Between samples A and C, the change in bacteria count was 7.5% (0.2–44%) in the LCS group and 8.3% (0.3–39%) in the FCS group (p = 0.55). We examined 62 subjects in each group (LCS and FCS), and samples A, B, and C were collected from all participants in both groups. Nasal bleeding occurred in 42/62 in the LCS group and 22/62 in the FCS group (p < 0.01). Conclusion: Cotton swab thickness did not impact disinfection efficacy, but large swabs increased the risk of nasal bleeding. We recommend FCS for nasal disinfection prior to NTI in ASA 1–2 patients, as they reduce bleeding risk without compromising disinfection. Trial registration: UMIN-CTR (registration no. Introduction: Epistaxis is common throughout pregnancy and is usually not a cause for concern; severe nosebleeds are rare. An increased rate of nasal bleeding was observed during the COVID-19 epidemic. Materials and Methods: The study sample comprised 3,362 pregnant women who sought care at the Gynecologic and Obstetrics Department/Al-Yarmouk Teaching Hospital and Al-Karkh Maternity Hospital between May 2020 and April 2021. All were asked to fill out a questionnaire. Results: Nine hundred forty-one pregnant women experienced an episode of epistaxis during the last pregnancy. One thousand seven hundred forty-eight pregnant women had a corona-positive history. Pregnant women with a positive history of coronavirus infection have a higher incidence of epistaxis (612 pregnant women) than pregnant women with a corona-negative history (329 pregnant women) P value (0.039%). Conclusions: Oxygen and blood-thinning drugs are the leading causes of the increased rate of nosebleeds among pregnant women during the Corona pandemic. [ABSTRACT FROM AUTHOR]

ObjectiveThis study aimed to develop and validate a dynamic nomogram to assess the risk of nasal bleeding after endoscopic transnasal transsphenoidal pituitary tumor resection.MethodsA retrospective analysis was conducted on patients who underwent endoscopic transnasal transsphenoidal pituitary tumor resection from June 2019 to June 2021. Univariate and multivariate logistic regression analyses were used to screen for independent risk factors for nasal bleeding from the training set. A multivariate logistic regression model was established, a nomogram was plotted, and it was validated in an internal validation set. The performance of the nomogram was evaluated based on the receiver operating characteristic (ROC) curve, calibration curve, and decision curve analysis (DCA).ResultsThe nomogram indicators included anticoagulant use, sphenoid sinus artery injury, nasal irrigation, platelet count (PLT), and constipation. The predictive model had an area under the ROC curve of 0.932 (95% CI: 0.873–0.990) and 0.969 (95% CI: 0.940–0.997) for the training and validation sets, respectively, indicating good discrimination. The calibration curve showed good consistency between the actual and predicted incidence of nasal bleeding (p > 0.05). DCA indicated that the nomogram had good clinical net benefit in predicting postoperative nasal bleeding in patients.ConclusionIn summary, this study explored the incidence and influencing factors of nasal bleeding after endoscopic transnasal transsphenoidal pituitary tumor resection and established a predictive model to assist clinical decision-making.

