1. Classical and integrated multicomponent geothermometry at the Tengchong geothermal field, Southwestern China

Abstract

To reconstruct deep fluid chemical composition and increase the confidence in estimated reservoir temperatures, a more integral geothermometry method was compared to other classical geothermometers. Here, we apply the integrated multicomponent geothermometry (IMG) method using the GeoT code to estimate reservoir temperatures at the Tengchong geothermal field in Southwestern China. Results show reservoir temperatures calculated using the quartz geothermometer are closest to those estimated with the IMG method. The concentrations of Al and Mg, as well as selected minerals for geothermometry computations, are key factors for successfully using the IMG. Using the IMG method together with classical geothermometers can significantly increase confidence in reservoir temperature estimations. The methods presented and simulation program used here may be useful for analysis of other geothermal fields under similar conditions.

Abstract

Stroke is the leading cause of long-term disability in the United States. (1,2) More than two-thirds of nearly 7 million stroke survivors in the United States (2) have persistent impairment [...], Background. Peripheral sensory stimulation has been used in conjunction with upper extremity movement therapy to increase therapy-induced motor gains in patients with stroke. The limitation is that existing sensory stimulation methods typically interfere with natural hand tasks and thus are administered prior to therapy, requiring patients' time commitment. To address this limitation, we developed TheraBracelet. This novel stimulation method provides subthreshold (ie, imperceptible) vibratory stimulation to the wrist and can be used during hand tasks/therapy without interfering with natural hand tasks. Objective. The objective was to determine the feasibility of using TheraBracelet during therapy to augment motor recovery after stroke. Design. The design was a triple-blinded pilot randomized controlled trial. Methods. Twelve chronic stroke survivors were assigned to die treatment or control group. All participants completed 2-hour task practice therapy sessions thrice weekly for 2 weeks. Both groups wore a small vibrator on the paretic wrist, which was turned on to provide TheraBracelet stimulation for the treatment group and turned off for the control group to provide sham stimulation. Outcome measures (Box and Block Test [BBT] and Wolf Motor Function Test [WMFT]) were obtained at baseline, 6 days after therapy, and at follow-up 19 days after therapy. Results. The intervention was feasible with no adverse events. The treatment group significantly improved their BBT scores after therapy and at follow-up compared with baseline, whereas die control group did not. For WMFT, the group x time interaction was short of achieving significance. Large effect sizes were obtained (BBT d = 1.43, WMFT d = 0.87). No indication of desensitization to TheraBracelet stimulation was observed. Limitations. The limitation was a small sample size. Conclusions. TheraBracelet could be a promising therapy adjuvant for upper extremity recovery after stroke.

The Strengths and Difficulties Questionnaire (SDQ) and General Health Questionnaire (GHQ) were used to measure the mental health of the students, their parents and the teacher in charge of the class, respectively. A total of 6173 students (72.73%) with full mental health information from all three caretakers were included in the final analysis.Results: We found a significantly elevated risk of mental health symptoms in children when their mothers (odds ratios (OR)=2.30, 95% CI=1.93– 2.73), fathers (OR=2.08, 95% CI=1.73– 2.50) and teachers (OR=1.18, 95% CI=1.01– 1.39) reported poorer mental health, and the risk increased significantly with the number of the caretakers with mental symptoms. A father with poor mental health has both direct and indirect effects on a child’s emotional health, by worsening the influence of a mother’s poor mental health.Conclusion: All three caretakers have a significant negative influence on schoolchildren’s emotional well-being, in the order of mother > father > teacher. 21. The degradation of performance of a state-of-the-art skin image classifier when applied to patient-driven internet search

Abstract

Model Dermatology (https://modelderm.com; Build2021) is a publicly testable neural network that can classify 184 skin disorders. We aimed to investigate whether our algorithm can classify clinical images of an Internet community along with tertiary care center datasets. Consecutive images from an Internet skin cancer community (‘RD’ dataset, 1,282 images posted between 25 January 2020 to 30 July 2021; https://reddit.com/r/melanoma) were analyzed retrospectively, along with hospital datasets (Edinburgh dataset, 1,300 images; SNU dataset, 2,101 images; TeleDerm dataset, 340 consecutive images). The algorithm’s performance was equivalent to that of dermatologists in the curated clinical datasets (Edinburgh and SNU datasets). However, its performance deteriorated in the RD and TeleDerm datasets because of insufficient image quality and the presence of out-of-distribution disorders, respectively. For the RD dataset, the algorithm’s Top-1/3 accuracy (39.2%/67.2%) and AUC (0.800) were equivalent to that of general physicians (36.8%/52.9%). It was more accurate than that of the laypersons using random Internet searches (19.2%/24.4%). The Top-1/3 accuracy was affected by inadequate image quality (adequate = 43.2%/71.3% versus inadequate = 32.9%/60.8%), whereas participant performance did not deteriorate (adequate = 35.8%/52.7% vs. inadequate = 38.4%/53.3%). In this report, the algorithm performance was significantly affected by the change of the intended settings, which implies that AI algorithms at dermatologist-level, in-distribution setting, may not be able to show the same level of performance in with out-of-distribution settings. © 2022, The Author(s).

