The article presents the possibilities of radiation methods in the diagnosis of spinal echinococcosis based on publications and our own clinical observation. An electronic literature search was conducted using PubMed/ MEDLINE, Cochrane Library, ResearchGate databases; the review included case reports, retrospective studies, and literature reviews published from 1991 to 2023. A clinical case of a patient with spinal echinococcosis was described showing that for this disease, a comprehensive radiation examination using radiography, computed tomography and magnetic resonance imaging (MRI) provides the necessary information. MRI is the most informative method, since it gives excellent visualization of spinal canal structures and allows detecting a number of spinal echinococcosis characteristic manifestations.

Diagnostic imaging of oligometastatic disease is a difficult task, since it requires determining the exact locoregional stage of the underlying disease and assessing the condition of the entire body in terms of possible diffuse spread of distant metastases, which presupposes a fairly frequent combination of various instrumental methods. In each specific case, a more accurate imaging method is chosen depending on the type of tumor, time of diagnosis, possible previously implemented stage of special antitumor treatment, as well as the individual risk of developing distant metastases, the most characteristic for this type of tumor process. Today, along with some diagnostic techniques that were previously constantly used in everyday practice, but are gradually losing their long-standing clinical application, such imaging methods as contrastenhanced computed tomography (CT), magnetic resonance imaging, as well as metabolic and specific receptor imaging, for example, CT combined with positron emission tomography with 18F-fluorodeoxyglucose, are most commonly performed, alone or in various combinations.

The article presents the clinical observation of a patient born in 1965, who was examined and treated at the Russian Scientific Center of Roentgenoradiology for tumor of anterior bladder wall detected in May 2023 according to the results of radiation diagnostic methods (ultrasound, computed tomography, magnetic resonance imaging). Analysis of biopsy material obtained during cystoscopy revealed a highly differentiated mucinous urachal adenocarcinoma. Based on the diagnosis of malignant neoplasm of primary urinary tract (urachus) pT3N0M0 stage II, the surgical treatment was proposed. In June 2023, the Bricker procedure was performed: laparoscopic anterior pelvic exenteration with formation of intestinal reservoir. Control examination 3 months after showed no progression of the disease. Asymptomatic development of urachal carcinoma in early stages, peculiarities of its location in pelvic cavity and propensity to metastasis, as well as the rarity of such neoplasm indicate the need for earlier and more accurate verification of the diagnosis. This clinical case demonstrates characteristic clinical and imaging features of urachal adenocarcinoma, which may help radiologists, oncologists, and urologists in making diagnosis and choosing correct treatment tactics.

Objective: to study the informativity of the Ovarian-Adnexal Imaging-Reporting-Data System Magnetic Resonance Imaging (O-RADS MRI) in detection of ovarian cancer in the practice of a radiologist.Material and methods. The study included 271 women who underwent pelvic magnetic resonance imaging (MRI) in the period from August to November, 2021. Two radiologists with 6-year (Researcher 1) and 2-year (Researcher 2) experience retrospectively analyzed MR images, in which all ovarian masses were evaluated according to O-RADS MRI categorical risk scale from 0 to 5. The analysis of true positive, true negative, false positive and false negative rates according to MRI data compared to the reference data (histology or 1-year follow-up) was performed.Results. O-RADS MRI categorial risk scale had high diagnostic performance for the characterization of adnexal lesions (sensitivity 87.5% and 87.5%, specificity 97.84% and 96.75%, area under curve (AUC) 0.991 and 0.986 for Researchers 1 and 2, respectively), as well as good interreader agreement (Cohen’s kappa coefficient 0,83). Researchers’ mistakes were analyzed, as well as controversial issues of categorial affiliation of ovarian masses that caused false positive and false negative results. Visual aids for ovarian MRI in accordance with O-RADS MRI classification were made for quick orientation in the system to simplify creating the radiology report.Conclusion. O-RADS MRI categorial risk scale proved to be a reliable working tool for the communication between radiologist and gynecologist-oncologist. However, the question of categorial affiliation of a number of nosological names that are not reflected in O-RADS MRI and require further study continues to be debated.

