The aim of this current study was to establish a metaphyseal femoral non-union model in osteoporotic rats by comparing a power tool versus a manual tool for fracture creation. Twelve adult female Sprague Dawley rats were ovariectomized (OVX) and received a special diet for 6 weeks. Biweekly pQCT measurements confirmed a significant reduction in the cancellous and total bone mineral density in OVX rats compared to control (CTRL) animals. After 6 weeks, OVX rats underwent surgery creating a distal metaphyseal osteotomy, either using a piezoelectric- (n = 6) or a manual Gigli wire (n = 6) saw. Fractures were stabilized with a Y-shaped mini-locking plate. Within each group, three rats received Alginate directly into the fracture gap. OVX animals gained more weight over 8 weeks compared to CTRL animals. pQCT analysis showed a significant difference in the volumetric cancellous bone mineral density between OVX and CTRL rats. A histological examination of the osteoporotic phenotype was completed. Radiographic evaluation and Masson-Goldner trichrome staining with the piezoelectric saw failed to demonstrate bony bridging or a callus formation. New bone formation and complete healing were seen after 6 weeks in the Gigli group. For the creation of a metaphyseal atrophic non-union in the osteoporotic bone, a piezoelectric saw should be used.

Vascular endothelial growth factor (VEGF) signaling is crucial for a large variety of cellular processes, not only related to angiogenesis but also in nonvascular cell types. We have previously shown that controlling angiogenesis by reducing VEGF-A signaling positively affects tendon healing. We now hypothesize that VEGF signaling in non-endothelial cells may contribute to tendon pathologies. By immunohistochemistry we show that VEGFR1, VEGFR2, and VEGFR3 are expressed in murine and human tendon cells in vivo. In a rat Achilles tendon defect model we show that VEGFR1, VEGFR3, and VEGF-D expression are increased after injury. On cultured rat tendon cells we show that VEGF-D stimulates cell proliferation in a dose-dependent manner; the specific VEGFR3 inhibitor SAR131675 reduces cell proliferation and cell migration. Furthermore, activation of VEGFR2 and -3 in tendon-derived cells affects the expression of mRNAs encoding extracellular matrix and matrix remodeling proteins. Using explant model systems, we provide evidence, that VEGFR3 inhibition prevents biomechanical deterioration in rat tail tendon fascicles cultured without load and attenuates matrix damage if exposed to dynamic overload in a bioreactor system. Together, these results suggest a strong role of tendon cell VEGF signaling in mediation of degenerative processes. Background: Treatment of degenerating tendons still presents a major challenge, since the aetiology of tendinopathies remains poorly understood. Besides mechanical overuse, further known predisposing factors include rheumatoid arthritis, diabetes, obesity or smoking all of which combine with a systemic inflammation., Methods: To determine whether the systemic inflammation accompanying these conditions contributes to the onset of tendinopathy, we studied the effect of a systemic inflammation induced by an allergic episode on tendon properties. To this end, we induced an allergic response in mice by exposing them to a timothy grass pollen allergen and subsequently analysed both their flexor and Achilles tendons. Additionally, we analysed data from a health survey comprising data from more than 10.000 persons for an association between the occurrence of an allergy and tendinopathy., Findings: Biomechanical testing and histological analysis revealed that tendons from allergic mice not only showed a significant reduction of both elastic modulus and tensile stress, but also alterations of the tendon matrix. Moreover, treatment of 3D tendon-like constructs with sera from allergic mice resulted in a matrix-remodelling expression profile and the expression of macrophage-associated markers and matrix metalloproteinase 2 (MMP2) was increased in allergic Achilles tendons. The repair of large bone defects remains challenging and often requires graft material due to limited availability of autologous bone. In clinical settings, collagen sponges loaded with excessive amounts of bone morphogenetic protein 2 (rhBMP-2) are occasionally used for the treatment of bone non-unions, increasing the risk of adverse events. Therefore, strategies to reduce rhBMP-2 dosage are desirable. Silk scaffolds show great promise due to their favorable biocompatibility and their utility for various biofabrication methods. For this study, we generated silk scaffolds with axially aligned pores, which were subsequently treated with 10× simulated body fluid (SBF) to generate an apatitic calcium phosphate coating. Using a rat femoral critical sized defect model (CSD) we evaluated if the resulting scaffold allows the reduction of BMP-2 dosage to promote efficient bone repair by providing appropriate guidance cues. Highly porous, anisotropic silk scaffolds were produced, demonstrating good cytocompatibility in vitro and treatment with 10× SBF resulted in efficient surface coating. In vivo, the coated silk scaffolds loaded with a low dose of rhBMP-2 demonstrated significantly improved bone regeneration when compared to the unmineralized scaffold. Overall, our findings show that this simple and cost-efficient technique yields scaffolds that enhance rhBMP-2 mediated bone healing.

