RESUMO Objetivo: O objetivo deste estudo é avaliar se o método de Semmes-Weinstein tem resultados consistentes e verificar se existem diferenças de sensibilidade entre as regiões da parede abdominal. Métodos: Os monofilamentos de Semmes-Weinstein foram utilizados para avaliar a sensibilidade da pele à pressão abdominal em 20 voluntárias. A pele abdominal foi dividida em nove regiões. A avaliação da sensibilidade cutânea à pressão foi realizada três vezes em cada área, com um intervalo de uma semana entre as medidas. Resultados: Analisando-se as três medidas em cada área, isoladamente, não houve diferenças significativas no limiar pressórico. No entanto, o método de Semmes-Weinstein mostrou diferença estatisticamente significativa entre os valores pressóricos obtidos de cada região abdominal. Conclusões: Os monofilamentos de Semmes-Weinstein são consistentes para avaliar a sensibilidade da pele abdominal à pressão. Notou-se que o método de Semmes-Weinstein mostrou diferença estatisticamente significativa entre as diferentes áreas da região abdominal. Utilizando-se a metodologia do presente estudo, foi possível estabelecer um protocolo para reduzir o aspecto subjetivo, medindo a sensibilidade à pressão. ABSTRACT Objective: The aim of this study is to evaluate whether Semmes-Weinstein monofilaments could be used to reliably assess the sensitivities of different abdominal wall regions. Methods: Semmes-Weinstein monofilaments were used to evaluate skin sensitivity to abdominal pressure in 20 volunteers. The abdominal skin was divided into nine regions. The assessment of skin sensitivity to pressure was carried out three times in each area, with 1 week intervals between measurements. Results: No significant differences in the pressure threshold were observed when the three measurements in each region were analyzed separately. However, with the Semmes-Weinstein monofilament technique, a statistically significant difference was observed between the values of pressure obtained in each abdominal region. Conclusions: SemmesWeinstein monofilaments are a reliable tool in evaluating abdominal skin sensitivity to pressure. A statistically significant difference was observed between the different areas of the abdominal region with this technique. With the method reported in this study, it was possible to establish a protocol to reduce subjectivity and measure the skin sensitivity to pressure.

RESUMO Introdução: A correção da diástase dos músculos retos do abdome é realizada principalmente pela sua plicatura, procedimento que consome muito tempo cirúrgico. O objetivo deste estudo é avaliar a eficácia e o tempo necessário para a correção da diástase dos retos do abdome comparando a plicatura com sutura contínua em plano único à plicatura em dois planos. Método: Foram incluídas 20 mulheres com história de pelo menos uma gestação. Foram randomizadas em dois grupos, um submetido à plicatura dos retos do abdome em dois planos (controle) e outro à sutura em plano único contínuo (estudo) com mononylon 2-0. Mediu-se o tempo necessário para a realização de cada uma das técnicas. Para análise estatística, foram usados os testes não paramétricos de Mann-Whitney, Friedman e Wilcoxon, considerando estatisticamente significante p < 0,05. Todas as pacientes foram submetidas à ultrassonografia previamente à cirurgia, 3 semanas e 6 meses de pós-operatório. Resultados: A plicatura do grupo estudo mostrou-se tão eficaz quanto à do grupo controle, mantendo seu resultado em longo prazo, apresentando diferença estatisticamente significante (p = 0,018) na comparação das medidas pré e pós-operatórias. No grupo estudo, porém, a plicatura consumiu menor tempo cirúrgico para ser realizada, também com diferença estatisticamente significante (p = 0,002). Conclusão: Com este estudo, pode ser concluído que tanto a sutura em dois planos como a em plano único contínuo são eficazes na correção da diástase dos retos do abdome e mantêm o resultado por longo tempo. Concluiu-se também que a técnica de sutura contínua demanda menor tempo para ser realizada. ABSTRACT Introduction: The correction of diastasis of the rectus abdominis muscles is primarily performed using the plication technique, which involves a surgical procedure. To evaluate the efficacy and the surgical time necessary to correct diastasis of the rectus abdominis muscles using plication and sutures in one continuous or two planes. Method: Twenty women with at least one pregnancy were included in the study. The study participants were randomized into two groups: the control group was subjected to plication in two planes, and the experimental group was subjected to plication using suture in one continuous plane with mono-nylon 2-0. The time required to perform each of these techniques was measured. For statistical analysis, the nonparametric Mann-Whitney test, Friedman test, and Wilcoxon test were used, and p-values of < 0.05 were considered statistically significant. All patients underwent ultrasonography before surgery, and at 3 weeks and 6 months postoperatively. Results: The plication technique was similarly effective in the two groups, achieving a long-term surgical outcome; however, the differences in the ultrasound measurements in the preand postoperative periods were statistically significant (p = 0.018). In addition, plication was performed in a significantly shorter time in the experimental group (p = 0.002). Conclusion: Sutures in one continuous plane and in two planes were effective in correcting diastasis of the rectus abdominis muscles, with the maintenance of a long-term surgical outcome. In addition, the continuous suture technique can be performed in a shorter time.

