Abstract

Chronic obstructive pulmonary disease (COPD) and asthma are common lung diseases with heterogeneous clinical presentations. Lung imaging allows evaluations of underlying pathophysiological changes and provides additional personalized approaches for disease management. This narrative review provides an overview of recent advances in chest imaging analysis using various modalities, such as computed tomography (CT), dynamic chest radiography, and magnetic resonance imaging (MRI). Visual CT assessment localizes emphysema subtypes and mucus plugging in the airways. Dedicated software quantifies the severity and spatial distribution of emphysema and the airway tree structure, including the central airway wall thickness, branch count and fractal dimension of the tree, and airway-to-lung size ratio. Nonrigid registration of inspiratory and expiratory CT scans quantifies small airway dysfunction, local volume changes and shape deformations in specific regions. Lung ventilation and diaphragm movement are also evaluated on dynamic chest radiography. Functional MRI detects regional oxygen transfer across the alveolus using inhaled oxygen and ventilation defects and gas diffusion into the alveolar–capillary barrier tissue and red blood cells using inhaled hyperpolarized ¹²⁹Xe gas. These methods have the potential to determine local functional properties in the lungs that cannot be detected by lung function tests in patients with COPD and asthma. Further studies are needed to apply these technologies in clinical practice, particularly for early disease detection and tailor-made interventions, such as the efficient selection of patients likely to respond to biologics. Moreover, research should focus on the extension of healthy life expectancy in patients at higher risk and with established diseases.

To elucidate the progression of COPD, we performed a cluster analysis using the following DPM parameters: normal (DPMNormal), gas-trapping (DPMGasTrap), and emphysematous lesions (DPMEmph). Our findings revealed the characteristics of each cluster and the 3-year disease progression using imaging parameters.Patients and Methods: Inspiratory and expiratory chest computed tomography (CT) images of 131 patients with COPD were examined, of which 84 were followed up for 3 years. The percentage of low attenuation volume (LAV%) and the square root of the wall area of a hypothetical airway with an internal perimeter of 10 mm (√Aaw at Pi10) were quantitatively measured using inspiratory chest CT. A hierarchical cluster analysis was performed using the DPM parameters at baseline. Five clusters were named according to the dominant DPM parameters: normal (NL), normal-GasTrap (NL-GT), GasTrap (GT), GasTrap-Emphysema (GT-EM), and Emphysema (EM).Results: Women were predominantly diagnosed with GT. Forced expiratory volume in 1 s gradually decreased in the following order: NL, NL-GT, GT, GT-EM, and EM. DPMEmph correlated well with LAV%. 20. Aorta and main pulmonary artery segmentation using stacked U‐Net and localization on non‐contrast‐enhanced computed tomography images.

Abstract

Background: The contact between the aorta, main pulmonary artery (MPA), main pulmonary vein, vena cava (VC), and esophagus affects segmentation of the aorta and MPA in non‐contrast‐enhanced computed tomography (NCE‐CT) images. Purpose: A two‐stage stacked U‐Net and localization of the aorta and MPA were developed for the segmentation of the aorta and MPA in NCE‐CT images. Methods: Normal‐dose NCE‐CT images of 24 subjects with chronic thromboembolic pulmonary hypertension (CTEPH) and low‐dose NCE‐CT images of 100 subjects without CTEPH were used in this study. The aorta is in contact with the ascending aorta (AA) and MPA, the AA with the VC, the aortic arch (AR) with the VC and esophagus, and the descending aorta (DA) with the esophagus. These contact surfaces were manually annotated. The contact surfaces were quantified using the contact surface ratio (CSR). Segmentation of the aorta and MPA in NCE‐CT images was performed by localization of the aorta and MPA and a two‐stage stacked U‐Net. Localization was performed by extracting and processing the trachea and main bronchus. The first stage of the stacked U‐Net consisted of a 2D U‐Net, 2D U‐Net with a pre‐trained VGG‐16 encoder, and 2D attention U‐Net. The second stage consisted of a 3D U‐Net with four input channels: the CT volume and three segmentation results of the first stage. The model was trained and tested using 10‐fold cross‐validation. Segmentation of the entire volume was evaluated using the Dice similarity coefficient (DSC). Segmentation of the contact area was also assessed using the mean surface distance (MSD). The statistical analysis of the evaluation underwent a multi‐comparison correction. CTEPH and non‐CTEPH cases were classified based on the vessel diameters measured from the segmented MPA. Results: For the noncontact surfaces of AA, the MSD of stacked U‐Net was 0.31 ± 0.10 mm (p < 0.05) and 0.32 ± 0.13 mm (p < 0.05) for non‐CTEPH and CTEPH cases, respectively. For contact surfaces with a CSR of 0.4 or greater in AA, the MSD was 0.52 ± 0.23 mm (p < 0.05), and 0.68 ± 0.29 mm (p > 0.05) for non‐CTEPH and CTEPH cases, respectively. MSDs were lower than those of 2D and 3D U‐Nets for contact and noncontact surfaces; moreover, MSDs increased slightly with larger CSRs. However, the stacked U‐Net achieved MSDs of approximately 1 pixel for a wide contact surface. The area under the receiver operating characteristic curve for CTEPH and non‐CTEPH classification using the right main pulmonary artery (RMPA) diameter was 0.97 (95% confidence interval [CI]: 0.94–1.00). Conclusions: Segmentation of the aorta and MPA on NCE‐CT images were affected by vascular and esophageal contact. The application of stacked U‐Net and localization techniques for non‐CTEPH and CTEPH cases mitigated the impact of contact, suggesting its potential for diagnosing CTEPH. [ABSTRACT FROM AUTHOR]

