Digitalisation in railway networks harnesses digital technologies to optimise operations, leading to enhanced efficiency, and reduced energy consumption. By analysing real-time data, railways can predict maintenance needs, improve passenger experiences, and seamlessly integrate with other transport modes. As societies strive for sustainable transportation solutions, it is imperative to understand and collect digitalisation techniques to enhance efficiency and reduce the ecological footprint of railway networks. This paper serves as a snapshot of the current state of the art addressing the pivotal role of point cloud techniques in advancing railway digitalisation and providing valuable pointers for future research directions. Employing a systematic review approach, our study concentrates exclusively on research centred around railway assets and their digitalisation via point cloud data. We have themed the literature into pre-processing, modelling, and digital twinning. Within this review, we analyse diverse modelling and pre-processing techniques and categorise them for clarity. The digital twin techniques are also collected, though these techniques are scarce in the context of railway infrastructure and point clouds. The paper also presents a compilation of dataset statistics highlighting the scarcity of openly available railway-specific datasets. This scarcity considerably hampers the feasibility of research reproducibility and the comparative analysis of different approaches. Our conclusion reflects on the challenges encountered and proposes a course for future research. Particularly, we conclude that hybrid methodologies that combine machine learning with structure-based techniques hold substantial promise toward creating digital twins, considering the intrinsic characteristics of railway infrastructure.

Text-to-graphics systems encompass three types of tools: text-to-picture, text-to-scene and text-to-animation. They are an artificial intelligence application wherein users can create 2D and 3D scenes or animations and recently immersive environments from natural language. These complex tasks require the collaboration of various fields, such as natural language processing, computational linguistics and computer graphics. Text-to-animation systems have received more interest than their counterparts, and have been developed for various domains, including theatrical pre-production, education or training. In this survey we focus on text-to-animation systems, discussing their requirements, challenges and proposing solutions, and investigate the natural language understanding approaches adopted in previous research works to solve the challenge of animation generation. We review text-to-animation systems developed over the period 2001-2021, and investigate their recent trends in order to paint the current landscape of the field.

Abstract Virtual Reality (VR) and educational games are emerging technologies mediating a rapid transformation in the educational world. However, few studies have systematically analyzed Educational Virtual Reality Games (EVRGs) and how they have been applied in educational settings. This study reviewed 31 articles published in high impact journals and educational conference proceedings to unravel the technological, pedagogical, and gaming characteristics of contemporary EVRGs. The results show the predominance of Oculus Rift headsets and HTC Vive as the main technology used in EVRGs. Moreover, the analysis revealed that the pedagogical application of the majority of EVRGs was developed for all levels of education (e.g. tertiary, K-12, lifelong learning), with the specific target audience of each game based on the desired learning outcome. Furthermore, the application of EVRGs has primarily focused on out of class use, with healthcare education topics dominating the topics taught using EVRGs. Based on our findings, we highlight some key implications and suggestions to advance the field of EVRGs.

Education, training and isotopes production are the most important uses of the Moroccan 2 MW TRIGA Mark II reactor situated at the National Center for Energy Sciences and Nuclear Techniques (CNESTEN, Morocco). To develop new R&D projects in research reactors, the particular and advanced knowledge of neutron and photon flux distribution, within and around the reactor core, is crucial. In order to precisely preparing the experiments in the CNESTEN’s TRIGA reactor, a detailed model was developed using the 3D continuous energy Monte Carlo code TRIPOLI-4 and the continuous energy cross-section data from the JEFF3.1.1 nuclear data library. This new model was used to carry out preliminary neutron and photon calculations to estimate flux levels in the irradiation channels as well as to calculate kinetic parameters of the reactor, core excess reactivity, integral control rods worth and power peaking factors. As a first step of the validation of the model, the obtained results were compared with the experimental ones available in the Final Safety Analysis Report (FSAR) of the TRIGA reactor. A study is being carried out at the end of which the results will be published as an evaluated benchmark. Furthermore, this work aims at experimentally characterize the reaction rates in various irradiation channels inside and outside the reactor core. The measurements are carried out using the neutron activation technique. To set up the experimental design for the activation experiments a series of preliminary calculations were performed using the TRIPOLI-4 model to calculate the expected gamma flux/intensity levels of various materials after irradiations in different positions in the irradiation facilities. Different activation foils with known characteristics are then irradiated and the activity of several isotopes is measured with the Gamma Spectrometry Method. The measured relative reaction rates are then compared with the calculated ones evaluated through the new TRIPOLI-4 reactor model. Fairly good agreement was found, which indicates that the new computational model is accurate enough to reproduce experiments.

