Worldwide, patients on chronic peritoneal dialysis constititute approximately 15% of the total number of patients on dialysis. In Iran, very few people have access to chronic peritoneal dialysis, which is targeted by the Ministry of Health. This lack of access is primarily due to the high occurrence of mechanical complications. Improving catheterization procedures is an important way to extend the use of peritoneal dialysis in Iran. Thus, a prospective study was implemented to evaluate the outcome of a new laparoscopic technique for the insertion of peritoneal dialysis catheters under local anesthesia."n"n Methods: A total of 115 catheters (two-cuff, swan-necked, coiled) were inserted into the peritoneal cavity of 109 patients with end-stage chronic renal failure during a 16-month period. The method of insertion was a two-port laparoscopic technique with local anesthesia and sedation. All patients were followed for 12 to 28 months. We prospectively evaluated mechanical and infectious complications and survival rates of the catheters."n"n Results: The average age of the patients was 51.5 years (range: 15-84 years); 54.8% of these patients were female. The overall one-year and two-year catheter survival rates using this approach were 88% and 73%, respectively. Event-free catheter survival was 35%. The most common infectious and mechanical complications were peritonitis in 52 cases (45.2%) and temporary dialysate leakage in 10 cases (8.7%) respectively."n"n Conclusion: Laparoscopic insertion of peritoneal dialysis catheter with local anesthesia is a safe and simple procedure, giving reasonable rates of catheter survival and complications. Abstract

The relation between microchimerism and allograft tolerance is still a mystery. In this study we determined the presence of peripheral blood microchimerism (PBMC) in female renal transplant recipients from living male donors with second round polymerase chain reaction (PCR). Second round PCR was used to find Y chromocome products. The degree of PBMC in renal transplant recipinents must be below the rate of 1.104 and second round PCR provides the deterction of PBMC at the rate of 1.106. we divided our patients into two groups according to allograft function. Group 1 (16 patients) and normal allograft function. Group 2 (6 patients) had chronic allograft dysfunction. First PCR didn’t show PBMC. Second round PCR with SRY primers of Y chromosome showed PBMC in 13.22 (59%) of patients. PMBC was positive in 10.016 (62%) of patients in-group 1 and 3.6 (50%) of patients in group 2. There was acute rejection in 4.13 (30.7%) and 2.9 (22.2%) of patients with positive and negative PBMC, respectively, in our study, there was no significant correlation between the presence of PBMC and allograft function and the frequency or severity of rejection episodes.

Abstract

To delineate the incidence , epidemiology, etiology, and prognosis of acute renal failure (ARF) and to compaed the findings with international data, we prospectively studied 2360 patients, admitted at shariati hospital during a four months period ending on Jan, 6, 1996. One hundred and twenty-four (5.1%) patients meeting ARF crieteria were encountered. The most common cause of ARF was prerenal azotemia, nephrotoxins were the second common cause, and primary renal disease were the third.In-hospital mortality was 5.5% and 31% of this mortality was related to ARF. Poor prognostic factors were oliguria, maximum serum ceratinine (Cr) level, rise of Cr during admission, multiorgan failure,and sepsis.

7. A model for predicting peritoneal dialysis patients’ survival, using data mining algorithms

Abstract

Background: Peritoneal dialysis is one of the most commonly used treatment methods for the patients with end stage renal failure. In recent years, the mortality rate of patients under this treatment has decreased; however, long-term survival is still an important challenge for health systems. The present study aimed to predict the survival of continuous ambulatory peritoneal dialysis patients. Methods: In this retrospective study, according to the difference of relative importance of demographic characteristics, laboratory data, dialysis adequacy parameters and nutritional status in various patients, the factors affecting the survival of peritoneal dialysis patients have been identified by random forest algorithm. Then, the clinical and laboratory data of patients undergoing continuous ambulatory peritoneal dialysis treatment were evaluated retrospectively from July 1996 to April 2014 in 18 peritoneal dialysis centers, using multi-class one against all support vector machine (OAA-SVM) and multi-space mapped binary tree support vector machine (MBT-SVM) algorithms. Results: 3097 patients were studied with the mean age of 50.63±15.67 years and average follow-up time of 24.48±19.13 months. The results of the random forest algorithm have identified 35 factors as the most important predictors of peritoneal dialysis patient’s survival. Then, the prediction of peritoneal dialysis patients’ survival status was evaluated using one against all support vector machine and multi-space mapped binary tree support vector machine algorithms in 5 classes of patients including “still on peritoneal dialysis”, “transferred to hemodialysis”, “received a kidney transplant”, “died” and “improved kidney function”. The reliability of survival prediction algorithms were 51.99% and 89.57% respectively. Conclusion: An accurate prediction model would be a potentially useful way to evaluate patients’ survival at peritoneal dialysis that increased clinical scrutiny and timely intervention could be brought to bear. So, in this research, the multi-space mapped binary tree support vector machine algorithm has a high precision in predicting the survival of continuous ambulatory peritoneal dialysis patients considering multiple evaluation indices and different class distribution functions.

Abstract

Background & objectives: In the management of crush syndrome, the way of treating compartment syndrome is controversial. The aim of this study was to evaluate the outcomes of compartment syndrome treatment after Bam earthquake. Methods: Clinical and laboratory findings of all fasciotomized patients in Bam earthquake were analyzed. Then nonfasciotomized patients with same characteristics were selected and the outcomes of two groups were compared. Results: Fasciotomy for 107 injured out of 2962 hospitalized patients, was performed (46% female). Mean age of fasciotomized patients was 27.4±9.7 years (min=10, max=65 year). There were not significant differences in the final outcome of two groups with the same clinical, biochemical and demographic characteristics. 39. Distribution of albuminuria and low glomerular filtration rate in a rural area, shahreza, iran.

