International audience; Nous sommes confrontés à la pandémie COVID-19 qui est certes sans conséquences graves chez près de 85 % de la population générale mais qui peut engager le pronostic vital, plus particulièrement dans une population fragile. Le taux de mortalité estimé du COVID-19 est de 1 à 3 % soit entre 10 et 30 fois plus que le virus saisonnier de la grippe, et ceci sous réserve de données épidémiologiques imparfaites liées au fait que la recherche de SARS-CoV-2 n’est pas systématique dans une population asymptomatique, voire même symptomatique sans critères de gravité.

Abstract

National audience; Introduction - Rare ovarian tumors include complex borderline ovarian tumors, sex-cord tumors, germ cell tumors, and rare epithelial tumors. Indications and modalities of fertility preservation, infertility management and contraindications for hormonal contraception or menopause hormone therapy are frequent issues in clinical practice. A panel of experts from the French national network dedicated to rare gynaecological cancers, and of experts in reproductive medicine and gynaecology have worked on guidelines about fertility preservation, contraception and menopause hormone therapy in women treated for ovarian rare tumors. Methods - A panel of 39 experts from different specialties contributed to the preparation of the guidelines, following the DELPHI method (formal consensus method). Statements were drafted after a systematic literature review, and then rated through two successive rounds. Results - Thirty-five recommendations were selected, and concerned indications for fertility preservation, contraindications for ovarian stimulation (in the context of fertility preservation or for infertility management), contraceptive options (especially hormonal ones), and menopause hormone therapy for each tumor type. Overall, prudence has been recommended in the case of potentially hormone-sensitive tumors such as sex cord tumors, serous and endometrioid low-grade adenocarcinomas, as well as for high-risk serous borderline ovarian tumors. Discussion - In the context of a scarce literature, a formal consensus method allowed the elaboration of guidelines, which will help clinicians in the management of these patients.

Abstract

Abstract Purpose Colorectal cancer (CRC) screening has been shown to decrease CRC mortality. Organised mass screening programs are being implemented in France. Its perception in the general population and by general practitioners is not well known. Methods Two nationwide observational telephone surveys were conducted in early 2005. First among a representative sample of subjects living in France and aged between 50 and 74 years that covered both geographical departments with and without implemented screening services. Second among General Practionners (Gps). Descriptive and multiple logistic regression was carried out. Results Twenty-five percent of the persons(N = 1509) reported having undergone at least one CRC screening, 18% of the 600 interviewed GPs reported recommending a screening test for CRC systematically to their patients aged 50–74 years. The odds ratio (OR) of having undergone a screening test using FOBT was 3.91 (95% CI: 2.49–6.16) for those living in organised departments (referent group living in departments without organised screening), almost twice as high as impact educational level (OR = 2.03; 95% CI: 1.19–3.47). Conclusion CRC screening is improved in geographical departments where it is organised by health authorities. In France, an organised screening programs decrease inequalities for CRC screening.

Abstract

International audience; The prognosis of infracentimetric breast cancers (BC) is heterogeneous. The EURISTIC survey describes how French oncology specialists perceive the prognosis of pT1a,b pN0 BCs. A self-administered questionnaire has been sent to over 2000 French BC specialists. Six hundred and sixty-three physicians responded. Fifty-eight percent do not consider tumor size as a key prognostic criterion. They consider that the cutoff for poor prognosis is 22mm, 10mm and 7mm for hormone receptors (HRs)+, HER2+ and triple-negative (TN) tumors respectively. Eighty-three percent of respondents consider that a HR+ pT1a,b tumor has a good prognosis (21% and 8% for HER2+ and TN respectively). Factors perceived as most detrimental are: HER2 overexpression (29% of respondents); HR- (20%); high grade (20%); TN status (14%); high KI67 (5%); presence of lymphovascular invasion (3%); young age (2%) and high mitotic index (1%). For French specialists, immunohistochemical characteristics, in particular hormone and HER2 status, are strong prognostic factors in BCs below 1cm.

Cet ouvrage présente les communications du 12eCours supérieur francophone sur le cancer du sein qui s'est déroulé du 19 au 22Janvier 2011 au Palais des congrès de Nice. Ce Cours réunit chaque année près de 200 conférenciers, experts francophones du domaine, et plus de 700 participants. L'objectif est de faire le point sur l'actualité de la prise en charge de cette pathologie de plus en plus fréquente. La première journée du Cours était consacrée aux Soins Oncologiques de Support et aux Actualités en chirurgie. Trente oncologues ont présenté, à partir d'une veille bibliographique, l'état des connaissances dans toutes les disciplines couvertes : chirurgie, anatomopathologie, radiothérapie, traitements systémiques sans oublier d'aborder les prises en charge particulières des formes métastatiques, des patientes non ménopausées ou des patientes âgées. Enfin, au-delà des communications scientifiques, chaque année ce Cours donne lieu à des {laquo} Recommandations pour la Pratique Clinique{raquo} publiées en fin d'année qui sont une référence pour toute la profession.

