Aim. To analyze level of circulating biomarkers of plasma vascular endothelial growth factor (VEGF) and tumor necrosis factor-α (TNF-α) during 3-month therapy with a fixed-dose combination of ramipril/indapamide (Konsilar-D24, AO Vertex, Russia), as well as to evaluate the antihypertensive efficacy of a fixed-dose combination of ramipril/indapamide (Konsilar-D24, JSC “Vertex”, Russia) in hypertensive (HTN) patients with type 2 diabetes (T2D).Material and methods. This prospective open-label study included 44 patients (aged 35 to 60 years) of both sexes with essential grade 1-2 HTN and concomitant compensated T2D, who did not reach the target blood pressure (BP) level using single or dual antihypertensive therapy, as well as patients who did not take antihypertensives. All patients included in the study initially underwent a set of standard clinical, laboratory and functional examinations in accordance with the clinical guidelines for the management of patients with HTN and T2D, as well as an assessment of the level of C-reactive protein, VEGF and TNF-α. Patients were monitored and treated with Konsilar-D24 for 3 months.Results. In 93,2% of patients, individual target BP values were achieved during the first 2-4 weeks of therapy with a fixed combination of ramipril/indapamide (Konsilar-D24). In the subsequent 3-month follow-up, the average daily BP level in all patients ranged from 129/79 mm Hg to 110/70 mm Hg. Three-month Konsilar-D24 therapy showed a decrease in microalbuminuria: the median values of microalbuminuria decreased by 2 times, and the decrease in the maximum recorded values reached 40% of the baseline. Decrease in mean TNF-α values after 3-month therapy with Konsilar-D24 was 33% of the baseline values, while the maximum recorded values during the specified period decreased by 17%. Decrease in median VEGF values after 3-month Konsilar-D24 therapy was 28%, while the maximum value decreased by 7%, the minimum — by 8%.Conclusion. Konsilar-D24 improves the prognosis in hypertensive patients not only by reducing BP to target values, but also by reducing the level of VEGF and TNF-α biomarkers that determine the progression of endothelial dysfunction, diabetic retinopathy, and microalbuminuria.

Aim. In practice, to evaluate the efficacy, safety and long-term adherence to therapy with a fixed-dose combination of ramipril/indapamide (Konsilar-D24) in patients with grade 1-2 hypertension (HTN) who have not achieved blood pressure (BP) control with prior therapy or have not taken antihypertensive therapy.Material and methods. This multicenter open-label observational program included 524 patients with grade 1-2 HTN who did not take antihypertensive therapy or did not reach the target BP level with mono or dual antihypertensive therapy, as well as patients shifted to Konsilar-D24 therapy no later than two weeks before the start of the program. All patients signed a written informed consent to participate in the program. The safety analysis set includes all patients who have taken at least one dose of a fixed-dose combination of ramipril/indapamide and have visited physician at least once during the program. The effectiveness analysis set included all patients in the safety population who completed the study in accordance with protocol (n=511). Clinical systolic blood pressure (SBP), diastolic BP (DBP) and heart rate were assessed at baseline, as well as at 0,5, 1, 3 and 6 months of treatment. A post hoc subgroup analysis of changes in BP and heart rate was performed depending on age, sex and baseline body mass index.Results. The fixed-dose combination of ramipril with indapamide significantly reduced SBP and DBP after 2-week treatment (-20,9±10,1 mm Hg; pConclusion. Despite the limitations inherent in observational studies, the KONSONANS program has demonstrated high efficacy and safety of fixed-dose combination of ramipril/indapamide taken once a day in hypertensive patients. Ramipril/indapamide fixed-dose combination therapy significantly improved BP control and achieved even lower individual target BP levels in the majority of hypertensive patients.

Aim. To determine the feasibility of complex approach for the determination of individual tolerance to physical activity based on functional and biochemical markers, in order to choose an effective program of physical rehabilitation for patients with non-ST elevation acute coronary syndrome (NSTEACS) after percutaneous coronary intervention (PCI).Material and methods. A prospective study included 80 patients with NSTEACS who underwent coronary revascularization. One month after the acute coronary event and conducting PCI, patients were invited to participate in cardiac rehabilitation programs, including with the inclusion of exercise program. In order to select a program of physical rehabilitation, a general clinical examination, an assessment of the risk of cardiovascular complications in NSTEACS, and an individual tolerance to physical activity were carried out. Individual tolerance to physical activity was assessed by the results of a loading test and biochemical markers characterizing metabolic processes during aerobic exercise. A loading test was performed on the Marquette-Hellige 2000 treadmill (Bruse protocol). According to the results of the exercise test and biochemical markers, patients were recommended to controlled walking or prescribed a short course of exercises with an intensity of 60% of the threshold power.Results. At the end of the short cycle of the exercise program, a significant increase of the minute oxygen consumption (MOC) was observed in 72,5%. In 15,5% of the remaining patients the increase of MOC was insignificant, and in 12% MOC remained at the initial level. The analysis of biochemical markers in the dynamics of these patients showed consistently elevated values. The level of urea and creatinine increased by 16,7% compared with baseline afterload parameters. The level of creatine phosphokinase decreased by 16-24% of the afterload data. Myoglobin level was 106,2+8,4 pig/i vs baseline 90,2±6,2 gg/l. In addition, the total duration of exercise programs in these patients was less than stated.Conclusion. Thus, focusing on the initial level of the MOC in determining of load is not always possible to obtain the expected increase in physical performance. In this case, the assessment of biochemical blood changes allows to determine signs of overstrain under the influence of physical exertion, which makes it possible to correct the level of load.