The relevance of implementing artificial intelligence (AI) technologies in the diagnosis of breast cancer (BC) is associated with a continuing high increase in BC incidence among women and its leading position in the structure of cancer incidence. Theoretically, using AI technologies is possible both at the stage of screening and in clarifying BC diagnosis. The article provides a brief overview of AI systems used in clinical practice and discusses their prospects in BC diagnosis. Advances in machine learning can be effective to improve the accuracy of mammography screening by reducing missed cancer cases and false positives.

Background. Chest low-dose computed tomography (LDCT) is used in lung cancer screening, but the study data can also be used to assess the liver condition, including the hepatic steatosis (HS) detection. However, radiologists often do not pay attention to liver changes due to the focus on the chest. Objective: to determine the prevalence of HS during chest LDST among lung cancer screening patients using a computer vision (CV) system. Material and methods. For a retrospective study, 300 chest LDCT were taken from Moscow lung cancer screening in 2018–2020. Hepatic attenuation analysis was performed by CV, the values < 40 HU were considered as HS. The text protocols of CT scans were analysed and compared with decreased hepatic attenuation revealed by CV system. Results. 291 patients were analysed, the median age for the sample was 65 [61; 70] years. The mean hepatic attenuation was 55.6 ± 14.8 HU. Hepatic attenuation < 40 HU was found in 13% patients (23 (16.1%) males and 14 (9.5%) females), a statistically significant difference was revealed among these patients (p = 0.04). Six (4.2%) males and 4 (2.7%) females were at risk for HS (40–45 HU). The examination of text protocols showed no pathology discovered in all cases. Conclusion. The prevalence of CT signs for HS among the lung cancer screening group in Moscow was 13%. The absence of HS in text protocols highlights the importance of using CV systems in the routine practice.

Background. A wide variety of pulse sequences, the possibility of multiplanar imaging significantly increase the capabilities of magnetic resonance imaging (MRI) in diagnosing diseases of the female reproductive system. At the same time, the lack of a regulated scanning protocol, especially in patients who have undergone anticancer treatment, requires standardization of the technique and the search for an optimal set of pulse sequences that allows to visualize the vagina throughout its entire length and to perform differential diagnosis between continued tumor growth and post-radiation changes while maintaining an adequate examination time.Objective: to determine the optimal set of MRI pulse sequences for pelvic organs examination in patients with vaginal tumors and to form an original research protocol based on diagnostic information content.Material and methods. The study included 141 patients with vaginal tumors. A comparative analysis of four MRI protocols was carried out, built according to the principle “from simple to complex” (from native to multiparametric MRI).Results. A significant difference was obtained between all information content indicators of Protocol 4 compared to Protocol 1 (sensitivity p = 0.00006, specificity p = 0.00443, AUC p = 0.00000). Data analysis also showed a significant difference between sensitivity and AUC for Protocols 2 and 4 (p = 0.00150 and p = 0.00087, respectively), and Protocols 3 and 4 (p = 0.01333 and p = 0.01333, respectively).Conclusion. Significant increase in the information content of Protocol 4 compared to other protocols (sensitivity up to 93%, specificity up to 94%, accuracy up to 93%) indicates the expediency of the priority use of multiparametric MRI in the primary diagnosis of vaginal tumor lesions.

Will the detected lung foci in patients with a burdened oncological history (breast cancer) always be metastatic? In response to the question posed, we present a clinical case of pulmonary sarcoidosis mimicking the metastatic lesion of metaplastic breast cancer. And we remind of the need for a differential diagnosis, despite the oncological history of a patient and radiation diagnostics data.

Superior mesenteric artery syndrome, first reported by Karl Freiger von Rokitansky, is a rare disorder associated with the compression of the horizontal part of the duodenum between the mesenteric artery and the abdominal aorta. A rare clinical observation of the development of the superior mesenteric artery syndrome in a 19-year-old patient is presented to raise the awareness of doctors about this syndrome and the need for early diagnosis and treatment to prevent complications.