Tendons and tendon interfaces have a very limited regenerative capacity, rendering their injuries clinically challenging to resolve. Tendons sense muscle-mediated load; however, our knowledge on how loading affects tendon structure and functional adaption remains fragmentary. Here, we provide evidence that the matricellular protein secreted protein acidic and rich in cysteine (SPARC) is critically involved in the mechanobiology of tendons and is required for tissue maturation, homeostasis, and enthesis development. We show that tendon loading at the early postnatal stage leads to tissue hypotrophy and impaired maturation of Achilles tendon enthesis in Sparc -/- mice. Treadmill training revealed a higher prevalence of spontaneous tendon ruptures and a net catabolic adaptation in Sparc -/- mice. Tendon hypoplasia was attenuated in Sparc -/- mice in response to muscle unloading with botulinum toxin A. In vitro culture of Sparc -/- three-dimensional tendon constructs showed load-dependent impairment of ribosomal S6 kinase activation, resulting in reduced type I collagen synthesis. Further, functional calcium imaging revealed that lower stresses were required to trigger mechanically induced responses in Sparc -/- tendon fascicles. To underscore the clinical relevance of the findings, we further demonstrate that a missense mutation (p.Cys130Gln) in the follistatin-like domain of SPARC , which causes impaired protein secretion and type I collagen fibrillogenesis, is associated with tendon and ligament injuries in patients. MicroRNAs (miRNAs) have emerged as key regulators orchestrating a wide range of inflammatory and fibrotic diseases. However, the role of miRNAs in degenerative shoulder joint disorders is poorly understood. The aim of this explorative case-control study was to identify pathology-related, circulating miRNAs in patients with chronic rotator cuff tendinopathy and degenerative rotator cuff tears (RCT). In 2017, 15 patients were prospectively enrolled and assigned to three groups based on the diagnosed pathology: (i) no shoulder pathology, (ii) chronic rotator cuff tendinopathy, and (iii) degenerative RCTs. In total, 14 patients were included. Venous blood samples ("liquid biopsies") were collected from each patient and serum levels of 187 miRNAs were determined. Subsequently, the change in expression of nine candidate miRNAs was verified in tendon biopsy samples, collected from patients who underwent arthroscopic shoulder surgery between 2015 and 2018. Overall, we identified several miRNAs to be progressively deregulated in sera from patients with either chronic rotator cuff tendinopathy or degenerative RCTs. Importantly, for the several of these miRNAs candidates repression was also evident in tendon biopsies harvested from patients who were treated for a supraspinatus tendon tear. As similar expression profiles were determined for tendon samples, the newly identified systemic miRNA signature has potential as novel diagnostic or prognostic biomarkers for degenerative rotator cuff pathologies. © 2019 The Authors. Tendon disorders frequently occur and recent evidence has clearly implicated the presence of immune cells and inflammatory events during early tendinopathy. However, the origin and properties of these cells remain poorly defined. Therefore, the aim of this study was to determine the presence of cells in healthy rodent and human tendon tissue fulfilling macrophage-like functions. Using various transgenic reporter mouse models, we demonstrate the presence of tendon-resident cells in the dense matrix of the tendon core expressing the fractalkine (Fkn) receptor CX3CR1 and its cognate ligand CX3CL1/Fkn. Pro-inflammatory stimulation of 3D tendon-like constructs in vitro resulted in a significant increase in the expression of IL-1β, IL-6, Mmp3, Mmp9, CX3CL1 and epiregulin, which has been reported to contribute to inflammation, wound healing and tissue repair. Furthermore, we demonstrate that inhibition of the Fkn receptor blocked tendon cell migration in vitro , and show the presence of CX3CL1/CX3CR1/EREG-expressing cells in healthy human tendons. Tendinopathy is accompanied by a cascade of inflammatory events promoting tendon degeneration. Among various cytokines, interleukin-1β plays a central role in driving catabolic processes, ultimately resulting in the activation of matrix metalloproteinases and a diminished collagen synthesis, both of which promote tendon extracellular matrix degradation. Pulsed electromagnetic field (PEMF) therapy is often used for pain management, osteoarthritis, and delayed wound healing. In vitro PEMF treatment of tendon-derived cells was shown to modulate pro-inflammatory cytokines, potentially limiting their catabolic effects. However, our understanding of the underlying cellular and molecular mechanisms remains limited. We therefore investigated the transcriptome-wide responses of Il-1β-primed rat Achilles tendon cell-derived 3D tendon-like constructs to high-energy PEMF treatment. RNASeq analysis and gene ontology assignment revealed various biological processes to be affected by PEMF, including extracellular matrix remodeling and negative regulation of apoptosis. Further, we show that members of the cytoprotective Il-6/gp130 family and the Il-1β decoy receptor Il1r2 are positively regulated upon PEMF exposure. In conclusion, our results provide fundamental mechanistic insight into the cellular and molecular mode of action of PEMF on tendon cells and can help to optimize treatment protocols for the non-invasive therapy of tendinopathies.

