The aim of the present study was to validate the Omron HBP-1100-E professional blood pressure measuring device in accordance with the American Association for the Advancement of Medical Instrumentation in Iranian adults.Materials and Methods: Simultaneous blood pressure auscultator measurements were obtained by two observers using mercury sphygmomanometers as a reference, sequentially with a measurement by using the Omron HBP-1100-E device. Absolute device-reference blood pressure differences were categorized into three error categories (within 5, 10, and 15 mmHg), and mean device-reference blood pressure difference (standard deviation) was calculated and evaluated using the American Association for the Advancement of Medical Instrumentation criteria.Results: A total of 85 participants (250 paired readings) were enrolled to the study. 26.8%, 55.6%, and 79.6% of the device-reference blood pressure differences agreed to within 5, 10 and 15 mmHg, respectively, for systolic blood pressure, and 39.6%, 69.2%, and 81.6% of measurements for diastolic blood pressure, respectively, and failed to pass the protocol criteria. The mean device-reference blood pressure difference was 8.0 ± 13.1 mmHg for systolic BP and 2.2 ± 11.3 mmHg for diastolic BP, and was > 5.0 ± 8.0 mmHg (required criteria).Conclusion: Omron HBP-1100-E professional blood pressure monitor is not desirable for measuring the BP for Iranian adults as it overestimates blood pressure in this population.Keywords: validation, electronics, blood pressure monitor, automated devices, American Association for the Advancement of Medical Instrumentation

The present study aimed to determine whether serum 25-hydroxyvitamin D is independently associated with blood pressure in a large population-based study.Methods: The study was based on subjects from PERSIAN Guilan Cohort Study (PGCS), a prospective, population-based cohort study in Guilan, Iran. In 9520 men and women, aged 35– 70 years, serum 25-hydroxyvitamin D, systolic and diastolic blood pressure were measured. Multiple logistic and linear regression analyses were conducted with adjustments for demographic factors (age and gender), anthropometric characteristics (waist circumference and body mass index), lifestyle variables (physical activity, alcohol, and smoking consumption), and renal function (serum creatinine).Results: Fully adjusted linear regression analyses revealed a weak but statistically significant negative association between serum 25-hydroxyvitamin D levels and systolic blood pressure (β = – 0.02, 95% CI= − 0.052 to − 0.0001, P-value=0.04), whereas vitamin D status was not significantly associated with diastolic blood pressure (β = – 0.01, 95% CI= − 0.026 to 0.009, P-value=0.3). Serum 25-hydroxyvitamin D status showed no significant association with the presence of hypertension (OR 1.09, 95% CI=0.94 to 1.25 for the lowest (25OHD < 12 ng/mL) versus the highest (25OHD ≥ 20 ng/mL) category).Conclusion: Lower serum vitamin 25 (OH) D levels were associated with higher systolic blood pressure; however, it was not associated with diastolic blood pressure and presence of hypertension.Keywords: vitamin D, blood pressure, Guilan cohort study, Iran

Abstract

Background: Considering the importance of the hand hygiene behaviors for the prevention of COVID-19 transmission, the current study was conducted to investigate the changes in hand-washing behaviors and its related factors among the northern Iranian population during the first peak and subsidence of COVID-19 pandemic period.Methods: This cross-sectional study was conducted during two periods in Guilan (Northern provinces of Iran). First period conducted at the first peak of COVID-19 pandemic in Iran (between 03/23/2020 and 03/30/2020), and second period conducted at the first subsidence of COVID-19 pandemic in Iran (between 05/3/20 and 05/10/2020). The questionnaire was completed by 571 adult participants. Data on frequency, procedure and circumstances of hand-washing and obsessive-like behaviors were collected. Potential correlates of reduction in frequency of hand washing were analyzed by multiple logistic regression.Result: Compared to peak of COVID-19 pandemic, frequency and procedure of hand-washing and obsessive-like behaviors were significantly reduced at the subsidence of COVID-19 pandemic (all pConclusion: Northern Iranian populations were found to decrease their frequency and quality of hand washing and obsessive-like behaviors over time from the first peak to first subsidence of COVID-19 pandemic. Here, the factors that independently predicted reduction in frequency of hand washing in male population were younger age and reduction in procedure of hand-washing and in female population was a negative family history of coronavirus disease. Thus, special attention should be paid to maintaining the general population’s perceived susceptibility to illness, especially in younger men during the pandemic.