Abstract

Background and Objective: Weight and muscle loss are predictors of poor outcomes in chronic obstructive pulmonary disease. However, to our knowledge, no study has investigated the predictors of longitudinal weight loss or its composition from functional and morphological perspectives. Methods: This longitudinal observational study with a median follow‐up period of 5 years (range: 3.0–5.8 years) included patients with COPD and ever‐smokers at risk of COPD. Using chest computed tomography (CT) images, airway and emphysematous lesions were assessed as the square root of the wall area of a hypothetical airway with an internal perimeter of 10 mm (√Aaw at Pi10) and the percentage of low attenuation volume (LAV%). Muscle mass was estimated using cross‐sectional areas (CSAs) of the pectoralis and erector spinae muscles, and fat mass was estimated using the subcutaneous fat thickness at the level of the 8th rib measured using chest CT images. Statistical analyses were performed using the linear mixed‐effects models. Results: In total, 114 patients were enrolled. Their body mass index remained stable during the study period while body weight and muscle CSA decreased over time and the subcutaneous fat thickness increased. Reduced forced expiratory volume in 1 s and peak expiratory flow (PEF) at baseline predicted the future decline in muscle CSA. Conclusion: Severe airflow limitation predicted future muscle wasting in patients with COPD and ever‐smokers at risk of COPD. Airflow limitation with a PEF slightly below 90% of the predicted value may require intervention to prevent future muscle loss. [ABSTRACT FROM AUTHOR]

Abstract

Chronic obstructive pulmonary disease (COPD) is predicted to become the third leading cause of death worldwide by 2030. A longitudinal study using CT scans of COPD is useful to assess the changes in structural abnormalities. In this study, we performed visualization and unsupervised clustering of emphysema progression using t-distributed stochastic neighbor embedding (t-SNE) analysis of longitudinal CT images, smoking history, and SNPs. The procedure of this analysis is as follows: (1) automatic segmentation of lung lobes using 3D U-Net, (2) quantitative image analysis of emphysema progression in lung lobes, and (3) visualization and unsupervised clustering of emphysema progression using t-SNE. Nine explanatory variables were used for the clustering: genotypes at two SNPs (rs13180 and rs3923564), smoking history (smoking years, number of cigarettes per day, pack-year), and LAV distribution (LAV size and density in upper lobes, LAV size, and density in lower lobes). The objective variable was emphysema progression which was defined as the annual change in low attenuation volume (LAV%/year) using linear regression. The nine-dimensional space was transformed to two-dimensional space by t-SNE, and divided into three clusters by Gaussian mixture model. This method was applied to 37 smokers with 68.2 pack-years and 97 past smokers with 51.1 pack-years. The results demonstrated that this method could be effective for quantitative assessment of emphysema progression by SNPs, smoking history, and imaging features.