The CNESTEN (National Center for Energy Sciences and Nuclear Technology, Morocco) operates a TRIGA Mark II reactor, which can reach a thermal maximum power at steady state of 2 MW. In reactors devoted to research and experiments, it is mandatory to characterize the neutron and photon fields in the irradiation positions. Together with a computational model of the core, it ensures the ability to reach the requested uncertainties when performing experiments, such as detectors testing, irradiation for hardening or nuclear data measurements. The neutron field of different irradiation positions has been characterized by dosimetry techniques and compared to the MCNP full model of the reactor. Preliminary photon propagation calculations are also performed with this model, but up to now, no experimental validation of the results exists. The aim of the newly set collaboration between CEA and CNESTEN is to characterize the gamma field of these positions. The first position investigated is the part of the NB1 tangential channel closest to the core. Among gamma measurements techniques, and according to the constraints arising from using this channel, it was chosen to use thermos- and optically stimulated luminescent detectors. This paper presents the experiments carried out in September 2018 as well as their results. Three detectors types were used: TLD400 (CaF2:Mn), TLD700 (7LiF:Mg,Ti) and OSLD (Al2O3:C). Measurements were performed in several steps: background measurements, transient measurements (divergence phase + SCRAM), and irradiation at steady state. In the end, these measurements will provide a dose as well as a gamma flux value for this position.

Objectives: This study aims to investigate the electrophysiological, scintigraphic, and histopathological effects of pitavastatin and its impact on functional status in rats with sciatic nerve injury., Materials and Methods: A total of 30 Wistar albino rats were divided into three equal groups including 10 rats in each group: sham group (no injury), control group (nerve injury induced), and pitavastatin group (nerve injury induced and 2 mg/kg of pitavastatin administered orally once a day for 21 days). Before and at the end of intervention, quantitative gait analysis with the CatWalk system and sciatic nerve conduction studies were performed. After the intervention, the gastrocnemius muscle was scintigraphically evaluated, and the sciatic nerve was histopathologically examined., Results: There was no significant difference in the sciatic nerve conduction before the intervention and Day 21 among the groups (p>0.05). According to the quantitative gait analysis, there were significant differences in the control group in terms of the individual, static, dynamic, and coordination parameters (p<0.05). Background/objective: Notalgia paresthetica (NP) is a sensory neuropathy characterized by localized pruritus and pain, presenting with or without a well-circumscribed hyperpigmented patch in the upper back. Abnormal sensations, such as burning, numbness, and paresthesia are often present in patients with NP. In this study, we clinically and radiologically analyzed patients with NP. The literature contains studies describing lidocaine treatments involving intravenous and topical applications for NP. We also investigated the effect of intradermal lidocaine injection on patients with NP., Methods: A total of 80 patients (45 patients with NP and 35 suffering from dorsalgia without NP) were included in the study. The age, gender and body mass index (BMI) of the patients, and the characteristics of their symptoms were recorded. The severity of pain and pruritus was assessed by the Visual Analog Scale (VAS). Radiography and magnetic resonance imaging of the spine were performed. In this study, we intradermally administered lidocaine diluted with saline into the upper back over three sessions. 1 cc 2% lidocaine was diluted with 5 cc 0.9% saline, and a total of 6 cc lidocaine mixture was obtained. The injection was performed locally at 1-cm intervals around the hyperpigmented patch and segmentally along the C2-T6 spinous processes. These patients were called for a follow-up at the second and fourth weeks and third month., Results: There was no statistically significant difference between the two groups in terms of age, BMI, VAS-pain score, and duration of symptoms (p > 0.05 for all). Forty-six cervical and/or thoracic degenerative changes or herniated nucleus pulposus (HNP) were detected in patients with NP. There was a significantly higher number of HNP at the C6-7 segment and cervical degenerative changes in the NP group (p < 0.05). The VAS-pain and VAS-pruritus scores were significantly decreased at all follow-up sessions, and improvement was sustained by lidocaine up to the third month., Conclusion: Cervical degenerative changes and HNP of the C6-7 segment seem to be contributing factors for NP. Local lidocaine can be effective for pain relief and pruritus in NP.