A rare case of jejunal lipoma complicated by intestinal intussusception in a 71-year-old man is presented. Abdominal computed tomography revealed intestinal intussusception caused by the jejunal wall lipoma, allowed to timely make a diagnosis and perform surgical treatment.

The jejunal diverticulosis is rare and accounts for approximately 0.5–1 % of the general population. Diagnosis of the pathology is difficult even in the presence of symptomatic complications (perforation, abscess, peritonitis, sepsis, intestinal obstruction and bleeding), therefore, it is often overlooked or delayed, which requires high awareness and vigilance of physicians. Emergency surgical interventions are performed due to the lack of clear clinical symptoms and reliable radiological data in an average of 25 % of patients with complications of diverticulosis. In the presented case, multislice computed tomography (MSCT) with intravenous contrasting revealed multiple diverticula of the jejunum with reactively altered walls, infiltration of the adjacent cellular tissue and small gas inclusions, indicating diverticulum microperforation. Esophagogastroduodenoscopy (EGD) was performed which confirmed the jejunum bleeding, and was followed by laparotomy which confirmed multiple jejunal diverticula with microperforation and bleeding. Thus, timely MSCT in combination with EGD allowed to avoid complications of jejunal diverticulitis.

The timely diagnosis of postoperative consequences through identification of intracranial foreign bodies enables prevention of complications and improvement in quality of life. The paper considers two clinical cases after surgical interventions to remove subdural and intracerebral hematomas of different ages. Computed tomography (CT) was performed on a 64-slice Somatom Perspective apparatus (Siemens, Germany) applying a reformative slice thickness of less than 0.6 mm. Magnetic resonance imaging (MRI) was carried out using a Magnetom Essenza 1.5 T system (Siemens, Germany). When wording the CT and MRT findings, it was suggested that each of the patients had an intracranial foreign body in the presence of postoperative changes. The CT and MRI findings were fully correlated with intraoperative data on the location, number, shape, and size of foreign bodies. The neuroimaging techniques are highly informative in identifying intracranial foreign bodies and are especially important for chronically critically ill patients because of the lack of productive contact with them due to speech problems and cognitive impairment. They assist in making a correct diagnosis and playing a leading role in neurosurgical practice, allowing the detection of an abnormality, the optimal planning of patient management tactics, and the prevention of possible complications.

Objective: to assess the percentage of mesh “titanium silk” implant shrinkage after inguinal hernia repair surgery in the late postoperative period using multi-slice computed tomography (MSCT). Material and methods. The comparative assessment of the long-term results of treatment in 90 patients with inguinal hernias was performed using MSCT. In 36 (40%) patients of Group 1 the titanium implant was used in Lichtenstein hernia repair surgery. In Group 2, 54 (60%) patients were operated by laparoscopic hernia repair surgery. On day 3 and 3 months after surgery every patient underwent MSCT with subsequent determination of the implant square. Results. The percent of mesh “titanium silk” implant shrinkage 3 months after surgery according to MSCT was 4.4% in Lichtenstein hernia repair group, and 8.3% in laparoscopic hernia repair group. According to Kruskal-Wallis test, there were no statistic differences of this indicator between two groups (p = 0,185). Conclusion. The analysis of long-term results of inguinal hernia repair surgery with titanium mesh implants using MSCT showed that implant square significantly decreases 3 months after surgery. There were no significant differences in implants shrinkage regarding the type of surgery. MSCT is an effective method for evaluating the size of mesh titanium implants after hernia repair surgery.

