In this retrospective study the authors analised the clinical data of thirty-one patients with necrotizing soft-tissue infection admited at Hospital Estadual Alberto Torres between january 2019 and december 2022. The authors concluded that necrotizing softtissue infection was more common in men and the genito perineal region was the most affected. Treatment is based on early and extensive debridament, antibiotic therapy and maintenance of the surgical wound clean and free of necrotic tissue. [ABSTRACT FROM AUTHOR]

4. Complicações necrosantes e intercorrências na rinomodelação com ácido hialurônico

Objetivo: Descrever os efeitos adversos, intercorrências e complicações do preenchimento com ácido hialurônico na região de nariz, indicando possíveis causas para os problemas apresentados após o procedimento. Metodologia: Trata-se de uma revisão integrativa realizada nas seguintes bases de dados: Medline via PubMed, SciELO e Lilacs, com artigos incluídos no período 2013 a 2023, utilizando os descritores, Ácido hialurônico; Necrose; Preenchedores dérmicos; Nariz. A busca integrada foi realizada unindo os descritores com o operador booleano “AND”. Foram analisadas 9 publicações. Resultados: Foi realizada a identificação dos trabalhos selecionados, elencando-se os dados relacionados à título, autor, ano de publicação, tipo de estudo, metodologia, principais intercorrências encontradas e a conclusão do estudo. Após análise dos estudos, observou-se que a ocorrência de complicações após a aplicação de ácido hialurônico não é frequente. Para que não ocorra o agravamento, é necessário que os primeiros sinais e sintomas sejam analisados para assim seguir com o protocolo mais adequado a fim de prevenir ou reverter. Conclusão: Seguindo passos simples de segurança e tendo um conhecimento anatômico refinado, os preenchedores podem ser uma boa ferramenta para um aprimoramento seguro e abrangente da rinomodelação.

5. Results of transarterial chemoembolization of hepatocellular carcinoma as a bridging therapy to liver transplantation.

Objective: To evaluate the degree of tumor necrosis after transarterial chemoembolization (TACE), used as a bridging therapy in patients awaiting liver transplantation, and its effect on survival. Materials and Methods: This was a retrospective cohort study involving 118 patients submitted to TACE prior to liver transplantation, after which the degree of tumor necrosis in the explant and post-transplant survival were evaluated. Results: Total necrosis of the neoplastic nodule in the explant was observed in 76 patients (64.4%). Of the patients with total necrosis in the explanted liver, 77.8% had presented a complete response on imaging examinations. Drug-eluting bead TACE (DEB-TACE), despite showing a lower rate of complications than conventional TACE, provided a lower degree of total necrosis, although there was no statistical difference between the two. By the end of the study period, 26 of the patients had died. Survival was longer among the patients with total necrosis than among those with partial or no necrosis (HR = 2.24 [95% CI: 0.91-5.53]; p = 0.078). Conclusion: In patients undergoing TACE as a bridging therapy, total tumor necrosis appears to be associated with improved patient survival. [ABSTRACT FROM AUTHOR]

Parafimose é a incapacidade de retrair o pênis em direção à bainha ou prepúcio. Para correção da enfermidade podem ser empregadas diversas técnicas, dentre elas a falopexia, que é um procedimento para criação de uma aderência permanente entre o corpo do pênis e mucosa prepucial. O objetivo deste trabalho é relatar o caso de um canino, macho, da raça Pitbull, sete meses de idade, 18 kg, não castrado, atendido no Hospital Veterinário da Universidade de Passo Fundo com queixa de incapacidade de retração peniana. O diagnóstico foi obtido através de inspeção visual. O paciente foi encaminhado para procedimento cirúrgico para falopexia e orquiectomia eletiva. O animal apresentou notável recuperação após procedimento cirúrgico. A parafimose compromete a circulação e faz com que o pênis se torne edemaciado, podendo progredir para trombose do corpo esponjoso e necrose.

Apresentamos um caso de arterite de células gigantes com amaurose, seguida de necrose da língua, diagnosticado clinicamente com boa resposta terapêutica ao corticoide oral. Salientamos que os casos de necrose da língua são extremamente raros. A suspeita diagnóstica de arterite de células gigantes é de fundamental importância e mesmo na impossibilidade de realizar a biópsia de artéria temporal, não se deve retardar o início do tratamento sob pena de acarretar sequelas definitivas ou manifestações mais graves da doença.

Prostate artery embolization is a new minimal invasive technic to combat benign prostate hyperplasia. This new technic also comes with new complications. In this case report, we look at four patients in whom a necrosis of the glans penis occurred after the embolization of their prostate arteries. The cause of this complication is not yet clear in all of the patients and further research is still necessary. Patients who are considering an embolization of their prostate arteries need to be informed about this complication because of the drastic aesthetic outcomes that can sometimes be seen. Envenomation due to a Loxosceles spider bite is a rare event in France and no case concerning the breast has yet been reported. We report the case of a 48-year-old woman who presented at Saint-Etienne University Hospital with a pulsatile pain in her left breast two days after feeling a fleeting bite. An abscess was diagnosed and antibiotics were prescribed. A few days later, a necrotic zone appeared around the areola and 75 percent of the surrounding skin was inflamed. The patient reported a persistent fever and a burning pain in her breast. Loxoscelism was finally concluded and surgical debridement was undertaken, followed by supervised wound therapy and local skin care. Complete healing was achieved after 4 weeks of treatment. The diagnosis of loxoscelism is generally presumptive, as the bite is usually painless and the spider rarely captured. Its diagnosis should be considered when an abscess responds poorly to antibiotics. [ABSTRACT FROM AUTHOR]

11. Biological and molecular characterization of tomato spotted wilt virus (TSWV) infecting Chrysanthemum in India.

Tomato spotted wilt virus (TSWV) infecting chrysanthemum (Dendranthema grandiflora L.) has been identified as a major constraint for chrysanthemum cut-flower industries in Southern India. The disease symptoms include veinal necrosis, necrotic spots on leaves, deformed flowers, and stem necrosis. The presence of TSWV in chrysanthemum plants collected from farms at Kotagiri, Nedugula, and Yercaud in Tamil Nadu State, India, was confirmed by the triple antibody sandwich-enzyme-linked immunosorbent assay (TAS-ELISA). The pathogenicity of TSWV was established for three isolates (TN1KO, TN2NE, and TN3YE) by mechanical inoculation of sap into chrysanthemum cuttings. Host range experiments in different host plants indicated that the isolates expressed symptoms typical of TSWV in all hosts assayed. Furthermore, Nicotiana tabacum L. had maximum virus titre among the tested host plants, and therefore was identified as the best indicator and the propagative host. The nucleocapsid (N) gene of the three studied isolates shared 99.2–100% identity at the amino acid level among themselves and 96.5–99.6% identity with reference TSWV isolates. The non-structural movement protein (NSm) gene of studied isolates shared 99.0% identity among themselves and 96.0%–100% identity at the amino acid level with reference TSWV isolates. The phylogenetic relationship analyzed based on the amino acid sequence identity of the N gene revealed that the studied isolate clustered as a single group with all the TSWV isolates taken for comparison. However, analysis based on the NSm gene indicated that the Indian isolates grouped with isolates from South Korea, the USA, Italy, Spain, and Australia. [ABSTRACT FROM AUTHOR]