Background: Isotretinoin (ISO) is a synthetic vitamin A derivative which has been used for treatment-resistant acne vulgaris. Although most musculoskeletal side effects of ISO are common, including myalgia, arthralgia, and back pain, sacroiliitis is one of the uncommon side effects. ISO-induced sacroiliitis usually completely resolves within a few months by the cessation of the drug., Case Presentation: In this paper, we present a 26-year-old female patient with chronic sacroiliitis that was probably induced by ISO and not resolved by the discontinuation of the drug., Conclusion: In this patient, sacroiliitis was overlooked for three years. Therefore, ISO usage should be considered in the differential diagnosis of sacroiliitis and low back pain.

Objection: Entrapment neuropathies are common in clinical practice. Early diagnosis and management of nerve compression is necessary to maintain limb function and to improve the patient's quality of life., Case Report: In this article, we reported a woman presenting with wrist drop as a result of acute radial nerve compression following strenuous activity involving the arms. The diagnosis was based on clinical and ultrasonographic findings. Once the diagnosis was made, activity modifications and systemic steroid were prescribed, and the patient made a near-complete recovery., Conclusion: Patients with acute wrist drop and sensorial loss should be examined in terms of arm overuse, and radial nerve compression should be confirmed by peripheral nerve ultrasound.

Background: Baker's cyst is a benign lesion that results from degenerative or inflammatory diseases of the knee joint. When Baker's cyst ruptures, it may simulate deep vein thrombosis known as Pseudothrombophlebitis syndrome with calf pain, swelling and redness. Pseudothrombophlebitis syndrome without thrombus in popliteal veins has distinct treatment choice than deep vein thrombus., Case Presentation: In this report, we presented a 47 year-old male rheumatoid arthritis patient with complaints of redness, pain and swelling on his right calf. Pseudothrombophlebitis syndrome was diagnosed due to ruptured Baker's cyst., Conclusions: We used musculoskeletal ultrasound for both differential diagnosis and treatment of pseudothrombophlebitis syndrome. Ultrasonography revealed massive fluid collection within muscle layers. 280 cc inflammatory fluid was aspirated simultaneously. We also emphasized the importance of ultrasonography in diagnosis and treatment of Pseudothrombophlebitis syndrome with this report.

Objectives: To investigate the relationship between enthesitis and disease activity, functional status, fatigue, joint mobility, radiological damage, laboratory parameter and quality of life in patients with ankylosing spondylitis (AS)., Methods: A total of 421 patients with AS (323 male and 98 female) who were included in the Turkish League Against Rheumatism Registry were enrolled in the study. The Bath AS Disease Activity Index (BASDAI), fatigue, the Bath AS Functional Index (BASFI), the Bath AS Metrology Index (BASMI), the Maastricht AS Enthesitis Score (MASES), AS quality of life (ASQoL), the Bath AS Radiology Index (BASRI) and erythrocyte sedimentation rate (ESR) were evaluated., Results: Enthesitis was detected in 27.3% of patients. There were positive correlations between MASES and BASDAI, BASFI and fatigue (p < 0.05). MASES was not correlated with BASRI, BASMI, ASQoL and ESR. The mean MASES score was 1.1 ± 2.4. The most frequent regions of enthesopathies were right iliac crest, spinous process of L5 and proximal to the insertion of left achilles tendon, respectively., Conclusions: Enthesitis was found to be associated with higher disease activity, higher fatigue, worse functional status and lower disease duration. As enthesitis was correlated with BASDAI, we conclude that enthesitis can reflect the disease activity in patients with AS.