The expert consensus of the Russian communities discusses the issues of comprehensive medical rehabilitation of patients with coronavirus disease 2019 (COVID-19). COVID-19 is a contagious infectious disease that can cause pulmonary, cardiac, motor, metabolic, neurocognitive and mental disorders, that is, multiple organ dysfunction. COVID-19 survivors, especially after a severe clinical course, face serious psychological and physical problems, posttraumatic stress, cognitive dysfunction, nutritional deficiencies and exacerbation of concomitant chronic diseases. Some patients, regardless of the COVID-19 severity, have a long clinical course of the disease (“chronic-COVID”, “long COVID”). There is evidence of post-acute COVID-19 syndrome. In this regard, patients after COVID-19 will need rehabilitation measures. The expert consensus of the Russian communities presents general principles, stages and aspects of medical rehabilitation after COVID-19, indications and contraindications for rehabilitation interventions. The paper includes recommendations on comprehensive cardiac and pulmonary rehabilitation, as well as specifics of rehabilitation care for different COVID-19 courses within the national three-stage rehabilitation system.

The review describes the experience in intermittent hypoxic conditioning protocols in rehabilitation programs for patients with cardiovascular diseases. Based on randomized controlled trials, the efficacy and safety of intermittent hypoxic conditioning in patients with cardiovascular disease, including those with multicomorbidity in clinical practice, have been analyzed. From a physiological and clinical standpoints, the review justifies the need for further controlled clinical trials on hypoxic conditioning with a longer follow-up period both to expand the clinical indications for this method and to develop optimal combinations with exercise within cardiac rehabilitation programs. Discovery of the key mechanisms of adaptation to oxygen concentration changes and further research on hypoxia physiology will expand the application of this method in clinical and rehabilitation medicine in cardiac patients.

Experts consensus of the Russian Cardiology Society (RSC)

Aim. To evaluate the readiness of acute coronary syndrome patients to distant physical rehabilitation (DPR) during the office rehabilitational counseling (ORC).Material and methods. Open retrospective study included data of 148 patients (117 males, 31 female), who had participated in ORC with the aim to include to DPR. Main part of ORC was conducted in the format of physician work with a computerized algorithm. Completeness of data to form the programs of physical rehabilitation (PR) was assessed manually with reviewing of the discharge summaries. Clinical status was assessed, and if necessary, the 6 minute walking test was conducted. Motivational component was assessed by the results of questionnaire “Program of calculation of motivational readiness of patients to adhere clinician recommendations”. Cognition was assessed with MMSE.Results. The part of “electronic” discharge summaries was 88,5%. Analysis of availability of epicrise data for algorithm modules showed that only 134 patients (97,3%) had complete data in clinical diagnosis. Also, an insufficiency was shown for number of stress tests with ECG registration: 22 (14,9%), but 96 (64,9%) with 6-minute test. In 30 patients (20,2%) the data on exercise tests was lacking. Data on PR regimen and related assessment of individual exercise tolerance was found in 34 (23% of ORC); and all who passed in-hospital stage. To the program of DPR about 1/3 (31,1%) of ORC participants were included. Under the framework of ORC, PR programs were created for all patients. By the results of investigation, a generalized clinical and instrumental characteristics of DPR patients were formulated, with added motivational and psychological specificities.Conclusion. It is found that in every 5th myocardial infarction patient discharged from hospital, there is lack of data on individual PR parameters that significantly reduces the ability of primary care physician in prescription and implementation of PR events. Usage of eHealth instruments at outpatient stage of cardiorehabilitation make it to solve these issues of ORC, which has as its tasks the PR programs formulation and evaluation of patient readiness for ORC participation.

Aim. To assess the neurohumoral state, specifics of clinical presentation, prevalence and prominence of cardiovascular and behavioral risk factors in males of young age with masked arterial hypertension (MAH).Material and methods. To the full coverage screening study, 643 participants included, age 18-30 y. o. (mean age 22). Complete clinical and laboratory, instrumental assessments performed, with analysis of neurohumoral activity by the levels of adrenaline, noradrenaline, angiotensin II. All participants were selected to groups: MAH (n=61), manifest AH (n=46), normotonics (n=536). Diagnostics of MAH was done in case of positive test of 30 second respiration pause and 24 hour blood pressure (BP) monitoring.Results. In males, MAH was found in 9,5% cases, manifest AH in 7,1%, normotonia in 83,4%. In MAH and manifest AH participants the level of adrenaline was higher than in normotonics: 4,55 pM/L [3,53; 5,1], 6,25 pM/L [4,75; 8,65] and 2,1 pM/L [1,2; 4,5], respectively. Males with MAH had almost 2 times higher level of noradrenaline (19,5 pM/L [18,8; 22,8]) comparing to “normotonics” (8 pM/L [6; 12,1]) and 1,5 times higher than in manifest AH (13,1 pM/L [11,5; 14,4]). Angiotensin II assessment showed that in MAH males it was higher (18,5 pM [15,75; 21,9]), than in normotonics (15,7 pM [13;18,3]), but lower than in manifest AH (20,15 pM [18,9; 21,9]). Food related behavior assessment as of a factor of sodium overconsumption, the higher values were found in MAH patients and in manifest AH (50% and 50%) comparing to normotonics (25%). Early age of smoking beginning found in 28% of MAH males, more prevalent than in normotonics (24%) and manifest AH (15%).Conclusion. In young males MAH is revealed more often than manifest AH. Development of MAH in young males is related to hypersympathicity with raised catecholamines level, disordered 24 hour rhythm “non-dipper”, disorder of BP variability and higher smoking level, than in normotonics.