Stevens-Johnson's syndrome is a rare dermatological entity and potentially fatal, which is characterized by a process of necrosis of the keratinocytes and detachment of the skin. The majority of the cases occur due to the use of pharmacological drugs, mainly beta-lactams, aromatic anticonvulsants and allopurinol. The diagnosis is basically clinical and the treatment is focused not only on removing the drug responsible for the disease but also with clinical support. This report has as its main goal to contribute to medical awareness about the syndrome mentioned through the description of a clinical case of a patient admitted in Santa Casa de Franca in 2019. 14. The action of subconjunctival cyclophosphamide in rabbits.

Objective: To observe clinically, in rabbits, the side effects of topical injection of subconjunctival cyclophosphamide, studying its role as an antifibrotic drug. Methods: Prospective study in 20 albino rabbits of New Zealand race. All rabbits were treated with cyclophosphamide, 10mg/ml in a volume of 0.3 ml, in the left eye through subconjunctival injection. They were evaluated for 1, 7, 30, and 60 days after the procedure. All the animals were examined for the detection of ocular reactions such as necrosis, hyperemia, chemosis, secretion, opacity, and iritis. Other side effects as changes in the behavior, in the feed, and the water consumption were also evaluated. Results: It was observed that from the 20 rabbits studied, three rabbits (15%) showed side effects only at the 24 hours analysis. One rabbit (5%) presented hyperemia, one rabbit (5%) had hyperemia associated with iritis, and one rabbit (5%) presented hyperemia associated with secretion. These reactions were not observed at 1, 7, 30, and 60 days. Conclusion: Cyclophosphamide subconjunctival injection induces minor side effects on the conjunctiva of rabbits such as hyperemia, associated with iritis and secretion. [ABSTRACT FROM AUTHOR]

15. Nécrose digitale : une complication rare au cours du lupus

La nécrose digitale est une complication rare du lupus. Elle est inaugurale de la maladie dans seulement 0,2% des cas. Les mécanismes physiopathologiques de survenue de gangrène digitale au cours du Lupus érythomateux systémique (LES) sont complexes et impliquent : une maladie thromboembolique surtout liée à la présence d´anticorps antiphospholipides, une athérosclérose précoce dont l´origine est multifactorielle ainsi qu´une vascularite aussi bien des petits que des gros vaisseaux pouvant mimer une vasculopathie occlusive. Ces hypothèses sont attestées par les différents résultats histologiques et d´imagerie réalisée chez ces patients. En effet, dans plusieurs cas rapportés, l´écho-Doppler, voire l´artériographie ont permis de mettre en évidence une occlusion ou sténose des artères digitales. De plus, les biopsies réalisées mettaient en évidence une vascularite des vaisseaux de moyen et de petit calibre même en présence d´un écho-Doppler normal. La gravité de cette atteinte justifie le recours à une corticothérapie, qui doit être précoce et à forte dose, associée à un agent immunosuppresseur. En cas de syndrome des antiphospholipides associé, une anticoagulation curative est nécessaire. Un traitement immunomodulateur type rituximab a également été instauré chez certains patients avec une bonne évolution. Ces thérapeutiques permettant d´améliorer le pronostic, pourraient diminuer de façon significative le risque d´amputation. Nous rapportons l´observation d´une femme de 30 ans qui présentait une nécrose du gros orteil gauche occasionnant son amputation et une nécrose du deuxième orteil gauche avec des lésions de vascularite. Les anticorps anti-nucléaires et anti-DNA fortement positif confirmant le diagnostic de lupus. Le reset du bilan immunologique était négatif.

A cicatrização adequada das feridas cirúrgicas é influenciada pela técnica operatória, pela presença de intercorrências pós-cirúrgicas e pelos hábitos de vida do paciente. Relatamos caso de paciente submetido a exérese de carcinoma basocelular por cirurgia micrográfica de Mohs e reconstrução com enxertia cutânea palpebral. No pós-operatório houve necrose do enxerto devido má perfusão do leito cirúrgico imputada ao tabagismo. O tabagismo interfere na embebição plasmática e neovascularização do enxerto, além de promover estresse oxidativo e disfunção endotelial. O cirurgião deve orientar a suspensão do tabagismo por pelo menos quatro semanas antes do procedimento, a fim de evitar maiores riscos de complicações.

18. Diaporthe eres causes stem cankers and death of young apple rootstocks in Canada.

In the spring of 2018, necrotic and canker symptoms were observed on stems of young apple trees that were bench-grafted by growers on four rootstocks (EMLA 9, Supporter 4, M9-T337 and Pajam 2) in Annapolis Valley, Nova Scotia. The objective of this study was to isolate and identify the causal organism of the disease on apple rootstocks. The diseased tissue obtained from eight infected plants of the four rootstocks showing visible necrotic and canker symptoms was used for microorganism isolation on potato dextrose agar (PDA). More than 90% of the isolates on PDA yielded a similar type of fungal colony. Based on the colony and spore morphology and presence of both α and β spores, Diaporthe was proposed as a major pathogen from the symptomatic tissue. Analysis of partial sequences of four unlinked loci DNA-lyase (APN2), histone-3 (HIS), translation elongation factor 1 alpha (TEF1-α) and the internal transcribed spacer (ITS) 5.8S region of rDNA of the representative fungal isolates from young apple trees confirmed its identity as D. eres (Phomopsis fukushii). The pathogenicity of the representative D. eres isolate was confirmed on 1-year-old young apple trees bench-grafted on four different rootstocks in greenhouse tests. Necrotic and sunken canker symptoms like those observed on the nursery-grown young apple trees appeared on stems of all inoculated plants. The necrotic and canker lesions were observed 3 days after inoculation; they spread both directions from the inoculation point and led to the death of the young plants after 4 weeks. To our knowledge, this is the first report of D. eres causing stem canker and death of young apple rootstock in Canada. [ABSTRACT FROM AUTHOR]

Parafinoma decorre da implantação de óleo mineral na pele, sendo caracterizado por reação inflamatória do tipo corpo estranho com período de latência variável. As lesões iniciais são nódulos endurecidos, que podem ulcerar, fistulizar ou necrosar. Esta técnica foi muito utilizada para fins estéticos no século XX, não sendo mais realizada por médicos. Contudo, ainda é empregada por profissionais não médicos ou pelos próprios pacientes, principalmente na região genital, para melhoria da prática sexual. Relata-se caso de implante de óleo mineral no pênis, realizado por profissional não médico para fins estéticos, que evoluiu com necrose após três anos.

