study days 14, 21, and 28. Clinical, radiographic, synovial fluid analysis, gross, histologic, histochemical, and biochemical findings were evaluated. Results—No adverse treatment-related events were detected. Induced osteoarthritis caused a substantial change in lameness, response to flexion, joint effusion, and radiographic findings, and of these, synovial fluid effusion was reduced with PSGAG, compared with control horses. No changes in clinical signs were seen with PSGAG or hyaluronan, compared with control horses. Histologically, the degree of synovial membrane vascularity and subintimal fibrosis was significantly reduced with PSGAG treatment, compared with controls. Histologically, significantly less fibrillation was seen with hyaluronan treatment, compared with controls. Conclusions and Clinical Relevance—Results indicated that PSGAG and hyaluronan had beneficial disease-modifying effects and are viable therapeutic options for osteoarthritis in horses.

Objective—To assess the clinical, biochemical, and histologic effects of topically administered diclofenac liposomal cream (DLC) in the treatment of horses with experimentally induced osteoarthritis. Animals—24 horses. Procedures—Osteoarthritis was induced arthroscopically in 1 middle carpal joint of all horses. Eight horses treated with DLC were given 7.3 g twice daily via topical application. Eight horses treated with phenylbutazone were given 2 g orally once daily. Eight control horses received no treatment. Evaluations included clinical, radiographic, magnetic reso-nance imaging, synovial fluid, gross, and histologic examinations as well as histochemical and biochemical analyses. Results—No adverse treatment-related events were detected. Horses that were treated with DLC or phenylbutazone had significant clinical improvement of lameness, unlike the control horses. Treatment with DLC induced significant improvement in staining and total articular glycosaminoglycan content, compared with no treatment. Treatment with phen-ylbutazone induced significant reduction in synovial fluid prostaglandin E2 concentration, compared with DLC and no treatment. Treatment with DLC induced significantly less radial carpal bone sclerosis and overall gross cartilage erosion, compared with phenylbutazone. Conclusions and Clinical Relevance—Results indicated that DLC had both clinical sign–modifying and disease-modifying effects. Only clinical sign–modifying effects were detected in association with phenylbutazone administration. Treatment with DLC had significant beneficial effects, compared with phenylbutazone, and no detrimental effects. Results suggested that DLC is a viable therapeutic option for horses with osteoarthritis.

Objective—To evaluate the use of a combination of avocado and soybean unsaponifiable (ASU) extracts for the treatment of experimentally induced osteoarthritis in horses. Animals—16 horses. Procedures—Osteoarthritis was induced via osteochondral fragmentation in 1 middle carpal joint of each horse; the other joint underwent a sham operation. Horses were randomly allocated to receive oral treatment with ASU extracts (1:2 [avocado-to-soybean] ratio mixed in 6 mL of molasses; n = 8) or molasses (6 mL) alone (placebo treatment; 8) once daily from days 0 to 70. Lameness, response to joint flexion, synovial effusion, gross and histologic joint assessments, and serum and synovial fluid biochemical data were compared between treatment groups to identify effects of treatment. Results—Osteochondral fragmentation induced significant increases in various variables indicative of joint pain and disease. Treatment with ASU extracts did not have an effect on signs of pain or lameness; however, there was a significant reduction in severity of articular cartilage erosion and synovial hemorrhage (assessed grossly) and significant increase in articular cartilage glycosaminoglycan synthesis, compared with placebo-treated horses. Conclusions and Clinical Relevance—Although treatment with ASU extracts did not decrease clinical signs of pain in horses with experimentally induced osteoarthritis, there did appear to be a disease-modifying effect of treatment, compared with findings in placebotreated horses. These objective data support the use of ASU extracts as a disease-modifying treatment for management of osteoarthritis in horses.

As use of magnetic resonance (MR) imaging evolves in veterinary medicine, the importance of field strength will be a continued source of debate. This article addresses the fundamental differences between high- and low-field MR imaging systems. The magnet and room construction, examination time, image quality, and implications on diagnostic accuracy are discussed. Although many injuries can be well characterized with both high- and low-field systems, a high-field system is required to identify certain lesions. Further studies are needed to define the difference in the detectability of lesions on high-field versus low-field MR imaging systems.

The femorotibial joints are a common source of lameness in Western performance horses. The objective of this prospective study was to compare the radiography, ultrasonography, computed tomographic arthrography (CTA), and arthroscopy findings in horses with lameness localized to the femorotibial joints. Twenty-five stifles in 24 horses were included and were evaluated with all four of these diagnostic methods. Defects detected in femorotibial joint structures were compared between diagnostic methods using a McNemar's test to evaluate for disagreement. Cranial medial meniscotibial desmopathy was most detected on arthroscopy (in 14/25 cases) and was only detected on ultrasonography in three out of 11 (27.3%) arthroscopically observed cases, but was detected on CTA in nine out of 12 (75%) arthroscopically observed cases. Medial meniscal injury located on the craniolateral border was most detected on arthroscopy (n = 9) and was detected on CTA in five cases, but on ultrasonography in 0 cases. Detection of articular cartilage defects on the medial femoral condyle was most detected with arthroscopy (24/25, 96% cases) and was also detected on CTA in 12/20 (60%) cases with a significant disagreement identified between modalities (P = 0.02). Cranial and caudal cruciate ligament defects were detected on CTA in 6/22 (27.3%) and 7/19 (36.8%) cases, respectively, and with arthroscopy in 3/25 (12%) and 2/25 (8%) cases, respectively. The use of CTA detected more defects in the cruciate ligaments, proximal tibia, and ligament entheses than the other diagnostic methods, but was not reliable for detection of articular cartilage damage on the medial femoral condyle.