Transnasal endoscopic sphenopalatine artery occlusion procedures are becoming the standard of care for intractable posterior epistaxis. Improved endoscopic anatomical features of the lateral nasal wall and endoscopic skill with high-resolution cameras result in a higher success rate of endoscopic intervention. To evaluate the safety and effectiveness of endoscopic cauterization of the sphenopalatine artery (ESPAC) in controlling intractable posterior nasal bleeding. This prospective cohort study enrolled patients with refractory posterior epistaxis from August 2016 to December 2019. The trial recruited patients between 18 and 65 years of age with a history of recurrent and refractory posterior epistaxis receiving endoscopic arterial cauterization due to conservative treatment failure. All of the cases involved bipolar cauterization. Recurrent nosebleeds must pause for at least three months for a procedure to be considered successful. In the first 30 days following surgery, complications are recorded. 415 patients with epistaxis received both inpatient and outpatient care. Transnasal ESPAC was necessary for 36 patients (11.5%). The most common comorbidity was hypertension accounting for 9 (23%) cases. Thus, 26 of 36 (72%) cases had a unilateral ESPAC, while 10 (28%) had a bilateral ESPAC. Twenty-two (61%) and ten (28%) patients had single and two branching patterns of the sphenopalatine artery, respectively. Septal correction and middle meatus antrostomy (44%) were the most performed additional procedures. During the three-month follow-up period, 35 patients in this study had epistaxis control; the success rate of ESPAC was 97.2%. There were no significant postoperative complications found. Endoscopic sphenopalatine artery cauterization is successful in controlling 97.2% of posterior epistaxis. It is safe and effective without any significant complications. [ABSTRACT FROM AUTHOR]

Objectives: Endovascular embolization has emerged as an effective treatment for intractable epistaxis. This systematic review and meta-analysis aimed to calculate the rates of success, rebleeds, and complications and to identify the etiologies and complications of patients who undergo endovascular embolization. Methods: This systematic review and meta-analysis was conducted per the guidelines set forth by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Articles were extracted from Scopus, PubMed, Web of Science, and Cochrane Central and were filtered by a systematic review process using Rayyan software. A random-effects model was used to quantify the rates success, rebleeds, and complications. Results: Forty-two studies were included, totaling 1660 patients. The pooled success rate was 89% (95% confidence interval [CI] 86%-92%) and the pooled rebleed rate was 19% (95% CI 16%-22%). The pooled minor complication rate was 18% (95% CI 11%-27%). The most common major complication was soft tissue necrosis followed by stroke. The most common minor complication was facial pain. No minor complications were reported to be permanent. Of the patients who failed initial embolization, 42% underwent repeat embolization and 34% underwent surgical arterial ligation. Conclusions: Endovascular embolization is an effective treatment for intractable epistaxis. Abstract Background Giant prolactinoma (> 4 cm in dimension) is a rare disorder. Invasive macroprolactinoma has the potential to cause base of skull erosion and extend into the nasal cavity or even the sphenoid sinus. Nasal bleeding caused by intranasal tumor extension is a rare complication associated with invasive giant prolactinoma. We report a case of giant invasive macroprolactinoma with repeated nasal bleeding as the initial symptom. Case presentation A 24-year-old man with an invasive giant prolactinoma in the nasal cavity and sellar region who presented with nasal bleeding as the initial symptom, misdiagnosed as olfactory neuroblastoma. However, markedly elevated serum prolactin levels (4700 ng/mL), and a 7.8-cm invasive sellar mass confirmed the diagnosis of invasive giant prolactinoma. He was treated with oral bromocriptine. Serum prolactin was reduced to near normal after 6 months of treatment. Follow-up magnetic resonance imaging showed that the sellar lesion had disappeared completely and the skull base lesions were reduced. Conclusion This case is notable in demonstrating the aggressive nature of untreated invasive giant prolactinomas which can cause a diagnostic difficulty with potential serious consequences. Early detection of hormonal levels can avoid unnecessary nasal biopsy. Early identification of pituitary adenoma with nasal bleeding as the first symptom is particularly important.

The information about condition as a part of the problem of municipal urgent ENT-help for public of Russian Federation is interesting for practicing physicians. The more often causes of nasal bleeding and organizing problems by their treatment are studied. Contemporary methods of treatment of those diseases are presented. With the aim considerable the improvement quality of medical help for patients by this type of urgent ENT-pathology and the organization roads of decision of this problem are proposed.

