Introduction. In recent years, healthcare-associated infections (HAIs) cause ever greater economic and social damage. Microorganisms circulating in medical institutions the causative agents of HAIs, become resistant to most antibiotics and many disinfectants. There is a need to create alternative mechanisms for controlling microorganisms. The purpose of the work is to determine the antagonistic activity of bacteria strains of the genus Bacillus in relation to test microorganisms, as a potential basis for the development of domestic detergents and disinfectants based on probiotics. Material and methods. The strains of Bacillus subtilis 2/10 VKPM B-2896; Bacillus subtilis VKPM B-1283; Bacillus licheniformis VKPM B-5397; Bacillus amyloliquefaciens VKPM B-5462 werethe main object of study. At the first stage of the studies, the antagonistic activity of the strains was studied by the well method, at the second stage of the studies, the antagonistic activity was studied at the test object, and at the third stage of the studies, the effectiveness of the composition containing B. subtilis VKPM B-1283, B.licheniformis VKPM B-5397 and sulfonol strains was studied. Results. As a result of studies, the antagonistic effect of bacilli against test microorganisms was revealed. When surfaces are contaminated with test microorganisms (S.aureus ATCC 6538-P, E.coli ATCC 10531, C. albicans ATCC 10231) and subsequent application of Bacillus spores, a significant reduction in the seeding rate with test microorganisms is observed after 24 hours by 97.07-100%. The use of B.subtilis VKPM B-1283 and B.licheniformis VKPM B-5397 spores together with a detergent component for treating surfaces contaminated with test microorganisms (S.aureus ATCC 6538-P, E.coli ATCC 10531, C.albicans ATCC 10231) provides a reduction in microbial seeding after 4 hours by 99.97 %. Conclusions. B.subtilis 2/10 VKPM B-2896, B.subtilis VKPM B-1283 and B.licheniformis VKPM B-5397 are promising for the use as probiotics, as the basis of detergents and disinfectants.

Policy management of institutes and business organizations’ activity, dealing with radiation sources, has more of a bulky and confusing complex of requirements at the present day. The requirements being very substantial, slow down economic growth of entire branches in the field of the beneficial use of radiation technologies, and, at the same time, do not provide the condition of radiation safety. Prime Minister Dmitriy Medvedev, without any coincidence, pointed out the problem during a plenary meeting as an overpressure of regulatory and supervisory bodies on the Russian private sector. According to him, the task can be solved with ‘a regulatory guillotine’. The upcoming article resembles the first attempt to analyze and sort reasons and factors, providing a glimpse of the current situation, search key elements that cause negative influence in the field taken. On the ground of analysis of regulatory documents, vast survey experience and estimation of radiation objects and technologies, authors suggest a new complex of requirements to provide radiation security, their breakdown of classes depending on danger level of radiation objects and operating with sources of ionizing radiation. The article presents the analysis of current regulatory documents on radiation hygiene field and radiation security, retrospective view on valid documents, the expertise of activity of regulatory and supervisory bodies as part of licensing of the activity with man-made sources of ionizing radiation.

A promising way to prevent spread of airborne infections with aerosol contagion and to reduce risks of healthcare-associated infections (HAI) is to preserve clean air inside premises (including medical organizations) via performing activities aimed at disinfecting air, ventilation and air-conditioning systems and via switching to up-to-date ecologically friendly disinfectants and their rotation. Epidemiological hazards for patients and medical personnel in medical organizations are caused by pathogenic and opportunistic pathogenic microorganisms that can be found in the air inside premises and on various surfaces; such micro-organisms are bacteria in their vegetative form (including tuberculosis agents), viruses, fungi that cause candidosis and dermato-phytes, mold fungi, spore bacteria. Aerosol disinfection is a well proven technique for treating the air inside premises in order to disinfect it and to reduce risks of infections spread; this technique involves transforming a disinfectant into a fine-disperse state with a specialized spraying device so that an aerosol is introduced into the air inside a premise. To work out efficient air disinfection regimes that involve application of relevant equipment and disinfectants, it is necessary to apply such techniques and conditions that would secure the most efficient application of aerosol disinfection in practical conditions. The authors have outlined basic organizational and methodical approaches to an objective assessment of how efficient and how safe aerosol disinfection technique is when it is applied in different premises, including medical organizations.

West Nilefever (WNF) is the most important arbovirus infectionfor the Volgograd region. The observed decrease in the WNF incidence from 2001 to 2006, in 2008 and2009, the reducing the number ofsevere forms ofdiseases only give evidence ofa temporary regression of the epidemic process in the territory of the Volgograd region. In the territory of the Volgograd region there are prerequisites and harbingers of the complication of epidemiological situation on West Nile fever. The aim of the performed in the Volgograd region epidemiological surveillance for WNF is to provide the information necessary and sufficient for the analysis of the epidemiological situation, as well as to determine the direction of policy and activities in public health and in making management decisions.

