Purpose: To assess the corneal endothelial morphometry in healthy elderly Hispanic eyes. Methods: A retrospective, cross-sectional, observational, descriptive, and analytical study was designed to assess the central region of the corneal endothelium by specular microscopy. A total of 241 eyes from 125 patients were studied. All eyes included were from healthy patients 65 or older (mean: 74.56± 6.74), without ocular disease or surgeries. Results: The mean cell density (MCD) was 2198.92±493.43 cells/mm2. 125 (51.87%) eyes had polymegethism, with a mean coefficient of variation (CV) of 42.89 ± 9.16%. 148 (61.41%) eyes presented pleomorphism, with an average of 46.25 ± 7.13% hexagonal cells. The mean corneal thickness was 527.55±28.37 μm. A statistically significant difference between age groups was found for MCD and pachymetry (P Conclusion: This study suggests that the elderly Hispanic population has a high prevalence of polymegethism, pleomorphism, and guttata. To provide a review for general practitioners and medical specialists about the most common eye complaints and ocular manifestations in a COVID-19 patient.Reviewed 50 articles referenced in 4 databases from 20 December 2019 to 16 September 2020.Of the 50 articles reviewed, 26 met the criteria for analysis and were included in the study. Among them were 5 reviews, 6 case series, 7 case reports, 4 cross-sectional studies, 2 systematic reviews, 1 cohort study and 1 correspondence. We found that the prevalence of ocular manifestations in patients during the course of COVID-19 varied between 0.8% and 31.6%, depending on the study design. Symptoms of acute conjunctivitis were the most frequent clinical findings. Acute phase reactants were also correlated to ocular manifestations and the severity of the systemic disease in many reports.Ocular manifestations are not uncommon in COVID-19 patients and have been associated with higher levels of acute phase reactants as well as a higher degree of severity of the systemic disease. We recommend that all physicians consider COVID-19 as a differential diagnosis in the presence of acute conjunctivitis with or without systemic symptoms.

All eyes included were from patients 65 or older (mean: 73.87±6.86), without ocular disease or surgeries.Results: The mean cell density (MCD) was 2268±597.0 cells/mm2. 44% of patients had polymegethism, with an average coefficient of variation (CV) of 42.04 ± 10.1%. 76% of patients had low pleomorphism, with an average of 42.3 ± 8.9% hexagonal cells. The mean corneal thickness was 537±38.2 μm. There was no statistically significant difference between age groups for MCD, CV, hexagonality and pachymetry (p=>.05). 27% of patients had corneal guttata.Conclusion: This study suggests that the elderly Hispanic population has a high prevalence of polymegethism, pleomorphism and guttata. Surprisingly, the oldest age group (>85) had an above average MCD, low polymegethism and good pleomorphism.

Background We report the case and management of a patient that started with primary cornea guttata symptoms after cataract surgery with trifocal intraocular lens (IOL) implant. The unusual postoperative presentation of the disease in association with a trifocal IOL resulted in increased corneal surface aberrations that resulted in visual dissatisfaction. Case presentation A 67 year-old male was evaluated due to blurred vision for two months, as well as a desire to stop using glasses in his everyday life. At the initial refraction, myopia, corneal astigmatism and nuclear cataract were diagnosed in both eyes. A nuclear cataract was diagnosed in both eyes. Phacoemulsification surgery was performed in OS, implanting a trifocal IOL. The postoperative refraction in OS was -1.25DCx120º, with a best-corrected visual acuity (BCVA) of 20/20. Six months later, the patient kept complaining of blurred vision and night glare. In slit lamp examination, corneal guttae were denoted and changes in corneal endothelium morphology suggested primary cornea guttata. A monofocal IOL exchange was performed in order to reduce corneal aberrations with excellent outcome at 2-year follow up. Conclusions Screening for endothelial dysfunction is a key element for successful outcomes in cataract surgery. Corneal guttae accent optical aberrations when in combination with premium IOLs.

Rosacea is a dermatologic condition that affects the midfacial region. This chronic inflammatory disease can present with ocular manifestations distinguished by meibomian gland dysfunction, dry eye, and in severe cases, corneal ulceration and perforation. Neurotrophic keratopathy is an ocular condition with a reduced or absent corneal sensation associated with a corneal epitheliopathy. We present a 68-year-old man with both ocular rosacea and neurotrophic keratopathy who developed a corneal perforation. It was managed successfully with a tectonic keratoplasty in addition to topical steroids, lubrication, and oral tetracyclines. The perforation is explained by the superposition of both pathologies which lead to dry eye syndrome, along with chronic inflammation of the ocular surface. The effective follow-up and treatment of any epithelial defect in these patients is a major factor for corneal melting and perforation prevention.