Soft tissue mineralization and epithelial ulceration are common findings in dogs with uremia, being commonly reported in the gastrointestinal tract, lungs and pleura. This report described a case of nasal mucosal mineralization and ulceration contributing to recurrent epistaxis in a dog with chronic renal failure and uremia. A dog with recurrent epistaxis accompanied by elevated urea and creatinine was hospitalized. Platelet count and coagulation tests were within normal limits. Chronic renal failure was diagnosed, and the dog was euthanized. On necropsy, the kidneys were small, with an irregular capsular surface. The nasal conchae were slightly reddish. Histopathology revealed chronic glomerulonephritis, with gastric mineralization and bilateral parathyroid hyperplasia. Vascular and basal lamina mineralization, epithelial ulceration and hemorrhage were seen in the nasal conchae. The observed findings indicated that nasal mineralization and ulceration were caused by uremia. The severity of histopathological findings suggested that nasal mineralization/ulceration may have caused or at least contributed to epistaxis in this dog. We hope to stimulate further investigations into possible association between uremia, nasal mucosa mineralization/ulceration and epistaxis in dogs. [ABSTRACT FROM AUTHOR]

Introduction. The number of patients with nasal bleedings (NB) remains quite high (14.7–20.5 %) among all patients in need of emergency ENT care. When using anticoagulants and disaggregants, the risk of developing hemorrhagic complications is always higher. Otorhinolaryngologists have been improving the methods of stopping NC for many years. Objective: a comparative study of the effect of different types of nasal tamponade on mucociliary transport (MCT) of the nasal mucosa in patients with NB on the background of antithrombotic therapy. Materials and methods. The effect of different types of nasal tamponade on MCT of the nasal mucosa in 156 patients with NC who developed on the background of antithrombotic therapy was studied. All patients were divided into two groups: the main (104 patients), in which NB cessation was performed by the combined method, which includes the use of a two-chamber hydraulic tampon of our own design in combination with Nosochem gel and thermal exposure, and the control group (52 patients), where the classic gauze tamponade was used. The criterion for assessing the functional state of the nasal mucosa was the time of MCT when performing the saccharin test. Results. Immediately after removal of tampons, a significant acceleration of the MCT rate was observed in patients of both groups in comparison with the conditional norm. BackgroundInternal carotid artery pseudoaneurysm (PSA) is a serious complication after radiotherapy for nasopharyngeal carcinoma, and once it ruptures and bleeds, it will seriously affect the patient's survival and prognosis. However, because of its relatively low incidence, many medical institutions lack experience in managing this type of emergency.Case informationIn this case report, we described two cases suffered ruptured internal carotid artery PSA after radiotherapy for nasopharyngeal carcinoma, including their history, diagnosis, and treatment. Both cases underwent emergency endovascular interventions, one of which with long-term healing after embolization of the PSA, and the other one with re-bleeding after embolization and was eventually stopped by embolization of the parent artery. Ultimately, both cases received timely and effective treatment.ConclusionThis case report detailed the diagnosis and treatment course of internal carotid artery PSA after radiotherapy for nasopharyngeal carcinoma, which enhanced the understanding of this emergency, and provided valuable information and experience for the treatment strategy of similar PSA on the internal carotid artery.

To reduce nasal bleeding, the nasal mucosa is treated with vasoconstrictors (epinephrine [E] or tramazoline [T]) prior to NTI. This study aimed to determine whether E or T is more effective and safe for reducing nasal bleeding during NTI. Methods This study was preregistered on UMIN-CTR after being approved by the IRB of the School of Dentistry at Aichi Gakuin University. Written consent was received from all the patients. Total 206 patients aged 20–70 years and classified as 1–2 on American Society of Anesthesiologists-physical status were scheduled to undergo general anesthesia with NTI. At last, 197 patients were randomly divided into two groups and treated with either E (n = 99; 3 patients were discontinued) or T (n = 98; 2 patient were discontinued). After induction of general anesthesia, each patient’s nasal mucosa was treated using either E or T. The E used in this study was BOSMIN® SOLUTION 0.1% (Daiichi-Sankyo Co., Ltd., Tokyo), and the T used in this study was