Occlusions of the basilar artery (BA), which are typically caused by thromboembolism and atherosclerosis, may account for as much as 27% of posterior circulation ischemic strokes. The symptoms of BA occlusion can range from temporary weakness or paraesthesia to near-complete paralysis, leading to a high risk of morbidity and mortality. CT and MRI are the imaging modalities of choice for identifying and locating the infarct area, ruling out any other lesions, and detecting vascular abnormalities. The combination of thrombectomy and stenting has been shown to increase the safety and durability of angioplasty without causing noticeably negative side effects, and it may be utilized as the first line of treatment for BA stenosis or occlusion in acute stroke.

The use of healthcare monitoring systems in hospitals and other health facilities has grown exponentially, and portable healthcare monitoring systems based on new technologies have emerged as a severe concern in several countries throughout the globe. The emergence of the Internet of Things (IoT) technology has contributed to the progress of healthcare from face-to-face counseling to telemedicine. The project aims to design a reliable health monitoring system utilizing the IoT, Arduino, and Android applications. It can be used to measure body temperature, heart rate, and level of oxygen in the blood in both hospitals and homes. The system is also capable to verify that the transmission of the sensor’s data to the host computer is done in real-time. The results show that the system reading is approximately 100% accurate as of the purchased measuring instruments or actual devices. Therefore, doctors and family members can now monitor and track the patient’s health anywhere through this smart health monitoring system.

Rasmussen’s aneurysm is a pulmonary artery pseudoaneurysm, secondary to the invasion of granular tissues to the pulmonary artery wall, causing massive hemoptysis. A 39-year-old male, with a history of pulmonary TB presented with massive hemoptysis, persistent cough, and dyspnea. Chest X-ray and Chest CT depicted active pulmonary tuberculosis and a co-infection with aspergillosis in the form of aspergilloma. Chest CT angiography (CTA) showed narrow-necked Rasmussen’s aneurysm with the feeding artery coming from the pulmonary artery of the apicoposterior segment and the systemic artery from the left thyrocervical trunk branch and left bronchial artery. Fluoroscopy-guided transarterial embolization with polyvinyl alcohol (PVA) and gel foam was performed from the left bronchial artery and thyrocervical trunk branch followed by transcatheter embolization with glue insertion via the pulmonary artery in the aneurysmal site. After the procedures the embolic agent completely filled the aneurysm and there were no complaints of hemoptysis during the follow-up. Appropriate chest CT angiography procedures can help diagnose Rasmussen’s aneurysm and become a road map for embolization. Transarterial catheter embolization from bronchial or non-bronchial systemic artery and pulmonary artery can be used as the treatment modality of choice for Rasmussen’s aneurysm in pulmonary tuberculosis with aspergillosis co-infection in the form of aspergilloma.

Gonore adalah infeksi menular seksual (IMS) yang dapat terjadi pada laki-laki dan perempuan yang disebabkan oleh bakteri diplokokus Gram negatif, Neisseria gonorrhoeae. Insiden infeksi gonore tahun 2014 di RSUD. Dr. Saiful Anwar Malang (RSSA) diketahui sebanyak 60 orang dari total 399 pasien IMS. Tahun 2015, tercatat 34 orang pasien infeksi baru gonore. Kejadian ini meningkat karena beberapa faktor di antaranya adalah pengetahuan dan hubungan seksual berisiko tinggi, yang tampak saat ini merupakan sebagian kecil dari keseluruhan pasien IMS. Tujuan penelitian adalah untuk mengetahui pengaruh tingkat pengetahuan terhadap hubungan seksual berisiko tinggi serta terhadap kejadian infeksi gonore di RSUD Dr. Saiful Anwar Malang. Populasi adalah pasien laki-laki terinfeksi gonore yang datang ke RSSA berjumlah 93 orang dan berdasarkan formula Lemeshow didapatkan. sampel sebanyak 36 responden. Pengambilan sampel dengan metode consecutive sampling. Path analysis (analisis jalur) dilakukan untuk menemukan penjelasan mengenai pengaruh langsung dan tidak langsung dari variabel tingkat pengetahuan, hubungan seksual berisiko tinggi, dan infeksi gonore berdasarkan beberapa pertimbangan teoritis serta pengetahuan peneliti yang ditampilkan dalam bentuk diagram jalur yang berfungsi untuk membantu dalam melakukan konseptualisasi masalah yang kompleks dan mengenai implikasi empirik dari teori yang sedang diuji. Hasil menunjukkan bahwa tingkat pengetahuan memiliki pengaruh yang signifikan dan berpengaruh positif terhadap hubungan seksual berisiko tinggi. Dapat disimpulkan bahwa tingkat pengetahuan terhadap hubungan seksual berisiko tinggi memiliki pengaruh yang signifikan terhadap infeksi gonore, namun tidak berpengaruh signifikan terhadap kejadian infeksi gonore. Serta, tingkat pengetahuan tidak berpengaruh signifikan terhadap infeksi gonore melalui hubungan seksual berisiko tinggi, dan hubungan seksual berisiko tinggi tidak berpengaruh signifikan terhadap kejadian infeksi gonore.