20. Gangrène de Fournier à Bujumbura : épidémiologie, facteurs étiologiques, traitement et pronostic

But : Déterminer les aspects épidémiologiques, étiologiques, thérapeutiques et évolutifs de la gangrène de Fournier. Patients et Méthodes : Il s'agissait d'une étude rétrospective et descriptive portant sur 10 patients ayant été traités dans quatre principaux hôpitaux publics de Bujumbura pour Gangrène de Fournier. Elle s'étend sur une période de deux ans. Résultats : La fréquence de la GF était de 0,102%. L'âge moyen était de 46,7 ans. Tous nos patients étaient masculins. Six malades sur 10 avaient consulté entre 5 et 15 jours après le début des signes. Les antécédents urogénitaux prédominent (40%). Les facteurs favorisants étaient le diabète (40%), la paraplégie (20%) et le VIH (10%). La GF secondaire représentait 7 cas dont 4 urogénitales, 2 cutanées et 1 proctologique. Les lésions se limitaient au périnée chez 80% des malades. L'examen bactériologique retrouvait l'Escherichia Coli (70%). Tous les décès (2 cas) avaient un ISGF de 10,5. L'association céphalosporine, aminoside et imidazole était principalement prescrite (60%). Le traitement chirurgical était essentiellement le débridement (90. La dérivation urinaire était faite chez tous les malades. Un séjour moyen était 93,6 jours. Les complications retrouvées étaient choc septique (20%), insuffisance rénale (40%) et anémie (20%). Conclusion : La GF reste une pathologie relativement rare. La consultation tardive explique le retard diagnostique. Les étiologies sont dominées par les causes urogénitales avec le diabète comme facteur de risque principal. La consultation précoce et le contrôle des facteurs favorisant réduiraient le taux de mortalité.

RESUMO Introdução: Ao longo das últimas duas décadas, houve um avanço exponencial no tratamento dos sinais causados pelo envelhecimento facial. A procura crescente por terapias menos invasivas estimulou a evolução dos biomateriais em direção ao produto ideal, buscando preencher os critérios de segurança, tais como biocompatibilidade e reversibilidade. O ácido hialurônico é o produto mais utilizado mundialmente para preenchimento facial, sendo rotineiro nos consultórios de cirurgia plástica. Mesmo com baixos índices de complicações, é prudente que o cirurgião plástico esteja atento aos sinais de oclusão vascular, pois a interrupção da evolução em direção à necrose e sequela permanente depende da rápida atuação médica. Sendo assim, o nosso serviço vislumbrou a necessidade da confecção de um protocolo de prevenção e tratamento, uma vez que tais intercorrências são graves e algumas vezes até mesmo irreversíveis. Métodos: Revisão sistemática da literatura entre janeiro de 2003 a janeiro de 2018, utilizando descritores de complicações vasculares após preenchimento facial com AH e o respectivo tratamento. Resultados: O preenchimento com AH apresenta baixo potencial de complicação quando realizado por profissionais habilitados. A hialuronidase, atualmente utilizada off-label, é capaz de hidrolisar o ácido hialurônico, mesmo na sua forma cross-linked. Se utilizada corretamente em tempo hábil, pode tratar possíveis complicações vasculares que naturalmente evoluiriam para danos irreversíveis. Para tanto, confeccionamos um protocolo de tratamento à luz das evidências atuais. Conclusão: Todo cirurgião plástico que atua com preenchimentos e ácido hialurônico, deve ter em mãos um protocolo e o material necessário para intervenção precoce. ABSTRACT Introduction: Over the past two decades, there has been an exponential advancement in treating signs of facial aging. The growing demand for less invasive therapies has stimulated the development of biomaterials toward better products, seeking to fulfill safety criteria, such as biocompatibility and reversibility. Hyaluronic acid (HA) is the most widely used facial filler worldwide, being routine in plastic surgery clinics. Even with low complication rates, it is prudent for the plastic surgeon to be attentive to the signs of vascular occlusion because the interruption of the progression towards necrosis and permanent sequelae depends on rapid medical action. Thus, our service saw the need to create a prevention and treatment protocol, since such complications are serious and sometimes even irreversible. Methods: A systematic review of the literature was conducted from January 2003 to January 2018, using descriptors of vascular complications after facial filling with HA and its treatment. Results: Filling with HA presents a low potential for complications when performed by qualified professionals. Hyaluronidase, which is currently used off-label, can hydrolyze HA, even in its cross-linked form. If used correctly in a timely manner, it can treat possible vascular complications that would progress to irreversible damage. Accordingly, we prepared a treatment protocol given the current evidence. Conclusion: Every plastic surgeon who works with fillers and HA must have a protocol and be aware of the necessary material for early intervention.

RESUMO Introdução: Terapia de pressão negativa ganha espaço na prática cirúrgica como intervenção para melhorar cicatrização. Pacientes pós-bariátricos submetidos a dermolipectomia abdominal apresentam maior risco de complicações locais. Há uma notável escassez de estudos brasileiros atuais acerca disso. O objetivo desse estudo é analisar a presença de complicações em pacientes submetidos a cirurgia de dermolipectomia pós-bariátrica com curativo de pressão negativa em incisões cirúrgicas fechadas. Método: Estudo descritivo que avaliou complicações de incisões cirúrgicas de 20 pacientes submetidos a cirurgia de dermolipectomia pós-bariátrica com terapia de pressão negativa. Dados tabulados no software Windows Excel e analisados no programa Statistical Package for the Social Sciences 18.0. Variáveis qualitativas foram apresentadas em frequência simples e quantitativas através de média, desvio padrão e amplitude. O estudo foi aprovado pelo CEP-UNISUL. Resultados: 20 pacientes submetidos a terapia de pressão negativa, sendo 80% (n=16) do sexo feminino, com idade média de 39,55 anos (±9,08). Incisão em âncora foi escolha em 70% (n=14) das cirurgias, com retirada média de tecido de 1940 gramas (±710,37) e tempo de hospitalização de 40,20 horas (±19,18), correspondendo a 1,66 diárias. Apenas 15% (n=3) dos pacientes apresentaram complicações (deiscência, seroma e hematoma, que aconteceram na mesma proporção). Não houve caso de necrose. Conclusão: Uso da terapia de pressão negativa em incisões cirúrgicas fechadas de dermolipectomia pós-bariátrica parece contribuir na redução das complicações pós-operatórias. ABSTRACT Introduction: Negative pressure therapy gains ground in surgical practice as an intervention to improve healing. Post-bariatric patients undergoing abdominal dermolipectomy are at increased risk of local complications. There is a notable dearth of current Brazilian studies on this. This study aims to analyze the presence of complications in patients undergoing post-bariatric dermolipectomy surgery with negative pressure dressing in closed surgical incisions. Method: Descriptive study that evaluated complications of surgical incisions in 20 patients undergoing post-bariatric dermolipectomy surgery with negative pressure therapy. Data tabulated in Windows Excel software and analyzed in the Statistical Package for the Social Sciences 18.0 program. Qualitative variables were presented in simple frequency and quantitative as mean, standard deviation, and amplitude. CEP-UNISUL approved the study. Results: 20 patients undergoing negative pressure therapy, 80% (n=16) female, mean age 39.55 years (±9.08). Anchor incision was chosen in 70% (n=14) of the surgeries, with an average tissue removal of 1940 grams (±710.37) and hospitalization time of 40.20 hours (±19.18), corresponding to 1,66 daily. Only 15% (n=3) of patients had complications (dehiscence, seroma, and hematoma, which occurred in the same proportion). There was no case of necrosis. Conclusion: The use of negative pressure therapy in closed surgical incisions of post-bariatric dermolipectomy seems to contribute to reducing postoperative complications.

