Abstract

BackgroundThe impact of the 2017 American College of Cardiology (ACC)/American Heart Association (AHA) guidelines for diagnosis and management of hypertension on the prevalence of hypertension in India is unknown.MethodsWe analyzed data from the Cardiac Prevent 2015 survey to estimate the change in the prevalence of hypertension. The JNC8 guidelines defined hypertension as a systolic blood pressure of ≥140 ​mmHg or diastolic blood pressure of ≥90 ​mmHg. The 2017 ACC/AHA guidelines define hypertension as a systolic blood pressure of ≥130 ​mmHg or diastolic blood pressure of ≥80 ​mmHg. We standardized the prevalence as per the 2011 census population of India. We also calculated the prevalence as per the World Health Organization (WHO) World Standard Population (2000-2025).ResultsAmong 180,335 participants (33.2% women), the mean age was 40.6 ​± ​14.9 years (41.1 ​± ​15.0 and 39.7 ​± ​14.7 years in men and women, respectively). Among them, 8,898 (4.9%), 99,791 (55.3%), 35,694 (11.9%), 23,084 (12.8%), 9,989 (5.5%) and 2,878 (1.6%) participants belonged to age group 18-19, 20-44, 45-54, 55-64, 65-74 and ​≥ ​75 years respectively. The prevalence of hypertension according to the JNC8 and 2017 ACC/AHA guidelines was 29.7% and 63.8%, respectively- an increase of 115%. With the 2011 census population of India, this suggests that currently, 486 million Indian adults have hypertension according to the 2017 ACC/AHA guidelines, an addition of 260 million as compared to the JNC8 guidelines.ConclusionAccording to the 2017 ACC/AHA guidelines, 3 in every 5 Indian adults have hypertension.

6. Comparative evaluation of different brands of sacubitril/valsartan for the presence of co-crystallization.

Abstract

Background: Sacubitril/valsartan, marketed as Azmarda (manufactured by the innovator company) is a cocrystal consisting 6 sacubitril and valsartan molecules, along with sodium cations and water molecules. This formulation is considered a major mechanism of benefit of this molecule. This study was aimed to assess various brands of sacubitril/valsartan in the market for the presence of the cocrystal forms and compare them with Azmarda. a brand of sacubitril/valsartan manufactured by the innovator company. Methods: The study involved analysis of various marketed products containing sacubitril/valsartan tablets, including Azmarda. Both the Azmarda and marketed products were characterized using differential scanning calorimetry (DSC) and powder X-ray diffraction (PXRD). Sodium content and dissolution studies were also performed . Results: A total of 16 brands of marketed products including Azmarda were studied. During DSC studies, unique melting pattern was observed in Azmarda, with exothermic peaks of sacubitril/valsartan detected at 140°C and 102°C, respectively. PXRD analysis revealed that none of the products exhibited the same crystal lattice as Azmarda. Azmarda, with a sodium level of 1.773%, was comparable to brand 2 (1.61%), brand 4 (1.73%), and brand 7 (1.58%). Azmarda demonstrated 64% release of sacubitril and 57% release of valsartan within 30 minutes in 0.1N HCl. Brand 7, brand 12, and brand 13 showed 52%, 57%, and 52% release of sacubitril, respectively, and 45%, 52%, and 48% release of valsartan, respectively, in 30 minutes. Conclusion: The absence of identical crystal lattice structures, highlights absence of cocrystalsin generic formulations. Such variations may impact the bioavailability and efficacy, emphasizing the importance of ensuring consistent and reliable therapeutic outcomes. [ABSTRACT FROM AUTHOR]

Abstract

There is ongoing debate around rate versus rhythm control strategies for managing atrial fibrillation (AF), however, much of the data comes from Western cohorts. Kerala‐AF represents the largest prospective AF cohort study from the Indian subcontinent. To compare 12‐month outcomes between rate and rhythm control strategies. Patients aged ≥18 years with non‐transient AF were recruited from 53 hospitals across Kerala. Patients were stratified by rate or rhythm control. The primary outcome was a composite of all‐cause mortality, arterial thromboembolism, acute coronary syndrome or hospitalization due to heart failure or arrhythmia at 12 months. Secondary outcomes included bleeding events and individual components of the primary. Predictors of the composite outcome were analysed by logistic regression. A total of 2901 patients (mean age 64.6 years, 51% female) were included (2464 rate control, 437 rhythm control). Rates of the primary composite outcome did not differ between groups (29.7% vs 30.0%; p= .955), nor did any component of the primary. Bleeding outcomes were also similar (1.6% vs 1.9%; p= .848). Independent predictors of the primary composite outcome were older age (aOR 1.01; p= .013), BMI <18 (aOR 1.51; p= .025), permanent AF (aOR 0.78; p= .010), HFpEF (aOR 1.40; p= .023), HFrEF (aOR 1.39; p= .004), chronic kidney disease (aOR 1.36; p< .001), and prior thromboembolism (aOR 1.31; p= .014). In the Kerala‐AF registry, 12‐month outcomes did not differ between rate and rhythm control cohorts. No significant difference was found between rate and rhythm control approaches to atrial fibrillation management in the 1‐year follow‐up of the Kerala‐AF cohort. Independent predictors of adverse outcomes included age, underweight BMI, heart failure, and chronic renal failure.