Aim. To evaluate the prevalence of masked arterial hypertension (MAH) in young persons, as the prevalence and significance of cardiovascular risk factors, taken gender differences.Material and methods. A coverage screening study of 512 young persons age 19-30 y. o. was done, from institutional workers, with no specific professional harm exposures, of those 82,4% — normotonic and 17,6% have MAH. The participants underwent clinical laboratory and instrumental investigations, 30-second breath stop test.Results. In anthropometric measures, body mass index, waist circumference, hips volume were significantly higher (p6 times and in women >5 times higher than in normotonics of the same age.

Aim. To analyze, how is, under the framework of the Pilot Project, organized and being conducted the improvement of work of the known departments in exact medical institutions (MI) of RF for rehabilitation of cardiovascular diseases (CVD) patients at the three steps under the current “Order of medical care and medical rehabilitation”, and how do the scientifically proven and types and methods of cardiorehabilitation (CR) being implemented, and to try to elaborate on an exact algorithm for patients course.Material and methods. Into the Pilot Project, 17 MI included, from 13 entities of RF. For the analysis of effectiveness of organizational models of CR in MI the unified charts were developed making to assess the equality to the Order of the rehabilitation care provided to patients with CVD, of multidisciplinary team involved in care, the staff list and MI equipment. Specific charts included the units assessing the efficacy of work on three stages of CR: I stage — in resuscitation and intensive care unit or specialized cardiology department, II — in in-patient department, III — in polyclinics. The article presents data from CR analysis of Jan 2013 to Dec 2015. Results. The CR was organized at the stage I in 10 MI, stage II in 10 MI and stage III in 7 MI. By the year 2015, almost in all MI the multidisciplinary teams were shaped, including cardiologist, cardiorehabilitologist, exercise specialist, clinical psychologist, physiotherapist; the rooms for exercise were equipped, as the halls for education of patients. The start of rehabilitation process from ICU made it to decrease the remaining of patient on the bedside for 0,7 days in average (2013-2015) and for 1 day in cardiovascular department. About 67% myocardial infarction patients in average were transferred to the stage II of CR. In all MI there were educational Schools organized, and the explanatory talks with the relatives were done as well. By now, the project is on its way. Conclusion. The analysis of three-staged CR system for myocardial infarction patients according to current laws, showed significant improvement of rehabilitational activeness of the newly created multidisciplinary teams at all stages of CR, increase of the number of methods of CR and better fulfillment of those, as demonstrated the safety of proposed principals of physical rehabilitation according to the Russian clinical guidelines “Myocardial infarction with ST elevation: rehabilitation and secondary prevention” under the framework of the organizational model of CR.

Aim. To study effect of long-term controlled physical training (CPT) of moderate intensity on diastolic function, depending on the degree of recanalization, in patients with ischemic heart disease (ICD) with left ventricle diastolic dysfunction (DD) who underwent percutaneous coronary intervention (PCI). Material and methods. Patients with ICD (n=60, all males, aged from 49 to 58 years) who had undergone PCI last 3–6 weeks were included into the study. They were randomized into two groups depending on CPT availability in the rehabilitation program. Standard clinical examination, diastolic function assessment, exercise tolerance test were performed at baseline, after 6 and 9 months. Results. The long-term CPT of moderate intensity led to diastolic function improvement in 15.6% of patients after 6 months and 28.1% - in 9 months. Improvement of diastolic function in ICD patients with complete recanalization was observed after 6 months of CPT moderate intensity. Longer period of physical rehabilitation (9 months or more) was required for patients with incomplete recanalization. The improvement of diastolic function without CPT was observed after 6 months in 2 (7.1%) patients with a relaxation type of DD and complete recanalization. Normalization of diastolic function without CPT after 9 months was observed in 10.7% of patients and 3.5% of patients showed DD progression. Conclusion. Inclusion of the long-term CPT of moderate intensity into the rehabilitation program has cardioprotective effect in ICD patients with ischemic DD undergoing PCI.

Aim – to identify the most affordable way of physical training (PT) in patients with ischemic heart disease (IHD), promoting effective control of the intensity of inactivity and has a high level of adherence in physical training of CHD patients on an outpatient basis – outpatient rehabilitation stage. Materials and methods. The study included 76 male patients with coronary artery disease aged from 49 to 64 years. Results. Among the PE methods used in the study, moderate-intensity treadmill exercises at least thrice weekly are most effective as they promote the most increased exercise tolerance (ET), but in 3 months there is a noticeable reduction in adherence to this PE method. The heavyweight walking method assists in increasing the ET comparable to that of treadmill exercises and in effectively correcting the degree of hypodynamia and preserves high adherence to PE even after 6 months of their regular performance. Conclusion. Hypodynamia as a risk factor for cardiovascular diseases is common and encountered in 86 % of the CHD patients who have undergone percutaneous coronary interventions and myocardial infarction with the baseline exercise capacity being 7 METs or more, as evidenced by treadmill tests. Therefore this category of patients must be actively involved into the programs of cardiac rehabilitation and secondary prevention. On choosing PE methods, preference should be given to the method that aids in increasing ET and adherence to PT and in effectively correcting hypodynamia.

Currently, it is proved that physical training (FT) should be a compulsory component of programmes for complex rehabilitation of patients with coronary heart disease after PCI. However, given the heterogeneity of clinical status of patients after conducting PCI medical rehabilitation should be personalized as for the choice of intensity and exposure of FT and with obligatory monitoring of their effectiveness and safety. This greatly extends the possibilities of wide use of FT at the stage of rehabilitation in the real clinical practice.

