Abstract

Introduction: Clinically amyopathic dermatomyositis (CADM) with anti‐melanoma differentiation‐associated gene 5 (MDA5) antibody (Ab) with rapidly progressive interstitial lung disease (RP‐ILD) is often refractory for intensive immunosuppression. In this study, we verified the effectiveness and safety of plasma exchange (PEx) for this lethal disease. Methods: We retrospectively examined the clinical course and adverse effect (AE) of 12 patients with anti‐MDA5 Ab–positive CADM between January 2017 and December 2021 in our hospital. Results: Five out of six patients treated with simple PEx using fresh frozen plasma or 5% albumin survived with or without home oxygen therapy. Multiple PEx (15–20 times) were required to achieve satisfactory improvement as well as remission of CADM. The AEs caused by PEx were resolved using conventional methods. Conclusion: PEx might be a promising option for controlling the disease activity of anti‐MDA5 Ab–positive CADM with severe RP‐ILD and may contribute to better survival. [ABSTRACT FROM AUTHOR]

Abstract

ObjectiveAntimelanoma differentiation–associated gene 5 (anti-MDA5)–positive dermatomyositis with interstitial lung disease (DM-ILD) progresses rapidly and has a poor prognosis. Previously, we reported the efficacy of a combination therapy comprising high-dose glucocorticoids (GCs), calcineurin inhibitors (CNIs), and intravenous cyclophosphamide (IV CYC) in a multicenter clinical trial (UMIN000014344). In the present study, we evaluated the long-term outcomes and effects of induction therapy on the maintenance of remission.MethodsAll participants from our previous trial were followed up for > 5 years. Seventy-three other patients with anti-MDA5–positive DM-ILD from our institute were retrospectively integrated into the previous trial for further analysis. Sixty-eight patients achieved remission and survived for > 6 months. Based on the induction treatment, we classified the patients into 2 groups: (1) group T (n = 56), with triple combination therapy (GCs, CNIs, and IV CYC), and (2) group C (n = 12), with monotherapy/dual therapy. The recurrence-free and drug-withdrawal rates of immunosuppressive agents were compared.ResultsThe overall survival and recurrence-free survival rates at 5 years were 100% for the participants in the previous trial. The 5-year cumulative withdrawal rates for CNIs and GCs were 70% and 53%, respectively. In a comprehensive analysis, the recurrence-free rates in group T were higher than those in group C (90% vs 56%; P< 0.05). The drug-withdrawal rates of CNIs and GCs at 10 years in group T were also higher than those in group C (79% vs 0% and 43% vs 0%, respectively; P< 0.05).ConclusionTriple combination therapy in the induction phase can reduce the risk of recurrence and facilitate drug withdrawal in anti-MDA5–positive DM-ILD.

17. Long-term outcomes of refractory Takayasu arteritis patients treated with biologics including ustekinumab

Abstract

Biologics have been used to treat refractory Takayasu arteritis (TAK), but their efficacy and safety have not been sufficiently evaluated. We extracted clinical information from medical records for TAK patients who were treated with biologics including ustekinumab (UST) at Kyoto University Hospital. We also analysed the patient’s genetic backgrounds. Of 163 cases, 12 (7.4%) were treated with infliximab, tocilizumab, or UST (n = 3). Erythrocyte sedimentation rate (ESR), C-reactive protein levels (CRP), and prednisolone (PSL) dose were significantly decreased 12 months after the initiation of biologics. When compared with the 15 patients who were only treated with immunosuppressants (IS group), the change in ESR from baseline was significantly lower in the biologics group than in the IS group (−2 mm/h, p = .005). The proportion of patients with HLA-B*52 and the risk-type alleles of the SNP were similar in both groups. Among the biologics, TCZ showed the highest continuation rate. UST exhibited marginal effects on reducing ESR, CRP levels, and PSL dose. No adverse events were observed in patients with UST for approximately 3 years. Biological treatments resulted in a reduction in inflammatory markers and PSL dose in refractory TAK patients.

18. Self‐Organization of Remote Reservoirs: Transferring Computation to Spatially Distant Locations.

Abstract

Soft materials generate rich and diverse dynamics that can be used as computational resources based on the framework of physical reservoir computing. Herein, a method that exploits the dynamic coupling between soft structures and a water medium to allow for the transfer of computation to spatially distant locations is proposed. This technique is implemented by introducing the concept of remote reservoirs that can autonomously alter their physical constituents in real time rather than using reservoirs with predefined, fixed physical constituents. These remote reservoirs self‐organize by including many ad hoc physical substrates in the environment, making the framework more flexible for soft robotic applications. Using a simple experimental platform consisting of silicone rubber strips containing embedded flexible strain sensors, it is demonstrated that the dynamics of passive silicone rubber strips located at a distance from the actuation point can be successfully exploited for computation through generalized synchronization realized via the medium of water. Future application scenarios are illustrated, in which the proposed technique is applied in emergency situations as a communication tool for transferring a message to a location that humans cannot easily access. Abstract

