2. Predicting severe chronic obstructive pulmonary disease exacerbations using quantitative CT: a retrospective model development and external validation study.

BACKGROUND: Quantitative CT is becoming increasingly common for the characterisation of lung disease; however, its added potential as a clinical tool for predicting severe exacerbations remains understudied. We aimed to develop and validate quantitative CT-based models for predicting severe chronic obstructive pulmonary disease (COPD) exacerbations. METHODS: We analysed the Subpopulations and Intermediate Outcome Measures In COPD Study (SPIROMICS) cohort, a multicentre study done at 12 clinical sites across the USA, of individuals aged 40-80 years from four strata: individuals who never smoked, individuals who smoked but had normal spirometry, individuals who smoked and had mild to moderate COPD, and individuals who smoked and had severe COPD. We used 3-year follow-up data to develop logistic regression classifiers for predicting severe exacerbations. Predictors included age, sex, race, BMI, pulmonary function, exacerbation history, smoking status, respiratory quality of life, and CT-based measures of density gradient texture and airway structure. We externally validated our models in a subset from the Genetic Epidemiology of COPD (COPDGene) cohort. Discriminative model performance was assessed using the area under the receiver operating characteristic curve (AUC), which was also compared with other predictors, including exacerbation history and the BMI, airflow obstruction, dyspnoea, and exercise capacity (BODE) index. We evaluated model calibration using calibration plots and Brier scores. FINDINGS: Participants in SPIROMICS were enrolled between Nov 12, 2010, and July 31, 2015. Participants in COPDGene were enrolled between Jan 10, 2008, and April 15, 2011. We included 1956 participants from the SPIROMICS cohort who had complete 3-year follow-up data: the mean age of the cohort was 63·1 years (SD 9·2) and 1017 (52%) were men and 939 (48%) were women. Among the 1956 participants, 434 (22%) had a history of at least one severe exacerbation. For the CT-based models, the AUC was 0·854 (95% CI 0·852-0·855) for at least one severe exacerbation within 3 years and 0·931 (0·930-0·933) for consistent exacerbations (defined as ≥1 acute episode in each of the 3 years). Models were well calibrated with low Brier scores (0·121 for at least one severe exacerbation; 0·039 for consistent exacerbations). For the prediction of at least one severe event during 3-year follow-up, AUCs were significantly higher with CT biomarkers (0·854 [0·852-0·855]) than exacerbation history (0·823 [0·822-0·825]) and BODE index 0·812 [0·811-0·814]). 6965 participants were included in the external validation cohort, with a mean age of 60·5 years (SD 8·9). In this cohort, AUC for at least one severe exacerbation was 0·768 (0·767-0·769; Brier score 0·088). INTERPRETATION: CT-based prediction models can be used for identification of patients with COPD who are at high risk of severe exacerbations. The newly identified CT biomarkers could potentially enable investigation into underlying disease mechanisms responsible for exacerbations. Arterial claudication is a common manifestation of peripheral artery disease. This document focuses on necessary imaging before revascularization for claudication. Appropriate use of ultrasound, invasive arteriography, MR angiography, and CT angiography are discussed. The ACR Appropriateness Criteria are evidence-based guidelines for specific clinical conditions that are reviewed annually by a multidisciplinary expert panel. The guideline development and revision process support the systematic analysis of the medical literature from peer reviewed journals. Established methodology principles such as Grading of Recommendations Assessment, Development, and Evaluation or GRADE are adapted to evaluate the evidence. The RAND/UCLA Appropriateness Method User Manual provides the methodology to determine the appropriateness of imaging and treatment procedures for specific clinical scenarios. In those instances where peer reviewed literature is lacking or equivocal, experts may be the primary evidentiary source available to formulate a recommendation.