RESUMO O uso do silicone líquido industrial como material para modificação estética no contorno corporal é uma prática realizada de forma clandestina há cerca de 60 anos. Atualmente, a maioria dos relatos provém de países da Ásia e América do Sul e as vítimas são principalmente mulheres e transexuais. Devido ao grande número de casos com complicações, o uso do silicone industrial para fins estéticos nunca foi aprovado. Entretanto, continua a ser aplicado isoladamente ou associado a outros produtos, determinando graves complicações locais e sistêmicas. Relata-se um caso de óbito de paciente transexual após injeção de silicone industrial em coxas e glúteos. ABSTRACT The use of industrial liquid silicone as a material for aesthetic modification of body contour is a practice that has been carried out clandestine for about 60 years. Currently, most reports come from countries in Asia and South America, and the victims are mainly women and transsexuals. Due to the large number of cases with complications, the use of industrial silicone for aesthetic purposes has never been approved. However, it continues to be applied alone or associated with other products, determining severe local and systemic complications. We report a case of death of a transsexual patient after injecting industrial silicone in the thighs and buttocks.

RESUMO Introdução: A pressão positiva contínua nas vias aéreas nasais (CPAP) é uma forma não invasiva de ventilação utilizada em recém-nascidos prematuros em unidades de terapia intensiva. Porém, ela pode acometer o nariz destes pacientes, evoluindo até mesmo com isquemia e necrose columelar. Diversas técnicas são descritas para reconstruir a columela, como enxertos de pele, enxertos compostos, retalhos locais e retalhos livres; mas o enxerto composto condrocutâneo auricular tem se destacado. O objetivo deste trabalho é descrever um caso de necrose de columela por uso de CPAP com reconstrução utilizando enxertia condrocutânea auricular posterior. Relato do Caso: Paciente do sexo feminino, parda, com história de prematuridade e uso prolongado de CPAP quando recém-nascida devido à síndrome da membrana hialina, evoluiu com necrose de columela. Foi submetida à enxertia composta condrocutânea auricular posterior para reconstrução da columela. A paciente apresentou resultado cirúrgico satisfatório, evoluindo com 100% de vitalidade do enxerto. Discussão: A necrose columelar associada ao uso do CPAP pode ser estética e funcionalmente debilitante, e representa um desafio reconstrutivo. As opções para obter resultados aceitáveis são limitadas, porém o uso de enxertos de orelha é tecnicamente simples, utiliza tecidos doadores estruturalmente semelhantes, não provoca cicatrizes adicionais no nariz, é realizado em um tempo cirúrgico e geralmente tem um resultado muito bom. A enxertia composta auricular posterior para reconstrução columelar se mostrou segura, com resultado estético e funcional satisfatório e mínima morbidade da área doadora. ABSTRACT Introduction: Continuous positive pressure in the nasal airways (CPAP) is a non-invasive form of ventilation used in premature newborns in intensive care units. However, it can affect the nose of these patients, even evolving with ischemia and columellar necrosis. Several techniques are described to reconstruct the columella, such as skin grafts, composite grafts, local flaps, and free flaps, but the atrial chondrocutaneous graft has stood out. This study aims to describe a case of columella necrosis using CPAP with reconstruction using posterior atrial chondrocutaneous grafting. Case Report: A brown, female patient, with a history of prematurity and prolonged use of CPAP when she was born due to hyaline membrane syndrome, developed columella necrosis. The patient underwent posterior auricular chondrocutaneous grafting to reconstruct the columella. She presented a satisfactory surgical result, evolving with 100% graft vitality. Discussion: Columellar necrosis associated with the use of CPAP can be aesthetically and functionally debilitating, and represents a reconstructive challenge. The options for obtaining acceptable results are limited. However, the use of ear grafts is technically straightforward, uses structurally similar donor tissues, does not cause additional scarring on the nose, is performed in a surgical period, and generally has an excellent result. Posterior auricular composite grafting for columellar reconstruction proved safe, with satisfactory aesthetic and functional results and minimal morbidity in the donor area.

RESUMO Objetivo: relatar a experiência da Divisão de Cirurgia Plástica do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo no tratamento de lesões cutâneas por extravasamento acidental de drogas. Métodos: foram incluídos os pacientes com lesões por extravasamento de drogas num período de 18 meses. Os seguintes parâmetros foram avaliados retrospectivamente: idade, diagnósticos durante internação e comorbidades, níveis séricos de albumina e hemoglobina, local de internação, droga envolvida, segmento anatômico acometido, conduta terapêutica e óbito durante internação. Resultados: foram acompanhados 14 pacientes. A principal droga envolvida foi a noradrenalina (21%). Todos os pacientes foram submetidos a desbridamento da necrose tecidual. Três pacientes foram submetidos a retalhos após preparo do leito com terapia por pressão negativa, com bons resultados. Sete pacientes não tiveram tratamento definitivo de suas lesões por falta de condições clínicas. Conclusão: o tratamento definitivo por meio de retalhos se mostrou adequado para os casos de feridas por extravasamento de drogas em tecido subcutâneo, quando há exposição de estruturas nobres, em pacientes com condições clínicas favoráveis.