Abstract

Introduction. Chronic kidney disease (CKD) is becoming a major public health problem worldwide. A remarkable part of health budget is designated annually to control end-stage renal disease in most countries. The aim of this study was to screen for CKD among the general population of the rural area of Shahreza, in the central region of Iran. Materials and Methods. In a study of rural area around Shahreza, Iran, in 2009, a total of 1400 participants aged over 30 years old were selected by systematic randomized sampling. Glomerular filtration rate (GFR) was used as an index of kidney function and albuminuria, as an index of kidney damage. The simplified Modification of Diet in Renal Disease Study equation was used for estimation of GFR. Results. A GFR less than 60 mL/min/m2 was found in 4.7% of the study population (1.8% in men and 6.1% in women). Microalbuminuria and macroalbuminuria were present in 16.2% of the participants (15% of men and 16.8% of women). Pyuria and hematuria rates were 12.3% and 12.6%. The prevalence of a GFR less than 60 mL/min/1.73 m2 was significantly increasing by age groups in both genders. Conclusions. Considering its high prevalence, CKD needs measures to identify the disease sooner and requires an active national screening program to identify patients in earlier stages. 40. Epidemiology of culture-negative peritonitis in Iranian patients on continuous ambulatory peritoneal dialysis.

Abstract

Introduction. Culture-negative peritonitis is a major challenge in the treatment of peritonitis in continuous ambulatory peritoneal dialysis (CAPD). This study aimed to evaluate the culture-negative peritonitis in patients from the Iranian CAPD Registry. Materials and Methods. Data of 1472 patients from 26 CAPD centers were analysed. Peritonitis was defined as any clinical suspicion together with peritoneal leukocyte count of 100/mL and more. Results. The patients had been on PD for a mean of 500 ± 402 days. There were a total of 660 episodes of peritonitis observed among 299 patients (peritonitis rate of 1 episode in 34.1 patient-months). Excluding patients with both negative and positive culture results, there were 391 episodes of peritonitis in 220 patients (174 culture-positive episodes in 97 patients and 217 culture-negative episodes in 123). The 1- to 4-year patient survival rates were 85%, 75%, 69%, and 59% for the patients with culture-positive peritonitis, and 92%, 78%, 73% and 63% for the patients with culture-negative peritonitis, respectively (P = .34). The technique survival rates were 90%, 57%, 42%, and 27% and 95%, 85%, 74%, and 40%, respectively (P = .001). On follow-up, there were higher rates of active PD patients, lower rates of PD dropouts, and higher rates of kidney transplantation in patients with culture-negative peritonitis compared to those with culture-positive peritonitis. Conclusions. In our patients, the prevalence of culture-negative peritonitis was high (55.9%). Patient survival with culture-negative peritonitis was comparable to those with culture-positive peritonitis and technique survival was higher among those with culture-negative peritonitis. Abstract

INTRODUCTION. Fasciotomy may increase the morbidity and mortality in patients with crush-induced acute kidney injury (AKI), by creating an open wound, increasing the risk of bleeding, coagulopathy, and potentially fatal sepsis. This study evaluates the outcomes of fasciotomy in these patients after Bam earthquake in Iran. MATERIALS AND METHODS. We reviewed medical records of victims of Bam earthquake complicated with crush-induced AKI. Demographic, biochemical, and clinical data of patients who underwent fasciotomy were evaluated and compared with other patients with AKI. RESULTS. Fasciotomy was performed for 70 of 200 patients with crush-induced AKI (35.0%). There were no significant differences regarding sex, age, time under the rubble, and muscle enzymes level between these patients and those without fasciotomy. They did not experience higher rates of disseminated intravascular coagulopathy, sepsis, adult respiratory distress syndrome, amputation, and dialysis session. Neither did they have a longer hospitalization period or higher death rate. CONCLUSIONS. This study showed that fasciotomy did not have any deteriorating effect on morbidity and mortality of patients with crush-induced AKI after Bam earthquake. [ABSTRACT FROM AUTHOR]

42. Frequency, risk factors, and outcome of acute kidney injury following bone marrow transplantation at Dr Shariati Hospital in Tehran.

Abstract

INTRODUCTION. Bone marrow transplantation (BMT) is a major modality for malignant and hematologic disorders. This procedure is associated with a high morbidity and mortality such as acute kidney injury (AKI). Many factors, such as therapeutic agents, irradiation, and graft versus host disease (GVHD) can cause AKI. Bone marrow transplantation conditioning therapy in Iran is based on drugs such as busulfan and cyclophosphamide and without irradiation therapy. The aim of this study was to evaluate the frequency, risk factors, and mortality of AKI among patients who underwent BMT. MATERIALS AND METHODS. Acute kidney injury was defined as doubling serum creatinine from baseline at any time during the first 180 days posttransplant. The risk of AKI in relation to non-total-body-irradiation-based conditioning regimen, type of graft (allograft and autograft), comorbidities, GVHD, drug toxicity, and veno-occlusive disease were examined in 375 patients with BMT. RESULTS. One hundred and forty-two patients (37.6%) developed AKI at a median of 18 days after transplant. A higher frequency of AKI was observed in patients who received cyclosporine A (40%), patients with allograft BMT (42.1%), and those who developed gastrointestinal GVHD (47.3%) .The remainder AKI cases were associated with amphotericin B, veno-occlusive disease, and hemolytic-uremic syndrome. CONCLUSIONS. The frequency of AKI in our patients with BMT remained high. Cyclosporine A and amphotericin B and the presence of GVHD and veno-occlusive disease increased the risk of AKI within the first 180 days after BMT. [ABSTRACT FROM AUTHOR]
