Abstract

Minor histocompatibility (H) antigens are allogeneic target molecules having significant roles in alloimmune responses after human leukocyte antigen-matched solid organ and stem cell transplantation (SCT). Minor H antigens are instrumental in the processes of transplant rejection, graft-versus-host disease, and in the curative graft-versus-tumor effect of SCT. The latter characteristic enabled the current application of selected minor H antigens in clinical immunotherapeutic SCT protocols. No information exists on the global phenotypic distribution of the currently identified minor H antigens. Therefore, an estimation of their overall impact in human leukocyte antigen-matched solid organ and SCT in the major ethnic populations is still lacking. For the first time, a worldwide phenotype frequency analysis of ten autosomal minor H antigens was executed by 31 laboratories and comprised 2,685 randomly selected individuals from six major ethnic populations. Significant differences in minor H antigen frequencies were observed between the ethnic populations, some of which appeared to be geographically correlated.

Abstract

Background: \ud Spontaneous coronary artery dissection (SCAD) is an increasingly recognized cause of acute coronary syndromes (ACS) afflicting predominantly younger to middle-aged women. Observational studies have reported a high prevalence of extracoronary vascular anomalies, especially fibromuscular dysplasia (FMD) and a low prevalence of coincidental cases of atherosclerosis. PHACTR1/EDN1 is a genetic risk locus for several vascular diseases, including FMD and coronary artery disease, with the putative causal noncoding variant at the rs9349379 locus acting as a potential enhancer for the endothelin-1 (EDN1) gene.\ud \ud Objectives: \ud This study sought to test the association between the rs9349379 genotype and SCAD.\ud \ud Methods: \ud Results from case control studies from France, United Kingdom, United States, and Australia were analyzed to test the association with SCAD risk, including age at first event, pregnancy-associated SCAD (P-SCAD), and recurrent SCAD.\ud \ud Results: \ud The previously reported risk allele for FMD (rs9349379-A) was associated with a higher risk of SCAD in all studies. In a meta-analysis of 1,055 SCAD patients and 7,190 controls, the odds ratio (OR) was 1.67 (95% confidence interval [CI]: 1.50 to 1.86) per copy of rs9349379-A. In a subset of 491 SCAD patients, the OR estimate was found to be higher for the association with SCAD in patients without FMD (OR: 1.89; 95% CI: 1.53 to 2.33) than in SCAD cases with FMD (OR: 1.60; 95% CI: 1.28 to 1.99). There was no effect of genotype on age at first event, P-SCAD, or recurrence.\ud \ud Conclusions: \ud The first genetic risk factor for SCAD was identified in the largest study conducted to date for this condition. This genetic link may contribute to the clinical overlap between SCAD and FMD.

5. Cytokine gene polymorphisms: methods of detection and biological significance

Abstract

In recent years, several functional polymorphisms, particularly, SNPs have been identified in cytokines and their receptor genes that regulate levels of cytokine expression. These have been implicated as immune prognostic markers in diseases, including differential response to therapy and as biomarkers of graft outcome following organ and stem cell transplantation. Population distribution of cytokine gene polymorphisms (CGPs) reveals significant variations in allele frequencies in different ethnic groups and this might explain, to some extent, the observed differences in SNP associations with various diseases and immune-pathologies. A number of molecular methods are available for defining CGPs. These include PCR-SSP, AFLP, Taqman probe assays as well as sequencing based typing. Of these, the PCR-based sequence-specific primer based test is the most widely accepted technique. This chapter describes steps involved in this procedure along with sources for procuring essential reagents. An important aspect of CGP analyses is the correct interpretation of results particularly determination of their multilocus haplotypes.

Abstract

The lack of epidemiologic data on invasive Streptococcus pyogen es infections in many developing countries is concerning, as S. pyogenes infections are commonly endemic in these areas. Here we present the results of the first prospective surveillance study of invasive Streptococcus pyogenes infections in India. Fifty-four patients with invasive S. pyogenes infections were prospectively enrolled at two study sites, one in the north and one in the south of India. Sterile-site isolates were collected, and clinical information was documented using a standardized questionnaire. Available acute-phase sera were tested for their ability to inhibit superantigens produced by the patient's own isolate using a cell-based neutralizing assay. The most common clinical presentations were bacteremia without focus (30%), pneumonia (28%), and cellulitis (17%). Only two cases of streptococcal toxic shock syndrome and no cases of necrotizing fasciitis were identified. Characterization of the isolates revealed great heterogeneity, with 32 different emm subtypes and 29 different superantigen gene profiles being represented among the 49 sterile-site isolates. Analyses of acute-phase sera showed that only 20% of the cases in the north cohort had superantigen-neutralizing activity in their sera, whereas 50% of the cases from the south site had neutralizing activity. The results demonstrate that there are important differences in both clinical presentation and strain characteristics between invasive S. pyogenes infections in India and invasive S. pyogenes infections in Western countries. The findings underscore the importance of epidemiologic studies on streptococcal infections in India and have direct implications for current vaccine developments.

Abstract

During the Ninth International Histocompatibility Workshop (1984), several laboratories reported the results of restriction fragment length polymorphism (RFLP) studies that used HLA class II gene probes (1-6). It was apparent from this work and other reports in the literature (7-12) that this new technique could provide a powerful tool for the study of HLA region polymorphism. One of the aims of the Tenth International Histocompatibility Workshop (1987) was to provide a reference standard of RFLP patterns using a battery of restriction enzymes and a panel of well characterized lymphoblastoid B cell lines. These cell lines were chosen because they were recognized to be HLA-D homozygous by the laboratories of origin. Homozygosity was a characteristic that allowed their use in cellular typing and would facilitate the interpretation of the results of the RFLP analysis.
