10. Impact of Controlling Nutritional Status Score on Mortality in Elderly Patients with Idiopathic Pulmonary Fibrosis.

Abstract

Background: There are only a few reports on the nutritional status and mortality of patients with idiopathic pulmonary fibrosis (IPF). As such, this study aims to investigate the relationship between controlling nutritional status (CONUT) and the mortality of elderly patients with IPF. Methods: A total of 170 IPF patients aged ≥65 years old who visited the rehabilitation department of our hospital between July 2014 and July 2021 (mean age: 75.7 ± 6.3 years, sex (male/female): 138/32, %FVC: 78.3 ± 18.3%) were retrospectively analyzed. The Kaplan–Meier method and log-rank test were applied. Furthermore, using a Cox proportional hazards model with multivariate analysis, we analyzed the relationship between all-cause mortality and baseline characteristics including CONUT. Results: Based on the CONUT score, the normal group included 101 cases, the mild group included 58 cases, the moderate group included 11 cases, and the severe group had 0 cases. There were 49 cases of all-cause mortality events, suggesting that the mortality of the moderate group was significantly poorer than that of the normal and mild groups (p < 0.05). Furthermore, multivariate analysis identified GAP stage (HR: 5.972, 95%CI: 2.901~12.291, p < 0.0001), mMRC scale (HR: 0.615, 95%CI: 0.389~0.971, p = 0.009), and CONUT (HR: 2.012, 95%CI: 1.192~3.395, p = 0.037) as factors significantly influencing mortality. Conclusions: Severe malnutrition was not observed in elderly patients with IPF. Moderate malnutrition was associated with a significantly higher risk of all-cause mortality, suggesting that CONUT is an important indicator for predicting mortality. [ABSTRACT FROM AUTHOR]

Abstract

Notch signaling plays various key roles in cell fate determination during CNS development in a context-dependent fashion. However, its precise physiological role and the localization of its target cells remain unclear. To address this issue, we developed a new reporter system for assessing the RBP-J-mediated activation of Notch signaling target genes in living cells and tissues using a fluorescent protein Venus. Our reporter system revealed that Notch signaling is selectively activated in neurosphere-initiating multipotent neural stem cells in vitro and in radial glia in the embryonic forebrain in vivo. Furthermore, the activation of Notch signaling occurs during gliogenesis and is required in the early stage of astroglial development. Consistent with these findings, the persistent activation of Notch signaling inhibits the differentiation of GFAP-positive astrocytes. Thus, the development of our RBP-J-dependent live reporter system, which is activated upon Notch activation, together with a stage-dependent gain-of-function analysis allowed us to gain further insight into the complexity of Notch signaling in mammalian CNS development. Abstract

SMARCA4 -deficient thoracic sarcoma (SMARCA4-DTS) is a recently described entity of thoracic sarcomas with an undifferentiated rhabdoid morphology and SMARCA4 inactivation. Regardless of some reports about the histopathological findings so far, there have been only a few reports about the cytological features. In this article, we present the pathological features of 2 SMARCA4-DTS cases, including the cytological findings. Histopathologically, the tumor cells showed atypical loosely cohesive large epithelioid cells focally with geographic necrosis. Some cells were characterized by rhabdoid cells. Both patients showed intrathoracic masses with a history of smoking, and loss of SMARCA4 expression was confirmed with histopathological specimens. Immunohistochemically, tumor cells of both cases were at least focally positive for cytokeratin, CD34, CD99, synaptophysin, SOX2, and SALL4. In addition, tumor cells demonstrated significantly reduced expression of BRG1/SMARCA4 and SMARCA2. In conclusion, SMARCA4-DTS should be taken into consideration in the differential diagnosis of tumors with undifferentiated rhabdoid morphology involving the thoracic region. [ABSTRACT FROM AUTHOR]

Aims/Introduction: Although increases in urinary protein excretion generally precede a decline in the glomerular filtration rate, non‐proteinuric renal impairment is common in patients with diabetes. In the present study, we examined the relationship between indices of arterial stiffness and renal function in type 2 diabetic patients without proteinuria. Methods: Blood sampling, 24‐h urine collection, brachial–ankle pulse wave velocity, and 24‐h ambulatory blood pressure monitoring were performed in type 2 diabetic patients without overt proteinuria. The ambulatory arterial stiffness index was calculated as (1 – the regression slope of diastolic/systolic ambulatory blood pressure). Abstract

本稿では、2000年9月に実施された東京版総合社会調査(TGSS) のデータに基づき、パーソナルネットワークが個人の職業的地位の達成において果たす役割について検討する。個人が保有する人間関係を社会関係的資源と捉え、その資源の活用が就職時に有利な効果をあらわすことは、多くの先行研究において示されてきた。しかし、関係的資源がどのような機能を果たすかについての一貫した知見は得られていない。本稿では、関係的資源の保有と利用の関連性、および、それぞ、れの異なった役割に着目し、地位達成における関係的資源の機能について追究する。TGSSの対象者には複数の間柄について、「日頃から何かと頼りにし、親しくしている」人々の数を挙げてもらっている。そのデータをもとに、個人がどのようなネットワークを保有しているかを測定し(保有ネットワーク)、どのような方法で就職したかという入職経路(ネットワークの利用)との関連性を調べた。その結果、知人の紹介を通じて就職した人々とそれ以外の経路で就職した人々の保有するネットワークには、その規模において有意差が見られなかった。しかし、ネットワークを利用して就職した個人のネットワークは、そうでない人と比べると、より多くの間柄の人々を含む多様なネットワークであることがわかった。さらに、関係的資源が就職時に果たす役割について、社会的資本(social capital) としての機能と人的資本(human capital)としての機能について分析した結果、ネットワークの利用が社会的資本として個人の職業的地位に有利に影響を与えるだけでなく、保有するネットワークが人的資本としても効果をあらわす可能性があることが示唆された。, This paper investigates how social resources contribute to attaining one's occupational status. Previous research indicated that one's personal networks may be viewed as social capital through which one can obtain high status jobs. Social resources such as one's personal networks can also be seen as human capital that employers might find desirable for their potential value to the company. Social resources, therefore, can serve either or both as social capital and/or human capital. Using as ample of the General Social Survey conducted in Tokyo (Tokyo GSS) in 2000, the analyses in this paper intend to distinguish the benefit of the use of social resources (social capital effect) from the benefit of their possession (human capital effect), as well as investigate the relationship between them. It was found that individuals with diverse networks consisting of various relationships are more likely to utilize social resources when obtaining jobs. Having a larger network, however, was not shown to increase the chance of social resource utilization. The analyses also showed that those whose obtained jobs via acquaintances are more likely to hold higher positions within the company, consistent with the social capital effect. Possession of a large and/or diverse network is also shown to lead to a higher position, indicating the possibility of the human capital effect. These results suggest that the diversity of a network may allow one's social resources to function as both social and human capital, while having a large network without diversity only serves as human capital.