A previous study revealed the association between susceptibility to Takayasu arteritis (TAK) and a single nucleotide polymorphism (SNP) rs6871626 located in IL12B, which encodes interleukin (IL)-12p40, a common component of IL-12p70 and IL-23. We investigated the expression of these cytokines in patients with TAK, stratifying them into those with or without the risk allele at the rs6871626 SNP. Plasma levels of IL-12p40, IL-12p70, and IL-23 were quantified in 44 patients with TAK and 19 healthy controls (HCs) by enzyme-linked immunosorbent assays. Monocytes were obtained from 20 patients with TAK and 14 HCs, treated with interferon-γ (IFN-γ) and lipopolysaccharide, and then supernatant cytokines were quantified. In addition, the ratio of IFN-γ+or IL-17A+cells to CD4+T cells was measured by flow cytometric analysis of peripheral blood mononuclear cells. The levels of plasma IL-12p40, plasma IL-12p70, and supernatant IL-12p70 were significantly higher in patients with TAK than in HCs, whereas there were no significant differences in the levels of plasma IL-23, supernatant IL-23, or supernatant IL-12p40. The levels of plasma IL-12p70, supernatant IL-12p40, and supernatant IL-12p70 were significantly higher in patients with the risk allele than in those without. The ratio of CD4+IFN-γ+cells was significantly higher in patients with the risk allele, whereas CD4+IL-17A+cells showed no differences. The rs6871626 SNP in IL12Bmay influence the increased expression of IL-12p40 and IL-12p70. These enhanced cytokines might play roles in the pathophysiology of TAK.

Abstract

Polypyrrole (PPy) and polyaniline (PAn) were investigated for cathode materials of rechargeable lithium batteries. PPy films prepared with PF6(-) anion and/or Platinum substrate precoated with nitrile butadiene rubber(NBR) were excellent cathode materials because of rough and/or highly oriented film structure. PAn films were successfully prepared from non-aqueous propylene carbonate solution containing aniline, CF3COOH and Lithium perchlorate. Its acidity strongly affects the anion doping-undoping behavior. The PAn cathode prepared in high acidic solution (e.g. 4:1 ratio of acid:aniline) gives the excellent battery performance.

Abstract

Polyaniline films electro-deposited from acidified non-aqueous media were investigated for their electropolymerization behavior and charge-discharge characteristics. The solution system used for the electropolymerization of polyaniline films was CF3COOH acidified propylene carbonate, which was previously found to be the best system for the preparation of highly electroactive conducting polymer film. Electroactive polyaniline films only form in solutions wherein the mole ratio of acid (CF3COOH) to aniline monomer is greater than one. As the acidity is increased, the resultant polyaniline film exhibits greater electroactivity and better charge-discharge performance. Keywords: Polyaniline; Redox polymer; Nonaqueous electrolytes. Abstract

Objectives: We aimed to clarify the clinical implication of ultrasound (US)-detected foot joint inflammation in tightly controlled patients with rheumatoid arthritis (RA)., Methods: We evaluated bilateral foot joints (second to fifth metatarsophalangeal joints of forefoot; tarsometatarsal, cuneonavicular and midtarsal joints of midfoot) of 430 RA patients for synovitis using Power Doppler (PD) imaging by US. We made a cross-sectional and a 3-year longitudinal analysis about the associations of US-detected synovitis with clinical, laboratory and radiographic data as well as foot-specific outcomes using a self-administered foot evaluation questionnaire (SAFE-Q)., Results: The US-detected foot synovitis was seen in 28% of patients. The US-detected synovitis was closely related to 28 joint-disease activity score (DAS28) more in the forefoot than in the midfoot, while related to joint destruction in both. Multiple regression analyses showed significant associations between midfoot PD positivity and SAFE-Q in the remission group. SAFE-Q was worsened after the 3-year interval, but PD positivity at baseline did not contribute to the changes. Abstract

Objectives: Biologics have been used to treat refractory Takayasu arteritis (TAK), but their efficacy and safety have not been sufficiently evaluated., Methods: We extracted clinical information from medical records for TAK patients who were treated with biologics including ustekinumab (UST) at Kyoto University Hospital. We also analysed the patient's genetic backgrounds., Results: Of 163 cases, 12 (7.4%) were treated with infliximab, tocilizumab, or UST (n = 3). Erythrocyte sedimentation rate (ESR), C-reactive protein levels (CRP), and prednisolone (PSL) dose were significantly decreased 12 months after the initiation of biologics. When compared with the 15 patients who were only treated with immunosuppressants (IS group), the change in ESR from baseline was significantly lower in the biologics group than in the IS group (-2 mm/h, p  = .005). The proportion of patients with HLA-B*52 and the risk-type alleles of the SNP were similar in both groups. Among the biologics, TCZ showed the highest continuation rate. UST exhibited marginal effects on reducing ESR, CRP levels, and PSL dose. No adverse events were observed in patients with UST for approximately 3 years., Conclusions: Biological treatments resulted in a reduction in inflammatory markers and PSL dose in refractory TAK patients.

Abstract

Objective: Although recent clinical trials showed that ultrasound (US) remission is not required to achieve good outcomes at the group level, it currently remains unclear whether the prognosis of individual patients in clinical remission, but not US remission, i.e. those with subclinical sonographic synovitis (SSS), is favorable. However, it is no longer acceptable to perform US on all patients in order to identify those with SSS. Therefore, the present study was initiated to elucidate the conditions under which SSS is frequently detected., Methods: In total, 563 consecutive RA patients were recruited. Bilateral 2-5 MCP, wrist, ankle, and 2-5 MTP joints were scanned by US, and Gray scale and Power Doppler (PD) images were scored semi-quantitatively. Clinical data were obtained by physicians who were blind to US results. Changes in the modified Total Sharp Score (mTSS) of tocilizumab (TCZ) users were calculated., Results: A total of 402 patients were included. SSS was more frequently detected in patients with more severe joint deformity, even if they were in remission. In contrast, a high Patient Global Assessment of Disease (PtGA) did not reflect SSS. Furthermore, the relationship between PtGA and PD scores was weak. Although the frequency of SSS was high in TCZ user, the presence of SSS in TCZ users not always results in the progression of mTSS., Conclusions: While remission is overestimated in patients with severe joint deformity, underestimations may occur in those who do not fulfill remission criteria because of a high PtGA.