Objective: to comparatively analyze overall survival (OS) in patients with non-small cell lung cancer (NSCLC) with affected lymph nodes (pN1) of the lung root after surgical and combination treatment with postoperative radiotherapy (PORT). Subjects and methods. OS was studied in 310 patients with grade 2-3 NSCLC. (pT1a-4N1M0): in 101 patients after lobectomy/bilobectomy, pulmonectomy with ipsilateral mediastinal lymphadenectomy and in 209 patients after combination treatment with the similar surgical volume and hypofractionated PORT (a single focal dose (SFD) of 3 Gy; 5 fractions per week, a cumulative focal dose (CFD) of 36-39 Gy (43.2-46.8 Gy-eq)) or classical fractionation (SFD2 Gy, 5 fractions per week, CFD44 Gy). An analysis was carried out in the groups of patients younger and older than 60 years with central or peripheral cancer, squamous cell carcinoma or adenocarcinoma, with different tumor grading according to the T criterion (T1-4). Results. PORT in radically operated patients with NSCLC increased 5- and 10-year OS rates only in central squamous cell lung cancer (56.1% and 39.5% vs.25.4% and 4.3%, p = 0.002). This group receiving combination therapy showed a statistically significant increasing trend in 5-year OS rates for both pT1-2 tumors (57.5% vs. 21.3%, respectively, p = 0.013) and pT3-4 tumors (53.9% versus 26.0%; p = 0.044), so did patients younger than 61 years (65.5% vs.29.4%, p = 0.008) and those over 60 years old (47.5% vs.21.3%, p = 0.047). Patients with peripheral squamous cell carcinoma or lung adenocarcinoma at any site exhibited no statistically significant increase in OS after PORT. In general, the 5- and 10-year OS rates in the compared groups were statistically significantly higher in the PORT group (47.9% and 28.9% vs. 27.1% and 11.4, p = 0.006). None of the analyzed subgroups showed a decrease in OS after PORT. Conclusion. In patients with pN1 NSCLC who had radical surgery via lobectomy/bilobectomy or pulmonectomy with ipsilateral mediastinal lymphadenectomy, PORT can be recommended only for those with central squamous cell carcinoma, regardless of tumor size and age group. In other subgroups of patients with pN1 NSCLC, PORT can only be performed within the scientific protocols. The expediency of PORT after bilateral mediastinal lymph node dissection needs to be investigated.

The paper presents two clinical cases of patients with giant renal angiomyolipomas (AML), in one of whom its course was complicated by intratumoral hemorrhage. It describes key diagnostic criteria for computed tomography (CT), as well as the distinctive features of other neoplasms that should undergo a differential diagnosis. The similar clinical presentations and morphological characteristics of different renal neoplasms can cause certain diagnostic difficulties; however, the carefully collected historical data and distinctive criteria allow AML to be identified. Due to its high sensitivity and specificity, abdominal contrast-enhanced CT is an effective imaging technique in the detection and differential diagnosis of giant renal AML.

The paper considers an algorithm for the diagnostic study of patients with cancer of different localization, the possibility and feasibility of involving imaging techniques in radiation therapy and brachytherapy (BT) planning. The most widely used ultrasound in BT planning does not exclude a subjective assessment of the boundaries of the tumor and organs at risk, as well as the inaccuracies caused by endostatic devices. Magnetic resonance imaging (MRI) provides a high level of achievement of the criteria necessary for BT planning, but it has financial constraints and the need for individual selection of research conditions. Computed tomography (CT) is the most effective technique for visualizing the tumor, endostatic devices, and organs at risk for BT 3D planning. The involvement of MRI and positron emission tomography with CT in BT planning is a promising area that requires equipping with a mathematical tool for double and triple contouring of the tumor and organs at risk and for defining protection priorities. The prescribed and used focal dose should be controlled by direct dosimetry methods to ensure quality assurance.

Gastric neuroendocrine tumors commonly called carcinoids arise from enterochromaffin cells of the stomach and are rare. Recently, their incidence has increased, which may be due to the improvement of diagnostic and therapeutic capabilities. The article describes a rare clinical case of gastric carcinoid 23 years after surgical removal of gastric cardia cancer.

