Abstract

Trading Places is about urban land markets in African cities. It explores how local practice, land governance and markets interact to shape the ways that people at society's margins access land to build their livelihoods. The authors argue that the problem is not with markets per se, but in the unequal ways in which market access is structured. They make the case for more equal access to urban land markets, not only for ethical reasons, but because it makes economic sense for growing cities and towns. If we are to have any chance of understanding and intervening in predominantly poor and very unequal African cities, we need to see land and markets differently. New migrants to the city and communities living in slums are as much a part of the real estate market as anyone else; they're just not registered or officially recognised. This book highlights the land practices of those living on the city's margins, and explores the nature and character of their participation in the urban land market. It details how the urban poor access, hold and trade land in the city, and how local practices shape the city, and reconfigures how we understand land markets in rapidly urbanising contexts. Rather than developing new policies which aim to supply land and housing formally but with little effect on the scale of the need, it advocates an alternative approach which recognises the local practices that already exist in land access and management. In this way, the agency of the poor is strengthened, and households and communities are better able to integrate into urban economies.

20. Core housing, enablement and urban poverty : the consolidation paths of households living in two South African settlements

Abstract

The objective of this thesis was to enhance the understanding of both the formation of core housing settlements in South Africa and the consolidation processes which take place after residents have occupied the housing. Whilst the usually unassisted, consolidation efforts of residents of informal settlements and mass built government housing have been well studied, there were relatively few studies which gave a comprehensive understanding of longer term physical consolidation in settlements which had been designed and built with a view to extension. The central research question was whether core housing should be accepted as a sufficiently supportive institutional and physical framework for allowing or enabling households to counter situations of urban poverty, achieve adequate housing, and integrate themselves into the city. Two case studies were chosen which represented the delivery of core housing at scale, one being Inanda Newtown in Durban (about 4000 houses occupied from 1981), and the other being Khayelitsha in Cape Town (about 5000 houses occupied in 1985). Some 444 respondent households were interviewed in 1996 using a mainly quantitative method, as were stakeholders involved in the initial production and ongoing support of the areas. The study was retrospective and comparative of the two settlements. Inanda Newtown represented the delivery of larger, shell houses and lower levels of service, where an NGO had supported the process for a protracted period and established a local authority also supportive of consolidation. Khayelitsha represented the delivery of smaller core houses with higher levels of service and an unsupportive institutional context. A critical realist framework was adopted to enhance the understanding of the power relations between the various urban actors involved in the production, support and consolidation of core housing, and thus the causal mechanisms which shaped the personal projects of households as they attempted to achieve their own housing consolidation projects. In the conclusion the relevance of the findings to the current South African policy context and to global development thinking was discussed.

Importance The patterns of disease recurrence after resection of pancreatic ductal adenocarcinoma with adjuvant chemotherapy remain unclear. Objective To define patterns of recurrence after adjuvant chemotherapy and the association with survival. Design, Setting, and Participants Prospectively collected data from the phase 3 European Study Group for Pancreatic Cancer 4 adjuvant clinical trial, an international multicenter study. The study included 730 patients who had resection and adjuvant chemotherapy for pancreatic cancer. Data were analyzed between July 2017 and May 2019. Interventions Randomization to adjuvant gemcitabine or gemcitabine plus capecitabine. Main Outcomes and Measures Overall survival, recurrence, and sites of recurrence. Results Of the 730 patients, median age was 65 years (range 37-81 years), 414 were men (57%), and 316 were women (43%). The median follow-up time from randomization was 43.2 months (95% CI, 39.7-45.5 months), with overall survival from time of surgery of 27.9 months (95% CI, 24.8-29.9 months) with gemcitabine and 30.2 months (95% CI, 25.8-33.5 months) with the combination (HR, 0.81; 95% CI, 0.68-0.98;P = .03). The 5-year survival estimates were 17.1% (95% CI, 11.6%-23.5%) and 28.0% (22.0%-34.3%), respectively. Recurrence occurred in 479 patients (65.6%); another 78 patients (10.7%) died without recurrence. Local recurrence occurred at a median of 11.63 months (95% CI, 10.05-12.19 months), significantly different from those with distant recurrence with a median of 9.49 months (95% CI, 8.44-10.71 months) (HR, 1.21; 95% CI, 1.01-1.45;P = .04). Following recurrence, the median survival was 9.36 months (95% CI, 8.08-10.48 months) for local recurrence and 8.94 months (95% CI, 7.82-11.17 months) with distant recurrence (HR, 0.89; 95% CI, 0.73-1.09;P = .27). The median overall survival of patients with distant-only recurrence (23.03 months; 95% CI, 19.55-25.85 months) or local with distant recurrence (23.82 months; 95% CI, 17.48-28.32 months) was not significantly different from those with only local recurrence (24.83 months; 95% CI, 22.96-27.63 months) (P = .85 andP = .35, respectively). Gemcitabine plus capecitabine had a 21% reduction of death following recurrence compared with monotherapy (HR, 0.79; 95% CI, 0.64-0.98;P = .03). Conclusions and Relevance There were no significant differences between the time to recurrence and subsequent and overall survival between local and distant recurrence. Pancreatic cancer behaves as a systemic disease requiring effective systemic therapy after resection. Abstract

Under apartheid, black African households could not own land or homes in most major urban centres in South Africa. This limited residential mobility and locked many households into state rental accommodation in townships. Homeownership for all South Africans was restored in the mid-1980 s and the Group Areas Act was repealed in 1991. Democracy opened up economic opportunities previously unavailable to black people. This paper investigates the effect on black middle-class South African households’ residential mobility and housing careers. A retrospective cross-sectional survey of 244 such homeowners in the Mangaung Metropolitan Municipality determined their last five housing states. Changes in housing state indicated a steady improvement in housing quality, but tenure changes were not necessarily unidirectional—some had reverted to rental. More than 85% of the study participants had used mortgages to finance their housing career. Very few had financed their housing using own savings, an inheritance, or sale of a previous house, and not many had used the government subsidy. Abstract

In line with international trends, South Africa's post‐apartheid government has embarked on a housing policy that emphasises asset building. Homeownership has become a dominant policy for assisting the historically disadvantaged Black population. This paper looks at two property markets in Mangaung, a metropolitan municipality that includes Bloemfontein. We analysed property activity on 2,545 residential stands in former Black and former White areas in Mangaung since 1990. Our findings show two distinct markets. The formal property market in former Black areas shows a low number of transactions, low value and slow turnover. In the former White areas, a housing investment made between 1990 and 2016 would have yielded an annual return of 14 per cent, but in the former Black areas only 7 per cent.
