COVID-19 was identified by an International Classification of Diseases, tenth revision, clinical modification (ICD-10-CM) diagnosis code of U07.1. The primary outcome was HCU (ie, emergency department (ED) visits, readmissions, rehabilitation/skilled nursing facility (SNF) visits, outpatient office visits, and telemedicine visits) nine months post-discharge after COVID-19 hospitalization (from here on “post-discharge”) in patients with COPD compared to HCU of patients without COPD. Poisson regression modeling was used to calculate relative risk (RR) and confidence interval (CI) for COPD, adjusted for the other covariates.Results: We identified a cohort of 160,913 patients hospitalized with COVID-19, with 57,756 discharged home and 14,622 (25.3%) diagnosed with COPD. Patients with COPD had a mean age of 75.48 years (± 9.49); 55.5% were female and 70.9% were White. Patients with COPD had an increased risk of HCU in the nine months post-discharge after adjusting for the other covariates. Risk of ED visits, readmissions, length of stay during readmission, rehabilitation/SNF visits, outpatient office visits, and telemedicine visits were increased by 57% (RR 1.57; 95% CI 1.53– 1.60), 50% (RR 1.50; 95% CI 1.46– 1.54), 55% (RR 1.55; 95% CI 1.53– 1.56), 18% (RR 1.18; 95% CI 1.14– 1.22), 16% (RR 1.16; 95% CI 1.16– 1.17), and 28% (RR 1.28; 95% CI 1.24– 1.31), respectively. Younger patients (ages 18 to 65 years), women, and Hispanic patients with COPD showed an increased risk for post-discharge HCU.Conclusion: Patients with COPD hospitalized with COVID-19 experienced increased HCU post-discharge compared to patients without COPD.Keywords: COPD, Healthcare Utilization disease, COVID-19

The most neutron-rich boron isotopes 20B and 21B have been observed for the first time following proton removal from 22N and 22C at energies around 230 MeV/nucleon. Both nuclei were found to exist as resonances which were detected through their decay into 19B and one or two neutrons. Two-proton removal from 22N populated a prominent resonance-like structure in 20B at around 2.5 MeV above the one-neutron decay threshold, which is interpreted as arising from the closely spaced 1-,2- ground-state doublet predicted by the shell model. In the case of proton removal from 22C, the 19B plus one- and two-neutron channels were consistent with the population of a resonance in 21B 2.47+-0.19 MeV above the two-neutron decay threshold, which is found to exhibit direct two-neutron decay. The ground-state mass excesses determined for 20,21B are found to be in agreement with mass surface extrapolations derived within the latest atomic-mass evaluations., Comment: Accepted for publication in Physical Review Letters. The low-lying unbound level structure of the halo nucleus $^{19}\textrm{C}$ has been investigated using single-neutron knockout from $^{20}\textrm{C}$ on a carbon target at 280 MeV/nucleon. The invariant mass spectrum, derived from the momenta of the forward going beam velocity $^{18}\textrm{C}$ fragment and neutrons, was found to be dominated by a very narrow near threshold ($E_\textrm{rel}$ = 0.036(1) MeV) peak. Two less strongly populated resonance-like features were also observed at $E_\textrm{rel}$ = 0.84(4) and 2.31(3) MeV, both of which exhibit characteristics consistent with neutron $p$-shell hole states. Comparisons of the energies, measured cross sections and parallel momentum distributions to the results of shell-model and eikonal reaction calculations lead to spin-parity assignments of $5/2^+_1$ and $1/2^-_1$ for the levels at $E_x$ = 0.62(9) and 2.89(10) MeV with $S_n$ = 0.58(9) MeV. Spectroscopic factors were also deduced and found to be in reasonable accord with shell-model calculations. The valence neutron configuration of the $^{20}\textrm{C}$ ground state is thus seen to include, in addition to the known $1s^2_{1/2}$ component, a significant $0d^2_{5/2}$ contribution. The level scheme of $^{19}\textrm{C}$, including significantly the $1/2^-_1$ cross-shell state, is well accounted for by the YSOX shell-model interaction developed from the monopole-based universal interaction., Comment: Physics Letters B, in press