In patients with coronary heart disease (CHD), ischemic preconditioning facilitates the mechanisms of cardioprotection and metabolic adaptation to non-fatal ischemia, which is particularly important for the individuals with reduced coronary flow reserve. Development of new cardiac rehabilitation programmes, based on the phenomenon of ischemic preconditioning, is a promising non-pharmacological method of CHD management.

Aim. To study effects of long-term, moderate-intensity controlled physical training (CPT) on blood pressure (BP) level and metabolic disturbances (MD) in young men with Stage I arterial hypertension (AH). Material and methods. The study included 49 men aged 18-45 years, with I Stage AH and MD, adhering to nonpharmaceutical lifestyle modification recommendations. Twenty-three patients additionally underwent long-term, moderate-intensity CPT for at least 6 months. Every 3 months, 24-hour BP monitoring, Doppler echocardiography (EchoCG), treadmill test, anthropometry with body mass index (BMI) calculation, lipid and carbohydrate profile assessment were performed. Results. Systematic moderate-intensity CPT course lasting for at least 3 months promoted decrease in systolic and diastolic BP (SBP, DBP). Long-term (6 months or longer) moderate-intensity CPT significantly reduced SBP and DBP levels, normalized SBP and DBP variability, restored circadian BP profile, beneficially influenced lipid and carbohydrate metabolism. Conclusion. Long-term, systematic moderate-intensity CPT in young men with Stage I AH demonstrated antihypertensive, anti-atherogenic, and metabolically neutral effects.

Aim. To study correlations between blood pressure (BP) level and autonomous dysfunction symptoms. Material and methods. The study included 84 males aged 18-45, with mean systolic BP (SBP) level of 110-139 mm Hg, and diastolic BP (DBP) level of 70-89 mm Hg. The standard physical and instrumental examination, 24-hour BP monitoring, and modified orthostatic test were performed. Results. In 80% of the patients with high normal (HN) BP, there were some clinical symptoms observed, e.g. headache and vertigo. Circadian BP profile was characterized by increased load index and elevated mean BP during all day, comparing to normotensive subjects. HN BP group was heterogeneous: in 42.3% of cases, BP variability was increased, nighttime BP was abnormally decreased, and hemodynamics reaction in orthostatic test was exaggerated. Conclusion. In young males with HN BP, autonomous dysfunction symptoms, associated with increased BP variability and abnormally increased nighttime BP drop, can be regarded as a complex of early cardiovascular disease predictors.

Patients surviving non-ST-segment-elevation acute coronary syndrome constitute a group of high coronary risk and sudden death risk and are therefore in particular need of adequate and proper rehabilitation and secondary prevention including physical training. 122 patients surviving non-Q-wave myocardial infarction or an episode of unstable angina have been studied, with mild to moderate arterial hypertension (mean age 52. 26± 0. 56 years), in the period of stabilization 3-8 weeks following the onset of the acute period. The main group (80 subjects) underwent controlled systematic physical training of moderate intensity for 3 months, leading to improved central hemodynamical parameters, left ventricle diastolic function, increased tolerance to physical stress, promoting blood pressure decrease, which, in overall, improved the clinical course of the disease and led to early restoration of working capacity, decreased invalidation, decreased the risk of cardiovascular complications.

Aim. To study the effectiveness of ambulatory rehabilitation and prevention programmes in patients after percutaneous coronary intervention (PCI), with the use of computerized medical decision support system (MDSS) facilitating the choice of controlled physical training (CPT) regimen and the delivery of adequate recommendations on physical activity (PA) levels. Material and methods. This 12-month study included 194 patients (124 men and 70 women; mean age 53,5±3,6 years) with stable coronary heart disease (CHD), who underwent PCI (coronary angioplasty or coronary stenting). The CPT regimen was chosen, using the computerized MDSS. To perform the analysis of standard medical decisions, 25 doctors participated in a questionnaire survey. Results. The patients from the CPT group, demonstrated a significant increase in exercise capacity (EC) and mean exercise duration, an improvement in quality of life (QoL), and good pharmaceutical therapy compliance throughout the rehabilitation period. As shown by the questionnaire survey data, standard medical decisions were based, on average, on three clinical parameters; methodological errors were the most prevalent ones. Conclusion. The use of rehabilitation programmes, based on computerized MDSS, in ambulatory post-PCI patients ensures not only high effectiveness of rehabilitation and prevention measures, but also safety of physical training.

