Abstract

Background and purpose: Clinical outcomes of multiple sclerosis (MS) patients affected by coronavirus disease 2019 (COVID-19) have been thoroughly investigated, but a further analysis on main signs and symptoms and their risk factors still needs attention. The objective of this study was to group together and describe based on similarity the most common signs and symptoms of COVID-19 in MS patients and identify all factors associated with their manifestation. Method: Logistic and linear regression models were run to recognize factors associated with each pooled group of symptoms and their total number. Results: From March 2020 to November 2021, data were collected from 1354 MS patients with confirmed infection of COVID-19. Ageusia and anosmia was less frequent in older people (odds ratio [OR] 0.98; p=0.005) and more in smoker patients (OR 1.39; p=0.049). Smoke was also associated with an incremental number of symptoms (OR 1.24; p=0.031), substance abuse (drugs or alcohol), conjunctivitis and rash (OR 5.20; p=0.042) and the presence of at least one comorbidity with shortness of breath, tachycardia or chest pain (OR 1.24; p=0.008). Some disease-modifying therapies were associated with greater frequencies of certain COVID-19 symptoms (association between anti-CD20 therapies and increment in the number of concomitant symptoms: OR 1.29; p=0.05). Differences in frequencies between the three waves were found for flu-like symptoms (G1, p=0.024), joint or muscle pain (G2, p=0.013) and ageusia and anosmia (G5, p < 0.001). All cases should be referred to variants up to Delta. Conclusion: Several factors along with the choice of specific therapeutic approaches might have a different impact on the occurrence of some COVID-19 symptoms.

Abstract

Contemporary game studies have opened to an increasingly deep problematisation of video game worlds, both from arepresentational and an interactional point of view: there is now widespread talk of how video games contribute to buildinterpretations of the world, and trajectories to convey ideologically, politically, historically, and socially defined positions.In this context, studies on the relationship between video games and gender or ethnic representations are increasinglypresent (and decisive). On the other hand, the relationship between video games and disability, although equally complexand delicate, is surprisingly little studied: not only is the number of academic studies on the subject very low, but quan‐titative evaluations on the issue tend to be rare.This contribution aims, on the one hand, to offer a synthetic view of the studies published so far about disability andvideo games; to provide keys to interpretation and reading to stimulate a more lively debate on the subject; and, finally,to offer an overview of the accessibility solutions that video games offer in order to better interpret the phenomenon.

12. Automatic Generation of Domain-Aware Control Plane Logic for Software Defined Railway Communication Networks

Abstract

The emergence of 5G technologies opens up new opportunities for railway communications. One of the foundational aspects of 5G architecture is its control-plane programmability, which can be achieved through Software Defined Networking (SDN). In railway scenarios, this can be used to dynamically reconfigure the network for a more effective and efficient management of communication flows produced by moving trains. The paper presents a framework for integrating modelling and analysis tools into a programmable control plane specifically tailored to railway communications. We introduce the concept of domain-awareness in the network control plane as an SDN-enabled feature that allows achieving application-specific advantages besides those purely expressed in terms of key performance indicators such as the quality of service. We propose a reference architecture in which domain-awareness in the control plane is obtained by considering information gathered by network devices and ad-hoc communication gateways that are able to detect relevant signalling events. In the architecture, the actual behaviour of the SDN controller is governed by applications that are able to react to specific triggers and re-configure network devices accordingly. We also provide a methodological framework based on model-driven engineering and formal methods, including dynamic state machines, for the automatic generation of SDN control plane logic.

Abstract

Background and purpose Clinical outcomes of multiple sclerosis (MS) patients affected by coronavirus disease 2019 (COVID-19) have been thoroughly investigated, but a further analysis on main signs and symptoms and their risk factors still needs attention. The objective of this study was to group together and describe based on similarity the most common signs and symptoms of COVID-19 in MS patients and identify all factors associated with their manifestation. Method Logistic and linear regression models were run to recognize factors associated with each pooled group of symptoms and their total number. Results From March 2020 to November 2021, data were collected from 1354 MS patients with confirmed infection of COVID-19. Ageusia and anosmia was less frequent in older people (odds ratio [OR] 0.98; p = 0.005) and more in smoker patients (OR 1.39; p = 0.049). Smoke was also associated with an incremental number of symptoms (OR 1.24; p = 0.031), substance abuse (drugs or alcohol), conjunctivitis and rash (OR 5.20; p = 0.042) and the presence of at least one comorbidity with shortness of breath, tachycardia or chest pain (OR 1.24; p = 0.008). Some disease-modifying therapies were associated with greater frequencies of certain COVID-19 symptoms (association between anti-CD20 therapies and increment in the number of concomitant symptoms: OR 1.29; p = 0.05). Differences in frequencies between the three waves were found for flu-like symptoms (G1, p = 0.024), joint or muscle pain (G2, p = 0.013) and ageusia and anosmia (G5, p < 0.001). All cases should be referred to variants up to Delta. Conclusion Several factors along with the choice of specific therapeutic approaches might have a different impact on the occurrence of some COVID-19 symptoms.