- Abstract
Background: Testicular torsion is a rare condition in canines, affecting any animal, regardless of age and racial predisposition. The possible cause, when talking about young dogs, is often unreliable, and can be associated with traumatic processes or excessive physical activity. Dogs with spermatic cord twist usually show signs of acute and sudden pain in one of the testicles that can be propagated to the abdomen; besides edema, heat and scrotal flush. The absence of blood flow toward the affected testicle can be observed, however, the definitive diagnosis can only be confirmed through a surgical procedure or necropsy. This situation is considered a medical emergency that requires surgical intervention for resolution and has treatment of choice bilateral orchiectomy. The objective of this study was to report the case of a testicular torsion inside the scrotal pouch of a young dog. Case: A 6-month-old male Poodle dog, weighing 6.40 kg, was attended at the Metropolitan Veterinary Polyclinic, located in the municipality of Caucaia, Ceará, Brazil. During anamnesis it was reported by the owner of the animal an suddenly increase in volume of the scrotal pouch and right testicle. The physical examination observed normal mucous membranes, lymphadenopathy, capillary refill time of 2 s, temperature of 39.2°C, heart rate of 72 beats per minute and 68 breaths per minute. Hematologic test was requested, creatinine, dosage of ALT and ultrasound as complementary exams. The result of hemogram presented thrombocytopenia (150,000/mm3) and the presence of giant platelets. Besides, a biochemical series presented no alterations. On the Doppler ultrasound, the right testicle proved to show loss of normal structure, enlarged scrotum, hyperechoic epididymal region with adjacent liquid and absence of local vascularization. Pre-scrotal orchiectomy was performed and an enlarged right testicle was observed with blackened coloration and torsion of spermatic cord, that, possibly resulted in ischemia and testicular necrosis. On that occasion, the testicle was displaced to the incision line and maintained until the previous incision in the tunica albuginea, spermatic fascia and tunica vaginalis, parietal and visceral, in this order. After the identification of the torsioned spermatic cord, a ligature was made in order to remove testicle. The process was accomplished in both testicles. The contralateral gonad was found without alterations. The patient returned after fifteen days of surgical procedure for evaluation and presented evident improvement of overall clinical overview and surgery recuperation. Discussion: In spite of the testicular torsion be more associated to cases of senile and criptorquid animals, the present overview occurred in a testicle located in the interior of the scrotal sac in an extremely young animal. This can be related to a rupture of scrotal ligament, due to trauma or excessive physical effort, which fits overall overview of the patient in evidence. This results as presented in this report, in venous heart attack with testicle and epidimo becoming enlarged, congestioned with necrosis points. The rapid establishment of therapeutical surgery of orquiectomy can avoid the patients' instability and evolution of the overall clinical overview. The rarity in literature of the reports of testicle torsion at the clinic of small animals demonstrated the importance, need for further investigation and previous knowledge regarding the origin of the abnormality. The ultrasound and surgical treatment of orquiectomy demonstrated an alternative capable and efficient to produce a favorable prognostic of this report. [ABSTRACT FROM AUTHOR]

34. Ulcères nécrotiques sous antivitamine K, sans déficit en protéine C et S associé.

Résumé Introduction Les patients sous antivitamines K (AVK) représentent 1 % de la population en France. Nous rapportons un cas d’ulcères de jambe nécrotiques atypiques sous AVK. Observation Une patiente de 84 ans se présentait en consultation pour des ulcères nécrotiques de jambe, évoluant depuis 5 semaines, d’apparition spontanée, après l’introduction de fluindione. Le bilan étiologique était négatif, notamment le bilan de thrombophilie. La biopsie cutanée retrouvait un aspect compatible avec un pyoderma gangrenosum. L’évolution était favorable après arrêt de la fluindione et relais par apixaban avec cicatrisation complète en 5 mois. Conclusion Nous rapportons donc un cas original d’ulcères de jambe sous AVK sans déficit en protéine C ou S avec une histologie de type pyoderma-like. Les cas rapportés d’ulcères sous AVK sont rares et la physiopathologie est mal connue. L’implication des AVK doit être évoquée devant des ulcères nécrotiques sans étiologie spécifique retrouvée. Introduction Patients treated by vitamin K antagonists (VKA) represent 1% of the population in France. We report a case of atypical necrotic leg ulcers induced by VKA. Case report A 84-year-old woman was referred to our dermatology department because of necrotic leg ulcers that developed for the past 5 weeks, and appeared spontaneously after the introduction of a VKA, fluindione. The etiological assessment was non contributive, in particular the search for thrombophilic factors. The skin biopsy found an aspect compatible with pyoderma gangrenosum. The outcome was favorable after discontinuing the fluindione and the switch to apixaban. A complete healing was obtained in 5 months. Conclusion We report an original case of necrotic leg ulcers induced by VKA without deficit of protein C or S, with a pyoderma like histology. Reported cases of ulcers induced by VKA are uncommon and the physiopathology is not well known. The involvement of VKA should be evoked in case of necrotic leg ulcer without specific etiology found. A panleucopenia felina é uma importante doença infectocontagiosa de felinos domésticos, principalmente em animais com menos de um ano de idade. Este trabalho descreve os achados clinicopatológicos e o diagnóstico imuno-histoquímico de 33 casos de panleucopenia felina. Os principais sinais clínicos relatados foram vômito, diarreia e anorexia. As alterações mais frequentes na necropsia foram mucosa intestinal avermelhada (16/33), evidenciação das placas de Peyer (14/33) e conteúdo intestinal liquefeito (7/33). Os achados histológicos mais frequentes no intestino foram necrose (33/33) e infiltrado inflamatório linfo-histiocitário na mucosa (31/33), fusão (27/33) e atrofia de vilosidades (26/33). Em órgãos hematopoiéticos as alterações se caracterizavam principalmente por necrose e rarefação celular. Obteve-se resultado imuno-histoquímico positivo para parvovírus em 84,85% dos casos analisados. O intestino delgado foi o melhor órgão para detecção viral, com imunomarcação em 84,85%. Dentre os órgãos linfoides, o baço apresentou o melhor resultado, com 47,37% dos cortes analisados positivos. A pesquisa revelou importantes lesões no intestino delgado e em órgãos linfoides e a técnica da imuno-histoquímica demonstrou-se eficiente na detecção do parvovírus.