Goal. The pilot project to organize or conduct performance improvement of existing structural units in specific hospitals (LU) Russia to provide rehabilitative care for patients with cardiovascular diseases in three stages under the current "Procedure for the provision of medical care for patients in medical rehabilitation", to introduce in practice these units tested scientifically sound approaches, forms and methods of cardio-rehabilitation, and try to create some patients routing algorithm. Material and methods. The pilot project includes 17 of the medical institutions of 13 regions of Russian Federation. Single special cards that allow to evaluate the appropriate order on the medical rehabilitation provided rehabilitation assistance to cardiac patients, a multidisciplinary team involved in the provision of rehabilitation assistance, staffing and equipping MI To analyze the effectiveness of organizational models in cardiorehabilitation MI have been developed. Special cards consisted of three sections, each of which allows you to evaluate the effectiveness of each of the three stages cardiorehabilitation: Stage I - unit intensive care unit (BRIT), and a specialized cardiology department; Stage II - stationary cardiorehabilitation department; Stage III - patient department cardiorehabilitation. The article presents the results of a three-year analysis from January 2013 to December 2015. Results. The pilot project was organized with the cardiorehabilitational help stage I in 10 DR, II stage - a 10-LU and Phase III - 7 DR. By 2015, almost all of Latvia to provide assistance in cardiorehabilitational area, multidisciplinary teams consisting of a cardiologist, cardiologist, rehabilitator, medical physical culture (physical therapy) specialist, physical therapy instructor-methodologist, psychotherapist, clinical psychologist, physiotherapist were formed. They were provided and equipped with facilities to practice physical therapy, room for educational schools. Home rehabilitation process BRIT has reduced the patient's stay in the bed of 0.7 days (2013 to 2015) and for 1 day in the cardiology department. On average, 67% of patients with acute myocardial infarction (AMI) were translated into Phase II cardiorehabilitaяtion in which they have become actively involved in the physical rehabilitation program, which is based on a classic exercise therapy, dosage walking in the room, physical training on simulators. Each MI educational schools were organized for patients actively pursued discussions with the relatives. Special complexity of the project has caused the organization phase III cardiorehabilitation. The project is currently ongoing. Conclusion. The analysis of the organization of a three-stage kardioreabilitatsionnoy care system with AMI patients considering the use of staffing, equipment and methodology, according to the current normative law, showed a significant increase in recreational activities created by multidisciplinary teams in all three stages of cardio-rehabilitation, increasing the number of used rehabilitation methods and improve the quality of their performance, as well as demonstrated safety of the proposed principles of physical rehabilitation, according to Russian clinical guidelines "Acute myocardial infarction with ST-segment elevation ECG: rehabilitation and secondary prevention", in the framework of the proposed organizational models cardiorehabilitation.

Цель. Изучить влияние длительных контролируемых физических тренировок умеренной интенсивности - ФТУИ (60% от толерантности к физической нагрузке) на уровень артериального давления (АД) у лиц мужского пола молодого возраста с маскированной артериальной гипертензией (АГ). Материал и методы. Обследованы 48 пациентов с маскированной АГ, имеющих не более 2 факторов риска, в возрасте от 18 до 36 лет, которым согласно алгоритму врачебной тактики «Национальных рекомендаций по профилактике, диагностике и лечению АГ» были рекомендованы динамическое наблюдение с амбулаторным мониторингом АД в течение 3 мес и немедикаментозная коррекция образа жизни. Пациенты были рандомизированы на 2 группы. Пациенты 1-й группы (n=25, средний возраст 27,9±4,7 года, АД 145,3±8,1/91,0±4,8 мм рт. ст.) выполняли врачебные рекомендации по коррекции образа жизни. Пациенты 2-й группы (n=23, средний возраст 29,1±4,8 года, АД 144,8±6,8/92,9±4,8 мм рт. ст.) выполняли врачебные рекомендации по коррекции образа жизни и контролируемые ФТУИ на беговой дорожке ATEMIBT-5400 3-5 раз в неделю в течение 3 мес. Динамическая оценка АД производилась по результатам показателей, полученных при суточном мониторировании АД (СМАД). Результаты. Через 3 мес наблюдения у пациентов 1-й группы отмечалось понижение систолического АД (САД) на 3,5±0,92 мм рт. ст. и диастолического (ДАД) - на 1,5±0,61 мм рт. ст. У пациентов 2-й группы после курса ФТУИ отмечалось снижение САД на 8,2±1,25 мм рт. ст. и ДАД - на 3,7±0,81 мм рт. ст. Целевой уровень АД у пациентов 1-й группы был достигнут у 29,7%, у 37,9% АД не изменилось. У пациентов 2-й группы в 58,8% определялся целевой уровень АД, у 41,2% АД снизилось, но целевого не достигло. Нормализация показателей вариабельности АД отмечалась у 27,0% 1-й группы, у остальных выходила за пределы нормы. Во 2-й группе нормализация вариабельности САД и ДАД отмечалась у 52,2%, у 17,6% - нормализация только ДАД и 11,8% - только САД. Во всех группах достоверно улучшились показатели качества жизни по шкалам физического и социального функционирования и общественного здоровья, особенно значимо у пациентов 2-й группы. Заключение. Длительные контролируемые ФТУИ приводят к достоверному снижению САД и ДАД, нормализации вариабельности как САД, так и ДАД, способствуют повышению качества жизни у лиц молодого возраста мужского пола с АГ 1-йстепени.