The early and complete diagnosis of lung carcinoid tumors is of great interest in clinical oncology, since this is the basis for the possibility of using options for organ-sparing surgical treatment. According to the 2015 WHO classification, carcinoids belong to the group of neuroendocrine tumors and are divided into two types: a typical carcinoid and an atypical one. Based on the data available in the literature, there are from 0.2 to 2 cases per 100,000 population. The paper considers the possibilities of radiation studies in the early diagnosis of this tumor, as well as those of determining the tactics, type, and scope of surgical treatment.

Acute mesenteric ischemia is an acute disruption of blood supply to part of the intestine, which, if untreated, leads to bowel wall necrosis and a patient’s death. Computed tomography (CT) plays a leading role in detecting mesenteric ischemia and in making the initial diagnosis, especially in patients with chronic critical illness, when productive contact is impossible. The final diagnosis is established during surgery or postmortem examination. Timely diagnosis and surgical treatment are a basic tool to reduce high mortality rates from this disease. The article presents the possibilities of CT in imaging and making the primary diagnosis of mesenteric ischemia and mesenteric thrombosis.

Computed tomography for coronavirus infection (COVID-19) is effective not only in making a diagnosis, but also in timely and accurately detecting some complications of this disease in different organs and systems. The paper shows various complications of coronavirus infection that a radiologist may face in practice, which develop both in the natural course and due to therapy for COVID-19, including hemorrhagic and thrombotic events in coagulopathy, pneumothorax, and pneumomediastinum as a result of the direct cytotoxic effect of SARS-CoV-2 on pneumocytes, and barotrauma during mechanical ventilation, as well as pathological fractures due to osteoporosis, including steroid osteoporosis that has developed during therapy with glucocorticosteroids. It considers the main causes and pathogenesis of various complications of coronavirus infection.

Neurofibromatosis type 2 is a rare genetic disease that occurs with a frequency of approximately 1 in 50,000 newborns and is characterized by benign tumors, mainly bilateral schwannomas of the VIII cranial nerve. It is inherited by an autosomal dominant type without sex. Clinical symptoms are associated with damage to the VIII pair of cranial nerves, skin manifestations are less common than with neurofibromatosis type 1. The tumor can be one-sided and can be combined with neurofibromas, meningiomas (intracranial and spinal), schwannomas of other cranial nerves, astrocytomas and ependymomas of the spinal cord. In detecting tumors and making a primary diagnosis, the leading role is played by instrumental diagnostic methods: magnetic resonance imaging (MRI) and computed tomography (CT). The final diagnosis is made after genetic analysis. The article presents the possibilities of CT and MRI in imaging and diagnosis of type neurofibromatosis type 2.

Objective: to evaluate the role and place of MRI lung screening in the detection of pneumonia and patient routing.Material and methods. 500 MRI tests were performed in the period from November 1, 2020 to December 15, 2020 on the basis of more than 20 diagnostic centers of the federal MRI-Expert network. Two groups were identified. In Group 1 (control, n = 50) MRI studies were compared with CT studies in a single time interval of 0–2 days; 25 patients had a positive PCR test for SARS-CoV-2 and 25 patients were conditionally healthy. In Group 2 (effective, n = 450) mediastinal MRI was performed with lung screening on Philips 1.5 T and Siemens 1.5 T devices. The statistical analysis of the medical data base was performed.Results. During the study, mandatory and additional MRI scanning programs were identified: for Philips 1.5 T tomograph, the mandatory programs are sSSh_fb: tra and cor; THRIVE: tra (inhale/exhale); for Siemens 1.5 T – T2_BLADE: cor, tra, sag. When comparing the visual data of the control group (n = 25) with a positive PCR test in a single time interval of 0–2 days, the fact of detecting MR changes coincided with CT changes in 76% of cases (n = 19). The group of patients were examined by only one method – MRI-screening of the lungs (n = 475), pathological changes in the lungs were detected in 44% (n = 209). Localizations of the pathological changes were as follows: bilateral changes – 70.3% (n = 147), right-side changes – 19.6% (n = 41), left-side changes – 10% (n = 21); lower parts 26.8% (n = 26), average departments – 10% (n = 21), the upper divisions – 14.8% (n = 31), mid-lower 29.6% (n = 62), total – 18.6% (n = 39). The ranges of the area of lesion of the pulmonary parenchyma on MRI were graded according to MRI criteria into 4 groups (< 25%, 25 – 50%, 50 – 75%, > 75%). The collective radiation dose decrease was calculated for a group of patients (n = 450), which averaged 2.025 man-Sv for 1.5 months, of which 0.077 man-Sv were control studies.Conclusion. Based on the results obtained, the analyses of the possibilities of the MRI method in displaying pulmonary changes was performed. The method of MRI screening of the lungs is recognized as a possible alternative to computed tomography for dynamic monitoring in conditions of a shortage of CT records or the inability to perform it, and as a tool to reduce the collective effective dose of radiation to the population.

