Reduced risk of recurrent myocardial infarction in homozygous carriers of the chromosome 9p21 rs1333049 C risk allele in the contemporary percutaneous coronary intervention era: a prospective observational study. BMJ Open 2014;4:e005438. doi: 10.1136/bmjopen-2014-005438, Objectives: Chromosome 9p21 single nucleotide polymorphism (SNP) is a susceptibility variant for acute myocardial infarction (AMI) in the primary prevention setting. However, it is controversial whether this SNP is also associated with recurrent myocardial infarction (ReMI) in the secondary prevention setting. The purpose of this study is to evaluate the impact of chromosome 9p21 SNP on ReMI in patients receiving secondary prevention programmes after AMI. Design: A prospective observational study. Setting: Osaka Acute Coronary Insufficiency Study (OACIS) in Japan. Participants: 2022 patients from the OACIS database. Interventions: Genotyping of the 9p21 rs1333049 variant. Primary outcome measures: ReMI event after survival discharge for 1 year. Results: A total of 43 ReMI occurred during the 1 year follow-up period. Although the rs1333049 C allele had an increased susceptibility to their first AMI in an additive model when compared with 1373 healthy controls (OR 1.20, 95% CI 1.09 to 1.33, p=2.3*10-4), patients with the CC genotype had a lower incidence of ReMI at 1 year after discharge of AMI (log-rank p=0.005). The adjusted HR of the CC genotype as compared with the CG/GG genotypes was 0.20 (0.06 to 0.65, p=0.007). Subgroup analysis demonstrated that the association between the rs1333049 CC genotype and a lower incidence of 1 year ReMI was common to all subgroups. Conclusions: Homozygous carriers of the rs1333049 C allele on chromosome 9p21 showed a reduced risk of 1 year ReMI in the contemporary percutaneous coronary intervention era, although the C allele had conferred susceptibility to their first AMI.

29. Role of atrial contraction and synchrony of ventricular contraction in the optimisation of ventriculoarterial coupling in humans.

Abstract

OBJECTIVE--To examine the effects of pacing modes on the interaction between the left ventricle and arterial system in humans. DESIGN--The slope of the end systolic pressure-volume relation (end systolic elastance), effective arterial elastance, the ratio of effective arterial elastance to end systolic elastance, and mechanical energy efficiency were compared under different pacing modes (atrial, atrioventricular, and ventricular). PATIENTS--Nine male patients with sick sinus syndrome who had cardiac catheterisation for diagnosis and to see whether they needed a pacemaker. INTERVENTIONS--A conductance catheter with tip-manometer was inserted into the left ventricle to obtain pressure-volume loops, and two pacing catheters were inserted into the right atrium and into the right ventricle respectively. RESULTS--End systolic elastance was lower in atrioventricular pacing than in atrial pacing, but effective arterial elastance was not significantly different. End systolic elastance was lower in ventricular pacing than in atrioventricular pacing, and effective arterial elastance was higher in ventricular pacing than in atrioventricular pacing. Consequently the ratio of effective arterial elastance to end systolic elastance was lowest in atrial pacing and highest in ventricular pacing, and mechanical energy efficiency was highest in atrial pacing and lowest in ventricular pacing. CONCLUSIONS--Atrial contraction and synchronous ventricular contraction independently optimise ventriculoarterial coupling in terms of a transfer of energy. 30. Positive Isovolumic Relaxation Velocity Detected by a Spectral Tissue Doppler Mapping Technique as an Indicator of Coronary Artery Disease: A Prospective Study

Abstract

Background: A positive myocardial velocity during isovolumic relaxation phase (V"I"R) detected by spectral tissue Doppler imaging has been shown to indicate ischemic myocardium. We sought to examine the diagnostic value of positive V"I"R for detecting coronary artery disease (CAD) in patients with chest pain and apparently normal left ventricular contraction. Methods: A total of 138 patients (74 men, age 69 +/- 8 years) underwent spectral Doppler tissue imaging measurements at the annular and mid left ventricular levels in the apical 4- and 2-chamber views (8 points/patient) at rest in addition to standard echocardiography. Subsequently, patients underwent thallium-201 myocardial perfusion single photon emission computed tomography (n = 98) and/or coronary angiography (n = 60). The duration of positive V"I"R was also assessed. CAD was diagnosed by having more than 75% diameter stenosis in coronary angiography. Results: CAD was present in 41 patients (30%). The duration of positive V"I"R was distributed from 0 to 280 milliseconds with the median value of 100 milliseconds. Presence of positive V"I"R predicted CAD with sensitivity of 80% and specificity of 61%. Conclusions: Positive V"I"R detected by spectral tissue Doppler imaging is a useful indicator of CAD in patients with apparently normal left ventricular contraction and chest pain.