- Abstract
Corneal sequestration is a relatively common pathology in cats with fondness the brachycephalic breeds, without distinction of gender and age. Its cause isn't still well defined, however it is known that relates to chronic trauma to the cornea, such as dry keratoconjunctivitis dry and eyelid defects. It has also been observed the presence of this pathology in animals affected by feline herpes virus-1 (FHV-1), exposed to prior administration of topical corticosteroids and surgical intervention keratectomy network. Clinical examination found signs are usually pain, epiphora and blepharospasm. The cornea begins to have an area with plate appearance, with golden color to blackish. This is limited to occupy the central corneal region in the anterior half of the stroma, also having the power reach the Descemet's membrane. The treatment adopted will depend on the degree and time of the injury, and this may be drug or surgical. The recovery period is also dependent on two factors and the lesion can be optimized when chosen corneal surgical removal of the affected area. [ABSTRACT FROM AUTHOR]

Introduction: Endoscopic surgery is the standard treatment for benign prostatic hyperplasia (BPH) refractory to pharmacological treatments. In order to reduce invasiveness and preserve sexual function, prostatic artery embolization (PAE) has been developed. However, the technical difficulties of carrying out this procedure and the results, which have yet to be confirmed, mean that it is not currently recommended. The seriousness of the complications generated should lead to a reflection on the benefit-risk balance. The objective is to report a case of penile ischemia after embolization of the prostatic arteries., Material and Method: A severe complication following prostatic artery embolization (PAE) is reported with its clinical and paraclinical evaluation before and after the procedure as well as the therapeutic management., Results: Penile necrosis following prostatic artery embolization was reported in a 75-year-old patient despite an attempt of deobstruction. Lower urinary tract symptoms worsened postoperatively, associated with glans necrosis and refractory erectile dysfunction., Conclusion: The place of PAE in the therapeutic arsenal of BPH needs to be confirmed. This innovative technique exposes the patient to potentially severe risks such as penile ischemia, not encountered in conventional endoscopic surgical treatment. RESUMO A reação cutânea induzida por radiação (RCIR) é geralmente caracterizada por edema, hiperemia, fibrose, ulceração, dor e prurido na pele. Sabe-se que a radiação interrompe o processo normal de divisão e regeneração celular, resultando em dano que pode envolver prejuízo na função das células endoteliais, inflamação e até morte celular. A recuperação do dano tecidual pela radiação depende de múltiplos fatores relativos ao procedimento realizado e também intrínsecos ao paciente. Apresentamos caso atípico de RCIR, cujas lesões apresentaram comportamento muito imprevisível e de difícil manejo clínico. Além disso, ressalta-se a importância da intervenção cirúrgica neste caso, fundamental para o tratamento adequado do paciente.

RESUMO Introdução: Fraturas complexas e extensas lesões de pele estão cada vez mais comuns devido aos traumas de alta energia. Uma alternativa para o tratamento dessas lesões nos membros inferiores é a utilização do retalho sural. Métodos: Esse foi um estudo retrospectivo, analítico-descritivo de análise exploratória documental de pacientes submetidos ao retalho sural em um hospital de referência em trauma do norte de Santa Catarina, Brasil. Foram analisados a idade, sexo, lateralidade, causa, local e tamanho da lesão, uso de tunelização e enxerto de pele, complicações e seus fatores de risco, além do manejo de tais complicações. Resultados: A amostra do estudo foi composta por 16 pacientes, com média de idade de 44,4 anos, 87,5% eram do sexo masculino. A causa da lesão mais prevalente foi trauma (75,0%) e o local da lesão foi mais prevalente na tíbia distal (43,8%). Em 50,0% dos casos os fatores de risco para as complicações estavam presentes, sendo que pacientes com diabetes mellitus e tabagistas exibiram 5 vezes mais chances de apresentar tais complicações. Necrose parcial teve uma prevalência de 25,0%, sendo que em 18,8% foi realizado apenas debridamento e em 6,3% enxertia. Conclusão: O retalho sural é uma boa alternativa para a cobertura de lesões dos membros inferiores devido ao bom índice de sucesso, mas não está livre de complicações. Tais complicações são mais prevalentes em pacientes que possuem fatores de risco como o tabagismo e diabetes mellitus. ABSTRACT Introduction: Complex fractures and extensive skin lesions are increasingly common due to high-energy traumas. An alternative for treating these lesions in the lower limbs is the use of the sural flap. Methods: This was a retrospective, analytical-descriptive study of exploratory documental analysis of patients submitted to the sural flap in a trauma reference hospital in northern Santa Catarina, Brazil. Age, sex, laterality, cause, place, and size of the lesion, use of tunneling and skin grafting, complications and their risk factors, and the management of such complications were analyzed. Results: The study sample consisted of 16 patients with a mean age of 44.4 years; 87.5% were male. The cause of the most prevalent lesion was trauma (75.0%), and the site of the lesion was more prevalent in the distal tibia (43.8%). In 50.0% of the cases, risk factors for complications were present, and patients with diabetes mellitus and smokers were five times more likely to present such complications. Partial necrosis had a prevalence of 25.0%, and in 18.8%, only debridement was performed, and 6.3% grafting was performed. Conclusion: The sural flap is a good alternative for covering lower limbs lesions due to its good success rate, but it is not free of complications. Such complications are more prevalent in patients who have risk factors such as smoking and diabetes mellitus.

Severe dengue virus (DENV) infection is characterized by exacerbated inflammatory responses that lead to endothelial dysfunction and plasma leakage. We have recently demonstrated that Toll-like receptor 2 (TLR2) on blood monocytes senses DENV infection leading to endothelial activation. Here, we report that non-infectious immature DENV particles, which are released in large numbers by DENV-infected cells, drive endothelial activation via the TLR2 axis. We show that fully immature DENV particles induce a rapid, within 6 hours post-infection, inflammatory response in PBMCs. Furthermore, pharmacological blocking of TLR2/TLR6/CD14 and/or NF-kB prior to exposure of PBMCs to immature DENV reduces the initial production of inter alia TNF-α and IL-1β by monocytes and prevents endothelial activation. However, prolonged TLR2 block induces TNF-α production and leads to exacerbated endothelial activation, indicating that TLR2-mediated responses play an important role not only in the initiation but also the resolution of inflammation. Altogether, these data indicate that the maturation status of the virus has the potential to influence the kinetics and extent of inflammatory responses during DENV infection.