Abstract

Analytical and experimental investigation into dynamic properties of glued-laminated (glulam) beams reinforced with various lay ups of e-glass reinforced epoxy polymer (GRP) composites are discussed. Three unreinforced glulam (control) beams and 11 glulam beams reinforced with GRP on their bottom surface were clamped in a cantilever condition and tested using an instrumental hammer. The natural frequencies of all beams determined by the impact dynamic (hammer) method using power spectral density (PSD) were used to estimate damping ratio of all the beams through both time domain and frequency domain methods. In the time domain analysis, the method of logarithmic decrement analysis (LDA) and the Hilbert transform analysis (HTA) were considered; in the frequency domain analysis, both moving block analysis (MBA) and half-power bandwidth (HPB) were applied to the data and a new approach HPB method with a polynomial correction function was numerically established. A comparison between the results obtained from all the methods of analysis and a new approach shows that this method may improve the accuracy of the HPB method in evaluation of vibration damping properties of glulam reinforced composite beams because glulam beams have high level damping ratios and that this method should be modified especially when used with materials having high level damping ratios. L'examen analytique et experimental des proprietes dynamiques des poutres lamellees-collees renforcees de diverses couches de composes polymeres epoxy renforces de verre E (PRV) est aborde. Trois poutres lamellees-collees non renforcees (reference) et onze poutres lamellees-collees renforcees de PRV sur leur face inferieure ont ete serrees en position de porte-a-faux puis mises a l'epreuve en utilisant un marteau instrumente. Les frequences naturelles de toutes les poutres, determinees par la methode d'impact dynamique (marteau) utilisant la densite spectrale de puissance, ont ete utilisees pour estimer le rapport d'amortissement de toutes les poutres par les methodes du domaine temps et des frequences. Dans l'analyse du domaine temps, la methode d'analyse par decrement logarithmique (<< LDA >>) et l'analyse de la transformee de Hilbert (<< HTA >>) ont ete utilisees. Dans l'analyse du domaine des frequences, l'analyse par blocs mobiles (<< MBA >>) et la largeur a demi-puissance (<< HPB >>) ont ete appliquees aux donnees et une nouvelle approche de la methode << HPB >> avec une fonction de correction polynomiale a ete determinee numeriquement. A systematic experimental investigation was conducted to evaluate the damping ratios of uureinforced glulam beams, and glulam beams reinforced with E-glass reinforced epoxy polymer (GRP) plates. The methods considered were the logarithmic decrement, the Hilbert transform, the moving block, and the half band power (HBP) methods. A modification to the HPB method is proposed to improve the method's accuracy when considering materials with relatively high damping ratios. The influence of the lay up sequence, thickness, and through-the-depth location of the GRP plate on damping of the composite beams is also investigated. The results obtained from our experimental and numerical investigations show that the proposed modification to the HPB method could effectively improve the accuracy of the method when considering the vibration damping of composite materials possessing relatively high level of damping. The unanticipated destruct of more of the steel moment frames in Northridge earthquake, altered class of regard to the beamto- column connections in moment frames. Panel zone is one the significant part of joints which, it-s stiffness and rigidity has an important effect on the behavior and ductility of the frame. Abstract

Introduction: Helicobacter Pylori infection is one of the most common infectious diseases. It can result in chronic gastritis, peptic and duodenal ulcers, MALT lymphoma and gastric cancer. The H-pylori infection can lead to a wide range of diseases and the cost spent on the eradication is much less than the cost spent on the treatment of complications of the infection Objective: Study the efficacy of quadruple therapy (Amoxicillin, Bismuth, Omeprazole and Clarithromycin) on the eradication of Helicobacter Pylori infection. Materials and Methods: Between March 2011 and December 2011, we conducted a Quazi- experimental study on Helicobacter pylori infectious patients in Anzali. They were included after doing an endoscopy. Eight weeks after taking four-drugs regimen, Urea Breath Test was done to check the rate of eradication. Result: The majority of the patients were females (58.7 %). The most common symptoms before the treatment were flatulence (76 %) and epigastric pain (74.9 %). The eradication rate was 89.9 %. There were significant differences in all of the symptoms while the difference between gender and eradication rate was non-significant. Conclusion: Regardless of few side effects, all symptoms of the patients were treated significantly. As the eradication rate was 89.9 %, this quadruple drug regimen was effective on the eradication of the H-pylori. [ABSTRACT FROM AUTHOR]