Free-breathing cardiac MRI schemes are emerging as competitive alternatives to breath-held cine MRI protocols, enabling applicability to pediatric and other population groups that cannot hold their breath. Because the data from the slices are acquired sequentially, the cardiac/respiratory motion patterns may be different for each slice; current free-breathing approaches perform independent recovery of each slice. In addition to not being able to exploit the inter-slice redundancies, manual intervention or sophisticated post-processing methods are needed to align the images post-recovery for quantification. To overcome these challenges, we propose an unsupervised variational deep manifold learning scheme for the joint alignment and reconstruction of multislice dynamic MRI. The proposed scheme jointly learns the parameters of the deep network as well as the latent vectors for each slice, which capture the motion-induced dynamic variations, from the k-t space data of the specific subject. The variational framework minimizes the non-uniqueness in the representation, thus offering improved alignment and reconstructions.

Bilinear models that decompose dynamic data to spatial and temporal factors are powerful and memory-efficient tools for the recovery of dynamic MRI data. These methods rely on sparsity and energy compaction priors on the factors to regularize the recovery. The quality of the recovered images depend on the specific priors. Motivated by deep image prior, we introduce a novel bilinear model whose factors are represented using convolutional neural networks (CNNs). The CNN parameters are learned from the undersampled data off the same subject. To reduce the run time and to improve performance, we initialize the CNN parameters. We use sparsity regularization of the network parameters to minimize the overfitting of the network to measurement noise. Our experiments on free breathing and ungated cardiac cine data acquired using a navigated golden-angle gradient-echo radial sequence show the ability of our method to provide reduced spatial blurring as compared to low-rank and SToRM reconstructions.

We introduce a generative smoothness regularization on manifolds (SToRM) model for the recovery of dynamic image data from highly undersampled measurements. The model assumes that the images in the dataset are non-linear mappings of low-dimensional latent vectors. We use the deep convolutional neural network (CNN) to represent the non-linear transformation. The parameters of the generator as well as the low-dimensional latent vectors are jointly estimated only from the undersampled measurements. This approach is different from traditional CNN approaches that require extensive fully sampled training data. We penalize the norm of the gradients of the non-linear mapping to constrain the manifold to be smooth, while temporal gradients of the latent vectors are penalized to obtain a smoothly varying time-series. The proposed scheme brings in the spatial regularization provided by the convolutional network. The main benefit of the proposed scheme is the improvement in image quality and the orders-of-magnitude reduction in memory demand compared to traditional manifold models. To minimize the computational complexity of the algorithm, we introduce an efficient progressive training-in-time approach and an approximate cost function. These approaches speed up the image reconstructions and offers better reconstruction performance.

Unlike the popular CNN approaches that require extensive fully-sampled training data that is not available in this setting, the parameters of the CNN generator as well as the latent vectors are jointly estimated from the undersampled measurements using stochastic gradient descent. We penalize the norm of the gradient of the generator to encourage the learning of a smooth surface/manifold, while temporal gradients of the latent vectors are penalized to encourage the time series to be smooth. The main benefits of the proposed scheme are (a) the quite significant reduction in memory demand compared to the analysis based SToRM model, and (b) the spatial regularization brought in by the CNN model. We also introduce efficient progressive approaches to minimize the computational complexity of the algorithm.

11. Variational manifold learning from incomplete data: application to multislice dynamic MRI

Current deep learning-based manifold learning algorithms such as the variational autoencoder (VAE) require fully sampled data to learn the probability density of real-world datasets. Once learned, the density can be used for a variety of tasks, including data imputation. However, fully sampled data is often unavailable in a variety of problems, including the recovery of dynamic and high-resolution MRI data considered in this work. To overcome this problem, we introduce a novel variational approach to learn a manifold from undersampled data. The VAE uses a decoder fed by latent vectors, drawn from a conditional density estimated from the fully sampled images using an encoder. Since fully sampled images are not available in our setting, we approximate the conditional density of the latent vectors by a parametric model whose parameters are estimated from the undersampled measurements using back-propagation. We use the framework for the joint alignment and recovery of multislice free breathing and ungated cardiac MRI data from highly undersampled measurements. Most of the current self-gating and manifold cardiac MRI approaches consider the independent recovery of images from each slice; these methods are not capable of exploiting the inter-slice redundancies in the datasets and require sophisticated post-processing or manual approaches to align the images from different slices. By contrast, the proposed scheme is able to align the multislice data and exploit the redundancies. Experimental results demonstrate the utility of the proposed scheme in dynamic imaging alignment and reconstructions.