- Abstract
Beverages are an important part of the diet, but their environmental impact has been scarcely assessed. The aim of this study was to assess how changes in beverage consumption over a one-year period can impact the environmental sustainability of the diet. This is a one-year longitudinal study of 55-75-year-old participants with metabolic syndrome (n = 1122) within the frame of the PREDIMED-Plus study. Food and beverage intake were assessed using a validated food frequency questionnaire and a validated beverage-specific questionnaire. The Agribalyse® 3.0.1 database was used to calculate environmental impact parameters such as greenhouse gas emission, energy, water, and land use. A sustainability beverage score was created by considering the evaluated environmental markers. A higher beverage sustainability score was obtained when decreasing the consumption of bottled water, natural and packed fruit juice, milk, and drinkable dairy, soups and broths, sorbets and jellies, soft drinks, tea without sugar, beer (with and without alcohol), and wine, as well as when increasing the consumption of tap water and coffee with milk and without sugar. Beverage consumption should be considered when assessing the environmental impact of a diet. Trial registration: ISRCTN, ISRCTN89898870. Registered 5 September 2013.

Netherton syndrome (NS) is characterized by ichthyosis linearis circumflexa, trichorrhexis invaginata, and atopic diathesis. It is also considered a probable primary immunodeficiency as many patients have shown increased tendency for infections and abnormal levels of various immunoglobulins. Primary defect in NS is mutation in SPINK5 (serine protease inhibitor Kazal-type 5) gene which leads to the defective expression and function of lymphoepithelial Kazal-type-related inhibitor. We describe a case of a 7-year-old male born out of first-degree consanguineous marriage with findings of novel mutation, noninvolvement of the hair shaft, and unusual histopathology.