Tendons harbor various cell populations, including cells displaying classical adult mesenchymal stromal cell criteria. Previous studies have shown that a tenogenic phenotype is more effectively maintained in a 3D cell culture model under mechanical load. This chapter describes a method to isolate tendon-derived cells from rat Achilles tendons and the subsequent formation of 3D-embedded cell cultures. These tendon-like constructs can then be analyzed by various means, including histology, immunohistochemistry, qPCR, or standard protein analysis techniques.

It is widely accepted that partial nitrification by ANAMMOX has the potential to become one of the key processes in wastewater treatment. However, large greenhouse gas emissions have been panobserved in many cases. A novel mesh separated reactor, developed to allow continuous operation of deammonification at smaller scale without external biomass selection, was compared to a conventional single-chamber deammonification sequencing batch reactor (SBR), where both were equally-sized pilot-scale reactors. The mesh reactor consisted of an aerated and an anoxic zone separated by a mesh. The resulting differences in the structure of the microbial community were detected by next-generation sequencing. When both systems were operated in a sequencing batch mode, both systems had comparable nitrous oxide emission factors in the range of 4% to 5% of the influent nitrogen load. A significant decrease was observed after switching from sequencing batch mode to continuous operation.

The fate of four widely used veterinary antibiotics (doxycycline, flumequine, oxytetracycline and tetracycline) during manure upcycling was investigated at laboratory and pilot scale. The pilot was operated continuously, while the laboratory scale in batch mode. Both set-ups consisted of anaerobic digestion, ammonia stripping and a solid liquid separation step. A partial nitritation anammox process was used to treat the laboratory scale effluent. In the pilot installation, pig manure as feed, natural occurring antibiotics levels were reduced by 92% for doxycycline, 88% for flumequine, 95% for oxytetracycline and 100% for tetracycline. In the laboratory scale set-up, antibiotic free sludge was used and the four substances were spiked. The input antibiotics concentration was reduced by 85% for doxycycline, 46% for flumequine, 97% for oxytetracycline and 100% for tetracycline. In both set-ups the centrifuge cake was identified as the major emission pathway for residual antibiotics. A new approach to perform partial nitritation-anammox in a single tank was investigated. The tank incorporated a mesh (opening size 1.0 × 1.2 mm) as permeable barrier to create two distinct reaction zones (aerated and anoxic). The study reports on the operation and optimization of a 13 L laboratory scale reactor to treat sludge reject water with an NH 4 -N concentration of ∼750 mg·L -1 . Performance throughout 250 days at increasing nitrogen loading rates is presented. The maximum loading rate applied was 1.5 kg NH 4 -N·m -3 ·d -1 at a hydraulic retention time of 12 h. Typical composition of the effluent was ∼50 mg·L -1 NH 4 -N; <5-10 mg·L -1 NO 2 -N and ∼60 mg·L -1 NO 3 -N. The corresponding average N removal rates were 85% for total nitrogen and 90% for NH 4 -N, respectively. Process control was very simple. It comprised only regulation of the aeration rate to maintain a pre-set pH (7.1) in the nitritation zone. Performance data clearly indicate that the investigated reactor configuration offers distinct advantages over currently installed processes. It demonstrated high robustness without the need for sophisticated process control. Apparently, the use of a permeable mesh to establish different reaction conditions in a single reactor provides new features of high potential.