Western Nile fever (WNF) is an arboviral infection of the most relevance to the Volgograd Region. The observed decreasing incidence of WNF in 2001-2006, 2008, and 2009 and a decline in the number of its severe forms are only suggestive of the temporary regression of the epidemic process in the Volgograd Region. There are prerequisites and forerunners for a complication of the epidemiological Western Nile fever situation in the Volgograd Region. The goal of the epidemiological survey of Western Nile fever is to provide necessary and sufficient information for the analysis of the epidemiological situation and to define a line of public health policy and actions when making managerial decisions.

Specific aims of environment biologic pathogen control by applying special disinfectologic means and technologies are formulated. The main components of disinfectologic process are examined: molecular-biologic properties of targets--their sensitivity (resistance) to biocidal agents; biocidal molecular-chemical properties of disinfection means; toxicological properties of the latter by their effect on human organism as the object of epidemiologic protection. Perspectives of improvement of disinfectologic technologies by using composition potential of different actively operating molecular substances are discussed.

Based on the analysis of adopted in sanitary science methodology for the study and evaluation of the biological action and hygienic significance of environmental factors there was shown the need for the use of laws and methodology of classical epidemiology to ensure the chemical safety in the recognition and elimination of the group of diseases of chemical etiology, outbreaks of similar non infectious diseases. It is stated that individually small "non-toxic" doses of chemicals, but "loading" a large population and forming a large collective dose apparently can be implemented in a stochastic population health damage, detectable only on the basis of large-scale epidemiological studies to assess the risk. These approaches are presented to be especially important for agents in the environment, optimal for which is not absence, but the presence for provision of necessary target effect (pesticides, disinfectants and other household products, etc.). The main task of the epidemiological analysis of local toxic "bursts" and large-scale "epidemics" of this kind is an optimization of the complex of measures implemented by an adequate assessment of their medical, social and economic performance.

The necessity of further development of disinfectology as a branch of science that studies regularities of nonspecific prophylaxis of infectious and parasitic diseases by impact on pathogenic biological objects and their carriers is justified. The importance and topicality of nonspecific prophylaxis of infectious diseases, the necessity to take into account significant factors that characterize epidemic process during selection and application of means and methods of nonspecific prophylaxis are specified. Data that are necessary to be taken into account during development of means, methods and technologies of disinfectologic prophylaxis of infectious and parasitic diseases are presented.

It is shown that valid annual planning of the activities of the State Epidemiological and Sanitary Control Service oriented on social well-being parameters helps determine the role of each specialist of the staff, the scope of the measures and their sequence, thus warranting effective work of the service.

Use of a multi-level automated information system has been scientifically validated for the State Sanitary Epidemic Surveillance Service of the Russian Federation in recent years. Such a system ensures rapid collection, processing, and analysis of information on the sanitary and epidemic status of the population, activities of sanitary and epidemiological institutions, prediction and simulation of sanitary epidemic situations, management of the service. It helped automate the routine processes of professional, financial, and managing activities of the service institutions.

Structural and functional changes in the system of laboratory provision of the State Sanitary and Epidemiological Service under modern economic conditions had a positive impact on the number and quality of laboratory studies. The concept of rearrangement of laboratory service for the period of up to the year 2000 and its staff and material provision improved the quality of laboratory work and eventually the quality of sanitary epidemiological surveillance at the very first stages of its realization.

The authors offer a scientifically-based rationale for the system of management of the State Sanitary and Epidemiological Service and its institutions under modern socioeconomic conditions in Russia. Organizational models of the structure of State Sanitary and Epidemiological Surveillance Centers are offered and their functions determined for various levels of management. A concept of development and organizational structure of the service and its institutions is proposed, aimed at provision of sanitary and epidemiological well-being in the Russian Federation.

An outbreak of purulent meningitides in a hospital ward for preterm babies, caused by Serratia marcescens strain of serovar 05/13 with multiple resistance, is described. Data on the results of the long-term observation of the ward showed that during three months preceding the outbreak the consecutive spread of the infective strain and its colonization of the intestine of children occurred. At the moment of the outbreak S. marcescens 05/13 was the dominating intestinal microflora in 37% of children in the ward and constituted 30% of the total aerobic flora in the intestine of the examined children. No S. marcescens strains were isolated from the feces and urine of the medical personnel and mothers. The importance of the observation of microflora colonizing newborn infants in the ward for the evaluation and prognostication of the epidemiological situation is discussed.

The system of the sanitary and epidemiological surveillance of infectious diseases comprises three aspects: supply of information, epidemiological and sanitary-hygienic surveillance. The system ensures the reliable epidemiological diagnosis, close collaboration in the work of specialists of sanitary and epidemiological departments, the differentiated surveillance of epidemiologically important objects, and the effectiveness of measures aimed at decreasing the infectious diseases incidence rates.
