Abstract

Abstract Background Acute pyelonephritis (APN) is a common complication of ureteral obstruction caused by urolithiasis, and it can be lethal if it progresses to septic shock. We investigated the clinical characteristics of patients undergoing emergency drainage and assessed risk factors for septic shock. Methods A retrospective study was performed of 98 patients (101 events) requiring emergency drainage at our urology department for obstructive APN associated with upper urinary tract calculi from January 2003 to January 2011. Clinical characteristics were summarized, and risk factors for septic shock were assessed by logistic regression analysis. Results Objective evidence of sepsis was found in 64 (63.4%) events, and 21 events (20.8%) were categorized as septic shock. Ninety-six patients recovered, but 2 patients died of septic shock. Multivariate analysis revealed that age and the presence of paralysis were independent risk factors for septic shock. Conclusions APN associated with upper urinary tract calculi is a severe disease that should be treated with caution, particularly when risk factors are present.

Abstract

The ability to perform quantum error correction is a significant hurdle for scalable quantum information processing. A key requirement for multiple-round quantum error correction is the ability to dynamically extract entropy from ancilla qubits. Heat-bath algorithmic cooling is a method that uses quantum logic operations to move entropy from one subsystem to another, and permits cooling of a spin qubit below the closed system (Shannon) bound. Gamma-irradiated, $^{13}$C-labeled malonic acid provides up to 5 spin qubits: 1 spin-half electron and 4 spin-half nuclei. The nuclei are strongly hyperfine coupled to the electron and can be controlled either by exploiting the anisotropic part of the hyperfine interaction or by using pulsed electron-nuclear double resonance (ENDOR) techniques. The electron connects the nuclei to a heat-bath with a much colder effective temperature determined by the electron's thermal spin polarization. By accurately determining the full spin Hamiltonian and performing realistic algorithmic simulations, we show that an experimental demonstration of heat-bath algorithmic cooling beyond the Shannon bound is feasible in both 3-qubit and 5-qubit variants of this spin system. Abstract

A 22-year-old woman who was referred to our hospital presented with a complaint of urinary incontinence since childhood. Abdominal contrast-enhanced computed tomography revealed complete duplication of the left ureter. In addition, the upper half of the left kidney showed poor contrast, accompanied by signs of hydronephrosis. Magnetic resonance urography failed to show an opening between the upper half of the left kidney and the ectopic ureter. Cystoscopy revealed two normally positioned ureteral orifices. After intravenous injection of indigo carmine, however, the dye became evident in the vagina. Thus, she was diagnosed to have urinary incontinence due to complete duplication of the left ureter and an ectopic ureteral opening into the vagina. Transcatheter arterial embolization of the upper half of the kidney, the origin of the ectopic ureter, immediately relieved the patient of urinary incontinence. Although patients with BKPyVAN usually received a reduction of immunosuppressive agents, the majority of these patients undergo the loss of the graft kidney without any effective treatment afterward. Therefore, development of more effective therapy for BKPyVAN is eagerly expected.Patients and Methods: Among patients who underwent a kidney transplantation between January 2016 and April 2019 at our hospital, there were five cases of BKPyVAN. After the initial diagnosis, all patients discontinued administration of mycophenolate mofetil (MMF), which was not enough to diminish decoy cells in urine cytology test. Therefore, all patients received additional intravenous immunoglobulin (IVIG) (100 mg/kg/day) therapy for five days and were evaluated for the therapeutic effect of IVIG with immunohistochemical examination using re-biopsy samples of the graft kidney.Results: After IVIG therapy, 2 cases showed negative decoy cells in urine and 3 cases showed a drastic decrease of plasma BK virus load. Importantly, simian virus (SV) 40 large T antigens diminished after IVIG administration in all cases, which degraded polyomavirus nephropathy classification.Conclusion: Although it is difficult to treat BKPyVAN after kidney transplant, IVIG therapy was considered to a promising treatment to improve severity of BKPyVAN especially in cases that dose reduction of immunosuppressive agents was ineffective.Keywords: BK virus, BK virus nephropathy, intravenous immunoglobulin; IVIG, kidney transplantation, polyomavirus nephropathy classification

Abstract

A 20-year-old woman had urinary incontinence since childhood. She self-managed her symptoms by using incontinence pads and she admitted never having been to a urologist. When she consulted a urologist for cystitis, ultrasonography could not locate the presence of a right kidney. She was suspected of having a contracted kidney and was referred to our hospital for further examinations and treatment. An enhanced computed tomography scan showed a contracted right kidney, which was located on the surface of the inferior vena cava. Magnetic resonance imaging showed that the right ureter extended into the vagina. Cystoscopy showed the absence of a right ureteral orifice, and an ectopic orifice was identified in the vagina. Laparoscopic nephroureterectomy was performed based on the diagnosis of a hypoplastic kidney with an ectopic ureter. We removed the ureter as far as the vagina because a residual ureteral segment could cause infection. Postoperatively, the patient had no complications, and her uterine artery was successfully saved. The patient achieved complete continence after the operation.