In Austria, 1,840 wastewater treatment plants (WWTPs) with design size >50 population equivalent (PE) serve about 95% of the population. The remaining 5% of the population live in single houses and small settlements that require on site and decentralized wastewater treatment technologies. There is no common database on small WWTPs with design size <50 PE; thus data had to be collected from the Austrian federal states and compiled in a database. The total number of small WWTPs in Austria is about 28,700 comprising 1,300 WWTPs with design size 51-500 PE and 27,400 with design size <50 PE, respectively. The total number of treatment wetlands implemented in Austria is 5,450. Due to legal requirements (nitrification), only vertical flow wetlands are implemented in Austria. From the 5,450 treatment wetlands, about 100 are of design size larger than 50 PE and about 2,800 treatment wetlands have a design size of 5-10 PE. The peak of wetland implementation was in the years 2007-2011 with 2,200 implemented systems in 5 years. Since about 2000, about 30-40% of the new implemented small WWTPs are treatment wetlands.

Impact of strip gas composition on side stream ammonia stripping, a technology aiming at the reduction of high ammonia levels in anaerobic reactors, was investigated. Evaluation of the effect of oxygen contact during air stripping showed a distinct, though lower than perceived, inhibition of anaerobic microflora. To circumvent, the feasibility and possible constraints of biogas and flue gas as alternatives in side stream stripping were studied. Experiments, with ammonia bicarbonate model solution and digestate, were conducted. It was demonstrated that the stripping performance is negatively correlated to the CO2 level in the strip gas with a progressive performance loss towards higher concentrations. In contrast to biogas with its high CO2 content, the efficiency reduction observed for flue gas was significantly less pronounced. The later provides the additional benefit that its high thermal energy can be re-utilized in the stripping unit and it is therefore considered a viable alternative for air., (Copyright © 2015 The Authors. It is still not proven that treatment of sewage in a wastewater treatment plant (WWTP) is (in every case) environmentally friendly. To address this matter, we have applied a state-of-the-art life cycle assessment (LCA) to an energy self-sufficient WWTP in Strass (Austria), its supply chain and the valorization of its 'products': produced electricity out of biogas from sludge digestion and the associated stabilized digestate, applied as agricultural fertilizer. Prominent aspects of our study are: a holistic environmental impact assessment, measurement of greenhouse gas emissions (including N2O), and accounting for infrastructure, replacement of conventional fertilizers and toxicity of metals present in the stabilized digestate. Additionally, the environmental sustainability improvement by implementing one-stage partial nitritation/anammox (e.g. DEMON(®)) and co-digestion was also assessed. DEMON on the digesters reject water leads to a considerable saving of natural resources compared to nitritiation/denitritation (about 33% of the life cycle resource input), this through the lowering of sludge consumption for N-removal, and thus increasing electricity production via a higher sludge excess. However, its N2O emission could be restrained through further optimization as it represents a large share (30-66%) of the plants' damaging effect on human health, this through climate change. The co-substrate addition to the digester resulted in no significant improvement of the digestion process but induced net electricity generation. If respective amounts of conventional fertilizers are replaced, the land application of the stabilized digestate is environmentally friendly through prevention of natural resource consumption and diversity loss, but possibly not regarding human health impact due the presence of toxic heavy metals, mainly Zn, in the digestate. The outcomes show that the complete life cycle results in a prevention of resource extraction from nature and a potential mitigation of diversity loss (though for some impact categories no quantification of associated diversity loss is possible) but it also leads to a damaging effect on human health, mainly via climate change and heavy metal toxicity. Since it is for now impossible to aggregate the impact to these different aspects in a sound manner, it is not yet possible to consider in this case the studied system as environmentally friendly. The ATV-A-131 guideline and the design approach published in 'Wastewater Engineering, Treatment and Reuse (WE)' are widely used for the design of activated sludge plants. They are both based on simplified steady-state assumptions tailored to the boundary conditions of temperate climates. Using design guidelines beyond the designated temperature range may lead to inappropriate results. The objectives of this paper are (1) to summarise temperature relevant differences between ATV-A-131 and WE; (2) to show the related design components; and (3) to demonstrate a procedure for design parameter adaptation for a full-scale activated sludge plant located in a warm climate region. To gain steady-state data required for wastewater treatment plant (WWTP) design according to ATV-A-131 and WE, full-scale plant data were acquired for a period of 6 months as a basis for analyses and adaptation. Mass balances were calculated for the verification of the measurements and for analysing excess sludge production. The two approaches showed relevant temperature related differences. WE default application resulted in lower deviation in the mass balance results for excess sludge production. However, with the adaptation of the heterotrophic decay rates for both approaches and the inert organic and mineral solids fraction additionally for ATV-A-131, a good fit to the observed excess sludge production could be achieved.

