Abstract

To assess the image quality of multiple threshold display (MTD) as a new technique for generating three-dimensional (3D) pulmonary computed tomographic (CT) angiographic images.We used MTD, a type of shaded surface display (SSD) offering the selection of multiple thresholds and transparencies, to reconstruct 3D-CT angiograms from enhanced helical CT data sets in 33 patients with lung disease. In MTD, eight thresholds of CT values are selected, and transparency is assigned to each. The selected voxels, ranging from -600 to 1,000 Hounsfield Units, were divided into eight classes, and transparency ranging from 0 to 100% was assigned to each. The CT scanner employed was a Toshiba Xvigor. MTD and SSD images were generated by using an Xtension with a Sun SPARC station 20, and they were compared by two radiologists.The image quality of MTD images was superior to that of SSD images (p0.01), because the MTD images demonstrated clearly both the major and small vessels.MTD is a useful technique for 3D pulmonary CT angiography.

Abstract

To evaluate the usefulness of three-dimensional (3D) CT angiography of the portal vein obtained by using a multiple threshold display, 3D reconstructions were performed in 15 patients. The CT scanner employed was the Toshiba Xvigor. A volume of 150 ml of lopamidol 300 mg1/ml was administered at 3.3 ml/sec intravenously. Portal venous phase helical scanning was initiated 80 seconds after the start of the injection. Helical CT data were acquired using up to 25 continuous 1.0-sec rotations during a single breath-hold with an X-ray beam width of 7 mm and a couchtop movement speed of 7 mm/sec. Axial images were reconstructed at a section interval of 3 mm. Both the shaded surface display (SSD) and multiple threshold display (MTD) were generated by using Xtension with Sparc20. MTD was performed as follows. First, voxels, with higher CT values than that of liver parenchyma, were selected. Then, selected voxels were divided into eight parts, which were each assigned gradations of white to grey. The highest part of selected voxels, with higher CT values than that of the second branch of the portal vein, were set to white and a transparency of 0%. The other seven parts were each assigned transparencies of more than 0%. MTD images were compared with SSD in 15 cases by two radiologists. MTD images were superior to SSD images in quality, because MTD diminished surrounding artifacts due to liver parenchyma and enabled small vessels to be depicted clearly. Based on the above results, it was considered that MTD was a useful method for 3D CT angiography using enhanced helical CT.

Abstract

To evaluate the usefulness of three-dimensional (3D) imaging of pulmonary nodules from helical scan CT images, 3D reconstructions were performed in 23 patients, using a CEMAX VIPstation. These cases included 15 lung cancers, six metastatic lung cancers, an aspergilloma and a tuberculoma. The equipment used was a Toshiba CT system, the X force. Helical scan CT data were acquired using up to 20 continuous 1.5 sec rotations with an X-ray beam width of 5 mm and a couchtop movement speed of 5 mm/1.5 sec, and during a single breath-hold. Axial images were reconstructed at a section interval of 2 mm. Helical scan CT permits axial images to be reconstructed at any desired position within the scanned area, and provides images without interslice gaps caused by respiratory movement. Therefore, high-quality 3D images can be obtained from these data. Concerning the optimum threshold range of CT number of 3D reconstruction, we clarified the lower and upper limits (lower/upper), as follows: 1) Solid pulmonary nodule: (-700-(-)400/-100 HU) 2) Tumor invaded to pleura or chest wall: (-700-(-)400/-200 HU) 3) Pulmonary nodule with cavity: (-700-(-)400/50 HU) 4) Small pulmonary nodule (10 mm): (-750-(-)600/-100 HU) In all cases, it was possible not only to demonstrate abnormal findings three-dimensionally, but also to grasp anatomical relationships among the pulmonary nodule, bronchi, vessels and chest wall. In conclusion, it was considered that 3D CT imaging provided additional anatomical information and was very useful.