A wireless sensor network is preferable to a wired one for a smart building monitoring system since it is less expensive. The building monitoring system employs a hybrid protocol based on the LEACH concept for extending the life of sensor networks and the HRP protocol for efficient data collecting and connection with the base station. The building's infrastructure can support a huge number of sensors organised into multiple clusters. During the first stage, while the other sensors are dormant, the cluster head sensor activates a subset of member sensors to collect data. This extends the lifespan of the sensor network. Several more sensors are engaged to collect additional data for the Cluster Head in the event of an anomaly. Neural networks feature the right separation of information from cluster member nodes and communication between them. Information Collection Nodes (ICN) and Energy Search Optimization Monitoring Nodes (EOSMN)are added to each cluster in order to monitor energy levels and isolate data from cluster member Nodes in order to lessen the workload on the cluster head node. Modified minimum spanning tree algorithm selects the shortest and most advantageous path between two end nodes. The Energy search optimization algorithm examines the node information each time data is delivered to eliminate data loss entirely. During data transmission, any node can travel from its initial position, consume energy, and transform into a busy node to aid in the transmission of another node. Using the EOSMN-MSPE - method, the shortest path between the source and target nodes is determined. A network simulation is conducted using the NS2 software to develop the ESOMN algorithm. Validation and evaluation are performed on the simulation's outcomes. A hybrid protocol is developed to extend the lifespan of the Building Monitoring System and assure effective transmission. CRRT procedures vary depending on the country, region, and institution. It is not clear whether the dose of TAZ/PIPC for use in Japan can be determined based on studies conducted in other countries. Therefore, in this study, we examined the suitability of recommended dose in Japan. Methods: The study subjects consisted of 10 patients who received TAZ/PIPC during CRRT in the intensive care unit of Hyogo College of Medicine, Nishinomiya, Japan. We used a one-compartment model to characterize and parameterize the pharmacokinetics of TAZ/PIPC because their blood levels were eliminated monoexponentially. Results: Compared with the data of healthy adults, the half-lives (t1/2) of both PIPC and TAZ were prolonged while their clearance rates decreased. Conclusion: For the continuous hemodiafiltration procedure adopted in Japan, we concluded that the dose and frequency were appropriate because the patients who received PIPC/TAZ 2.25 g twice a day during continuous hemodiafiltration maintained appropriate blood levels of both PIPC and TAZ. - Abstract
Research related to sustainable diets is is highly relevant to provide better understanding of the impact of dietary intake on the health and the environment.To assess the association between the adherence to an energy-restricted Mediterranean diet and the amount of CO2 emitted in an older adult population.Using a cross-sectional design, the association between the adherence to an energy-reduced Mediterranean Diet (erMedDiet) score and dietary CO2 emissions in 6646 participants was assessed.Food intake and adherence to the erMedDiet was assessed using validated food frequency questionnaire and 17-item Mediterranean questionnaire. Sociodemographic characteristics were documented. Environmental impact was calculated through greenhouse gas emissions estimations, specifically CO2 emissions of each participant diet per day, using a European database. Participants were distributed in quartiles according to their estimated CO2 emissions expressed in kg/day: Q1 (≤2.01 kg CO2), Q2 (2.02-2.34 kg CO2), Q3 (2.35-2.79 kg CO2) and Q4 (≥2.80 kg CO2).More men than women induced higher dietary levels of CO2 emissions. Participants reporting higher consumption of vegetables, fruits, legumes, nuts, whole cereals, preferring white meat, and having less consumption of red meat were mostly emitting less kg of CO2 through diet. Participants with higher adherence to the Mediterranean Diet showed lower odds for dietary CO2 emissions: Q2 (OR 0.87; 95%CI: 0.76-1.00), Q3 (OR 0.69; 95%CI: 0.69-0.79) and Q4 (OR 0.48; 95%CI: 0.42-0.55) vs Q1 (reference).The Mediterranean diet can be environmentally protective since the higher the adherence to the Mediterranean diet, the lower total dietary CO2 emissions. - Abstract
Unbound states in C17 were investigated via one-neutron removal from a C18 beam at an energy of 245 MeV/nucleon on a carbon target. The energy spectrum of C17, above the single-neutron decay threshold, was reconstructed using invariant mass spectroscopy from the measured momenta of the C16 fragment and neutron, and was found to exhibit resonances at Er=0.52(2), 0.77(2), 1.36(1), 1.91(1), 2.22(3) and 3.20(1) MeV. The resonance at Er=0.77(2) MeV [Ex=1.51(3) MeV] was provisionally assigned as the second 5/2+ state. The two resonances at Er=1.91(1) and 3.20(1) MeV [Ex=2.65(2) and 3.94(2) MeV] were identified, through comparison of the energies, cross sections and momentum distributions with shell-model and eikonal reaction calculations, as p-shell hole states with spin-parities 1/21− and 3/21−, respectively. A detailed comparison was made with the results obtained using a range of shell-model interactions. The YSOX shell-model Hamiltonian, the cross-shell part of which is based on the monopole-based universal interaction, was found to provide a very good description of the present results and those for the neighbouring odd-A carbon isotopes – in particular for the negative parity cross-shell states.

Iontophoresis is a non-invasive method to improve drug delivery by the application of an electric field. The iontophoresis process causes deeper penetration of ions using electric current. The drug delivered through iontophoresis was found to be around 10 to 2,000 times more than conventional forms of delivery. The better results were shown by alternating current (AC) than conventional constant current (DC) iontophoresis. The preparation used in iontophoresis should be soluble in water, of a small voltage, and prone to ionization. More mobility is seen with smaller particles. Iontophoresis could increase the diffusion of drugs into dentin, enamel, and other oral tissues. The chief drugs delivered or studied by iontophoresis in dentistry are non-steroidal anti-inflammatory drugs, local anesthetics, anti-bacterial drugs, and fluorides. To enhance the ability of drug transfer nanomaterials were introduced. Under the impact of iontophoresis, remineralizing nanomaterial can be injected at larger concentrations in the deeper layer of incipient caries. Due to the size of nanocomplexes, it is possible that they will diffuse into the body of the subsurface lesion and enter the porosities to improve remineralization utilizing the iontophoresis approach. The concept of the application of an electric current for drug delivery was introduced several years ago in clinical practice, research, and literature. This review focuses on iontophoresis application in dentistry, its mode of action, and how the technique can be utilized in a beneficial way.