Mountain refuges are an excellent example of public stand-alone infrastructures equipped with energy and water supply, and wastewater and waste disposal systems suited to operating under unfavourable conditions, often comprising lengthy distances for transportation or scarce resources. An international project was undertaken to evaluate the existing supply and disposal schemes at 100 mountain refuges both individually and in an integrated manner. On the basis of the results obtained guidelines to be applied in the sustainable planning, construction and operation of supply and disposal systems will be published in the near future. The present article focuses on solid waste management with an aim to the identification, quantification and critical discussion of operational issues and problems relating to waste management in mountain refuges. Transportation represents a crucial factor in terms of costs and climate relevance. The results obtained reveal how waste prevention and composting are capable of reducing waste quantities requiring transportation. However, it was also demonstrated that composting is frequently not performed in an appropriate manner and, although prohibited by law paper and cardboard are frequently burned.

The aim of this work was to give a quantitative description of the gaseous nitrogen and carbon emissions of a full-scale deammonification plant (DEMON system). Deammonification accounted for the net carbon sequestration of 0.16 g CO2/g NO2-N. Both nitrogen dioxide (NO2) and nitric oxide (NO) were minor trace gases (<0.1% nitrogen output). However, in comparison, the nitrous oxide (N2O) emission (1.3% nitrogen output) was significant. The global warming potential of the N2O emissions from the DEMON were similar to those found in conventional simultaneous nitrification/denitrification systems; however, CO2 emissions in the investigated system were significantly lower, thereby lessening the overall environmental effect. This was the first time such an analysis has been performed on a DEMON system.

Planning, construction and operation of onsite wastewater treatment systems at mountain refuges is a challenge. Energy supply, costly transport, limited water resources, unfavourable climate and load variations are only some of the problems that have to be faced. Additionally, legal regulations are different between and even within countries of the Alps. To ensure sustainability, integrated management of the alpine infrastructure management is needed. The energy and water supply and the wastewater and waste disposal systems and the cross-relations between them were analysed for 100 mountain refuges. Wastewater treatment is a main part of the overall 'mountain refuge' system. The data survey and first analyses showed the complex interaction of the wastewater treatment with the other infrastructure. Main criteria for reliable and efficient operation are training, technical support, user friendly control and a relatively simple system set up. Wastewater temperature, alkalinity consumption and high peak loads have to be considered in the planning process. The availability of power in terms of duration and connexion is decisive for the choice of the system. Further, frequency fluctuations may lead to damages to the installed aerators. The type of water source and the type of sanitary equipment influence the wastewater quantity and quality. Biosolids are treated and disposed separately or together with primary or secondary sludge from wastewater treatment dependent on the legal requirements., (IWA Publishing 2008.)