In this study we investigated whether Tc-99m hexakis 2-methoxy isobutyl isonitnile (Tc-99m MIBI) single-photon emission computed tomography (SPECT) has a correlation with the multidrug resistance (MDRI) and multidrug resistance-associated protein (MRP1) gene expression levels in non-small cell lung cancer (NSCLC). Fifteen patients with NSCLC were studied. SPECT images were obtained 15 (early) and 120 (delayed) min after injection of Tc-99m MIBI. We chose only one transverse section and set the region of interest over the tumor and out of the body. The mean counts in the tumor on early and delayed images were corrected by using those in the background and represented as Te and Td, respectively. Resected tumor specimens were frozen with liquid nitrogen and each positive control cell line was cultured. After the total ribonucleic acid (RNA) was extracted from specimens and cell lines, the complimentary deoxyribonucleic acid (cDNA) was amplified by the reverse transcription-polymerase chain reaction (RT-PCR) method. Each product was electrophoresed and fluorointensity was measured. The gene expression level was represented as the ratio of that of the positive control cell line. Te and Td indicated a significant correlation with the MDR1 gene expression level (p = 0.015 and p = 0.022), but not the gene of MRP1 (p = 0.100 and p = 0.145). Abstract

Purpose: We performed technetium-99m hexakis-2-methoxyisobutylisonitrile (Tc-99m MIBI) single photon emission computed tomography (SPECT) in 23 patients with primary lung cancer between July 1993 and March 1996. We evaluated the relationships among the uptake ratio, retention index and the prognosis after radiation therapy and/or chemotherapy.Materials and Methods: Tc-99m MIBI SPECT was performed at 30 minutes and at 3 hours after intravenous injection of 600 MBq of Tc-99m MIBI with three gamma camera detectors (GCA-9300A/HG) on transverse SPECT images. Regions of interest were set in the area of abnormal uptake of Tc-99m MIBI and in the contralateral normal lung. The ratio of uptake in the lesion to that in the contralateral normal lung was obtained on early images (early ratio; ER) as well as delayed images (delayed ratio: DR). The retention index (RI) was calculated as follows: RI = (DR - ER)/ER x 100. The ratio was compared with survival time and prognostic factors.Results: There was no correlation between ER and DR. The patients with high RI survived longer than those with low RI (median survival, 19.4 months vs. 9.4 months; p = 0.0104 by the Mantel-Cox test).Conclusion: These results suggest that RI is the most useful among Tc-99m MIBI indices of primary lung cancer in predicting prognosis. Abstract

Serial changes in N-isopropyl-p[125I]-iodoamphetamine (125I-IMP) in mouse lungs were observed with a confocal laser scanning microscope. Male mice were intravenously injected with 125I-IMP and subjected to autoradiographic procedures 20 minutes, and 3 and 24 hours after injection. Differential interference contrast (DIC) images and confocal images were obtained with a confocal laser scanning microscope, and superimposed images were evaluated. Large numbers of silver grains were observed in the interstitium, bronchioles, and alveolar sacs 20 minutes after the injection, and lamellar distribution of the grains was observed on the ciliary surface. The numbers of silver grains in the interstitium and bronchioles had decreased 3 hours after the injection of 125I-IMP, but the numbers of silver grains in the alveolar spaces had not. Although small numbers of silver grains remained in both the bronchioles and alveolar sacs 24 hours after the injection, most of them had washed out. Confocal laser scanning microscopy is considered to be a useful procedure for studying the distribution of radioisotopes by microautoradiography, because it allows clear autoradiographs to be obtained in which tissues and silver grains are perfectly matched and all silver grains are in focus. 40. Intraarterial infusion chemotherapy with lipiodol-CDDP suspension for hepatocellular carcinoma.