INTRODUÇÃO: Os ferimentos descolantes de membros inferiores geralmente se caracterizam como lesões graves e apresentam dificuldades na decisão quanto ao tratamento cirúrgico mais adequado a ser instituído, se reposicionamento do retalho avulsionado ao leito da ferida ou ressecção do retalho, seguido de seu adelgaçamento e enxertia de pele. O propósito deste estudo foi desenvolver um modelo experimental de avulsão de retalhos cutâneos em membros inferiores de ratos e observar a viabilidade do retalho após seu reposicionamento ao leito de origem, com a finalidade de melhor estudar as alterações relacionadas ao ferimento e de testar modalidades terapêuticas em retalhos avulsionados. MÉTODO: Foram utilizados 90 ratos Wistar machos, subdivididos em 4 grupos experimentais. Foi delineado um modelo de avulsão de retalhos no membro inferior do rato, baseado em 4 pedículos diferentes: pedículo de fluxo proximal (G1), pedículo de fluxo distal (G2), pedículo de fluxo lateral (G3) e pedículo de fluxo medial (G4). RESULTADOS: A comparação entre as médias de área de necrose do retalho desenluvado evidenciou diferença estatística significativa entre os 4 grupos estudados (P < 0,0001). CONCLUSÕES: O grupo com pedículo de fluxo distal (G2) apresentou maior área de necrose em relação à área total do retalho, sendo o mais adequado para testar agentes terapêuticos no retalho avulsionado.BACKGROUND: The purpose of this study was to develop an experimental model for degloving injuries of the hind limbs of rats and observe flap viability after its relocation to the wound bed to better study the changes related to the injury and to test the therapeutic modalities in avulsed flaps. METHODS: Ninety male Wistar rats were divided into 4 experimental groups. A flap avulsion model on the lower limb of a rats was established, using 4 different pedicles: proximal flow pedicled flap (G1), distal flow pedicled flap (G2), lateral flow pedicled flap (G3), and medial flow pedicled flap (G4). RESULTS: Comparison between the mean necrotic area of the degloved flap showed statistically significant differences among the 4 groups (P < 0.0001). CONCLUSIONS: The group with the distal flow pedicled flap (G2) showed a higher necrotic area relative to the total flap area and it is the most suitable group for testing therapeutic agents in avulsed flaps.

The goal of this study was to determine the symptoms and microscopic damage caused by fluoride on Spondias dulcis, a fluoride-sensitive species. The plants were exposed to simulated fog with fluoride (0, 5, 10, 15 and 20 mg L-1) for 20 min daily during four consecutive days. Samples from leaflets without any apparent fluoride injury were collected to microscopic analysis. The percentage of necrosed leaf area was measured, and the level of pollutant in the dry matter from the basal and apical portions of the plant was determined. The necroses began 24 h after the first simulation mainly from the base of the leaflets. A higher level of necrosis was observed at the apical portion of the plants, a region of higher fluoride accumulation. The damage on the surface of the leaflets was characterized as plasmolysis, erosion of the epicuticular waxes and epidermal rupture. Structurally, the noticeable accumulation of granules and droplets green stained by toluidine blue in the spongy parenchima and the boundaries of ending veinlets was observed. The limb thickness reduction occurred due to plasmolysis in the mesophyll, showing an apparent correlation with the damage observed on the surface. The parameters observed in the laboratory are promising for field biomonitoring studies.O objetivo deste estudo foi determinar os sintomas e danos microscópicos causadas pelo flúor em Spondias dulcis, espécie sensível a essa substância. As plantas foram expostas a nevoeiros simulados com flúor (0, 5, 10, 15 e 20 mg L-1) por 20 minutos diários, durante quatro dias consecutivos. Amostras de folíolos sem qualquer lesão aparente ao flúor foram coletadas para análise microscópica. Mensurou-se a percentagem de área foliar necrosada e o teor do poluente na matéria seca das porções basal e apical da planta. As necroses surgiram 24 horas após a primeira simulação e tiveram início, principalmente, a partir da base dos folíolos. As necroses foram mais severas na porção apical das plantas, região com maior acúmulo de flúor. O dano na superfície dos folíolos foi caracterizado como plasmólise, erosão das ceras epicuticulares e ruptura da epiderme. Estruturalmente, observou-se considerável acúmulo de grânulos e gotas coradas de verde pelo azul de toluidina no parênquima lacunoso e nas proximidades das terminações xilemáticas. Houve redução da espessura do limbo devido a plasmólise no mesofilo, havendo aparente correlação com os danos observados na superfície. Os parâmetros observados em laboratório são promissores para estudos de biomonitoramento em campo.

INTRODUÇÃO: Hematoma é a complicação pós-operatória mais frequente em ritidoplastia. Sua ocorrência aumenta a morbidade e prejudica a recuperação. Pontos de adesão internos para fechamento das áreas descoladas em abdominoplastias evitam o surgimento de seroma. Baseados neste princípio, e com o objetivo de reduzir o número de pacientes com hematomas em ritidoplastia, foi desenvolvida tática cirúrgica análoga em que se confecciona uma rede hemostática de pontos contínuos e transfixantes de náilon 4-0, englobando a pele e o SMAS-platisma. MÉTODO: Foram incluídos no estudo 366 pacientes consecutivos, submetidos a ritidoplastia de pelo menos o terço médio da face, entre julho de 2009 e setembro de 2011. O grupo A, incluindo os primeiros 120 pacientes avaliados retrospectivamente, foi considerado controle. O grupo B foi constituído pelos demais 246 pacientes, operados com a tática cirúrgica proposta e avaliados prospectivamente. Observaram-se as incidências de hematoma, isquemia e necrose nas primeiras 72 horas de pós-operatório. RESULTADOS: No grupo A, 17 (14,2%) pacientes apresentaram hematoma, enquanto no grupo B não houve nenhum caso. A tática cirúrgica não aumentou significativamente a ocorrência de isquemia: 11 (9,2%) pacientes no grupo A e 16 (6,5%) no grupo B tiveram essa complicação (P < 0,3964). Também não houve alteração significativa na incidência de necrose, complicação observada em 3 (2,5%) pacientes do grupo A e em 4 (1,6%) do grupo B (P < 0,4298). CONCLUSÕES: A rede hemostática é método eficaz na prevenção de hematomas precoces em ritidoplastias. Essa tática cirúrgica não levou a aumento significativo da incidência de isquemia e necrose.BACKGROUND: Hematoma is the most common postoperative complication of rhytidoplasty. Its occurrence increases morbidity and impairs recovery. Internal sutures to close detached areas are used in abdominoplasty to prevent seromas. Taking this into account and in order to reduce the number of patients with hematomas post rhytidoplasty, we have developed a similar surgical procedure in which a hemostatic net is made of continuous nylon 4-0 transfixing stitches to include the skin and the superficial musculoaponeurotic system-platysma. METHODS: A total of 366 patients who underwent rhytidoplasty of at least the middle third of the face between July 2009 and September 2011 were included in the study. Group A was identified as the control group; it included the first 120 patients assessed retrospectively. The remaining 246 patients, who underwent the surgical procedure here proposed and who were evaluated prospectively formed group B. We observed the incidence of hematoma, ischemia, and necrosis in the first 72 postoperative hours. RESULTS: In group A, 17 (14.2%) patients developed hematomas, whereas none occurred in group B. The surgical procedure did not significantly increase the occurrence of ischemia: 11 (9.2%) patients in group A and 16 (6.5%) in group B had this complication (P < 0.3964). There was also no significant increase in the incidence of necrosis, which was observed in 3 (2.5%) patients in group A and 4 (1.6%) in group B (P < 0.4298). CONCLUSIONS: The hemostatic net is an efficient method for the prevention of early hematomas following rhytidoplasty. This surgical procedure did not result in a significant increase in the incidence of ischemia and necrosis.