38. Per cent low attenuation volume and fractal dimension of low attenuation clusters on CT predict different long-term outcomes in COPD

Abstract

Background Fractal dimension (D) characterises the size distribution of low attenuation clusters on CT and assesses the spatial heterogeneity of emphysema that per cent low attenuation volume (%LAV) cannot detect. This study tested the hypothesis that %LAV and D have different roles in predicting decline in FEV1, exacerbation and mortality in patients with COPD. Methods Chest inspiratory CT scans in the baseline and longitudinal follow-up records for FEV1, exacerbation and mortality prospectively collected over 10 years in the Hokkaido COPD Cohort Study were examined (n=96). The associations between CT measures and long-term outcomes were replicated in the Kyoto University cohort (n=130). Results In the Hokkaido COPD cohort, higher %LAV, but not D, was associated with a greater decline in FEV1 and 10-year mortality, whereas lower D, but not %LAV, was associated with shorter time to first exacerbation. Multivariable analysis for the Kyoto University cohort confirmed that lower D at baseline was independently associated with shorter time to first exacerbation and that higher LAV% was independently associated with increased mortality after adjusting for age, height, weight, FEV1 and smoking status. Conclusion These well-established cohorts clarify the different prognostic roles of %LAV and D, whereby lower D is associated with a higher risk of exacerbation and higher %LAV is associated with a rapid decline in lung function and long-term mortality. Combination of %LAV and fractal D may identify COPD subgroups at high risk of a poor clinical outcome more sensitively.

Abstract

Background: Excessive alcohol intake has been shown to be associated with cardiovascular disease via metabolic pathways. However, the relationship between alcohol intake and obesity has not been fully elucidated. We aimed to examine the association of alcohol consumption with fat deposition and anthropometric measures. Methods: From 2006–2008, we conducted a cross-sectional study in a population-based sample of Japanese men aged 40 through 79 years. Areas of abdominal visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) were calculated using computed tomography imaging. Based on a questionnaire, we classified participants into five groups according to weekly alcohol consumption, excluding former drinkers: non-drinkers (0 g/week), 0.1–160.9, 161–321.9, 322–482.9, and ≥483 g/week. Multivariable linear regression was used to estimate adjusted means of obesity indices for each group. Results: We analyzed 998 men (mean age and body mass index [BMI], 63.8 years and 23.6 kg/m2, respectively). Abstract

Aims: Calcification in the coronary and aortic arteries has been linked to cardiovascular morbidity and mortality. The pathophysiological influence of aortic artery calcification (AAC) differs from that of coronary artery calcification (CAC). We aimed to compare the relationships between CAC and AAC and cardiovascular disease (CVD) risk factors. Methods: We examined a random sample of 1035 Japanese men aged 40–79 years. CAC and AAC were measured by computed tomography and scored according to the Agatston method. Using a logistic regression, we calculated the odds ratio (OR) as being in the highest quintile (Q5) of the calcification score compared to the lower quintiles (Q1–Q4) per 1 standard deviation higher CVD risk factor. Models were simultaneously adjusted for age, body mass index (BMI), systolic blood pressure, smoking (pack-year), alcohol intake, hemoglobin A1c, uric acid, estimated glomerular filtration rate (eGFR), serum lipids, and C-reactive protein. Differences in ORs were investigated using a generalized estimating equation. We performed a multiple linear regression using log-transformed CAC and AAC values as dependent variables. Results: CAC and AAC were independently associated with age (OR, 95% CI: 2.30 [1.77–2.98] for CAC and 3.48 [2.57–4.73] for AAC), p for difference < 0.001), systolic blood pressure (1.29 [1.08–1.53] and 1.28 [1.07–1.54], p for difference = 0.270), and smoking (1.22, [1.04–1.43] and 1.34 [1.13–1.58]) p for difference = 0.071). Alcohol correlated with AAC only (1.17 [0.97–1.41] for CAC and 1.42 [1.16–1.73] for AAC, p for difference = 0.020). Conclusions: CAC and AAC were associated with similar CVD risk factors. The strength of association slightly differed between CAC and AAC.

Background: Quantitative CT shows promise as an outcome measure for cystic fibrosis (CF) lung disease in infancy, but must be accomplished at a dose as low as reasonably achievable. Objective: To determine the feasibility of ultra-low-dose CT for quantitative measurements of airway dimensions.