The biceps muscle located between the shoulder and ulnar articulations is attached to the bone structures with a tendon, so in case of traumatic impact, the damage may be both in the proximal and distal parts. Most tendon ruptures occur in the proximal biceps, less often distally, in the area of radial tuberosity. The anamnesis and examination data play an important role in the diagnosis of ruptures. But the instrumental diagnostics is used for complete verification of the diagnosis and exclusion of complications or other diseases. The most reliable and comprehensive information is provided by the method of magnetic resonance imaging (MRI). The article presents analysis of MRI results in a patient with a complete distal biceps tendon rupture with the tendon pulled upward and the muscle retracted. The MRI study could make a correct clear diagnosis, which allowed the patient to immediately contact a specialized professional and to receive qualified assistance in time. Thus, it is shown that high-field MRI makes it possible to solve the problems of identifying the pathological processes and determining their severity with high accuracy.

Multiple primary colon cancer is an urgent medical problem of our time, which is associated with a steady increment in the incidence of the above disease and with the high complexity of its diagnosis. According to the literary sources, the incidence of this form of cancer of the colon has recently amounted to as much as 17% among all multiple primary neoplasms and this organ is a very common site. The paper describes a clinical case confirming the need for a thorough diagnosis of multiple primary colon cancer, by applying radiation techniques to further define treatment policies.

Objective: to reveal a relationship of lung changes to time after radiotherapy (RT), its dose, and irradiated volume, by using a new procedure to quantitatively evaluate lung tissue density changes according to the data of dynamic X-ray computed tomography (X-ray CT) imaging.Subjects and methods. Data on 29 cancer patients who had received RT to the chest area using 3D planning, 2-5-Gy fractions, and a total focal RT dose of 16 to 84 Gy in the lung tissue were retrospectively selected. Each patient underwent at least two X-ray CT studies (before and after RT); a total of 83 studies were analyzed. The new diagnostic procedure developed by the authors was used to quantitatively analyze lung tissue density changes in the areas with a selected dose range.Results. On days 1 to 15-30 after completion of RT, the quantitative analysis revealed no significant density changes. On days 70-80, there was an increase in changes that were different from those in the nonirradiated areas. On 80 to 100-120 days, there was a decline of radiation reactions with preserved changes in the areas irradiated at a dose of 19-20 Gy. From 120 days onwards, the changes were preserved in the areas irradiated at a dose of over 32-37 Gy through the formation of post-radiation pulmonary fibrosis. In addition, eight (27.6%) patients treated with less than 20 Gy were detected to have changes associated with the initial density values, which can be regarded as an increased individual radiosensitivity.Conclusion. The time course of a quantitative change in the median lung tissue density in the areas irradiated at different doses allows radiation-induced lung injury to be detected at X-ray CT until the visual signs of pneumonitis appear. A continuous, quantitative scale of changes will assist in more reliably and accurately studying post-radiation changes at a later time, and the time course of spaciotemporal changes will be able to compare and quantify lung injury when exploring new treatment methods and policies.