Malignant primary and metastatic brain tumors are group of malignancies radiologically difficult to distinguish between one another. Meanwhile, the treatment regimens between the two entities are very different. The right regimen can maintain patient’s survival. MRI is the modality of choice for diagnosing brain tumors; although, malignant primary brain tumors and solitary metastases appear similar on conventional MRI. The difference in the pathophysiology of peritumoral edema in malignant primary and metastatic brain tumors has the potential for differentiation of the two entities. In malignant primary brain tumors, tumor cell infiltration occurs in the edema area, meaning that the peritumoral edema is narrower than that of the metastases. This study analyzed the ratio of peritumoral edema volume to tumor (EP/T volume ratio) in malignant primary and metastatic brain tumors by means of MRI examination with a cross-sectional design, using MRI data on FLAIR and T1WI sequences with contrast in malignant brain tumor of patients that have been pathologically proven. Then, volume contouring was performed on peritumoral edema (EP) and tumor (T), and comparation was done to obtain the EP/T volume ratio. The ratio of EP/T volume data in both groups was analyzed using the Mann–Whitney test with the SPSS 22 software. The results of statistical analysis revealed that the EP/T volume ratio of the malignant primary brain tumor group was smaller with a median value (max-min) of 1.1 (5.65-0.17) and in the metastatic group, 2.3 (64.03-0.09). Chest pain is a common clinical presentation in daily practice. Musculoskeletal origin is a rare etiology of chest pain, compared to a cardiorespiratory problem and often underrecognized. Fork-rib or bifid-rib is a rare anomaly which uncommonly present with clinical symptoms since in most cases fork-rib incidentally found during cadaveric dissection. Here we report a 27 years old man presenting with chest pain and radiographic examination showing bifid rib of the fifth left rib, without any abnormalities from physical examination and electrocardiography. The patient treated with intravenous painkiller and anticonvulsants. Fork-rib should be considered as a differential diagnosis for chest pain of musculoskeletal origin especially in young adults or chest pain precede with minor trauma.Â

Penelitian ini bertujuan untuk menilai efektivitas penggunaan modalitas ultrasonografi dan X-ray dalam mengevaluasi ukuran dan fase kalsium pada tendinitis kalsifik bahu dan mengkorelasikan dengan hasil pemeriksaan klinis berupa skor Constant dan Visual Analog Scale (VAS). Sebanyak 14 pasien (usia 58,6±9,74 tahun, 73% perempuan) mengalami kalsifikasi tendon bahu. Tingkat nyeri pasien diklasifikasikan sebagai tidak nyeri (0), ringan (1-3), sedang (4-7), dan berat (8-10). Kemampuan aktivitas sehari-hari diklasifikasikan bertingkat sebagai buruk ( 0,231). Dapat disimpulkan bahwa tidak terdapat hubungan yang signifikan antara temuan radiologis dengan derajat penurunan kemampuan aktivitas sehari-hari.

This study examines ALOS-2 PALSAR L-band dual-polarization (HH and HV) SAR data and Landsat-8 optical imagery for land cover classification. The SAR data has been preprocessed first, which included radiometric calibration, geocoding, and speckle filtering. The HH/HV band ratio has been used to create the third band, and thus a synthetic RGB SAR image was created. The Landsat-8 data was also preprocessed for the classification process. For land cover classification of both SAR and optical datasets, the supervised maximum likelihood classifier was used. Training samples were selected from the Landsat-8 optical imagery with the support of information available in Google Earth; the same pixel locations of training data were used to extract training data from SAR image as well. The Landsat-8 optical imagery was classified and also used for visual assessment of the SAR land cover classification results. Accuracy assessment has been done for both the results of SAR and Landsat-8 data. The SAR classified output gives us accuracy of 93.15% and the Landsat-8 classified map accuracy was 91.34%, while the Kappa coefficient for SAR and Landsat-8 classified images is 0.92 and 0.89, respectively. Classification limitations exist in some cases, such as roads being merged in vegetation areas and some of the barren land is merged in settlements. The land cover classification can be expected to be further improved using polarimetric decomposition methods and fusion of SAR data with optical data.