Abstract

Purpose: To quantitatively evaluate the usefulness of lipiodol-CDDP suspension (LCS) chemotherapy in hepatocellular carcinoma (HCC).Methods: CDDP (cis-diamminedichloroplatinum) powder was prepared by removing the water and NaCl from aqueous CDDP. Two quantities of prepared CDDP powder, 10 mg and 20 mg, were mixed with 1 ml each of iopamidol 300 mgI/ml (IP300) and lipiodol (LPD) using a high pressure pumping method, thus producing LCS. Thirty-two patients with HCC, who had good renal function [creatinine clearance (Ccr) 50 ml/min or more], received additional intraarterial infusion chemotherapy with LCS or LCS alone.Results: The most frequently observed CDDP powder sizes were 5.95-10.90 microm (average: 11.59 microm). The LCS obtained demonstrated a suspension of 2-12 microm (average 3.69 microm) immediately after mixing, and no significant changes were observed in LCS particle sizes 3 hr after mixing. Moreover, the sustained release with LCS was observed for up to 3 hr. Meanwhile, the peripheral free platinum concentration between intraarterial infusion chemotherapy with LCS and intraarterial infusion with the aqueous solution of CDDP, with respect to variance residence time (VRT), showed a significant difference, with a p value of 0.0382. The survival rate was 89.84% at 1 year, 73.78% at 2 years, and 68.51% at 3 years. Furthermore, the platinum concentration in the tumor was 25-95 times the concentration in the surrounding liver parenchyma.Conclusion: Good clinical results can be expected by applying LCS to HCC. 43. Iodine-123 metaiodobenzylguanidine myocardial scintigraphy for prediction of response to @b-blocker therapy in patients with dilated cardiomyopathy

Abstract

This study was performed to evaluate whether iodine-123 metaiodobenzylguanidine (MIBG) myocardial scintigraphy could predict the response to @b-blocker therapy in patients with nonischemic dilated cardiomyopathy (DCM). @b-Blocker therapy is effective in some patients with DCM. MIBG myocardial scintigraphy has also been suggested to be useful in evaluating the severity of myocardial damage in DCM. However, no data exist on whether MIBG imaging can be used to predict which patients with DCM will respond to @b-blocker therapy. We prospectively evaluated whether MIBG myocardial imaging was useful in predicting responses to @b-blocker therapy in patients with DCM. MIBG imaging was performed in 45 patients with DCM (35 men, 10 women, aged 13 to 68 years) before the start of bisoprolol. The heart to mediastinum (H/M) MIBG uptake ratio was evaluated on initial and delayed images, and the percent washout rate of myocardial MIBG was also obtained from these data. Of the 45 patients, 30 (67%) responded to @b-blocker therapy, whereas 2 were resistant and 13 showed progression of heart failure or died of heart failure. By logistic regression analysis, the H/M uptake ratio on delayed images was seen to be a good predictor of the response to @b-blocker therapy with a threshold of 1.7 (sensitivity = 91%, specificity = 92%, accuracy = 91%, positive and negative predictive value = 97% and 80%, respectively). These results indicate that an H/M ratio >1.7 on the delayed MIBG myocardial scintigraphic images provides a useful indication of whether patients with DCM will respond to @b-blocker therapy. 45. Do repolarization abnormalities in hypertrophic cardiomyopathy represent impaired fatty acid utilization?

Abstract

To identify the clinical significance of the isointegral body surface map of the QRST interval (QRST map) and the occurrence of repolarization abnormalities in patients with hypertrophic cardiomyopathy (HCM), the QRST map and signal-averaged electrocardiogram were evaluated in 50 patients with HCM, in 33 of whom the results were compared with nuclear images both for radioiodine-labeled fatty acid metabolism and for radiothallium perfusion. The QRST departure map was used to determine two parameters of difference between patient and control recordings: the subnormal area (the number of lead points at which the departure index values were negative and lay more than 2 SDs from the mean of the normal control group) and the subnormal minimum (the absolute value of the minimum in the departure map). Late potentials were detected in 6 (12%) of the 50 patients; they were observed in 3 of the 5 patients with dilated-phase HCM but in only 3 (7%) of the other 45 patients. The subnormal area and minimum values were lower in nonobstructive HCM than in dilated-phase HCM. Of the 33 patients examined by myocardial imaging, 28 (33%) had a filling defect or decreased uptake, as shown on fatty acid metabolic images, and 10 of the 28 also showed abnormal myocardial perfusion images, while the 18 others showed normal perfusion images. These 28 patients showed significantly larger values of the subnormal area and minimum than patients with normal results in both image tests, regardless of whether or not myocardial perfusion imaging abnormalities were present. The localization of filling defects or of decreased uptake presented in fatty acid metabolic images corresponded to the position of the minimum on the QRST departure map. These results suggest that the QRST map is useful for detection of repolarization abnormalities in HCM and that these abnormalities are highly related to impaired fatty acid utilization of the myocardium.