32. Circular Base Plates with a Gap under Large Eccentric Load.

Abstract

Circular base plates are commonly used for cylindrical columns, such as pylons in cabled-stayed bridges, lighting poles, electric power line posts, and even buildings. In some structures and conditions, a little gap is considered between the base plate and foundation. In this case, the whole forces are transmitted to the foundation via bolts. There are several explicit methods for design of rectangular base plates, which can be found in the AISC steel design guide series and in many other books, but an explicit method has not yet been presented for design of circular base plates. Therefore, in this study, we propose a procedure for designing circular base plates with large eccentric loads in which there is a little gap between base plate and foundation through analytical and numerical methods. It is assumed that the utmost compressive anchor bolt reaches to the allowable compressive stress and also critical section for calculation of base plate thickness to be the edge of the column. Comparison of theoretical and finite element analysis confirms the assumptions. Considered parameters in this study are the axial load and its eccentricity, thickness of base plate, and diameter of anchor bolts. It is concluded that the mentioned status is very useful for columns with large eccentric axial forces, because it causes the thickness of the base plate to be less in comparison with the other cases in which a gap is not considered. Analytical investigations into dynamic properties of glued-laminated beams reinforced with various lay ups of e-glass reinforced epoxy polymer composites are discussed The exp-function method is used to obtain generalized solitonary solutions and periodic solutions for nonlinear evolution equations arising in mathematical physics using symbolic computation. By using this method, natural frequency and dynamic responses of the wood-composite beams are obtained. The existing data obtained from our published work, the grindosonic instrument, and also the data from the beams tested using the power spectral density method is used for validity of analytical method. The data from finite element method as a numerical method was also employed. The comparison indicated that the proposed exp-function method could be effectively used to establish the natural frequency and dynamic properties of glulam reinforced composite beams when conducted in nonlinear equation forms. Abstract

Background: Understanding the magnitude of cancer burden attributable to potentially modifiable risk factors is crucial for development of effective prevention and mitigation strategies. We analysed results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019 to inform cancer control planning efforts globally. Methods: The GBD 2019 comparative risk assessment framework was used to estimate cancer burden attributable to behavioural, environmental and occupational, and metabolic risk factors. A total of 82 risk-outcome pairs were included on the basis of the World Cancer Research Fund criteria. Estimated cancer deaths and disability-adjusted life-years (DALYs) in 2019 and change in these measures between 2010 and 2019 are presented. Findings: Globally, in 2019, the risk factors included in this analysis accounted for 4·45 million (95% uncertainty interval 4·01-4·94) deaths and 105 million (95·0-116) DALYs for both sexes combined, representing 44·4% (41·3-48·4) of all cancer deaths and 42·0% (39·1-45·6) of all DALYs. There were 2·88 million (2·60-3·18) risk-attributable cancer deaths in males (50·6% [47·8-54·1] of all male cancer deaths) and 1·58 million (1·36-1·84) risk-attributable cancer deaths in females (36·3% [32·5-41·3] of all female cancer deaths). The leading risk factors at the most detailed level globally for risk-attributable cancer deaths and DALYs in 2019 for both sexes combined were smoking, followed by alcohol use and high BMI. Risk-attributable cancer burden varied by world region and Socio-demographic Index (SDI), with smoking, unsafe sex, and alcohol use being the three leading risk factors for risk-attributable cancer DALYs in low SDI locations in 2019, whereas DALYs in high SDI locations mirrored the top three global risk factor rankings. From 2010 to 2019, global risk-attributable cancer deaths increased by 20·4% (12·6-28·4) and DALYs by 16·8% (8·8-25·0), with the greatest percentage increase in metabolic risks (34·7% [27·9-42·8] and 33·3% [25·8-42·0]). Interpretation: The leading risk factors contributing to global cancer burden in 2019 were behavioural, whereas metabolic risk factors saw the largest increases between 2010 and 2019. Reducing exposure to these modifiable risk factors would decrease cancer mortality and DALY rates worldwide, and policies should be tailored appropriately to local cancer risk factor burden. Abstract