When using dynamic simulation for fine tuning of the design of activated sludge (AS) plants diurnal variations of influent data are required. For this application usually only data from the design process and no measured data are available. In this paper a simple method to generate diurnal variations of wastewater flow and concentrations is described. The aim is to generate realistic influent data in terms of flow, concentrations and TKN/COD ratios and not to predict the influent of the AS plant in detail. The present work focuses on the fate of two cancerostatic platinum compounds (CPC), cisplatin and carboplatin, as well as of two inorganic platinum compounds, [PtCl(4)](2-) and [PtCl(6)](2-) in biological wastewater treatment. Laboratory experiments modelling adsorption of these compounds onto activated sludge showed promising specific adsorption coefficients K(D) and K(OC) and Freundlich adsorption isotherms. However, the adsorption properties of the investigated substances were differing significantly. Adsorption decreased following the order cisplatin>[PtCl(6)](2-)>[PtCl(4)](2-)>carboplatin. LogK(D)-values were ranging from 2.5 to 4.3 , logK(OC) from 3.0 to 4.7. A pilot membrane bioreactor system (MBR) was installed in a hospital in Vienna and fed with wastewater from the oncologic in-patient treatment ward to investigate CPC-adsorption in a sewage treatment plant. During three monitoring periods Pt-concentrations were measured in the influent (3-250 microg l(-1) Pt) and the effluent (2-150 microgl(-1) Pt) of the treatment plant using ICP-MS. The monitoring periods (duration 30d) revealed elimination efficiencies between 51% and 63% based on averaged weekly input-output budgets. The derived logK(D)-values and logK(OC)-values ranged from 2.4 to 4.8 and from 2.8 to 5.3, respectively. Species analysis using HPLC-ICP-MS proofed that mainly carboplatin was present as intact drug in the influent and--due to low logK(D)--in the effluent of the MBR.

Carbon dioxide (CO(2)) online off-gas monitoring is useful to detect changes in biological activity for activated sludge systems especially under limited oxygen conditions like under simultaneous nitrification-denitrification (SND) where respirometric measurements are not applicable. So far, the influence of the bicarbonate system on the liquid-gas transfer of CO(2) prevented the wider use of off-gas CO(2) for monitoring purposes in wastewater treatment. The objective of the paper is to demonstrate a practical method to correct measured off-gas CO(2) as an indicator of biological activity by taking into account pH shifts (resulting in CO(2) release or retention) and changes in influent alkalinity. The simple model is based on the physicochemical system of the bicarbonate/CO(2) equilibrium and the liquid-gas mass transfer for aerated systems. Standard on-line measurements (pH, temperature, flow rates) and periodical alkalinity titration serve as input data to estimate the influence of the carbonate system on the CO(2) off-gas concentrations measured on-line. For a particular plant the CO(2) mass transfer coefficients are derived from measurements compared to the theoretical calculation from oxygen mass transfer. The model estimates the biological carbon dioxide production rate (CPR; heterotrophic activity) by the correction of the measured carbon dioxide transfer rate (CTR; C-flux by the off-gas) with the calculated inorganic carbon dioxide transfer rate (r(F)) considering bicarbonate consumption (autotrophic activity).

Simultaneous nitrification-denitrification (SND) allows biological nitrogen removal in a single reactor without separation of the two processes in time or space but requires adapted control strategies (anoxic/aerobic conditions). In this study, the formation of gaseous nitric oxide (NO(G)) and nitrogen dioxide (NO(2G)) was monitored for SND in relation to the oxidation-reduction potential (ORP) and nitrogen removal in a lab batch reactor and a pilot membrane bio-reactor (MBR). In addition hospital wastewater (COD/N(tot)>6:1) was treated on site for 1 year. The highest total nitrogen removal rates of max 90% were reached at 220-240mV ORP (given as E(h)) with corresponding maximal NO(G) emissions rates of 0.9microgg(-1)VSSh(-1). The maximal emission rates of NO(2G) (0.2microgg(-1)VSSh(-1)) were reached at the same ORP level and the NO(2G) emissions correlated to the nitrite accumulation in the activated sludge up to 5mgl(-1)NO(2L)-N. It was shown that this correlation was due to biological production and not due to pH-dependent chemical conversion. Therefore, NO(2G) can be used as additional control loop for ORP-controlled SND systems to avoid the inhibition of denitrification and high nitrite concentrations in the plant effluent.