TRAMAZOLIN Nasal Solution 0.118% AFP, (Alfresa Pharma Corporation, Osaka). E was diluted five times according to the package insert (final concentration of E = 0.02%), and T was used in its original solution. After 2 min, NTI was performed via the right nostril. Primary outcome were the presence of nasal bleeding (if bleeding was recognized at the posterior pharyngeal wall via nasal cavity during intubation, it was defined as bleeding) and the degree of bleeding (classified as none, mild, moderate, or severe). Secondary outcomes were arrhythmia, and hemodynamic (mean atrial pressure and heart rate) changes associated with vasoconstrictors. Results The presence of bleeding was comparable in both groups (12.5%, E; 14.5%, T; P = 0.63). No significant difference between the groups regarding the degree of bleeding (P = 0.78) was observed, with most patients having no bleeding (n = 84, E; n = 82, T). No severe bleeding and no arrhythmias induced by vasoconstrictor were observed in the two groups. Conclusions Nasal treatment with E or T shows no difference in nasal bleeding during NTI. Although no arrhythmia associated with E was observed in this study, it has been reported in literature. Background: During non-invasive respiratory support, administration of gases at high flow rates requires proper air conditioning to avoid upper airways complications. However, closed-loop control of temperature & relative humidity (T&RH) of mixed gases systems with wired-heated circuits are not commonly available in low-middle income countries (LMIC). Thus, a policy for close control of gas T&RH to overcome our system limitations was implemented. The objective of this study was to evaluate the impact of this intervention.Method: Single center retrospective study including preterm infants admitted to a level II unit (June 2019–2020). Close control of gas T&RH was implemented in Dec 2019 and consisted of starting with the heater at a set number higher than previously and followed by adjustments every 6 h based on intermittent temperature checks (target: 36.5°C–37°C). Humidity was controlled by adjusting the water level every 6 h and checking for the presence of condensation. Two groups were identified: infants treated with or without the close T&RH control. Primary outcome was the occurrence of nasal bleeding. Secondary outcomes were adherence to the policy, need to change the original mode of respiratory support or endotracheal intubation. Appropriate statistical tests were applied.Results: A total of 128 neonates were included: 63 managed without and 65 with close gas T&RH control. Rates of nasal bleeding decreased significantly with the close T&RH control, from 30% to 14% (p = 0.026) despite longer duration of non-invasive support (CPAP: 1.8 ± 1.9 vs 4.3 ± 5.8 days; p = 0.002 and NIPPV: 1.8 ± 1.4 vs 4.2 ± 5.2; p = 0.009). Adherence to the policy was 93%. Changes of the original mode of respiratory support also decreased significantly (14% vs 3%; 0.024) with no intubation.Conclusion: In the absence of advanced and expensive systems, a simple and low-cost intervention to optimize gas T&RH during non-invasive respiratory support in LMIC is feasible and associated with clinical improvements. Currently, standard practice includes the initial use of topical anesthetics and vasoconstrictors, followed by more invasive treatments such as nasal packing, cauterization or surgical ligation for refractory cases. Over the years several studies have investigated the potential use of topical Tranexamic Acid (TXA) in the management of epistaxis. We have conducted a meta-analysis to assess the efficacy of topical TXA versus other standard practices or placebo in the management of epistaxis. PubMed and Scopus databases were searched from inception to April 2021. We included randomized controlled trials and observational studies investigating the efficacy of TXA in bleeding cessation in epistaxis in adults. The primary outcome measured was the prevalence of bleeding cessation after treatment at first assessment. Other outcomes were bleeding reoccurrence between 24 and 72 h and at 7–8 days. A random-effects model was used to estimate odds ratio (OR) for outcomes. A total of eight studies were included in the analysis, including seven randomized trials and one retrospective study. We included a total of 1299 patients, 596 (46%) received TXA while 703 (54%) received control treatment (placebo, lidocaine plus vasoconstrictors or local anesthetics). Patients who were treated with TXA were 3.5 times (OR 3.5, 95% CI 1.3–9.7) more likely to achieve bleeding cessation at the first assessment. Patients treated with TXA had 63% (OR 0.37, 95% CI 0.20–0.66) less likelihood of returning due to rebleeding at 24–72 h. A 49-year-old Japanese man presented with a rare case of sinonasal leiomyosarcoma with left nasal bleeding for 12 