Abstract

To evaluate the usefulness of three-dimensional (3D) helical CT for the detection of abnormalities of the auditory ossicles, 3D helical CT of the middle ear was performed in seven patients with hearing disorder. It revealed that 4 patients had congenital deficiency of the auditory ossicles, 2 patients with chronic otitis media had shortening of the incus and one patient with head injury had doubtful fracture of the incus. This study indicated that 3D helical CT of the middle ear can represent the auditory ossicles objectively and can offer detailed diagnosis.

Abstract

The purpose of this study was to evaluate the clinical usefulness of three-dimensional (3D) images of the bronchi obtained using helical CT. Thirteen patients with lung cancer, one with tracheal diverticulum, and one with bronchial amyloidosis were examined. The CT scanner employed was the Toshiba Xforce. The helical CT scan cycle consisted of 20 continuous rotations, each requiring 1.5 sec, for a total scanning time of 30 sec. Scans were obtained using a 5-mm X-ray beam width, a 5-mm/1.5 sec couchtop sliding speed, and a 2-mm reconstruction interval. 3D images were reconstructed using a CEMAX VIPstation. The optimal lower and upper threshold CT values for 3D images of the bronchi were -650 and -100 HU, respectively, and 3D images clearly depicted endobronchial lesions. Cartilage crescents were also demonstrated, but longitudinal and circular mucosal folds could not be visualized. In conclusion, 3D images of the bronchi acquired using helical CT were useful in evaluating endobronchial lesions.

Abstract

To evaluate the usefulness of helical scanning CT of the upper abdomen, 30 patients with hepatic, adrenal, pancreatic and renal disease were examined using a Toshiba CT system, the Xforce. Helical scanning CT data were acquired using up to 20 continuous 1.5-second rotations, with a 1.5-3 ml bolus injection of contrast medium, and during a single breath-hold. Helical scanning provided both better data continuity and resolution than conventional scanning. The axial images with a slice thickness of 5 mm were not inferior in quality to equivalent images acquired by conventional CT, and multiplanar reconstruction images were superior. Targeted structures could be easily obtained because helical scanning CT permits image reconstruction in any direction. Using a CEMAX-VIPstation, we generated three-dimensional display images of high-density structures from the postcontrast data acquired with helical scanning. On these images, it was easy to observe the high-density tumors three-dimensionally and to determine the position of the tumors relative to the high-density surrounding organs. In conclusion, helical scanning CT was considered to be useful in clinical diagnosis involving the upper abdomen.

To evaluate the clinical usefulness of three-dimensional (3D) images of pulmonary nodules acquired using helical CT, 3D reconstructions were performed in 32 patients using a CEMAX VIPstation. The CT scanner employed was the Toshiba Xforce. Helical CT data were acquired using up to 20 continuous 1.5-sec rotations with an X-ray beam width of 5 mm and a couchtop movement speed of 5 mm/1.5 sec. Axial images were reconstructed at a section interval of 2 mm. The optimal lower and upper threshold CT values for 3D images were as follows: 1) solid pulmonary nodule (-700--400/-100 HU), 2) tumor invading pleura or chest wall (-700--400/-200 HU), 3) pulmonary nodule with cavity (-700--400/50 HU), 4) small pulmonary nodule (10 mm) (-750--650/-100 HU), and 5) arteriovenous malformation (180/500 HU). In all cases, it was possible not only to demonstrate abnormal findings three-dimensionally but also to grasp anatomical relationships among the pulmonary nodule, bronchi, vessels, and chest wall.