Abstract

Background: The MuSC-19 project is an Italian cohort study open to international partners that collects data on multiple sclerosis (MS) patients with COVID-19. During the second wave of the pandemic, serological tests became routinely available. Objective: To evaluate the seroprevalence of anti-SARS-CoV-2 antibodies according to the use of disease-modifying therapy (DMT) in a subset of patients included in the MuSC-19 data set who had undergone a serological test. Methods: We evaluated the association between positive serological test results and time elapsed since infection onset, age, sex, Expanded Disability Status Scale score, comorbidities and DMT exposure using a multivariable logistic model. Results: Data were collected from 423 patients (345 from Italy, 61 from Turkey and 17 from Brazil) with a serological test performed during follow-up. Overall, 325 out of 423 tested patients (76.8%) had a positive serological test. At multivariate analysis, therapy with anti-CD20 was significantly associated with a reduced probability of developing antibodies after COVID-19 (odds ratio (OR) = 0.20, p = 0.002). Conclusion: Patients with MS maintain the capacity to develop humoral immune response against SARS-COV-2, although to a lesser extent when treated with anti-CD20 drugs. Overall, our results are reassuring with respect to the possibility to achieve sufficient immunization with vaccination.

Abstract

Within Artificial Intelligence, Deep Learning (DL) represents a paradigm that has been showing unprecedented performance in image and audio processing by supporting or even replacing humans in defect and anomaly detection. The Railway sector is expected to benefit from DL applications, especially in predictive maintenance applications, where smart audio and video sensors can be leveraged yet kept distinct from safety-critical functions. Such separation is crucial, as it allows for improving system dependability with no impact on its safety certification. This is further supported by the development of DL in other transportation domains, such as automotive and avionics, opening for knowledge transfer opportunities and highlighting the potential of such a paradigm in railways. In order to summarize the recent state-of-the-art while inquiring about future opportunities, this paper reviews DL approaches for the analysis of data generated by acoustic and visual sensors in railway maintenance applications that have been published until August 31st, 2021. In this paper, the current state of the research is investigated and evaluated using a structured and systematic method, in order to highlight promising approaches and successful applications, as well as to identify available datasets, current limitations, open issues, challenges, and recommendations about future research directions.

Abstract

When referring to the study of Rutherford et al. (2015) on short- and long-term effects of rTMS in Alzheimer's disease, we erroneously stated in our article (Lefaucheur et al., 2020) that a clinical benefit on cognitive performance (tested by the Montreal Cognitive Assessment) was observed at weeks 2–3, only in the real stimulation condition during an initial sham-controlled 4-week period of treatment, and that then this benefit was prolonged by “2 additional weeks” of open-label real rTMS. In fact, this extended open-label treatment was administered for up to 19 months in multiple 2-week blocks with 2–7 months intervals between blocks and was not limited to a single additional 2-week block of treatment immediately following the original 4 weeks of treatment. Therefore, the results of that open-label extended follow-up study support the value of long-term maintenance treatment using multiple rTMS sessions rather than be interpreted as short-term 2-week extension.