We propose a deep self-learning algorithm to learn the manifold structure of free-breathing and ungated cardiac data and to recover the cardiac CINE MRI from highly undersampled measurements. Our method learns the manifold structure in the dynamic data from navigators using autoencoder network. The trained autoencoder is then used as a prior in the image reconstruction framework. We have tested the proposed method on free-breathing and ungated cardiac CINE data, which is acquired using a navigated golden-angle gradient-echo radial sequence. Results show the ability of our method to better capture the manifold structure, thus providing us reduced spatial and temporal blurring as compared to the SToRM reconstruction.

We introduce a kernel low-rank algorithm to recover free-breathing and ungated dynamic MRI from spiral acquisitions without explicit k-space navigators. It is often challenging for low-rank methods to recover free-breathing and ungated images from undersampled measurements; extensive cardiac and respiratory motion often results in the Casorati matrix not being sufficiently low-rank. Therefore, we exploit the non-linear structure of the dynamic data, which gives the low-rank kernel matrix. Unlike prior work that rely on navigators to estimate the manifold structure, we propose a kernel low-rank matrix completion method to directly fill in the missing k-space data from variable density spiral acquisitions. We validate the proposed scheme using simulated data and in-vivo data. Our results show that the proposed scheme provides improved reconstructions compared to the classical methods such as low-rank and XD-GRASP. - Abstract
This document summarizes the relevant literature for the selection of the initial imaging in five clinical scenarios in patients with suspected or known nonvariceal upper gastrointestinal bleeding (UGIB). These clinical scenarios include suspected nonvariceal UGIB without endoscopy performed; endoscopically confirmed nonvariceal UGIB with clear source but treatment not possible or continued bleeding after endoscopic treatment; endoscopically confirmed nonvariceal UGIB without a confirmed source; suspected nonvariceal UGIB with negative endoscopy; and postsurgical or post-traumatic nonvariceal UGIB when endoscopy is contraindicated. The appropriateness of imaging modalities as they apply to each clinical scenario is rated as usually appropriate, may be appropriate, and usually not appropriate to assist the selection of the most appropriate imaging modality in the corresponding clinical scenarios of nonvariceal UGIB. The American College of Radiology Appropriateness Criteria are evidence-based guidelines for specific clinical conditions that are reviewed annually by a multidisciplinary expert panel. The guideline development and revision process support the systematic analysis of the medical literature from peer reviewed journals. Established methodology principles such as Grading of Recommendations Assessment, Development, and Evaluation or GRADE are adapted to evaluate the evidence. The RAND/UCLA Appropriateness Method User Manual provides the methodology to determine the appropriateness of imaging and treatment procedures for specific clinical scenarios. In those instances where peer reviewed literature is lacking or equivocal, experts may be the primary evidentiary source available to formulate a recommendation. [ABSTRACT FROM AUTHOR]