months. He had no history of irradiation or malignancies, including retinoblastoma. Preoperative histological examination suggested vascular leiomyoma. Complete resection with endoscopic surgery was performed. Histological examination during the operation suggested that the tumor was a leiomyoma. However, immunohistochemical staining for α smooth muscle actin and desmin were helpful in establishing a definitive diagnosis of sinonasal leiomyosarcoma. The resection margins were positive for tumor cells. Staging with CT of the neck and thorax, ultrasound of the abdomen, and MRI of the head ruled out metastases. Second endoscopic tumor resection surgery was performed for positive resection margins. The patient's condition was successfully managed with additional excision, and he remains well with no evidence of recurrence and metastasis 36 months after treatment. Lobular capillary hemangiomas (LCHs) are benign vascular lesions of the skin, oral cavity, and, rarely, the nasal cavity that are histologically characterized by capillary proliferation and a lobular architecture. The etiology of LCH is not well understood, but the possible underlying factors include trauma, hormonal influences, and angiogenic growth factors. This case report describes the spontaneous complete regression of an LCH without further surgery although it was incompletely excised. To our knowledge, this is a rare case for a middle-aged man. A 79-year-old Japanese woman presented with a rare case of metastatic renal cell carcinoma to the left sphenoid sinus with left nasal bleeding. She had previously had right radical nephrectomy for renal cell carcinoma at the age of 64 years and brain and spinal cord infarction at 74 years. Endoscopic examination revealed no mass in the nasal cavity. CT and MRI revealed a tumor in the left sphenoid sinus. The size of the tumor increased gradually from 12 to 15 years after the radical nephrectomy. Complete resection with endoscopic surgery was performed without preoperative embolization. The tumor cells had clear cytoplasm and were arranged in a trabecular pattern lined by a layer of endothelial cells. These findings were identical to the pathological findings of the surgical specimen of the renal cell carcinoma from 15 years previous. A pathological diagnosis of metastatic renal cell carcinoma of clear cell type (grade 1) was made. PET-CT demonstrated no metastasis. The patient's condition was successfully managed with excision of the tumor, and she remains well with no evidence of recurrence and metastasis 36 months after treatment. Metastatic renal cell carcinoma to the sphenoid sinus is rare, but it might be considered in the differential diagnosis of masses in the paranasal sinus even long after initial treatment of renal cancer. The predisposition of horses to nasal bleeding significantly affects their performance and requires special control, particularly in the breeding work aimed at improving the breed and increasing its working abilities. One of the causes of such bleeding is spontaneous nucleotide substitutions in the D'-D3 domain of the von Willebrand factor. The presence and distribution of mutations in the D'-D3 domain of this factor (c.2826 A > C in exon 22, leading to the replacement of lysine by asparagine) in Russian horse breeds (Russian Trotter, Russian Saddle horse, Russian Heavyweight, Altai, and Karachai horse breeds) were analyzed. The highest frequency of this replacement was found in the Russian Trotter and Russian Heavyweight. We aimed to clarify the possible association of this mutation spread with the population and genetic relationship between horse breeds. The polylocus genotyping of 26 genomic DNA fragments flanked by inverted repeats sections of microsatellites (GA)9C, (AG)9C, and (GAG)6C (Inter Simple Sequence Repeats—ISSR-PCR markers) was performed, genetic distances were calculated, and corresponding dendrograms were constructed. It was found that the prevalence of the detected mutation is not related to the population and genetic proximity of the breeds. Historically, the Orlov horse breed had been involved in the breeding of the Russian Trotter and Russian Heavyweight. Therefore, it is possible that the high frequency of this mutation in the Russian Trotter and the Russian Heavyweight may be due to its presence in the Orlov horse breed that was the source for these breeds. Objectives: Epistaxis, especially posterior epistaxis, is occasionally refractory to treatment. In these cases, sphenopalatine artery surgeries, including cauterization and ligation, are required. Previous reports have demonstrated treatment results for these procedures but failed to provide high-level evidence. The aim of this study was to quantify the rates of failure and perioperative complications of these procedures by using a meta-analysis technique.Methods: We systematically