Основной целью использования контролируемых физических тренировок (КФТ) на этапе реабилитации у больных после чрескожного коронарного вмешательства (ЧКВ) является получение максимального долговременного кардиопротективного эффекта. Оценить эффективность проводимых реабилитационных программ можно с помощью функциональных маркеров, в то же время использование молекулярных маркеров может значительно повысить уровень индикации кардиопротективного эффекта ФТ в программах кардиореабилитации. Цель. Оценить динамику уровня молекулярных и функциональных маркеров при проведении комбинированных КФТ (интенсивностью 60 и 80%) и КФТ интенсивности (60%) на стационарном и постстационарном этапах у пациентов с неполной реваскуляризацией миокарда после ЧКВ и миокардиальной дисфункции. Материал и методы. В открытое проспективное исследование были включены 122 пациента с ишемической болезнью сердца (ИБС) после ЧКВ с частичной реваскуляризацией и положительными результатами парного стресс-теста с физической нагрузкой. Пациенты 1-й группы (n=71) на фоне стандартной медикаментозной терапии выполняли короткий курс (10 КФТ) ежедневных КФТ интенсивностью 80% в условиях реабилитационного стационара с последующим 12-недельным курсом умеренных КФТ в амбулаторных условиях. Пациенты 2-й группы (n=51) выполняли КФТ только интенсивности 60% на стационарном и амбулаторном этапах аналогичной продолжительности. В оценке кардиопротективного эффекта КФТ использовались как функциональные маркеры (максимальная депрессия сегмента ST, число отведений с депрессией сегмента ST≥1 мм, время восстановления сегмента ST до исходного уровня, эктопическая активность), так и молекулярные (ишемия-модифицированный альбумин - ИМА, свободные жирные кислоты - СЖК, мозговой натрийуретический пептид - N-про-МНП). Результаты. После 10-дневного курса КФТ в 1-й группе отмечались снижение уровня депрессии сегмента ST на 18,6% , во 2-й - на 14,7%; в 1-й группе уменьшение продолжительности восстановления сегмента ST на 15,7%, в группе сравнения - на 13,2% , снижение эктопической активности в 1,5 раза, в группе сравнения - в 1,1 раза. Это сопровождалось снижением уровня ИМА на 8,6% (127,2±23 нг/мл, исходно 139,6±12 нг/мл); в группе сравнения - на 7,07% (126,2±19 нг/мл, исходно 135,6±11,3 нг/мл). В 1-й группе уровень СЖК снизился на 13,5% (0,89 ммоль/л, исходно 1,03 ммоль/л), в группе сравнения - на 12,5% (0,98 ммоль/л, исходно 1,12%). Аналогично уровень N-про-МНП в 1-й группе снизился на 5,7% (45,7 пг/мл, исходно 48,48 пг/мл); в группе сравнения - на 4,1% (43,8 пг/мл, исходно 45,7 пг/мл). После 12-недельного курса умеренных КФТ в амбулаторных условиях у пациентов 1 и 2-й групп при сравнении по уровню прироста функциональных маркеров достоверно значимых изменений не выявлялось, в то же время уровень молекулярных маркеров ишемии достоверно снизился в 1-й группе. Заключение. Таким образом, комбинированные ФТ (интенсивностью 80 и 60%) и КФТ интенсивности (60%) на стационарном и постстационарном этапах у больных ИБС с частичной реваскуляризацией миокарда являются сравнимыми между собой по безопасности, но более эффективными в формировании кардиопротективного эффекта. Для оценки эффективности физической реабилитации у больных ИБС с частичной реваскуляризацией миокарда как на стационарном, так и на постстационарном этапе наблюдения целесообразно использовать аддитивный подход в оценке функциональных и молекулярных маркеров.

Цель. Разработка технологий и оценка безопасности новых видов продуктов на молочной основе, хлебобулочных и кондитерских изделий, обогащенных пищевыми волокнами (ПВ). Целесообразность проводимых исследований обоснована эффективностью использования ПВ в ежедневном рационе людей, в том числе пациентов с сердечно-сосудистыми заболеваниями (ССЗ), для уменьшения риска их развития. «Растворимые типы волокон могут оказать влияние на процессы усвоения пищевых веществ в тонкой кишке путем формирования гелей, которые уменьшают всасывание простых углеводов и насыщенных жирных кислот, тем самым оказывая влияние на уровень глюкозы и липидный спектр крови после приема пищи. Формирование гелей также замедляет секрецию желудочного сока, увеличивает чувство насыщения и ведет к снижению массы тела. Растворимые ПВ и устойчивые молекулы крахмала дополнительно влияют на микрофлору толстой кишки, что приводит к синтезу короткоцепочечных жирных кислот, которые опосредованно снижают уровень общего холестерина». Материал и методы. При выполнении работы использованы стандартные, общепринятые и оригинальные методы исследований, в том числе физико-химические (газожидкостная хроматография, атомно-адсорбционная спектроскопия, фотоколориметрия), реологические, микробиологические, биохимические, органолептические и др. На основании проведенных исследований создана серия оригинальных технологий и рецептур продуктов на молочной основе с ПВ: кисели на основе молочной сыворотки пастеризованные, коктейли на основе молочной сыворотки пастеризованные, десерты плодово-ягодные кислородные замороженные, сырники творожные с комплексом ПВ. Проводятся исследования по разработке технологий хлебобулочных изделий на основе муки из цельносмолотого зерна пшеницы с добавлением различных растительных источников сырья (гречневого и горохового пищевого волокна, полученного дезинтеграционно-волновым методом измельчения, а также семян сафлора). Разрабатывается ассортиментная линейка низкокалорийных кондитерских изделий с пищевыми волокнами. Результаты. Проведенные микробиологические исследования и исследования по безопасности продуктов показали, что они соответствуют требованиям Технического регламента таможенного союза 033/2013 «О безопасности молока и молочной продукции», Технического регламента таможенного союза 021/2011 «О безопасности пищевой продукции». Заключение. Следует отметить, что правильное питание занимает важнейшее место среди немедикаментозных способов профилактики ССЗ, в связи с чем разработанные продукты с ПВ, способствующие снижению дефицита эссенциальных веществ в организме и повышению качества жизни людей, могут быть рекомендованы в диетотерапии пациентов с ССЗ.