Background: Waist circumference (WC), or waist-to-hip ratio (WHR), potentially offers a more accurate reflection of intra-abdominal fat accumulation and could serve as a superior predictor of type 2 diabetes mellitus (T2DM) risk compared to BMI. The current study investigated the relationship between WHR and its influencing factors among diabetes patients enrolled in the Prospective Epidemiological Research Studies in Iran (PERSIAN) Guilan Cohort study (PGCS)., Method: In this cross-sectional study of 10,520 participants, 2,531 had T2DM. Waist and hip circumference, body mass index (BMI), underlying diseases, and demographical data of participants were recorded. Also, fasting blood sugar (FBS), low-density lipoprotein (LDL) cholesterol, high-density lipoprotein (HDL) cholesterol, and triglycerides (TG) were assessed. All data was analyzed using SPSS version 16; the significant level was < 0.05., Results: The mean age of participants was 51.52 ± 8.90 years, and 39.9% had a BMI between 25 and 30 kg/m 2 . The prevalence of diabetes was 24.1% (n = 2531). About 7628 (72.5%) individuals had abnormal WHR, and 2072 (19.7%) were diabetics. Among patients with diabetes, abnormal WHR was significantly associated with age over 50, female gender, higher BMI, and lower LDL (P < 0.05)., Conclusion: The study showed a higher prevalence of abnormal WHR in diabetic patients. 40. Evaluation of elevated serum liver enzymes and metabolic syndrome in the PERSIAN Guilan cohort study population.

Abstract

Objective: The purpose of this study is to evaluate the association between elevated serum liver enzymes and Metabolic Syndrome (MetS) in Prospective Epidemiological Research Studies of the Iranian Adults (PERSIAN) Guilan Cohort Study (PGCS) population., Methods: This cross-sectional study involved 10,519 individuals between the ages of 35 and 70 enrolled in the PGCS. The gathered data encompassed demographic information, anthropometric measurements, blood pressure, and biochemical indicators. MetS was defined by the National Cholesterol Education Program-Adult Treatment Panel III criteria (NCEP-ATP III). The associations between elevated liver enzymes and MetS were examined using logistic regression analysis. Odds ratio (OR) and 95 % confidence interval (CI) were calculated., Results: The prevalence of MetS was 41.8 %, and the prevalence of elevated alanine aminotransferase (ALT), aspartate aminotransferase (AST), gamma-glutamyl transferase (GGT), and alkaline phosphatase (ALP) were 19.4, 4.6, 11.6, and 5.1 %, respectively. In the unadjusted model, elevated ALT, AST, and GGT were associated with increased odds of MetS (OR = 1.55, 95 % CI: 1.41-1.71; OR = 1.29, 95 % CI: 1.07-1.55, and OR = 1.90, 95 % CI: 1.69-2.14, respectively). These associations remained significant for ALT and GGT after adjustment for some demographic and clinical characteristics (aOR = 1.31, 95 % CI: 1.17-1.46 and aOR = 1.30, 95 % CI: 1.14-1.49, respectively). In addition, the odds of MetS increased with the number of elevated liver enzymes, up to almost 1.32-fold among subjects with three/four elevated liver enzymes., Conclusion: The higher incidence of elevated liver enzymes was associated with an increased likelihood of MetS. Abstract

Although hematuria is not life-threatening, some could be the result of a more severe condition. Our objectives are to report on the prevalence and risk factors of asymptomatic microscopic hematuria (AMH) in the prospective epidemiological research studies of the Iranian adults (PERSIAN) Guilan cohort study (PGCS) population. This cross-sectional study was conducted from 2014 to 2017 and consisted of 10,520 individuals aged 35-70. Data collection was conducted using a questionnaire during a face-to-face interview. The urine analyses (UA) were done up to 2 h after sample collection. Based on a urine microscopy evaluation, AMH is defined as 3 or more red blood cells per high power field (HPF). Simple and multiple logistic regression analysis was conducted to explore factors associated with AMH. The prevalence of AMH in this study was 34.1% and was more prevalent in participants of older ages and female gender as well as those with low educational level, underweight-body mass index (BMI), high physical activity, smoking, alcohol consumption, and kidney stone disease. On the other hand, obesity, opium, and diabetes decreased the likelihood of AMH. The results of the present study shed light on the prevalence and risk factors of AMH and suggested that a significant portion of the study population is affected by AMH. Considering the lack of consensus on a definite clinical guideline for AMH in our country, the results of the present study could be used to design a unit algorithm for screening and therapy of AMH., (© 2024. Abstract