searched electronic databases and identified articles regarding epistaxis, sphenopalatine artery ligation, or cauterization. Pooled rebleeding and complication rates were calculated by using a random effects model.Results: A total of 896 cases of sphenopalatine ligation or cauterization for epistaxis were analyzed. Pooled rebleeding rates for the entire cohort, cauterization group, and ligation group were 13.4% (95% confidence interval [CI] 10.0-17.8, P < 0.001), 7.2% (95% CI 4.6-11.0, P < 0.001), and 15.1% (95% CI 9.8-22.5, P < 0.001), respectively. Pooled perioperative complication rates for the entire cohort, cauterization group, and ligation group were 8.7% (95% CI 4.9-15.1, P < 0.001), 10.2% (95% CI 3.8-24.5, P < 0.001), and 6.4% (95% CI 1.8-20.9, P < 0.001), respectively.Conclusion: Overall, sphenopalatine surgery for refractory epistaxis is an effective method because of its low rates of failure and complications. Aneurysmal bone cysts (ABCs) are non-neoplastic, highly vascularised bone lesions that rarely involve the craniofacial complex. We describe the first case of an ABC involving the inferior turbinate in a 17-month-old boy who attended our Pediatric Emergency Department because of recurrent epistaxis, and discuss the diagnostic and therapeutic work-up in children with nasal masses. Background: Despite the modern society, nasal bleeding has been increased these days in Saudi Arabia people these days due to multiple factors. People knowledge about this important situation is not quit enough. Attitude toward nasal bleeding is different from one person to another which may reflect the outcomes of nasal bleeding. Objectives: Nasal bleeding (epistaxis) is causing trouble to the patients in a multiple aspect. This study aimed at assessing the prevalence of nasal bleeding (epistaxis) in Saudi Arabia and to know the participants attitude towards it. Methods: A cross-sectional study on the prevalence of nasal bleeding (epistaxis) and the attitude towards it was randomly investigated among men and women (392 participants) within different social media platforms in Saudi Arabia during the period from January to March 2018. Results: in 27% of the participants have been suffering from nasal bleeding, only 17.4% seek medical help, 16.3% tried traditional medicine to stop the nasal bleeding, 5.6% are using anticoagulants, 49% had a positive family history of nasal bleeding and 9.4% had a chronic illness. Epistaxis, commonly known as nasal bleeding, ranks among the most prevalent emergencies encountered in otorhinolaryngology. The etiology of epistaxis is multifaceted, arising from both local and systemic factors. In Saudi Arabia, a country with a relatively high prevalence of epistaxis, understanding the level of awareness and attitudes toward first aid management of epistaxis is of paramount importance. This systematic review aims to bridge this knowledge gap by evaluating the awareness of and attitudes toward epistaxis first aid in Saudi Arabia. This systematic review and meta-analysis adhered to Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) guidelines. A comprehensive electronic search was executed across PubMed, Google Scholar, and Web of Science databases, encompassing studies published between January 2015 and July 2023. The study included exclusively cross-sectional studies, assessing awareness and attitude toward epistaxis first aid in Saudi Arabia across all populations and studies in English. The 17 selected studies were all published after October 2017, with three published in the year of this systematic review (2023). Sample sizes exhibited substantial variability, ranging from 57 to 2,441 individuals. Despite widespread awareness of epistaxis, the general population often disregards it as a minor health issue. This discrepancy highlights the importance of addressing epistaxis seriously, given the potential for severe bleeding as a medical emergency. The review of 17 studies revealed significant variations in epistaxis awareness levels, influenced by factors such as age, gender, and varying sample sizes. Notably, higher awareness levels were observed in studies involving the general Saudi population and those employing self-administered questionnaires. The average awareness and knowledge of epistaxis and its management among Saudi residents were moderate, with an estimated awareness level of 63%. Background Epistaxis, or nosebleeds, is a widespread medical condition that can be effectively managed with appropriate first aid. Understanding the general public's knowledge and practices about this is crucial. Objectives This study sought to evaluate the awareness and practice regarding first aid for epistaxis within the general population of the Jazan region in Saudi Arabia. Methods A cross-sectional survey was administered from April through June 2023, using