RESUMO Pectus excavatum (PE) é uma deformidade torácica congênita, caracterizada como uma depressão profunda no esterno. O transplante autólogo de gordura tem sido utilizado para fins estéticos, principalmente na face, e recentemente ganhou relevância nas cirurgias torácica e das mamas. O objetivo do presente estudo é apresentar um caso de PE leve associado a hipomastia. Uma paciente de 24 anos compareceu à consulta para mamoplastia de aumento por hipomastia, mas o exame clínico também revelou um PE leve associado que não foi percebido pela paciente. O planejamento cirúrgico incluiu a mamoplastia de aumento e a transferência de gordura autóloga. Foi utilizado um implante mamário de silicone de 260ml, e uma quantidade total de 250ml de gordura foi injetada na região esternal e no contorno medial inferior das mamas. Não houve complicações durante o período de acompanhamento de 12 meses. A associação de mamoplastia de aumento e transplante de gordura no tratamento da deformidade de PE revelou-se uma opção minimamente invasiva, boa, segura e com alta satisfação da paciente. ABSTRACT Pectus excavatum(PE) is a congenital chest deformity characterized by deep depression in the sternum. Autologous fat transplantation has been used for aesthetic purposes, mainly on the face, and has recently gained relevance in thoracic and breast surgeries. The present study aims to present a case of mild PE associated with hypomastia. A 24-year-old female patient attended the consultation for breast augmentation due to hypomastia, but the clinical examination also revealed an associated mild PE that the patient did not notice. Surgical planning included breast augmentation and autologous fat transfer. A 260ml silicone breast implant was used, and 250ml of fat was injected in the sternal region and the lower medial contour of the breasts. There were no complications during the 12-month follow-up period. The combination of augmentation mammoplasty and fat transplantation in treating PE deformity proved to be a minimally invasive, good, safe option with high patient satisfaction.

Capsular contraction is a frequent complication following breast augmentation. On the other hand, capsular weakness, a not widely recognized complication, may occur around the implant. A weak capsule allows the migration of the prosthesis to the lateral region of the thoracic region or inferiorly, towards the abdomen, due to gravitational forces. The cause of capsular weakness remains unresolved. Implant malposition, with lateral or downward displacement, breast asymmetry, improper contour, with implants moving in the pocket that compromise the aesthetic outcome of breast augmentation and require surgical correction may be different symptoms from the same clinical problem. Capsular weakness is a short or mid-term complication of breast augmentation. Most techniques aim to correct the malposition by making sutures to increase the resistance to the displacement of the implant, rearrange the structures using the capsule as flaps to remodel the envelope of the new pocket, obtaining a more stable and reliable result. In this article, four cases of displacement of breast prosthesis with capsular weakness are described and the surgical treatment that included a capsulotomy and capsulorraphy is described.