Background: Ultrasound is an important method to determine the volume of the gallbladder and check its structure. Considering the variation in the size and volume of the gallbladder in disease and physiological conditions, determining the volume of the gallbladder is clinically valuable. This study was carried out to evaluate the gallbladder volume and its association with patients' demographic data in the Prospective Epidemiological Research Studies of Iranian Adults (PERSIAN) Guilan cohort study (PGCS) population., Methods: In this cross-sectional study, 957 individuals aged 35-70 participated in determining the gallbladder volume by a radiologist based on the ultrasound method. The demographical data were collected using a questionnaire. After fasting for 12 h, the ultrasound was performed with an Ultrasonic device (Sonix SP series) with a 3.5 to 5 MHz probe., Results: The total frequency of gallbladder lesions was 2.2%. The results showed a significant association between marriage and gender with the presence or absence of lesions in the studied participants (P < 0.05). Also, significant differences were reported between the volume of gallbladder and gender, body mass index (BMI), social and economic status (SES), metabolic equivalent of task (MET), history of cardiovascular disease (CVD), and hypertension (P < 0.05). The results of a linear regression represented a significant association between gender, BMI, MET, and CVD and the mean volume of the gallbladder (P < 0.05). However, there was no significant association between the presence or absence of a lesion and the individuals' average gallbladder volume (P > 0.05)., Conclusion: According to our results, gender, BMI, MET, and CVD were significantly associated with gallbladder volume., (© 2023. Abstract

Background: Diabetes mellitus (DM) is known as one of the most prevalent non communicable diseases with high cost of health services in the world. Present study was conducted to assess the frequency of high risk people for diabetes mellitus based on American Diabetes Association (ADA) risk score among Iranian people., Methods: Present study was a cross sectional study on non-diabetic subjects aged 35-70 years from 10,520 PERSIAN Guilan Cohort Study (PGCS). ADA risk score was calculated for every individual through an online calculator. Receiver operating characteristic (ROC) curves was used to assess diagnostic accuracy of the anthropometric indices to identify individuals with high risk ADA score for developing DM, represented by the area under the curve (AUC)., Results: From 7989 study subjects, ADA risk score found 3874 (48.5%) and 1912 (23%) at risk for developing PreDM and DM, respectively. The results of ROC curve analyses showed the highest diagnostic value was related to waist circumference (WC) in total population and Waist to Height Ratio in both sex (0.695 total, 0.743 female, 0.744 male). The cut-points of WC in total population to identifying high risk group were 97 cm., Conclusions: A considerable number of populations were classified as high ADA risk for developing DM and PreDM that provide the importance of prevention strategies. 44. Prevalence of Tinnitus and Its Associated Factors in the PERSIAN Guilan Cohort Study.

Abstract

Introduction: Tinnitus can be associated with many auditory and non-auditory factors, and its prevalence varies widely in the literature. There is no large sample of published data on tinnitus prevalence and its associated factors in Iran. Here, we analyzed the PERSIAN Guilan Cohort Study (PGCS) data and reported the prevalence of tinnitus and some of the risk factors related to tinnitus in the Iranian population., Materials and Methods: This cross-sectional study was conducted on 10520 men and women between 35 and 70 years old. The prevalence of tinnitus and associations between tinnitus and age, sex, habitat, marital status, employment status, socioeconomic status, educational level, lifestyle habits, and comorbid diseases were examined using simple and multiple logistic regression analyses., Results: The prevalence of tinnitus was 6.4% in this study. Based on the adjusted analysis, only older age (odds ratio: 2.60, 95% confidence interval: 1.88 - 3.60), residency in a rural area (odds ratio: 1.22, 95% confidence interval: 1.03 - 1.44), cigarette smoking (odds ratio:1.33, 95% confidence interval: 1.04 - 1.72), and having other comorbidities (odds ratio: 2.75, 95% confidence interval: 2.19 - 3.44) were related to tinnitus. In addition, the results of subgroup analyses by sex were mostly consistent with the overall analysis., Conclusions: Our results revealed that the prevalence of tinnitus in the north of Iran is comparable with other communities. Age and other comorbidities were among the most related factors to tinnitus.