Il contributo si inserisce nel dibattito dei processi educativi e le rappresentazioni di genere, intrecciando tale filone di studi con gli approcci della media education. Essere maschio o femmina, come identità socialmente e cultural-mente costruite, deve fare i conti con le stratificazioni tra vecchie e nuove narrazioni, nonché silenzi, impregnati spesso di stereotipi che sfociano in pregiudizi e sessismi, per proporre nuove contro-narrazioni, nuove visioni. Una complessità narrativa rappresentata anche da uno storytelling che diventa crossmediale, ovvero il passaggio di uno stesso contenuto viene veicolato da un media ad un altro transmediale, per cui i contenuti di una storia vengono diversificati nei linguaggi e negli elementi dei media, creando una narrazione che può definirsi "convergente". Diventa, dunque, centrale, per un professionista dei processi socio-educativi e culturali, avere strumenti di analisi per costruire percorsi formativi di conoscenza critica e consapevole rispetto alle narrazioni stereotipate dei generi. Costruire l'empowement delle nuove generazioni significa partire da ciò che i bambini, le bambine e i ragazzi e le ragazze sanno, per condurli oltre e permettere loro di costruirsi uno sguardo attento, riflessivo, critico sui diversi media che attraversano quotidianamente le loro vite – dai cartoon, agli albi illustrati; dai giocattoli ai videogiochi; dai libri di narrativa alle serie tv e film – incoraggiandoli a documentare, analizzare e riflettere sui propri usi e su come sono rappresentate le identità in quegli specific media.

Abstract

This volume aims at offering a contribution to the studies on youth participation and active citizenship, according to a perspective that places both constructs in relation to the urban space dimension, understood as the scene of the expression of participation practices and the exercise of citizenship. The study of a city, developed with an interdisciplinary approach, shows how local governments promote specific political visions that justify equally peculiar ways of providing education: raising awareness, in this regard, is crucial and inevitably involves education professionals, the exclusive designers of inclusive processes and active citizenship. The theoretical analysis of the concepts of participation and citizenship implies an analysis of the “youth issues” from a historical-political and legislative point of view, showing that the analytical categorisations applied to youth and participation studies may result in a specialised vocabulary with negative connotations, and in a collection of educational projects developed differentially, absolutely distant from youth cultural practices. Finally, the results of a qualitative research carried out in Pescara, which involved students aged 15 to 19 years and was aimed at investigating the social representations of participation and citizenship, will be discussed.

Abstract

Objective: To evaluate the performance of a Random Forest (RF) classifier on Transcranial Magnetic Stimulation (TMS) measures in patients with Mild Cognitive Impairment (MCI). Methods: We applied a RF classifier on TMS measures obtained from a multicenter cohort of patients with MCI, including MCI-Alzheimer's Disease (MCI-AD), MCI-frontotemporal dementia (MCI-FTD), MCI-dementia with Lewy bodies (MCI-DLB), and healthy controls (HC). All patients underwent TMS assessment at recruitment (index test), with application of reference clinical criteria, to predict different neurodegenerative disorders. The primary outcome measures were the classification accuracy, precision, recall and F1-score of TMS in differentiating each disorder. Results: 160 participants were included, namely 64 patients diagnosed as MCI-AD, 28 as MCI-FTD, 14 as MCI-DLB, and 47 as healthy controls (HC). A series of 3 binary classifiers was employed, and the prediction model exhibited high classification accuracy (ranging from 0.72 to 0.86), high precision (0.72–0.90), high recall (0.75–0.98), and high F1-scores (0.78–0.92), in differentiating each neurodegenerative disorder. By computing a new classifier, trained and validated on the current cohort of MCI patients, classification indices showed even higher accuracy (ranging from 0.83 to 0.93), precision (0.87–0.89), recall (0.83–1.00), and F1-scores (0.85–0.94). Conclusions: TMS may be considered a useful additional screening tool to be used in clinical practice in the prodromal stages of neurodegenerative dementias.

Abstract

Objective: This study was undertaken to assess the impact of immunosuppressive and immunomodulatory therapies on the severity of coronavirus disease 2019 (COVID-19) in people with multiple sclerosis (PwMS). Methods: We retrospectively collected data of PwMS with suspected or confirmed COVID-19. All the patients had complete follow-up to death or recovery. Severe COVID-19 was defined by a 3-level variable: mild disease not requiring hospitalization versus pneumonia or hospitalization versus intensive care unit (ICU) admission or death. We evaluated baseline characteristics and MS therapies associated with severe COVID-19 by multivariate and propensity score (PS)-weighted ordinal logistic models. Sensitivity analyses were run to confirm the results. Results: Of 844 PwMS with suspected (n = 565) or confirmed (n = 279) COVID-19, 13 (1.54%) died; 11 of them were in a progressive MS phase, and 8 were without any therapy. Thirty-eight (4.5%) were admitted to an ICU; 99 (11.7%) had radiologically documented pneumonia; 96 (11.4%) were hospitalized. After adjusting for region, age, sex, progressive MS course, Expanded Disability Status Scale, disease duration, body mass index, comorbidities, and recent methylprednisolone use, therapy with an anti-CD20 agent (ocrelizumab or rituximab) was significantly associated (odds ratio [OR] = 2.37, 95% confidence interval [CI] = 1.18–4.74, p = 0.015) with increased risk of severe COVID-19. Recent use (<1 month) of methylprednisolone was also associated with a worse outcome (OR = 5.24, 95% CI = 2.20–12.53, p = 0.001). Results were confirmed by the PS-weighted analysis and by all the sensitivity analyses. Interpretation: This study showed an acceptable level of safety of therapies with a broad array of mechanisms of action. However, some specific elements of risk emerged. Abstract