Cytostatic agents are applied in cancer therapy and subsequently excreted into hospital wastewater. As these substances are known to be carcinogenic, mutagenic and toxic for reproduction, they should be removed from wastewater at their source of origin. In this study the fate and effects of the cancerostatic platinum compounds (CPC) cisplatin, carboplatin, oxaliplatin, 5-fluorouracil (5-FU) and the anthracyclines doxorubicin, daunorubicin and epirubicin were investigated in hospital wastewater. Wastewater from the in-patient treatment ward of a hospital in Vienna was collected and monitored for the occurrence of the selected drugs. A calculation model was established to spot the correlation between administered dosage and measured concentrations. To investigate the fate of the selected substances during wastewater treatment, the oncologic wastewater was treated in a pilot membrane bioreactor system (MBR) and in downstream advanced wastewater treatment processes (adsorption to activated carbon and UV-treatment). Genotoxic effects of the oncologic wastewater were assessed before and after wastewater treatment followed by a risk assessment. Monitoring concentrations of the selected cytostatics in the oncologic wastewater were in line with calculated concentrations. Due to different mechanisms (adsorption, biodegradation) in the MBR-system 5 - FU and the anthracyclines were removed < LOD, whereas CPC were removed by 60%. In parallel, genotoxic effects could be reduced significantly by the MBR-system. The risk for humans, the aquatic and terrestrial environment by hospital wastewater containing cytostatic drugs was classified as small in a preliminary risk assessment.

Antineoplastic agents are applied in cancer therapy and end up in hospital wastewater by human excretions. In this study, the raw wastewater of the sewer of the oncologic in-patient treatment ward of the Vienna University Hospital was monitored for 98 d over 2 years for the cytostatics 5-fluorouracil (5-FU), doxorubicin (DOX), epirubicin, and daunorubicin. In a next step, the elimination of the drugs by a membrane-bio-reactor system was investigated. In addition, their fate in wastewater and elimination by activated sludge was investigated with radio-labelled substances. During the monitoring periods, concentration levels ranging from <8.6 to 124 microgl(-1) for 5-FU and from <0.26 to 1.35 microgl(-1) for DOX were determined. The concentrations analysed fitted the lower ranges calculated by an input-output model. Treatment of oncologic wastewater in the membrane bio-reactor as well as the analysis of the effluents of the Vienna University Hospital resulted in concentrations below the limit of detection. Investigations with radio-labelled compounds showed that 5-FU is eliminated from the liquid phase below the limit of detection. But, up to 25% of radio-labelled equivalents of the drug's amount were found in the gaseous phase and only a marginal part in the solid phase, this indicates that at least one part of the drug is biodegraded. For the anthracyclines more than 90% was eliminated from the liquid phase. In this case, adsorption to suspended solids seems to be the major elimination pathway, as up to 30% of the radio-labelled equivalents of the drug's amount was detected in the solid phase. Our results indicate that the investigated anticancer drugs are eliminated by sewage treatment plants, either by biodegradation or adsorption.

Platinum originating from the excreted cancerostatic platinum compounds (CPC) cisplatin, carboplatin and oxaliplatin was monitored over a period of 28 days in the wastewater of the oncologic ward of the Vienna University Hospital. Concentration levels ranging from 4.7 to 145 microg L(-1) were measured by inductively coupled plasma mass spectrometry (ICP-MS). An average ratio of weekly drug emission/drug consumption of 0.27+/-0.12 was assessed. Model studies were carried out for fundamental understanding of CPC interaction with the solid phases present at different stages of the water cycle. Wastewater and activated sludge were spiked with CPC at concentration levels as found in the sewer of the oncologic ward. The platinum concentration remaining in the tested solution was measured after 24 h of incubation. Depending on pH, the three substances exhibited considerably different adsorption rates in wastewater. At pH 7, cisplatin was adsorbed by 88%, whereas only 26% of carboplatin and 54% of oxaliplatin were removed from the aqueous phase. Adsorption by activated sludge was higher, less affected by pH variation and comparable for all investigated CPC (96% for cisplatin, 70% for carboplatin and 74% for oxaliplatin at pH 6.8). In a next step, the dependence of CPC adsorption was tested for wastewater and activated sludge of different sampling sites. Strong variations were found only for wastewater, whereas activated sludge showed more consistent elimination rates (average values: cisplatin 92%, carboplatin 72%, and oxaliplatin 78%). These findings indicate that the major part of the excreted CPC is adsorbed by the solid phase in the water cycle and is thus expected to be removed from the wastewater by sewage treatment plants.
