15. Trend of accesses to the ED of Teaching Hospital of Tuscany for bronchiolitis 2018-2023: new emergency

Abstract

Introduction: Acute bronchiolitis is the leading cause of lower respiratory tract infection and hospitalisation in children less than one year old worldwide. The aim of our study is to analyse paediatric accesses of children with bronchiolitis to the Emergency Department (ED) of Teaching Hospital (AOUS), Santa Maria alle Scotte of Siena, Tuscany Methods: A retrospective observational study was conducted on the accesses performed at the ED of the AOUS of Siena by children under 18 years of age suffering from bronchiolitis from September 2018 to April 2023. Results: There were 36031 patients between 0 and 18 years old in the Emergency Department, 383 of which presented bronchiolitis (age 4.8 months C.I.:3.5-6 months.; 54% male). Those who accessed the ED with a higher priority code were more likely to be subsequently admitted (O.R.:2.6; C.I.:1.3-5.1; p<0.01). Those who accessed the ED with symptoms of bronchiolitis during the weekend were less likely to have been sent from community medicine services or professionals (O.R:0.1; C.I:0.0-0.5; p<0.001). Children below 1 year old were more likely to access the ED with respiratory distress symptoms (O.R.:2.6; C.I.:1.5-4.3; p<0.001). Finally, those who accessed the ED with bronchiolitis were more likely to be admitted than those who accessed for other conditions (O.R:24.5; C.I.:19.4-31; p<0.001). Conclusions: It is necessary to invest protocols integrating hospital services and community medicine in order to achieve a timely diagnosis and to reduce the accesses to the ED of children presenting mild, non-severe form of bronchiolitis  in order to avoid the overload of hospital services

22. The Angelina Jolie Effect -- Impact on Breast and Ovarian Cancer Prevention: A Systematic Review of Effects after the Public Announcement in May 2013

Abstract

Background: On 14 May 2013, Angelina Jolie (AJ), revealed herself to be the carrier of a BReast CAncer 1 (BRCA1) gene mutation and announced her decision to undergo a prophylactic mastectomy, followed by a laparoscopic bilateral salpingo-oophorectomy. This review explores the impact of the "Angelina Jolie Effect" in order to reveal whether her announcement led to a change in the attitude of patients, and in decisions regarding healthcare options focusing especially on referrals, on genetic tests and on prophylactic mastectomies. Methods: Between January-February 2017, we performed a systematic search in PubMed using the key search term "Angelina Jolie". We searched for studies published between 2013 and 2017, reporting data on number of BRCA1/2 tests, number of referrals for breast or ovarian cancer and number of performed mastectomies, before and after AJ's disclosure. We considered eligible for inclusion all cross-sectional, retrospective, prospective studies written in English. Results: The literature search yielded 27 publications. After the analysis of title, abstracts and full text, we identified eight manuscripts for inclusion in the review. The studies were conducted from 2011 to 2015 in the USA, Austria, Australia, Canada and the UK. The announcement generated an increase of referrals for breast/ ovarian cancer with peaks of +285%, an increase of BRCA tests with a peak of +80%, but did not lead to a significant increase of prophylactic mastectomies. Patients with a lower level of education asked for information about cancer, and most patients became more aware of breast reconstruction post-mastectomy. Conclusion: Celebrity disclosures, such as AJ's revelation of her BRCA status and her decision for a prophylactic mastectomy, can influence patients' behaviour leading to important effects on attitudes towards screening. Organisations should assist patients appropriately, but at the same time should consider the available resources, and should interface with journalists and the social media in order to guarantee the validity of the information.

Abstract

BACKGROUND: In 2022, a new outbreak of the Mpox virus occurred outside of Africa, its usual endemic area. The virus was detected in European, American, Asian, and Oceanian countries where Mpox is uncommon or had not been reported previously and where the spread was rapid. The study aims to compare the case definition and the indications for contact tracing in case of Mpox infection among the World Health Organization (WHO), the European Centre for Disease Prevention and Control (ECDC), and four European Countries. METHODS: From August 2022 to November 2022, we conducted research, first on the WHO and ECDC official websites and then on the official websites of the Ministry of Health or National Health Agencies of four European Countries (Italy, France, Spain, and Portugal). All reports found were compared to enlighten the differences in the definition of the case and indications for contact tracing. RESULTS: The WHO divides the case definition into four categories: suspected, probable, confirmed, and discarded, while the ECDC divides cases into confirmed and probable. The ECDC defines contact as close and others, while the WHO divides it into high, medium, and minimal risk. The four countries analyzed show heterogeneity in both the case definitions and the indications for contact tracing. CONCLUSIONS: Our analysis revealed heterogeneity in the case definition between the WHO and ECDC. Different countries followed different indications or have given their indications for both the case definition and contact tracing indications. Harmonization strengthens public health preparedness and response and creates unified communication.