Abstract

Bisphosphonates (BP) are the current standard of care for preventing malignant skeletal related-events. Recent reports have documented the relationship between osteonecrosis of the jaws (ONJ) and the use of BPs. Based on the opinion of experts, the purpose of our analysis was to summarize current knowledge, to propose therapeutic options, and to define areas of research. Identified risk factors were long-lasting exposure to BPs, intravenous nitrogen-containing BPs, and poor dental status. Three major hypotheses could explain the genesis of ONJ: excess of bone turnover inhibition, antiangiogenic effect, and local infection. Before the onset of therapy, the dental status must be controlled, and followed during treatment. Dental procedures could worsen the risk of ONJ, and indications must be well evaluated. When an ONJ occurs, the management should be adapted according to its extent. Thereby, a customization of BP therapy should be applied taking into account the aggressiveness of the underlying disease.

Abstract

International audience; INTRODUCTION: The practices of screening and the parameters influencing these practices are not well known in France. The objectives of the Edifice study were to analyze a large cohort of patients and doctors in order to further characterize these parameters. PATIENTS AND METHODS: The study was performed by the Institute TNS Healthcare-SOFRES, and included 2 parallel studies: 1) on 1 609 healthy persons representative of the global French population and aged 40 to 75 years (N = 1 509), with an over representation of patients aged 50 to 74 years living in the 22 pilot French departments pilots; 2) on 600 generalist practitioners. Data were collected and analyzed by the expert panel... RESULTS: Ninety-three, 25, 36 and 6% of the patients in the general population declared to have performed at least one a screening exam for breast, colon, prostate, and lung carcinoma respectively. Seventy, 20, 60 and 4% of GP declare to propose systematically to a 40-75-year-old patient a screening test for breast, colon, prostate, or lung cancer. For breast cancer screening the adhesion of the GP is independent of the date of implementation of a general screening in their own regions, while for colorectal screening, 34 and 20% of the patients living in the pilot versus other departments were screened. Overall, prostate cancer screening is recommended by the GP panel for 77.1% of patients aged 50 to 75 years. CONCLUSIONS: This study shows a good adhesion of screening procedures for GP and patients, shows that screening is improved by general screening policy in colorectal cancer, but that prostate cancer screening practices exceed what is recommended according to evidence based medicine.

Pr Moise Namer, exerçant au Centre Azuréen de Cancérologie à Mougins-Nice est considéré comme l'un des meilleurs experts du cancer du sein dans le monde. Il organise chaque année un Cours sur les actualités du cancer du sein suivi par 700 médecins qui donne ensuite lieu à des recommandations très prisées par la communauté scientifique.

Abstract

Le 10e Cours francophone supérieur sur le cancer du sein s'est déroulé du 18 au 20 Janvier 2007 à l'Hôtel Mas d'Artigny à Saint-Paul-de-Vence. Durant les deux premières journées, toutes les étapes importantes de la prise en charge des cancers du sein ont été déclinées. L'analyse du risque de voir survenir un cancer du sein a été explorée dans sa diversité suivie par les résultats du dépistage organisé et les problèmes du diagnostic. Les traitements locorégionaux ont précédé les traitements adjuvants sans oublier les patientes dont la tumeur présentait une hyperexpression de HER2. Une session a été consacrée aux cancers métastatiques et une autre aux personnes âgées. Comme à chaque cours, nous nous sommes penchés sur l'avenir en dissertant sur la classification moléculaire et sur le passé avec la très belle conférence du Pr David Khayat sur le bilan du Plan Cancer. Durant l'année 2006, divers groupes d'experts se sont réunis plusieurs fois et ont écrit des rapports sur les situations dues aux thérapeutiques modernes comme l'état osseux qui fait suite aux hormonothérapies actuelles ou l'état cardiaque qui fait suite à l'utilisation des anthracyclines et du trastuzumab. Ce livre recueille près de 50 articles dont l'ambition est de faire le point en 2007 sur cette pathologie fréquente et en constante évolution.

Abstract

En janvier 2005, le 9e Cours de Saint-Paul-de-Vence, consacré au Cancer du Sein a été dominé par l'établissement des « Recommandations pour la Pratique Clinique ». Les cinq sujets de ces recommandations, comme la prise en charge du creux axillaire, les traitements néo-adjuvants, la prise en charge des patientes dont la tumeur sur-exprime HER2, l'hormonothérapie adjuvante et enfin la chimiothérapie adjuvante ont été largement abordés. Comme à chaque cours, certains domaines incontournables ont été traités tels que le dépistage organisé, la radiologie conventionnelle et l'IRM, les facteurs pronostiques et prédictifs. Les cancers canalaires et les cancers lobulaires ont été comparés d'une manière remarquable dans trois communications qui ont été réalisées par l'école de Saint-Cloud. Les données sur les cancers in situ ont été actualisées. Enfin, des conseils ont été donnés pour l'utilisation des différents référentiels, qu'ils soient locaux, régionaux ou nationaux.