Оценка качества жизни (КЖ) наряду с клиническими признаками становится одним из основных критериев оценки эффективности лечения и реабилитации, особенно при заболеваниях, требующих длительной, многолетней терапии, и может являться индикатором в выборе эффективных методов реабилитации у больных ишемической болезнью сердца (ИБС). Цель. Изучить КЖ на III этапе (амбулаторном) реабилитации с использованием длительных физических тренировок (ФТ) у больных ИБС, перенесших острое коронарное событие. Материал и методы. В исследование были включены 39 больных ИБС среднего возраста 49,8±6,7 года, мужского пола, через 6-8 нед после перенесенного острого инфаркта миокарда с подъемом сегмента ST, рандомизированные в 2 группы: основная (20 больных) и контрольная (19 больных). Больные в группах были сравнимы по возрасту, клиническим характеристикам, уровню образования. Все больные были включены в комплексную реабилитационную программу, пациенты основной группы выполняли контролируемые ФТ в течение 6 мес. Для выбора программ ФТ использовалась программа для ЭВМ «Выбор программы физической реабилитации для пациентов с инфарктом миокарда с подъемом cегмента ST» (свидетельство Роспатента №2016618410 от 28.07.2016), в основу которой положены Российские рекомендации «Реабилитация и вторичная профилактика у больных, перенесших острый инфаркт миокарда с подъемом сегмента ST» (2014 г.). КЖ изучалось с использованием опросника ВОЗ-КЖ-100 (WHO-QL-100). Результаты. Через 6 мес выполнения реабилитационных мероприятий у больных определялось увеличение толерантности к физическим нагрузкам как в основной, так и контрольной группах на 25,8 и 12,1% соответственно, хотя суммарный показатель КЖ увеличился только в основной группе - на 12,6%, а в контрольной группе снизился на 4,1%. При анализе показателей жизненных субсфер по опроснику ВОЗ-КЖ-100 у больных основной группы отмечалась стойкая положительная динамика. Улучшение физической сферы PH на 13,7% было обусловлено достоверным повышение показателя как субсферы «жизненная активность и энергия» на 12%, так и других субсфер. Улучшение психологической сферы PS отмечалось в большей степени за счет роста показателя F4 «положительные эмоции» - на 26,3%. При анализе сферы независимости LI определялась стойкая тенденция к повышению показателя «удовлетворенность повседневной активностью, подвижностью, способностью к работе» (6,3%) и «независимость от лечения и лекарств». В сфере социальных взаимоотношений SR положительные изменения в большей степени касались субсфер личных отношений (11,5%) и сексуальной активности (5,4%). Иная ситуация наблюдалась у больных контрольной группы через 6 мес: показатель физической сферы имел тенденцию к снижению, показатель, характеризующий удовлетворенность доступностью и качеством медицинской и социальной помощи, снизился даже на 14%, показатели психологической сферы и социальных взаимоотношений оставались без изменений. Заключение. Включение в программу реабилитации на амбулаторном этапе длительных ФТ оказывает положительное влияние на физическое, психологическое и духовное состояние больных ИБС, способствует повышению уровня независимости, улучшению социальных взаимоотношений, повышению трудовой активности и успешной социальной адаптации, в свою очередь, улучшение психологического статуса повышает приверженность больного выполнению реабилитационных программ, а анализ КЖ отражает эффективность проведенных реабилитационных мероприятий.

Адаптация к факторам среды, таким как холод, физические нагрузки, гипоксия в горах, высокие температуры, дозированный стресс и т.д., эмпирически использовалась для повышения резистентности организма к повреждающим факторам задолго до начала систематических исследований ее защитных эффектов и механизмов. После открытия Ф.З.Меерсоном перекрестных защитных эффектов адаптации особое внимание привлекла адаптация к периодической гипоксии (АПГ), обладавшая наибольшим количеством таких эффектов. Адаптация к гипо - и нормобарической гипоксии в настоящее время особенно успешно применяется в клинике для профилактики и лечения сердечно-сосудистых заболеваний и модификации факторов риска. В частности, АПГ устойчиво уменьшает ишемические и неишемические аритмии, улучшает липидный профиль у пациентов с гиперхолестеринемией и повышает толерантность к физическим нагрузкам. У пациентов улучшается субъективное самочувствие и уменьшается количество и продолжительность эпизодов депрессии сегмента ST . В ряде клинических исследований было показано, что АПГ способствует потере массы тела у пациентов с ожирением. Важным терапевтическим эффектом АПГ является ее антигипертензивный эффект, который вначале казался неожиданным в свете хорошо известных данных о том, что периодическая гипоксия, сопровождающая апноэ во сне, напротив, способствует повышению артериального давления. Однако к настоящему времени уже убедительно доказано, что главными детерминантами направленности эффекта периодической гипоксии являются ее интенсивность и продолжительность, и поэтому в зависимости от режима периодической гипоксии ее эффекты могут быть прямо противоположными. Недавние исследования показали, что не только АПГ, но и адаптация к ишемии миокарда обладает выраженным кардиопротекторным эффектом и способствует повышению толерантности к физическим нагрузкам у кардиологических пациентов. Экспериментальные исследования на животных позволили выявить основные механизмы кардиопротекторных эффектов АПГ, среди которых ключевыми являются активация антиоксидантной защиты за счет периодической умеренной активации свободнорадикальных процессов, повышение эффективности энергопродукции в клетках, ограничение Са-перегрузки, предупреждение дисфункции эндотелия коронарных и некоронарных сосудов за счет активации синтеза и депонирования оксида азота, усиление экспрессии белков теплового шока и увеличение плотности микрососудистой сети в скелетных мышцах, головном мозге и миокарде. Таким образом, АПГ является перспективным нефармакологическим средством, которое может использоваться у кардиологических пациентов в качестве дополнения к лекарственной терапии, а в некоторых случаях, возможно, и самостоятельно.