45. Gender-related differences in the association of serum levels of vitamin D with body mass index in northern Iranian population: the PERSIAN Guilan Cohort Study (PGCS).

Background: Vitamin D (VD) insufficiency has now become a global problem throughout the world. The association between increasing body mass index (BMI) and VD insufficiency has attracted great attention in recent researches. The aim was to study if BMI was independently associated with serum 25-hydroxy VD in a large population-based study, specify by gender., Methods: Data on 9520 adults 35 years and older participating in PERSIAN Guilan Cohort Study (PGCS) were used. Serum levels of 25-hydroxy VD less than 20 ng/mL (50 nmol/L) was used as a measure of VD inadequacy. Multiple logistic and linear regression analyses were used to estimate the strength of the association between VD and BMI before and after adjusting for demographic factors and lifestyle variables., Results: After adjustment in male population, overweight (adjusted OR = 1.2, P < 0.05) and obese (adjusted OR = 1.4, P > 0.05) individuals were more likely to have VD inadequacy than normal weight counterparts. Moreover, there was a weak inverse linear association between BMI and serum 25 (OH) D levels in males (β = -0.14, P value > 0.05). In contrast, no significant associations between BMI and serum 25 (OH (D levels were observed in females. In male population, higher BMI were associated with lower serum vitamin 25 (OH) D levels., Conclusions: However, association between BMI and VD level was not observed in female population. Abstract

Background: The combination of sofosbuvir and daclatasvir has shown preliminary efficacy for hospitalized patients with COVID-19 in four open-label studies with small sample sizes. This larger trial aimed to assess if the addition of sofosbuvir/daclatasvir to standard care improved clinical outcomes in hospitalized patients with COVID-19., Methods: This was a placebo-controlled, double-blind, randomized clinical trial in adults hospitalized with COVID-19 at 19 hospitals in Iran. Patients were randomized to oral sofosbuvir/daclatasvir 400/60 mg once-daily or placebo in addition to standard of care. Patients were included if they had positive PCR or diagnostic chest CT, O2 saturation <95% and compatible symptoms. The primary outcome was hospital discharge within 10 days of randomization. Secondary outcomes included mortality and time to clinical events. The trial is registered on the Iran Registry of Clinical Trials under IRCT20200624047908N1., Results: Between July and October 2020, 1083 patients were randomized to either the sofosbuvir/daclatasvir arm (n = 541) or the placebo arm (n = 542). No significant difference was observed in the primary outcome of hospital discharge within 10 days, which was achieved by 415/541 (77%) in the sofosbuvir/daclatasvir arm and 411/542 (76%) in the placebo arm [risk ratio (RR) 1.01, 95% CI 0.95-1.08, P = 0.734]. In-hospital mortality was 60/541 (11%) in the sofosbuvir/daclatasvir arm versus 55/542 (10%) in the placebo arm (RR 1.09, 95% CI 0.77-1.54, P = 0.615). No differences were observed in time to hospital discharge or time to in-hospital mortality., Conclusions: We observed no significant effect of sofosbuvir/daclatasvir versus placebo on hospital discharge or survival in hospitalized COVID-19 patients., (© The Author(s) 2021. 48. Dietary supplements consumption and its association with socioeconomic factors, obesity and main non-communicable chronic diseases in the north of Iran: the PERSIAN Guilan Cohort Study (PGCS).

Abstract

Background: Dietary supplements (DSs) use have become a growing trend worldwide, and it may be affected by demographic and sociocultural factors. Some people use supplements with the thought that they can improve their health, reduce symptoms and prevent disease. The aim of the present study was to define the frequency of DS use and its association with socioeconomic factors among participants with selected main non-communicable chronic diseases (NCDs) (diabetes, cardiovascular disease (CVD), hypertension (HTN), cancers, and obesity in the north of Iran., Methods: This large cross-sectional study was conducted as a part of the PERSIAN Guilan cohort study. Supplement use during last year and its type, demographic factors, socioeconomic status, lifestyle habits were asked by face-to-face interview. The history of chronic disease was defined by a trained team. Data were analyzed using SPSS. The chance of supplement use according to demographic, socioeconomic, and lifestyle variables and history of chronic disease was analyzed by logistic regression., Results: 10,520 men and women aged 35-70 years in Some'e Sara County (including urban regions and 39 villages) were studied. About 25% of participants consumed DSs. The highest consumption of DS was calcium/vitamin D (11.1%), ferrous sulfate (8.8%), and vitamin D pearl or ampoule (7.7%). The highest percent of the history of chronic disease was central obesity (62.7%), HTN (43.2%), and general obesity (32.7), respectively. After adjustment for confounders, those with female gender, the highest age ranges (55-65 and > 65 years), high academic education, living in urban regions, and good economic status were more likely to be DSs consumers; however, married and smoker subjects were more likely to consume DS. Participants who had a history of diabetes, HTN, CVD, Obesity, and Central Obesity were more likely to intake DS in comparison with healthy subjects., Conclusion: This study showed that a quarter of the participants were DS users. Female sex, older age groups, and higher educated participants, and among chronic disease, patients with HTN, CVD, and diabetes were more likely to be users of any DS., (© 2021. The Author(s).)

