25. Changes in Skin Perfusion Pressure After Hyperbaric Oxygen Therapy Following Revascularization in Patients With Critical Limb Ischemia: A Preliminary Study.

Abstract

Hyperbaric oxygen (HBO) therapy promotes wound healing in patients with ischemic disease; however, HBO-induced changes in skin peripheral circulation have not been evaluated in clinical practice. Here, we investigated these changes in patients with critical limb ischemia (CLI), with a focus on the angiosome of crural blood vessels with blood flow improved by endovascular therapy (EVT). Six patients with CLI and ulcers who were treated with HBO after EVT (7 limbs; 1 patient had ulcers in the bilateral limbs) and 3 healthy subjects (6 limbs) were enrolled. HBO therapy was performed at 2 atm under 100% oxygen for 90 min per session. Skin perfusion pressure (SPP) was measured in the dorsum and sole of the foot 1 hour before (pre-SPP) and after (post-SPP) HBO therapy. ΔSPP was calculated as post-SPP minus pre-SPP. SPP measurement regions were divided into those that did (direct region) and did not (indirect region) correspond to the vascular angiosome in which angiography findings of the crus were improved after EVT; i.e., when the anterior tibial artery was effectively treated with EVT, the dorsum was the direct region and the sole was the indirect region, and vice versa when the posterior tibial artery was treated. In the direct, indirect, and healthy subject groups, the ΔSPPs were 20.5±8.7 (p=0.002), –6.4±10.9, and –15.1±18.1 (p=0.014), respectively; that of the direct group was significantly greater than that of the other groups. These results suggest that short-term improvement of the peripheral circulation by HBO therapy was significant in patients with successful revascularization. [ABSTRACT FROM AUTHOR]

Abstract

中国電力(株)の水力発電所における鋼構造物の多くは設置から長期間が経過しており, 今後老朽化がさらに進展していくことが予想される。これらの鋼構造物の健全度を正確に評価し, 適切な更新・補修を計画的に実施していく必要があるが, このような健全度評価には鋼構造物の応力状態の把握が不可欠である。そこで, 代表的な鋼構造物であるゲートの現在および将来の応力状態を把握するため, 「維持管理用解析モデル」による3次元有限要素解析を実施し, 実測値と比較することでその妥当性を検証した。この手法により, 実際に24発電所のゲー卜の健全度診断を実施し, その結果をもとに長期的なゲート更新計画を策定した。

Abstract

海洋環境における円形鋼管杭の腐食にともなう強度低下の一予測法を示した．まず，防食工の損傷・劣化を考慮できる防食工の劣化モデルと腐食表面凹凸形状の経年変化を作成するモデルを作成し，これらのモデルを用いて海洋環境における円形鋼管杭の圧縮強度の経年変化を予測した．腐食表面作成モデルの支配パラメータは，19年間海洋環境に暴露された鋼管杭の腐食表面座標計測結果から決定し，FEMおよび簡易強度評価式により圧縮強度の将来予測を行った．その結果，ここで提案した鋼管杭の圧縮強度の経年変化予測法は，電気化学的な腐食メカニズムは考慮していないものの，数少ない支配パラメータにより実現象に近い鋼腐食表面形状を表現できるとともに，腐食進展にともなう強度の低下予測ができることを示した．, This paper presents a future prediction method of strength deterioration of circular steel column corroded along with aging in marine environment. Corroded surfaces along with aging are made by the two models. One model makes surface unevenness caused by corrosion progress and the other expresses deterioration of paint to prevent rust. And based on the corroded surface made by the models, the remaining strength after a passing period can be predicted by FEM analyses or other formulae evaluating the strength. It is concluded from results that the prediction method proposed herein is persuasive enough to express the actual phenomena of corrosion and can predict the compressive strength of circular steel tube along with aging.