PURPOSE: To investigate the influence of intravenous nonselective cyclooxygenase inhibitor, ketoprofen (keto), on kidney histological changes and kidney cytokines, tumor necrosis factor-α (TNF-α) and interleukin-1 (IL-1), levels after hemorrhage of 30% of volemia (three times 10%, intervals of 10 min) in rats. METHODS: Under sevoflurane (sevo) anesthesia, sevo and sevo+keto groups (10 rats each) were instrumented for Ringer solution (5mL/kg/h) administration and mean arterial pressure (MAP) evaluation, plus keto (1.5mg/kg) administration in sevo+keto group in the beginning of anesthesia. Rectal temperature was continuously measured. The baseline data of temperature and MAP were collected at the first hemorrhage (T1), the third hemorrhage (T2) and 30min after T2 (T3). Bilateral nephrectomy was achieved for histology and immunohistochemistry. RESULTS: In both groups, temperature and MAP diminished from initial values. Hypothermia was greater in sevo group (p=0.0002). Tubular necrosis was more frequent in sevo group (p=0.02). The studied cytokines were equally present in the kidneys of both groups. CONCLUSION: Ketoprofen was more protective to the rat kidney in condition of anesthesia with sevoflurane and hypovolemia, but it seems that TNF-α and IL-1 were not involved in that protection.OBJETIVO: Investigar a influência do inibidor não-seletivo da ciclooxigenase, cetoprofeno (ceto) intravenoso, em alterações histológicas e dos níveis das citocinas renais - fator α de necrose tumoral (TNF- α) e interleucina 1 (IL-1) - após hemorragia de 30% da volemia (10%, três vezes, em intervalos de 10 min). MÉTODOS: Sob anestesia com sevoflurano (sevo), os grupos sevo e sevo+ceto (10 ratos cada) foram preparados cirurgicamente para leitura de pressão arterial média (PAM) e administração de solução de Ringer (5 mL/kg/h) e de cetoprofeno (1,5 mg/kg), no início da anestesia, no grupo sevo+ceto. Mediu-se temperatura retal continuamente. Os valores de temperatura e PAM foram observados antes da primeira hemorragia (T1), após a terceira hemorragia (T2) e 30 min após T2 (T3). Realizada nefrectomia bilateral nos dois grupos para análise histológica e imuno-histoquímica. RESULTADOS: Nos dois grupos, temperatura e PAM diminuíram com relação aos valores basais. Hipotermia foi mais acentuada no grupo sevo (p=0,0002). Necrose tubular foi mais frequente no grupo sevo (p=0,02). As citocinas estiveram igualmente presentes nos rins dos dois grupos. CONCLUSÃO: Cetoprofeno foi mais protetor no rim de rato durante anestesia com sevoflurano e hipovolemia, porém parece que TNF- α e IL-1 não estão envolvidas nessa proteção.

A síndrome da dilatação volvo gástrica (DVG) é uma condição grave, de caráter agudo, que confere alto índice de óbito em pequenos animais. A etiologia não está completamente estabelecida e, em contrapartida, diversas possibilidades de tratamento são descritas. A DVG causa grave redução na perfusão tecidual, afetando vários órgãos, incluindo os sistemas respiratório e cardiovascular. Este estudo tem como objetivo realizar uma revisão de literatura sobre a patogenia desta síndrome e seu tratamento.The syndrome of gastric dilatation volvulus (GDV) is a severe condition of acute character, which gives a high rate of death in small animals. The etiology is not fully established, however, several treatment options have been described. The DVG causes severe reduction in tissue perfusion, affecting many organs, including the respiratory and cardiovascular systems. This study aims to conduct a comprehensive literature review of the pathogenesis of this syndrome as well as its treatment.

BACKGROUND: Microcirculation dysfunction, as a consequence of localized vascular insufficiency, is considered to be one of the dominant causes of surgical flap necrosis. Several vasoactive drugs have been tested for the pharmacological treatment of tissue ischemia, with varying degrees of success. This study aimed to assess the impact of buflomedil and sildenafil on the viability of random skin flaps in rats. METHODS: Caudally pedicled skin flaps (10 x 3 cm) were created on the backs of rats. The animals were randomly assigned, in groups of 10, to three treatment groups: one group served as the vehicle control group, one group received buflomedil (10 mg/kg/d, orally), and a third group received the same dosage of sildenafil. Following seven days of dosing, the animals were sacrificed, and the viable flap area was determined. RESULTS: The average viable flap area for each group was: 16.2 ± 3.56 cm² (control group), 17.69 ± 2.54 cm² (buflomedil group), and 18.28 ± 3.74 cm² (sildenafil group). Data analysis by the Kruskal-Wallis test failed to show a statistically significant difference between the three groups. CONCLUSIONS: Neither buflomedil nor sildenafil showed a reduction in the necrotic area of random skin flaps in rats.INTRODUÇÃO: A insuficiência no aporte sanguíneo e a consequente disfunção gerada no fluxo da microcirculação são consideradas causas dominantes de sofrimento de um retalho cirúrgico. Várias drogas vasoativas têm sido testadas para o tratamento farmacológico da isquemia tecidual, porém com graus variáveis de sucesso. Este estudo teve como objetivo avaliar a influência do buflomedil e do sildenafil na viabilidade de retalhos cutâneos ao acaso, em ratos. MÉTODO: Foram confeccionados retalhos cutâneos no dorso de ratos, com dimensões de 10 x 3 cm e base caudal. Foram utilizados 30 ratos, divididos em três grupos de 10 ratos cada: um grupo que recebeu apenas o veículo da solução (grupo controle); um grupo que recebeu buflomedil (grupo buflomedil); e um terceiro grupo que recebeu sildenafil (grupo sildenafil). A via de administração foi a oral e a dose foi de 10 mg/kg/dia para cada droga, durante sete dias. Ao final desse período, os animais foram sacrificados, sendo realizada a determinação das áreas viáveis dos retalhos. RESULTADOS: A média das áreas viáveis dos retalhos foi de 16,2 ± 3,56 cm² para o grupo controle, de 17,69 ± 2,54 cm² para o grupo buflomedil, e de 18,28 ± 3,74 cm² para o grupo sildenafil. A análise dos dados pelo teste de Kruskal-Wallis não demonstrou significância estatística entre os três grupos. CONCLUSÕES: A utilização do buflomedil e do sildenafil demonstrou não diminuir a área de necrose de retalhos randomizados em ratos.