Background Metabolic acidosis is associated with accelerated progression of chronic kidney disease (CKD). Whether treatment of metabolic acidosis with sodium bicarbonate improves kidney and patient survival in CKD is unclear. Methods We conducted a randomized (ratio 1:1). open-label, controlled trial (NCT number: NCT01640119. www.clinicaltrials.gov) to determine the effect in patients with CKD stage 3–5 of treatment of metabolic acidosis with sodium bicarbonate (SB) on creatinine doubling (primary endpoint), all-cause mortality and time to renal replacement therapy compared to standard care (SC) over 36-months. Parametric, non-parametric tests and survival analyses were used to assess the effect of SB on these outcomes. Results A total of 376 and 364 individuals with mean (SD) age 67.8 (14.9) years, creatinine clearance 30 (12) ml/min, and serum bicarbonate 21.5 (2.4) mmol/l were enrolled in SB and SC, respectively. Mean (SD) follow-up was 29.6 (9.8) vs 30.3 (10.7) months in SC and SB. respectively. The mean (SD) daily doses of SB was 1.13 (0.10). 1.12 (0.11). and 1.09 (0.12) mmol/kg*bw/day in the first, second and third year of follow-up, respectively. A total of 87 participants reached the primary endpoint [62 (17.0%) in SC vs 25 (6.6%) in SB, p Conclusion In persons with CKD 3–5 without advanced stages of chronic heart failure, treatment of metabolic acidosis with sodium bicarbonate is safe and improves kidney and patient survival.

Abstract

Technology development in the field of the Internet of Things (IoT) and more specifically in Low-Power Wide-Area Networks (LPWANs) has enabled a whole set of new applications in several fields of Intelligent Transportation Systems. Among all, smart-railways represents one of the most challenging scenarios, due to its wide geographical distribution and strict energy-awareness. This paper aims to provide an overview of the state-of-the-art in LPWAN, with a focus on intelligent transportation. This study is part of the RAILS (Roadmaps for Artificial Intelligence integration in the raiL Sector) research project, funded by the European Union under the Shift2Rail Joint Undertaking. As a first step to meet its objectives, RAILS surveys the current state of development of technology enablers for smart-railways considering possible technology transfer from other sectors. To that aim, IoT and LPWAN technologies appear as very promising for cost-effective remote surveillance, monitoring and control over large geographical areas, by collecting data for several sensing applications (e.g., predictive condition-based maintenance, security early warning and situation awareness, etc.) even in situations where power supply is limited (e.g., where solar panels are employed) or absent (e.g., installation on-board freight cars).

L’infanzia contemporanea, fin dai primi mesi di vita, vive in ambienti nei quali sono presenti e vengono utilizzati personal devices quali smartphone e tablet. L’osservazione e l’esplorazione degli oggetti tecnologici e del loro uso entra perciò nelle esperienze precoci dei bambini e delle bambine e le influenza: nuovi linguaggi, nuove competenze, nuove modalità ludiche, nuovi ruoli (attore e spettatore). L’ambiente digitale diventa così parte della quotidianità dei più piccoli che, anche da soli, giocano con le app, guardano video, scattano foto, ascoltano musica, in differenti contesti. La pedagogia (e non solo) si interroga sulle opportunità e i rischi di una (sovra)esposizione agli strumenti tecnologici e all’uso di giochi digitali nei piccolissimi, evidenziando sfide, contraddizioni, timori, entusiasmi. Quale dunque il ruolo dei servizi educativi 0-6? E quello delle famiglie?