Background: Cardiac applications in radiation therapy are rapidly expanding including magnetic resonance guided radiation therapy (MRgRT) for real‐time gating for targeting and avoidance near the heart or treating ventricular tachycardia (VT). Purpose: This work describes the development and implementation of a novel multi‐modality and magnetic resonance (MR)‐compatible cardiac phantom. Methods: The patient‐informed 3D model was derived from manual contouring of a contrast‐enhanced Coronary Computed Tomography Angiography scan, exported as a Stereolithography model, then post‐processed to simulate female heart with an average volume. The model was 3D‐printed using Elastic50A to provide MR contrast to water background. Two rigid acrylic modules containing cardiac structures were designed and assembled, retrofitting to an MR‐safe programmable motor to supply cardiac and respiratory motion in superior‐inferior directions. One module contained a cavity for an ion chamber (IC), and the other was equipped with multiple interchangeable cavities for plastic scintillation detectors (PSDs). Images were acquired on a 0.35 T MR‐linac for validation of phantom geometry, motion, and simulated online treatment planning and delivery. Three motion profiles were prescribed: patient‐derived cardiac (sine waveform, 4.3 mm peak‐to‐peak, 60 beats/min), respiratory (cos4 waveform, 30 mm peak‐to‐peak, 12 breaths/min), and a superposition of cardiac (sine waveform, 4 mm peak‐to‐peak, 70 beats/min) and respiratory (cos4 waveform, 24 mm peak‐to‐peak, 12 breaths/min). The amplitude of the motion profiles was evaluated from sagittal cine images at eight frames/s with a resolution of 2.4 mm × 2.4 mm. Gated dosimetry experiments were performed using the two module configurations for calculating dose relative to stationary. A CT‐based VT treatment plan was delivered twice under cone‐beam CT guidance and cumulative stationary doses to multi‐point PSDs were evaluated. Results: No artifacts were observed on any images acquired during phantom operation. Phantom excursions measured 49.3 ± 25.8%/66.9 ± 14.0%, 97.0 ± 2.2%/96.4 ± 1.7%, and 90.4 ± 4.8%/89.3 ± 3.5% of prescription for cardiac, respiratory, and cardio‐respiratory motion profiles for the 2‐chamber (PSD) and 12‐substructure (IC) phantom modules respectively. In the gated experiments, the cumulative dose was <2% from expected using the IC module. Real‐time dose measured for the PSDs at 10 Hz acquisition rate demonstrated the ability to detect the dosimetric consequences of cardiac, respiratory, and cardio‐respiratory motion when sampling of different locations during a single delivery, and the stability of our phantom dosimetric results over repeated cycles for the high dose and high gradient regions. For the VT delivery, high dose PSD was <1% from expected (5–6 cGy deviation of 5.9 Gy/fraction) and high gradient/low dose regions had deviations <3.6% (6.3 cGy less than expected 1.73 Gy/fraction). Conclusions: A novel multi‐modality modular heart phantom was designed, constructed, and used for gated radiotherapy experiments on a 0.35 T MR‐linac. Our phantom was capable of mimicking cardiac, cardio‐respiratory, and respiratory motion while performing dosimetric evaluations of gated procedures using IC and PSD configurations. Time‐resolved PSDs with small sensitive volumes appear promising for low‐amplitude/high‐frequency motion and multi‐point data acquisition for advanced dosimetric capabilities. Illustrating VT planning and delivery further expands our phantom to address the unmet needs of cardiac applications in radiotherapy. [ABSTRACT FROM AUTHOR]

