18. Development of a Cognitive Behavioural Therapy-Based Guided Self-Help Intervention for Adults with Intellectual Disability

Abstract

Background: Despite strong evidence for cognitive behaviour therapy (CBT) in treating mental health, its use, thus far, has been limited for people with intellectual disabilities. This study describes a CBT-based guided self-help (CBT-GSH) manual for individuals with intellectual disability, and focus groups explore the views of clinicians, therapists, support staff and managers. Material and methods: Using a qualitative methodology, an expert team adapted the manual. Focus groups provided feedback, followed by thematic content analysis for modifications. Results: Participants supported using the manual, with varying views about the delivery. Quality of relationships and competence of the administrator determined the best person to deliver the treatment. Heterogeneity in the intellectual disability population was a challenge to delivering manual-based interventions. Participants made suggestions about language and organization. Conclusions: Amendments were made to the manual in line with expert feedback. An evaluation is warranted to test for feasibility, delivery, acceptability and efficacy.

Abstract

Background: South Asian (SA) Canadians are disproportionately affected by higher rates of mood and anxiety disorders. SA Canadians with depression report significant barriers to accessing mental health care and the highest proportion of unmet mental health needs. The Mental Health Commission of Canada (MHCC) advocates for culturally and linguistically relevant services for SA Canadians. Culturally adapted cognitive behavior therapy (CaCBT) has shown to be more effective than standard cognitive behavior therapy (CBT). Adapting CBT for the growing SA population in Canada will ensure equitable access to effective, culturally-appropriate mental health interventions. Method: The study used a qualitative design to elicit stakeholder consultation via in-depth interviews. This study is reported using the criteria included in Consolidated Criteria for Reporting Qualitative Studies (COREQ). The analysis follows an ethnographic approach and was informed by the principles of emergent design. Results: Five themes were identified from the analysis, (i) Awareness and preparation: factors that impact the individual's understanding of therapy and mental illness. (ii) Access and provision: SA Canadians' perception of barriers, facilitators, and access to treatment. (iii) Assessment and engagement: experiences of receiving helpful treatment. (iv) Adjustments to therapy: modifications and suggestions to standard CBT. (v) Ideology and ambiguity: racism, immigration, discrimination, and other socio-political factors. Conclusions: Mainstream mental health services need to be culturally appropriate to better serve SA Canadians experiencing depression and anxiety. Services must understand the family dynamics, cultural values and socio-political factors that impact SA Canadians to reduce attrition rates in therapy. [ABSTRACT FROM AUTHOR]

22. Adaptation of cognitive behaviour therapy for depression in Pakistan

Background: Cognitive Behaviour Therapy (CBT) in its current form might not be applicable in Non Western cultures. Differences between western and non western cultures have been reportedly widely. Psychotherapy was developed in the west and is underpinned by many beliefs and practices which might be specific only to the Western culture. However, in order to modify CBT we need to understand whether the concepts associated with the CBT might cause conflicts among people who receive therapy, the barriers in giving therapy and the views of the patients. This project was carried out mainly in Pakistan to adapt CBT for depression. Aims: To find out if CBT can be successfully adapted in a Non Western culture. Methods: This was a mixed methods Study. The project consisted of two phases. In the first phase a series of studies were carried out, including interviews with psychologists, patients and group discussions with university students about their views regarding concepts underlying therapy. In the second phase a CBT for depression manual was modified using guidelines which were developed on the basis of studies carried out in the first phase. This manual was then tested in a small pilot project using a Randomised Controlled Trial (RCT) design. Results: We were able to find themes and subthemes, on the basis of studies in first phase of the project, which were used to modify a CBT for depression manual. We developed an adaptation framework on the basis of the identified factors. This framework consisted of three broad themes (name theme) with each subdivided into seven sub themes. The pilot study showed that therapists trained for a short period and under supervision can deliver CBT using a manual. Results of pilot showed that modified CBT is more effective than 'care as usual' in reducing symptoms of depression. Conclusions: The study demonstrates that for CBT to be effective in Non Western cultures, it needs modification. This can be achieved using small scale qualitative studies locally, which explore experience of therapists working in a given culture as well as by exploring the views of patients. Further information can be obtained by talking to the members of that community about concepts underlying CBT. However, these are preliminary findings and further research needs to be done to explore this area further.

50. Psychological Morbidity in Children 18 Months after Kashmir Earthquake of 2005

Abstract

A severe earthquake occurred in Kashmir in 2005. The epicentre was close to Muzzafarabad. We collected data on over 1,100 children 18 months after the earthquake to look at symptoms of PTSD and behavioural and emotional problems using well established questionnaires. We found that 64.8% of children had significant symptoms of PTSD. Girls were more likely to suffer from these symptoms. The proportion of children suffering from emotional and behaviour difficulties was 34.6%. This percentage was not different from other studies of children from Pakistan within areas which were not affected by the earthquake. The rate of emotional symptoms was higher in girls while hyperactivity was more frequent in boys. This pattern is similar to other studies from across the world.
