3. The Clinical Effects of Acupuncture in Patients with Parkinson's Disease: A Randomized Controlled Trial

Abstract

【目的】パーキンソン病に対する鍼治療の臨床効果と,鍼治療が脊髄前角細胞や大脳に及ぼす影響を検討した.【対象】パーキンソン病患者26例を無作為に,標準鍼治療群13例(平均68.0歳)と低頻度鍼治療群13例(平均72.8歳)に分け,治療効果を比較した.【方法】治療効果の評価は,各鍼治療期間の前後に,1)パーキンソン症状はUPDRS,2)運動症状はTUG(歩行バランス能力),FRT(姿勢保持能力),大腿四頭筋筋力,3)うつ症状はGDS,4)QOLはPDQ-39を測定した.鍼治療の治効機序の検討のため,5)筋電図で,脊髄前角細胞の興奮性をF波,大脳を経由しパーキンソン症状と関連する長潜時反射,6)血液検査で神経栄養因子(IGF-1,IGFBP-3),炎症関連物質(Hs-CRP,TNF-α,IL-6)を測定した.【結果】両治療群で治療効果に差はなかったが,各治療群で治療期間の前後において,UPDRSとTUGの改善が認められた.標準鍼治療群で,FRTの改善,F波出現率の低下が認められた.また,標準鍼治療群で,LLRの振幅が低下した症例では,パーキンソン症状や運動症状の改善する傾向が示された.【考察】両治療群でパーキンソン症状や運動症状の改善が認められた.鍼治療は,脊髄前角細胞の興奮性亢進を低下させ,大脳基底核から大脳皮質や脳幹への過剰出力に対する抑制効果を有する可能性が考えられた.(著者抄録)

Abstract

2017年度, 【目的】軽度認知障害を有する高齢者の認知機能低下に対する鍼治療の効果を，Aβシークエスタータンパク質を指標に比較試験の下で，その有効性を検討した． 【対象】軽度認知障害を有する高齢者 17 名とした．脳トレーニングに加えて鍼治療を行う群 9 名（鍼治療群）と，脳トレーニングのみで経過観察を行う群 8 名（対照群）にブロックランダム割り付けした． 【方法】鍼治療群は三焦鍼法をベースとした週 1 回の鍼治療を 3 ヶ月間，計 12 回行った．主要評価項目は MoCA-J スコア，副次評価項目は血清 Aβシークエスタータンパク質（TTR，ApoA1，C3），MPI スコア，HDL-C 値とした． 【結果】主要評価項目では対照群と比べ，鍼治療群の MoCA-J スコア（Z=-2.153，r=0.52，p=0.031）は有意に増加した．副次評価項目では対照群と比べ，鍼治療群の ApoA1 値（Z=-2.168，r=0.53，p=0.030）が有意に増加した．対照群と比べ，鍼治療群の TTR 値（Z=-1.733，r=0.42，p=0.083）とHDL-C 値（Z=-1.498，r=0.36，p=0.134）は有意な増加は認められなかったが，効果量は「中」を示した． 【考察】鍼治療は，MCI 高齢者の認知機能を改善させ，鍼治療による MCI の認知機能の改善に関しては，Aβシークエスタータンパク質の活性化が治効機序の一端とすることが示唆された．

Abstract

Chronic obstructive pulmonary disease (COPD) involves a decrease in respiratory function and limits daily activities. We report a COPD patient whose respiratory symptoms were improved by acupuncture treatment. A 66-year-old man visited Gifu University Hospital with breathlessness on exercise. Despite medication, his general condition worsened. A series of acupuncture treatments was subsequently started on October 27, 2001. He had level III breathlessness on JRS classification prior to acupuncture treatment, and his respiratory function parameters were VC: 3.54 L, FEV1: 1.19 L, FEV1% (G): 33.6% and %FEV1: 45%. The basic meridian points used were KI 3 (Fuliu), LU 9 (Taiyuan), LU 1 (Zhongfu), CV 12 (Zhongwan), CV 4 (Guanyuan), BL 13 (Feishu), and BL 23 (Shenshu). The acupuncture needles were retained for ten minutes in each session. This research design was used to detect the specific efficacy of acupuncture treatment After ten acupuncture treatments over two months, his walking distance, Borg scale and respiratory function were improved compared with before treatment. These findings suggest that acupuncture treatment may be efficacious for advanced cases of COPD.