Abstract

SrTiO3 is a well-studied n-type metal oxide based thermoelectric (TE) material. In this work, the first-principles calculation of La-doped SrTiO3 has been performed using the density functional theory. In addition, high TE properties of bulk SrTiO3 material have been achieved by introducing nanoscale porosity and optimizing carrier concentration by La doping. The X-ray diffraction, atomic resolution scanning transmission electron microscopy imaging, and energy-dispersive X-ray spectrometry results show that La has been doped successfully into the lattice. The scanning electron microscopy images confirm that all the samples have nearly similar nanoscale porosities. The significant enhancement of electrical conductivity over the broad temperature range has been observed through optimization of La doping. Additionally, the samples possess very low thermal conductivity, which is speculated because of the nanoscale porosity of the samples. Because of this dual mechanism of doping optimization and nanoscale porosity, there is a remarkable improvement in power factor, 1 mW/m2K from 650 to 800 K, and figure of merit, zT of 0.26 at 850 K, of the sample, 22 at. % La-doped SrTiO3.

Abstract

To evaluate the racial and ethnic differences in prevalence of germline pathogenic variants (PVs) and the effect of race and ethnicity on breast cancer (BC) risk among carriers, results of multigene testing of 77 900 women with BC (non-Hispanic White [NHW] = 57 003; Ashkenazi-Jewish = 4798; Black = 6722; Hispanic = 5194; and Asian = 4183) were analyzed, and the frequency of PVs in each gene were compared between BC patients (cases) and race- and ethnicity-matched gnomAD reference controls. Compared with NHWs, BRCA1 PVs were enriched in Ashkenazi-Jews and Hispanics, whereas CHEK2 PVs were statistically significantly lower in Blacks, Hispanics, and Asians (all 2-sided P < .05). In case-control studies, BARD1 PVs were associated with high risks (odds ratio > 4.00) of BC in Blacks, Hispanics, and Asians; ATM PVs were associated with increased risk of BC among all races and ethnicities except Asians, whereas CHEK2 and BRIP1 PVs were associated with increased risk of BC among NHWs and Hispanics only. These findings suggest a need for personalized management of BC risk in PV carriers based on race and ethnicity.

Abstract

PURPOSE: To determine the contribution of germline pathogenic variants (PVs) in hereditary cancer testing panel genes to invasive lobular carcinoma (ILC) of the breast. MATERIALS AND METHODS: The study included 2,999 women with ILC from a population-based cohort and 3,796 women with ILC undergoing clinical multigene panel testing (clinical cohort). Frequencies of germline PVs in breast cancer predisposition genes (ATM, BARD1, BRCA1, BRCA2, BRIP1, CDH1, CHEK2, PALB2, PTEN, RAD51C, RAD51D, and TP53) were compared between women with ILC and unaffected female controls and between women with ILC and infiltrating ductal carcinoma (IDC). RESULTS: The frequency of PVs in breast cancer predisposition genes among women with ILC was 6.5% in the clinical cohort and 5.2% in the population-based cohort. In case-control analysis, CDH1 and BRCA2 PVs were associated with high risks of ILC (odds ratio [OR] > 4) and CHEK2, ATM, and PALB2 PVs were associated with moderate (OR = 2-4) risks. BRCA1 PVs and CHEK2 p.Ile157Thr were not associated with clinically relevant risks (OR < 2) of ILC. Compared with IDC, CDH1 PVs were > 10-fold enriched, whereas PVs in BRCA1 were substantially reduced in ILC. CONCLUSION: The study establishes that PVs in ATM, BRCA2, CDH1, CHEK2, and PALB2 are associated with an increased risk of ILC, whereas BRCA1 PVs are not. The similar overall PV frequencies for ILC and IDC suggest that cancer histology should not influence the decision to proceed with genetic testing. Similar to IDC, multigene panel testing may be appropriate for women with ILC, but CDH1 should be specifically discussed because of low prevalence and gastric cancer risk.
