This guideline for the diagnosis and treatment of squamous-cell carcinoma and adenocarcinoma of the esophagus was developed and managed by the German Guideline Program in Oncology (GGPO) of the Association of the Scientific Medical Societies in Germany (AWMF), German Cancer Society (DKG), and German Cancer Aid (DKH). The guideline commission comprised multidisciplinary experts from various professional associations and organizations involved in the management of esophageal cancer, as well as a patient representative. Quality of the evidence is presented using Oxford evidence-based medicine system, and recommendations were graded in a formal consensus process using a recommendation grading scheme.

Purpose: Therapeutic options for patients with brain metastases (BM) increase. While these lead to considerable survival effects in subgroups, there is limited knowledge about characteristics, prognosticators and treatment effects in patients with BM and short survival. Methods: Patients with a survival time of ≤ 6 months (short-term survivors, STS), diagnosed with BM between 2009–2021 at a large tertiary cancer center were analysed. Clinical and treatment characteristics, pathological data and causes of death were documented. Descriptive statistics, treatment-specific univariate Kaplan-Meier estimator analyses and multivariate Cox regression were performed. Background and purpose: Many patients with solid tumors develop brain metastases (BM). With more patients surviving long-term, preservation of neurocognitive function gains importance. In recent years, several methods to delay cognitive deterioration have been tested in clinical trials. However, knowledge on the extent to which these neuroprotective strategies have been implemented in clinical practice is missing. Materials and methods: We performed an online survey regarding treatment patterns of BM in German-speaking countries, focused on the use of neuroprotective approaches. The survey was distributed among radiation oncologists (ROs) registered within the database of the German Society for Radiation Oncology (DEGRO). Results: Physicians of 78 centers participated in the survey. Whole brain radiotherapy (WBRT) is still preferred by 70 % of ROs over stereotactic radiotherapy (SRT) in patients with 6–10 BM. For 4–5 BM WBRT is preferred by 23 % of ROs. The fraction of ROs using hippocampal sparing (HS) in WBRT has increased to 89 %, although the technique is used on a regular basis only by a minority (26 %). The drug memantine is not widely prescribed (14% of ROs). A trend was observed for university hospitals to implement neuroprotective approaches more frequently. Conclusion: There is considerable heterogeneity regarding the treatment of BM in German-speaking countries and a general standard of care is lacking. Neuroprotective strategies are not yet standard approaches in daily clinical routine, although usage is increasing. Further clinical trials, as well as improvement of technical opportunities and reimbursement, might further shift the treatment landscape towards neuroprotective radiation treatments in the future.

Zusammenfassung Hintergrund  In F�llen der subtotalen Amputation der Ohrmuschel kann h�ufig erfolgreich ohne Mikrochirurgie replantiert werden. Ziel der Arbeit ist es, die dieser klinischen Beobachtung zugrunde liegende Gef��anatomie zu verifizieren. Patienten und Methoden  Vier F�lle von erfolgreich behandelten subtotalen Ohrmuschelabrissen werden vorgestellt. Die Gef��versorgung des �u�eren Ohrs wurde durch Latexinjektion an 13 Leichenohren untersucht. Ergebnisse  Ein �ber dem Tragus abgehender Endast der A. temporalis superficialis wurde dargestellt, der im oberen Rand der Ohrmuschel mit der Helixarkade anastomosiert. Unterhalb des Tragus wurde ein weiterer Seitenast der Temporalarterie identifiziert. Schlussfolgerung  Die Ohrmuschel kann im Falle einer subtotalen Amputation �ber eine Gewebebr�cke oberhalb oder unterhalb des Tragus �berleben. Der Erhalt eines Seitenastes der A. temporalis superficialis erscheint f�r die Versorgung der Ohrmuschel ausreichend und erm�glicht eine erfolgreiche nicht-mikrochirurgische operative Versorgung.