49. Food Behaviors and Its Association with Hypertension and Cardiovascular Diseases in Sowme'eh Sara (North of Iran): The PERSIAN Guilan Cohort Study (PGCS).

Abstract

Food behavior and dietary pattern are major parameters affecting the health status of people. The objective of this study was investigation of food behaviors in people inhabited in North of Iran to find out association of the effective indices (related to salt and fried foods) with hypertension and other cardiovascular diseases (CVDs). This large cross-sectional study was conducted on 10,520 participants and their sociodemographic, medical, and food behavior information were evaluated. Association of salt intake with CVDs and stroke was studied by portfolio at risk (PAR) formula. Association of food behavior with hypertension and CVDs was surveyed by chi-square test. Differences were significant at P ≤0.05. Analysis of medical information revealed that 43.18% and 8.18% of the participants had hypertension and CVDs, respectively; mostly in women. Significant association was observed between salt-related behaviors and hypertension in the participants. Regarding salt intake, higher PARs were calculated in women than men (2.26 vs. 1.88 for PAR CVDs and 3.66 vs. 3.05 for PAR stroke ), and urban than rural inhabitants (4.56 vs. 3.66 for PAR CVDs and 7.27 vs. 5.88 for PAR stroke ). No significant contribution was observed between oil-related behaviors and CVDs. The majority of participants were well-informed about appropriate preparation and consumption of fried foods but not a healthy behavior about salt intake. Abstract

Background: The persistence of severe acute respiratory syndrome-coronavirus 2 (SARS-CoV-2) RNA in the body fluids of patients with the novel coronavirus disease 2019 (COVID-19) may increase the potential risk of viral transmission. There is still uncertainty on whether the recommended quarantine duration is sufficient to reduce the risk of transmission. This study aimed to investigate the persistence of SARS-CoV-2 RNA in the nasopharyngeal, blood, urine, and stool samples of patients with COVID-19., Methods: In this hospital-based longitudinal study, 100 confirmed cases of COVID-19 were recruited between March 2020 and August 2020 in Guilan Province, north of Iran. Nasopharyngeal, blood, urine, and stool samples were obtained from each participant at the time of hospital admission, upon discharge, 1 week after discharge, and every 2 weeks until all samples were negative for SARS-CoV-2 RNA by reverse transcription-polymerase chain reaction (RT-PCR) assay. A survival analysis was also performed to identify the duration of viral persistence., Results: The median duration of viral RNA persistence in the nasopharyngeal samples was 8 days from the first positive RT-PCR result upon admission (95% CI 6.91-9.09); the maximum duration of viral shedding was 25 days from admission. Positive blood, urine, and stool RT-PCR results were detected in 24%, 7%, and 6% of the patients, respectively. The median duration of viral persistence in the blood, urine, and stool samples was 7 days (95% CI 6.07-7.93), 6 days (95% CI 4.16-8.41), and 13 days (95% CI 6.96-19.4), respectively. Also, the maximum duration of viral persistence in the blood, urine, and stool samples was 17, 11, and 42 days from admission, respectively., Conclusion: According to the present results, immediately after the hospitalized patients were discharged, no evidence of viral genetic materials was found. Therefore, appropriate treatments were selected for the patients at this hospital. However, we recommend further investigations on a larger sample size in multi-center and prospective randomized controlled trials (RCTs) to evaluate the effects of different drugs on the shedding of the virus through body secretions.