Episacral lipoma is a small, tender subcutaneous nodule primarily occurring over the posterior iliac crest. Episacral lipoma is a significant and treatable cause of acute and chronic low back pain. Episacral lipoma occurs as a result of tears in the thoracodorsal fascia and subsequent herniation of a portion of the underlying dorsal fat pad through the tear. This clinical entity is common, and recognition is simple. The presence of a painful nodule with disappearance of pain after injection with anaesthetic, is diagnostic. Medication and physical therapy may not be effective. Local injection of the nodule with a solution of anaesthetic and steroid is effective in treating the episacral lipoma. Here we describe 2 patients with painful nodules over the posterior iliac crest. One patient complained of severe lower back pain radiating to the left lower extremity and this patient subsequently underwent disc operation. The other patient had been treated for greater trochanteric pain syndrome. In both patients, symptoms appeared to be relieved by local injection of anaesthetic and steroid. Episacral lipoma should be considered during diagnostic workup and in differential diagnosis of acute and chronic low back pain.

Objectives: This study aims to investigate the effect of acromioclavicular joint (ACJ) degeneration on orthopedic shoulder tests (OST) and to determine complementary role of ACJ injections in the treatment of subacromial shoulder impingements., Patients and Methods: At a secondary care center, 62 patients (56 females, 6 males; mean age 55.8±9.3 years; range 35 to 74 years) with the clinical and radiological diagnosis of shoulder impingement syndrome were enrolled to the study. Two injections were applied to ACJ and subacromial space. Patients were assessed through OSTs (Neer, Hawkins-Kennedy, empty can, full can, external rotation resistance, lift-off and cross-body adduction tests) at baseline and after injection to ACJ and subacromial space. Visual analog scale was used for overall pain., Results: Two injections and three sets of OSTs were performed on the patients. A significant difference was found in the all OSTs after ACJ injection (p<0.001). Visual analog scale scores improved significantly following both ACJ and subacromial injections (p<0.001)., Conclusion: Concomitant ACJ diseases should assessed carefully in the patients with rotator cuff lesions, as it may change the type of management approach.

A web-based application patient follow-up program was developed to create a registry of patients with ankylosing spondylitis (AS) by the Turkiye Romatizma Arastirma Savas Dernegi (TRASD) AS Study Group. This study describes the methodological background and patient characteristics. The patient follow-up program is a web-based questionnaire, which contains sections on socio-demographic data, anamnesis, personal and family history, systemic and musculoskeletal examination, laboratory and imaging data and treatment. Between October 1, 2007 and February 28, 2009, 1,381 patients from 41 centers were included in the registry (1,038 males [75.2%]; mean age 39.5 ± 10.7 years). Mean disease duration was 12.1 ± 8.5 years, and mean time from initial symptom to diagnosis was 5 ± 6.8 years (median 2 years). HLA-B27 positivity was detected in 73.7% of 262 patients tested. Manifestations of extraarticular involvement were anterior uveitis (13.2%), psoriasis and other skin and mucous membrane lesions (6%) and inflammatory bowel disease (3.8%). The prevalence of peripheral arthritis was 11.2%. In 51.7% of patients, the Bath AS Disease Activity Index was ≥4. But since our patients consisted of the ones with more severe disease who referred to the tertiary centers and needed a regular follow-up, they may not represent the general AS population. Disease-modifying anti-rheumatic drugs were being used by 41.9% of patients, with 16.4% using anti-TNF agents. TRASD-IP (Izlem Programi: Follow-up program) is the first AS registry in Turkey. Such databases are very useful and provide a basis for data collection from large numbers of subjects. TRASD-IP gives information on the clinical and demographic profiles of patients, and the efficacy and safety of anti-TNF drugs, examines the impact on quality of life, and provides real-life data that may be used in cost-effectiveness analyses.