a questionnaire that covered sociodemographic factors, knowledge of epistaxis, first aid practices for epistaxis, and any previous training received. Statistical analysis was performed using SPSS (IBM Corp., Armonk, NY), with chi-square tests to evaluate the variables' associations. Results The questionnaire was completed by 622 participants, predominantly females, Saudis, and individuals from the age group of 18 to 25 years. It was found that 60% of the participants had experienced epistaxis, but only 52% had received prior first aid training. Although the majority (91.8%) accurately defined epistaxis, a mere 40.8% correctly identified all the steps for first aid management of epistaxis. There was a notable insufficiency in understanding the causes, risk factors, and appropriate first aid steps. Participants' knowledge was evenly split, with approximately half exhibiting low knowledge (49.70%) and the remainder showing high knowledge (50.30%). Certain sociodemographic factors such as older age (p=0.028), Saudi nationality (p=0.045), and higher education (p=0.001) were linked with more experiences of epistaxis. Conversely, younger age (p=0.002), female gender (p=0.036), single status (p=0.001), prior experience with epistaxis (p=0.001), and higher overall knowledge (p=0.001) were associated with a higher likelihood of having received first aid training. Conclusions The study reveals significant gaps in the knowledge and practices of first aid for epistaxis among the general population in the Jazan region. Public awareness campaigns and educational programs are urgently needed, particularly for specific groups. Enhancing first aid knowledge could help alleviate the impacts of epistaxis. Spontaneous epistaxis under general anesthesia is a rare untoward event. Typically, mild epistaxis occurs from a nasal airway manipulation and usually resolves spontaneously or with external pressure to the affected nares for a short period of time. In case of epistaxis that is difficult to control with pressure or oxymetazoline, thorough evaluation should be considered. We present a case of a 12‐year‐old experiencing spontaneous nasal bleeding upon emergence difficult to control with noninvasive methods and required intervention by otolaryngology. AIM: To find an efficient and simple surgical procedure of the external dacryocystorhinostomy(EXT- DCR)in the treatment of chronic dacryocystitis.METHODS: A total of 270 cases were performed in this retrospective study. A comparison of clinical effects and hemorrhage complications between the experimental group, in which 189 eyes of 167 patients that underwent traditional EXT-DCR without packing, and the control group, in which 121 eyes of 103 patients that underwent traditional EXT-DCR with Vaseline gauze were performed. The postoperative follow-up ranged from 6 to 36mo(the mean follow-up was 15±3.6mo).RESULTS: There were no significant differences in the curative rates between the two groups(97.9% vs 97.5%, P>0.05). But the incidence rate of moderate or severe postoperative nasal bleeding was lower in the experimental group(15.9%, 30 eyes)than the control group(31.4%, 38 eyes)and PCONLUSION: The traditional EXT-DCR without packingis an effective procedure and is more effective in reducing moderate or severe postoperative bleeding when the intraoperative anastomose and the postoperative drainage are well done and the intraoperative bleeding is well controlled.

Conclusions: Transarterial embolization (TAE) appears to be a safe and effective treatment for patients with intractable epistaxis, despite different etiologies or angiography findings. Idiopathic epistaxis is prone to present with negative angiographic findings.Objective: To retrospectively evaluate the safety and effectiveness of TAE for intractable epistaxis, and focus on the factors of etiology and angiographic findings.Materials and methods: From March 2008 to December 2014, the data of 43 patients with intractable bleeding undergoing TAE were reviewed. The outcomes of interventional therapy were assessed according to different etiology (malignant or benign disease) and angiographic finding (positive or negative angiogram).Results: Positive angiographic findings were found in 11 of 12 cases with malignant diseases and 22 of 31 cases with benign diseases, respectively (p = 0.237). Among the 10 cases with negative angiographic findings, the negative angiography rate of idiopathic epistaxis was higher than that of epistaxis with definite etiology (p = 0.003). Bleeding was controlled successfully in all of the 43 patients after embolization. During the mean follow-up period of 24.0 ± 16.7 months, five patients relapsed. No significant difference was found in recurrence rates between malignant and benign diseases or between positive and negative angiography (p = 0.241,p = 0.704, respectively).