Neuroinflammation contributes to impaired cognitive function in brain aging and neurodegenerative disorders like Alzheimer's disease, which is characterized by the aggregation of pathological tau. One major driver of both age- and tau-associated neuroinflammation is the NF-κB and NLRP3 signaling axis. However, current treatments targeting NF-κB or NLRP3 may have adverse/systemic effects, and most have not been clinically translatable. In this study, we tested the efficacy of a novel, nucleic acid therapeutic (Nanoligomer) cocktail specifically targeting both NF-κB and NLRP3 in the brain for reducing neuroinflammation and improving cognitive function in old (aged 19 months) wildtype mice, and in rTg4510 tau pathology mice (aged 2 months). We found that 4 weeks of NF-κB/NLRP3-targeting Nanoligomer treatment strongly reduced neuro-inflammatory cytokine profiles in the brain and improved cognitive-behavioral function in both old and rTg4510 mice. These effects of NF-κB/NLRP3-targeting Nanoligomers were also associated with reduced glial cell activation and pathology, favorable changes in transcriptome signatures of glia-associated inflammation (reduced) and neuronal health (increased), and positive systemic effects. Collectively, our results provide a basis for future translational studies targeting both NF-κB and NLRP3 in the brain, perhaps using Nanoligomers, to inhibit neuroinflammation and improve cognitive function with aging and neurodegeneration. Rationale: Rates of emphysema progression vary in chronic obstructive pulmonary disease (COPD), and the relationships with vascular and airway pathophysiology remain unclear. Objectives: We sought to determine if indices of peripheral (segmental and beyond) pulmonary arterial dilation measured on computed tomography (CT) are associated with a 1-year index of emphysema (EI; percentage of voxels <−950 Hounsfield units) progression. Methods: Five hundred ninety-nine former and never-smokers (Global Initiative for Chronic Obstructive Lung Disease stages 0–3) were evaluated from the SPIROMICS (Subpopulations and Intermediate Outcome Measures in COPD Study) cohort: rapid emphysema progressors (RPs; n = 188, 1-year ΔEI > 1%), nonprogressors (n = 301, 1-year ΔEI ± 0.5%), and never-smokers (n = 110). Segmental pulmonary arterial cross-sectional areas were standardized to associated airway luminal areas (segmental pulmonary artery–to–airway ratio [PAAR
seg ]). Full-inspiratory CT scan–derived total (arteries and veins) pulmonary vascular volume (TPVV) was compared with small vessel volume (radius smaller than 0.75 mm). Ratios of airway to lung volume (an index of dysanapsis and COPD risk) were compared with ratios of TPVV to lung volume. Results: Compared with nonprogressors, RPs exhibited significantly larger PAARseg (0.73 ± 0.29 vs. 0.67 ± 0.23; P = 0.001), lower ratios of TPVV to lung volume (3.21 ± 0.42% vs. 3.48 ± 0.38%; P = 5.0 × 10−12 ), lower ratios of airway to lung volume (0.031 ± 0.003 vs. 0.034 ± 0.004; P = 6.1 × 10−13 ), and larger ratios of small vessel volume to TPVV (37.91 ± 4.26% vs. 35.53 ± 4.89%; P = 1.9 × 10−7 ). In adjusted analyses, an increment of 1 standard deviation in PAARseg was associated with a 98.4% higher rate of severe exacerbations (95% confidence interval, 29–206%; P = 0.002) and 79.3% higher odds of being in the RP group (95% confidence interval, 24–157%; P = 0.001). At 2-year follow-up, the CT-defined RP group demonstrated a significant decline in postbronchodilator percentage predicted forced expiratory volume in 1 second. Conclusions: Rapid one-year progression of emphysema was associated with indices indicative of higher peripheral pulmonary vascular resistance and a possible role played by pulmonary vascular–airway dysanapsis. Pulmonary arteriovenous malformations (PAVMs) occur in 30% to 50% of patients with hereditary hemorrhagic telangiectasia. Clinical presentations vary from asymptomatic disease to complications resulting from the right to left shunting of blood through the PAVM such as paradoxical stroke, brain abscesses, hypoxemia, and cardiac failure. Radiology plays an important role both in the diagnosis and treatment of PAVM. Based on different clinical scenarios, the appropriate imaging study has been reviewed and is presented in this document. The American College of Radiology Appropriateness Criteria are evidence-based guidelines for specific clinical conditions that are reviewed annually by a multidisciplinary expert panel. The guideline development and revision process support the systematic analysis of the medical literature from peer reviewed journals. Established methodology principles such as Grading of Recommendations Assessment, Development, and Evaluation or GRADE are adapted to evaluate the evidence. The RAND/UCLA Appropriateness Method User Manual provides the methodology to determine the appropriateness of imaging and treatment procedures for specific clinical scenarios. Abdominal aortic aneurysm (AAA) is a significant vascular disease found in 4% to 8% of the screening population. If ruptured, its mortality rate is between 75% and 90%, and it accounts for up to 5% of sudden deaths in the United States. Therefore, screening of AAA while asymptomatic has been a crucial portion of preventive health care worldwide. Ultrasound of the abdominal aorta is the primary imaging modality for screening of AAA recommended for asymptomatic adults regardless of their family history or smoking history. Alternatively, duplex ultrasound and CT abdomen and pelvis without contrast may be appropriate for screening. The American College of Radiology Appropriateness Criteria are evidence-based guidelines for specific clinical conditions that are reviewed annually by a multidisciplinary expert panel.