Abstract

Objective: The present systematic review assessed the efficacy of peri-procedurally administered trimetazidine in the prevention of contrast-induced nephropathy (CIN) in patients undergoing coronary interventions with contrast agents. Methods: We performed a systematic literature review of articles published in PubMed and Google Scholar by 7 December 2023 and included articles from the last 15 years that evaluated the efficacy of trimetazidine in preventing CIN in cardiac patients undergoing coronary intervention. Results: After title/abstract and full-text screening, this systematic review included 9 randomized controlled trials (N = 2158 patients) with two groups: Trimetazidine (60–70 mg/day 24 to 48 h before and up to 72 h after the procedure) with hydration and the control group with only hydration. A total of 234/2158 patients developed CIN (Incidence rate [IR], 10.8%) as per the CIN definition of the Contrast Media Safety Committee of the European Society of Urogenital Radiology. The incidence of CIN in the trimetazidine vs. control group was 6.4% (69/1083) vs. 15.4% (165/1075), and the odds ratio (95% CI) was 0.3753 (0.279–0.504). Conclusions: In conclusion, the trimetazidine group had a lower incidence of CIN. Trimetazidine offers a reno-protective effect and helps in reducing the CIN incidence in patients undergoing cardiac intervention. Peri-procedure administration of trimetazidine significantly decreases the risk of CIN in patients despite comorbidities. [ABSTRACT FROM AUTHOR]

Abstract

This Delphi method of consensus was designed to develop scientific statements for β-blockers in the continuum of cardiovascular diseases with a special focus on the role of bisoprolol. Eleven experienced cardiologists from across the Asia-Pacific countries participated in two rounds of the survey. In the first round, experts were asked to rate agreement/disagreement with 35 statements across seven domains regarding the use of β-blockers for treating hypertension, heart failure, coronary artery diseases, co-morbidities, as well as their safety profile, usage pattern, and pharmacokinetic variability. A consensus for a statement could be reached with >70% agreement. Except for seven statements, all attained consensus in the first round. In the second round that was conducted virtually, the experts re-appraised their ratings for the seven statements along with a critical appraisal of two additional statements that were suggested by experts in the preceding round. At the end of the second round, the final version included 36 statements (34 original statements, two statements suggested by experts, and the omission of one statement that did not attain consensus). The final version of statements in the second round was disseminated among experts for their approval followed by manuscript development. Attainment of consensus for almost all statements reconfirms the clinical benefits of β-blockers, particularly β1-selective blockers for the entire spectrum of cardiovascular diseases. Abstract

Objective: The objective of this study was to assess the effect of indapamide sustained release (SR) 1.5 mg in the treatment of hypertensive patients, untreated or uncontrolled on monotherapy with different antihypertensive agents, on blood pressure (BP) reduction and well-being. Patients and Methods: In a prospective multicenter study from 32 cities across India, 1545 patients between 40 and 70 years of age with untreated or uncontrolled hypertension (BP >140/90 mmHg) received indapamide SR 1.5 mg once daily as monotherapy, or in addition to the existing treatment for 90 days. The Nottingham general health questionnaire was used to assess the changes in well-being. Results: In 1545 hypertensive patients, with a mean (standard deviation) age of 56.86 years, 64.1% being men. Of those recruited, 29% were treatment naïve and 71% were receiving different antihypertensive medications. In terms of a sense of well-being, a lack of energy was reported in 56.6%, emotional problems in 61.6%, and disturbed sleep in 58.1% of the patients. On an intention to treat basis, 842 patients (54.5%; 95% confidence interval, 52.0–57.0) achieved BP control. The patients who achieved BP control (compared to those who did not) felt more energetic by 6.3% (8.2–4.3, P< 0.0001), emotionally better by 5% (6.3–3.6, P< 0.0001), and had improved sleep by 5.6% (7.4–4.1, P< 0.001). Conclusion: In hypertensive patients, untreated or uncontrolled on monotherapy with any of the major drug classes, the addition of indapamide SR 1.5 mg is effective in reducing BP and in improving their well-being.