Background and Purpose: Both adherence to the Mediterranean diet (MedDiet) and the use of metformin could benefit the cognitive performance of individuals with type 2 diabetes, but evidence is still controversial. We examined the association between metformin use and cognition in older adults with type 2 diabetes following a MedDiet intervention. Methods: Prospective cohort study framed in the PREDIMED-Plus-Cognition sub-study. The PREDIMED-Plus clinical trial aims to compare the cardiovascular effect of two MedDiet interventions, with and without energy restriction, in individuals with overweight/obesity and metabolic syndrome. The present sub-study included 487 cognitively normal subjects (50.5% women, mean +/- SD age of 65.2 +/- 4.7 years), 30.4% of them (N = 148) with type 2 diabetes. A comprehensive battery of neurocognitive tests was administered at baseline and after 1 and 3 years. Individuals with type 2 diabetes that exhibited a good glycemic control trajectory, either using or not using metformin, were compared to one another and to individuals without diabetes using mixed-effects models with inverse probability of treatment weights. Results: Most subjects with type 2 diabetes (83.1%) presented a good and stable glycemic control trajectory. Before engaging in the MedDiet intervention, subjects using metformin scored higher in executive functions (Cohen's d = 0.51), memory (Cohen's d = 0.38) and global cognition (Cohen's d = 0.48) than those not using metformin. However, these differences were not sustained during the 3 years of follow-up, as individuals not using metformin experienced greater improvements in memory (beta = 0.38 vs. beta = 0.10, P = 0.036), executive functions (beta = 0.36 vs. beta = 0.02, P = 0.005) and global cognition (beta = 0.29 vs. beta = -0.02, P = 0.001) that combined with a higher MedDiet adherence (12.6 vs. 11.5 points, P = 0.031). Finally, subjects without diabetes presented greater improvements in memory than subjects with diabetes irrespective of their exposure to metformin (beta = 0.55 vs. beta = 0.10, P < 0.001). However, subjects with diabetes not using metformin, compared to subjects without diabetes, presented greater improvements in executive functions (beta = 0.33 vs. beta = 0.08, P = 0.032) and displayed a higher MedDiet adherence (12.6 points vs. 11.6 points, P = 0.046). Conclusions: Although both metformin and MedDiet interventions are good candidates for future cognitive decline preventive studies, a higher adherence to the MedDiet could even outweigh the potential neuroprotective effects of metformin in subjects with diabetes.

The heaviest bound isotope of boron ¹⁹B has been investigated using exclusive measurements of its Coulomb dissociation, into ¹⁷B and two neutrons, in collisions with Pb at 220 MeV/nucleon. Enhanced electric dipole (E1) strength is observed just above the two-neutron decay threshold with an integrated E1 strength of B(E1)=1.64±0.06(stat)±0.12(sys) e² fm² for relative energies below 6 MeV. This feature, known as a soft E1 excitation, provides the first firm evidence that ¹⁹B has a prominent two-neutron halo. Internet of Things (IoT) is a very relevant technology used by internet networks to send and receive sensed data via a sensor. The same relates to common data communication except that sensors and microcontrollers are commonly used in IoT. It is supposed to explore, and there will be developing interest in the IoT framework which gives frequent IoT system capabilities. It connects us to the Internet and also helps us to reveal and manage the actual world by using sharing its info. IoT systems make use of real-world data, so device-collected data may also be a tool for cyber attack. The attack surface also expands as IoT expands and all the vulnerabilities present in the digital world flow through our modern world. DDoS attacks built on compromised IoT systems emerge as a serious problem. There are many technological solutions, but technology has changed a lot, so software solution can be in risk as well. The proposed system will serve as a prevention tool for DDoS attack and send the admin an alert when an attempt is made to hack the IoT device. In this proposed system, intend to provide a highly secured platform that will clean out all the unnecessary data without disrupting IoT’s normal operation.

Netherton syndrome (NS) is characterized by ichthyosis linearis circumflexa, trichorrhexis invaginata, and atopic diathesis. It is also considered a probable primary immunodeficiency as many patients have shown increased tendency for infections and abnormal levels of various immunoglobulins. Primary defect in NS is mutation in SPINK5 (serine protease inhibitor Kazal-type 5) gene which leads to the defective expression and function of lymphoepithelial Kazal-type-related inhibitor. We describe a case of a 7-year-old male born out of first-degree consanguineous marriage with findings of novel mutation, noninvolvement of the hair shaft, and unusual histopathology.