Magnetic resonance imaging (MRI) is becoming readily available to equine veterinarians. As with any new imaging modality it is necessary to understand the uses, limitations, and diagnostic capabilities. One of the most valuable uses of MRI in adult horses is the ability to image the soft tissues of the extremities, providing diagnostic information not possible with any other currently available imaging modality. MRI provides both anatomic and physiologic tissue characteristics allowing diagnosis of soft tissue and osseous injuries. This chapter provides an introduction to basic MRI principles: safety and technical considerations, physics, sequences, tissue appearance on images, and image interpretation. Specific examples of normal anatomic structures and structures with pathologic change are provided. Determining the normal appearance of structures with age and exercise is necessary for image interpretation and identification of pathologic change. Collaboration will expedite this process and benefit clinicians and patients. Soft tissue and osseous injuries characterized by using MRI will allow specific diagnosis, appropriate and targeted treatment, rehabilitation, and monitoring.

SummaryReasons for performing study To evaluate and compare the diagnostic capability of arthroscopy and ultrasonography for the detection of pathological change in equine stifle joints. Although descriptions of the arthroscopic and ultrasonographic boundaries of the normal femorotibial joint exist, there are few examples in the literature comparing the pathological changes observed with each imaging modality. Objectives To evaluate and compare arthroscopic and ultrasonographic examinations for characterising pathological change in the stifle joint. To describe how the results of arthroscopic and ultrasonographic examinations may differ in characterising the severity of lesions and to evaluate which lesions are best assessed with each modality. Study design Retrospective review of ultrasonographic and arthroscopic examinations. Methods The structures of the stifle joint were evaluated and graded for pathological change by scoring arthroscopic and ultrasonographic examinations. The presence and severity of the lesions were then compared between each modality. Results Medial meniscal lesions were detected more often with ultrasonography than with arthroscopy. Conversely, arthroscopy was better for detection of cranial medial meniscotibial ligament (CrMMTL) tearing. Articular cartilage defects were best detected with arthroscopy and periarticular osteophytes of the medial femoral condyle with ultrasonography. Four cases had defects within one of the patellar ligaments, all of which were only characterised with ultrasonography. Conclusions Ultrasonography and arthroscopy should be combined to best evaluate pathology of the stifle, since each modality has its own limitations depending on the location and type of lesion.

Objective—To determine the effect of limb positioning on the radiographic appearance of the distal and proximal interphalangeal joint spaces of the forelimbs of horses during evaluation of dorsopalmar radiographs. Design—Evaluation study. Animals—14 forelimbs from 9 adult horses. Procedures—Each horse was in standing position with its forelimbs positioned on blocks. Dorsopalmar radiographs of each foot were obtained with the forelimbs positioned squarely (the metacarpus of both forelimbs was perpendicular to the ground as determined by visual examination [abducted 0°]; baseline) and abducted 5° and 10°. The width of the space at the medial and lateral aspects of the distal and proximal interphalangeal joints (medial and lateral joint space width, respectively) was measured. Mediolateral joint balance was calculated as the difference between the widths of the lateral and medial joint spaces, and joint space width and mediolateral joint balance were compared among all 3 positions. Results—As the extent of limb abduction increased, the medial aspect of the proximal and distal interphalangeal joints became narrower, compared with the corresponding lateral aspect of those joints. For both the distal and proximal interphalangeal joints, the mediolateral joint balance differed significantly among all limb positions. Conclusions and Clinical Relevance—Forelimb position significantly affected the mediolateral joint balance of the interphalangeal joints of horses. Thus, it is crucial that the forelimbs of horses be squarely positioned when dorsopalmar radiographs are obtained for accurate evaluation of interphalangeal joint space and balance.

A randomized blinded placebo controlled trial was conducted to assess the clinical, biochemical and histological effects of a hyaluronan, sodium chondroitin sulfate and N-acetyl-D-glucosamine combination (PG) administered through an intra-articular (IA) route for the treatment of osteoarthritis (OA) at the time of injury. OA was induced in one carpal joint of each of 16 horses. Horses were designated placebo or IA PG treated. All horses were treated with 125 mg amikacin sulfate IA and 5 mL physiological saline in the middle carpal joint bilaterally on study Days 0 (after induction of OA), 7, 14 and 28, except the OA affected joint of the IA PG horses, which received 5 mL PG plus 125 mg of amikacin sulfate on similar days. Evaluations included clinical and radiographic, synovial fluid analysis, gross and histological examinations, as well as histochemical and biochemical analyses. The model induced a significant pathology that resulted in clinical disease. No adverse treatment-related events were detected in any of the horses. Intra-articular treatment of OA-affected joints with PG resulted in a transient 16% improvement in clinical pain (lameness scores) and evidence of improvement trends in bone proliferation radiographically as well as in the degree of full thickness articular cartilage erosion seen grossly when compared to placebo treated OA affected joints, although the vast majority of outcome parameters were not significantly different than controls. The findings support some potential clinical sign or disease modifying action of this compound administered IA at the tested dose and frequency.

Recheck magnetic resonance imaging (MRI) studies are critical to assess response to treatment and correctly determine both a horse's ability to return to exercise and an appropriate exercise program. As with any other modality, many diagnoses require additional monitoring. This principle applies to MRI despite the more significant financial investment and potential requirement for general anesthesia. This article explores the use of recheck MRIs in case management and proposes time frames for these studies based on the initial diagnosis.