Abstract

Background: Progress in the technique of TAVR requires good knowledge of the aortic root. With this aim new specialized software appears, with the ability of automated quantitative modeling of the AV and root from 3D TEE.The purpose of this study was to validate this model with the measurements made manually. Methods: Eight patients undergoing TAVR in our center where included. The diameters of the aortic annulus, sinotubular union (STU) and sinus of valsalva (SV) were measured by 2D TEE; diameters and areas of aortic annulus, STU and SV as well as anatomic aortic valve area were measured by 3D TEE. Afterwards, the images were analyzed using the new software (Figure 1). Results. We showed good correlation with aortic annulus diameter measured by 2D TEE (r:,832 p:,01) and excellent correlation with one of the aortic annulus diameter measured by 3D TEE (r:,941 p:,00). The same happened with the area (r:,720 p:,04). Regarding the measurements at SV level, the correlations between the diameters by 2D TEE and 3D TEE with the measurements obtained with the new model were the following (r:,771;p:,025) and (r:,797;p:,018). The correlation of the area was also good (r:,812 p:,014).An excellent correlation was found between the measurements at UST level. UST diameter by 2D TEE (r:,818;P:,013), by ETE3D (r:,800;p:,017) and area (r:,844;p:,008).Finally, the anatomic aortic valve area measured by the new model showed significant correlation with the 3D TTE (r:,830 p:,011). Conclusions. There is a proper correlation between manual and automated measurements analyzed by the new model. The feasibility of determine the TAVR results with geometric models based on image, prior to procedure, is one of the possibilities of this new software. Abstract

Although the C-reactive protein (CRP) concentration measured shortly after acute myocardial infarction (AMI) is associated with infarct size, its prognostic value is controversial. The reduction of CRP is accelerated by reperfusion. Therefore, the CRP concentration, measured during the stable phase of AMI in patients treated predominantly with reperfusion therapies, may be independent of infarct size and may predict long-term mortality. We studied 1,309 patients with AMI enrolled in the Osaka Acute Coronary Insufficiency Study between April 1999 and June 2001. CRP was measured during the stable phase (mean 25 days after AMI onset). The patients were followed for an average of 522 days. Reperfusion therapies were performed in 90% of the patients. Patients in the highest quartile of CRP values (> or =0.38 mg/dl) were older, had higher prevalences of diabetes mellitus, and had higher Killip classes than patients in the lower 3 quartiles (<0.38 mg/dl). Multivariate logistic regression analysis revealed that CRP was independently associated with age and the absence of revascularization therapies. Patients in the highest quartile had a higher long-term mortality rate than patients in the lower 3 quartiles (8.9% vs 2.0%; p <0.001). Multivariate Cox regression analysis revealed that the highest quartile of CRP values was an independent predictor of long-term mortality (hazard ratio 4.94, 95% confidence interval 1.13 to 21.6). We conclude that CRP measured during the stable phase of AMI is not associated with infarct size in the reperfusion era but is significantly associated with long-term mortality of AMI. Abstract

To elucidate the changes in oxidative metabolism in hibernating myocardium after coronary revascularization, we performed myocardial single-photon emission computed tomography with a free fatty acid analog, I-123 beta-methyliodophenylpentadecanoic acid (BMIPP), and thallium-201 before and 1 month after percutaneous transluminal coronary angioplasty (PTCA) in 11 patients with angina pectoris caused by single artery stenosis. All patients had improvement in wall motion after PTCA at the region with coronary stenosis; the wall motion abnormality score evaluated by left ventriculography decreased from 5.5+/-0.8 (mean +/- SE) to 2.1+/-0.9, p <0.01) after PTCA. The defect score of I-123 BMIPP images was significantly larger than that of thallium-201 images either before (14+/-1.3 vs 8.9+/-1.1, p <0.01) or 1 month after (7.4+/-1.5 vs 3.7+/-0.8, p <0.01) PTCA. The decrease in the defect score of both images was significant (p <0.01). Changes in the wall motion abnormality score showed a significant correlation with both the change in the defect score of thallium-201 images (r = 0.58, p < 0.01) and that of I-123 BMIPP images (r = 0.75, p <0.01). These results indicate that the metabolism of free fatty acid is impaired in hibernating myocardium, and that improvement in left ventricular function after successful PTCA is strongly associated with the recovery of oxidative metabolism. Abstract

Background: The relationship between baseline yellow plaque (YP) and vascular response after stent implantation has not been fully investigated., Methods: This was a sub-analysis of the Collaboration-1 study (multicenter, retrospective, observational study). A total of 88 lesions from 80 patients with chronic coronary syndrome who underwent percutaneous coronary intervention were analyzed. Optical coherence tomography (OCT) and coronary angioscopy (CAS) were serially performed immediately and 11 months after stent implantation. YP was defined as the stented segment with yellow or intensive yellow color assessed by CAS. Neoatherosclerosis was defined as a lipid or calcified neointima assessed by OCT. OCT and CAS findings at 11 months were compared between lesions with baseline YP (YP group) and lesions without baseline YP (Non-YP group)., Results: Baseline YP was detected in 37 lesions (42 %). OCT findings at 11 months showed that the incidence of neoatherosclerosis was significantly higher in the YP group (11 % versus 0 %, p = 0.028) and mean neointimal thickness tended to be lower (104 ± 43 μm versus 120 ± 48 μm, p = 0.098).