Objective: to analyze the possibilities of standard and additional methods for X-ray mammography (MG) in the comprehensive diagnosis of nonpalpable breast masses and preclinical cancers; to identify the important radiological signs of malignancy as predictors for the application of multislice spiral computed tomography/ magnetic resonance imaging (MSCT/MRI-MG), as well as invasive studies in premenopausal women. Material and methods. The investigation enrolled 423 pre-menopausal patients with nonpalpable breast masses; 129 women were found to have breast cancer. The diagnosis was verified by comprehensive clinical and instrumental examination using standard and additional methods for X-ray mammography, ultra-sonography, MRI, MSCT of the breast and biopsy. Significant radiological signs of malignancy of the lesions were detected and their diagnostic value was identified.Results. The use of additional diagnostic techniques for mammography contributes to the confident visualization of important radiological criteria for malignancy, enhances the informative value of by 5%, and promotes a decline in the number of unjustified biopsies by 11%. Conclusion. Problems in visualizing and categorizing nonpalpable breast masses necessitate additional diagnostic techniques for X-ray mammography. The identification and confident interpretation of important radiological criteria for benign or malignant lesions increase the diagnostic value of X-ray mammography: the sensitivity from 91% to 98%, specificity from 61% to 75%. MSCT-MG and MRI-MG demonstrate even higher efficiency: contrast-enhanced (CE) MSCT–MG (100% sensitivity, 99% specificity) and CE MRI–MG (100% sensitivity, 98% specificity). Sequential radiological and morphological studies improve the quality and efficiency of diagnosis of nonpalpable breast cancer.

The paper gives historical information on the foundation and establishment of the Russian X-Radiology Research Center, as well as the advances that have been fundamental in the further clinical, scientific, and educational activities of the center.It details information of the recent advances of the center in diagnosis, including radiation, clinical and molecular gene, and combined treatments for cancer patients, including surgery, polychemotherapy, and radiotherapy. The achievements of each subdivisions of the Center are given.The paper shows the latest advances of the Center and its workers in researches, which have been embodied in their dissertations, monographs, and guides.Of great importance is the educational process the Center’s workers are successfully doing in, by training specialists of different profile.

The paper describes the history of the Russian Research Center of Roentgenology and Radiology set up 90 years ago, the main advances of the researchers of the Institute in roentgenology and radiodiagnosis in historical perspective – from the last century up to the present day. It gives a brief list of the most important procedures and publications on radiodiagnosis, which have been developed and published by the Institute’s researchers.

Objective: to estimate the capabilities of magnetic resonance imaging (MRI) in revealing and estimating the extent of secondary vaginal involvement.Material and methods. Thirty patients with secondary vaginal tumor involvement underwent contrast-enhanced MRI. Examinations were made using a 1.5 T MRI. The protocol encompassed T2- and T1-weighted, diffusion-weighted, and dynamic contrastenhanced MRIs. To smooth out vaginal wall rigosity, the examination was made using an intracavitary MR-compatible applicator.Results. Vaginal metastatic involvement was detected in 30 patients with sustained malignancies at various sites. The largest group consisted of 16 (53.3%) patients with uterine corpus metastases. Secondary vaginal involvement was observed in 11 36.7%) patients with cervix uteri cancer, in 2 (6.7%) patients with progressive ovarian cancer, and in 1 (3.3%) patients with rectal cancer. With a significant tumor volume giving rise to thickening of the vaginal wall or its impaired slice differentiation, the portion of an inhomogeneously increased MR signal portion during T2-weighted MRI, which intensively accumulated the paramagnetic in an arterial and venous phase of dynamic contrast-enhanced MRI, was detected. There were no T2-weighted images of a hypointense muscular layer and paravaginal fat streaks during an invasive process. In our investigation, vaginal metastatic tumors which were enclosed by the mucous membrane and evident as shallow erosion, as shown by the data of clinical and рistological examinations, had no specific MRI signs in any of the pulse patterns, which is likely to be associated with the resolution of this technique.Conclusion. The proposed protocol of comprehensive magnetic resonance imaging makes it possible to clearly localize a tumor process into the vagina and to determine the volume and pattern of involvement in most patients, including whose who have a history of antitumor treatment.