В настоящее время неуклонный рост числа госпитализаций, обусловленных развитием осложнений среди больных с хронической сердечной недостаточностью (ХСН), определяет необходимость оптимизации как медикаментозного лечения, так и диетотерапии. Энтеральная оксигенотерапия в виде приема кислородных коктейлей может применяться для уменьшения проявлений синдрома хронической интоксикации, достижения иммуномодулирующего эффекта, повышения умственной и физической работоспособности, нормализации метаболических нарушений. Цель. Совершенствование технологии кислородного коктейля и создание новых видов кислородсодержащих продуктов повышенной пищевой ценности с улучшенными сенсорными характеристиками для дополнительной диетотерапии больных с ХСН.Разработаны рецептуры специализированных продуктов (кислородные коктейли, смузи, замороженные десерты) на основе молочной сыворотки и фруктово-ягодных наполнителей. Формирование пенной структуры в таких продуктах происходит благодаря пенообразующей способности сывороточных белков и использованию пищевых волокон, в том числе некрахмальных полисахаридов в качестве стабилизаторов полученной кислородной пены взамен сиропа корня солодки, так как прием солодки и препаратов на ее основе у больных с ХСН может вызывать повышение артериального давления, нарушение сердечного ритма и изменение выделительной функции почек. Известно, что белки сыворотки молока отличаются большим содержанием незаменимых аминокислот и серы по сравнению с казеином и считаются полноценными с точки зрения физиологии питания. Помимо белка молочная сыворотка содержит другие энергетические, пластические и регуляторные биологически активные вещества, необходимые пациенту для поддержания нормального состояния организма. Полисахариды, относящиеся к классу пищевых волокон, выполняют in vivo ряд важных биологических функций: участвуют в построении клеточных стенок и межклеточного матрикса, в регулировании обмена ионами между клеткой и ее окружением, являются для клетки энергетическим резервом. Полисахариды являются основными компонентами пищи, определяющими ее структуру, органолептические характеристики и функциональные свойства. Разработка технологии новых видов кислородсодержащих продуктов позволила обеспечить не только требуемые органолептические показатели и технологические характеристики, стабильность состава и потребительских свойств в процессе хранения, но и высокую пищевую и пониженную энергетическую ценность по сравнению с традиционным продуктом, что является особенно важным для больных с ХСН.Работа выполнена при поддержке гранта Президента РФ МК-3731.2013.4.

В Концепции развития здравоохранения до 2020 г. дистанционный мониторинг кардиоваскулярных заболеваний (КВЗ) и скрининг донозологических состояний представлены как наиболее перспективные направления. Создание мобильных информационных систем в области профилактики КВЗ определено как современное технологическое решение.Цель. Изучить возможности применения телемедицинской информационно-аналитической системы (ТИАС) «РеаПроф» для дистанционного скрининга и мониторинга факторов риска (ФР) у здоровых лиц и больных с КВЗ.Материал и методы. В работе использована ТИАС «РеаПроф» (ООО НПП «Волготех», Саратов), разработанная на основе «облачных» технологий для профилактики КВЗ. Скрининг ФР проводился с использованием мобильных персональных компьютеров в условиях лечебного учреждения, по месту учебы/работы обследуемых. Автоматизированная диагностика ФР осуществлялась в соответствии с доказательными критериями, изложенными в клинических рекомендациях. В рамках краткого профилактического консультирования давались рекомендации по здоровому образу жизни и необходимости углубленного обследования. Отклик среди обследуемых составил 65%: в группу мониторинга были включены пациенты с документированными КВЗ (n=12) и лица молодого возраста (n=25). Телемедицинский мониторинг ФР проводился в течение 2 мес, контролировались параметры, необходимые для коррекции лекарственной терапии у пациентов с диагностированными ФР и/или контроля динамики отдельных ФР при выполнении врачебных профилактических рекомендаций. Этап мониторинга включал отсроченные профилактические онлайн-консультации в соответствии с индивидуальными терапевтическими целями, информационные блоки мотивации в рамках опции «Школа пациента», предоставляемой системой. Оценивался уровень приверженности в процентах соответствия количества онлайн-отчетов пациентов всем врачебным назначениям. Результаты. Использование алгоритма углубленного профилактического консультирования в ТИАС позволяет автоматически формировать персонализированные программы мониторинга ФР и «Карты здоровья» на этапах первичной и вторичной профилактики. Результаты дистанционного мониторинга констатировали высокий уровень приверженности врачебным рекомендациям: студенты - 97%; молодые работающие пациенты - 45%; больные с КВЗ показали удовлетворительный уровень приверженности лекарственным (92%) и немедикаментозным рекомендациям (83%).Заключение. Применение современных технологических решений на основе «облачных» технологий расширяет возможности использования доказательных знаний в персонализации профилактических программ и мониторинге ФР КВЗ и донозологических состояний.

Клиническая эффективность программ физической реабилитации является залогом успешности реабилитационно-профилактических мероприятий у больных ишемической болезнью сердца (ИБС) после чрескожных коронарных вмешательств (ЧКВ).Цель. Провести сравнительный анализ клинической эффективности наиболее часто используемых на практике программ длительных физических тренировок (ФТ) при применении на амбулаторном этапе реабилитации у больных ИБС после ЧКВ.Материал и методы. В исследование включены 95 больных ИБС мужского пола в возрасте от 45 до 64 лет, из них 64 перенесли инфаркт миокарда и 91 выполнено ЧКВ. Больные не имели противопоказаний к назначению длительных ФТ и получали медикаментозную терапию согласно действующим стандартам. Пациенты случайным образом были рандомизированы в 4 группы, сопоставимые по клиническому статусу и результатам ЧКВ. В 1-й группе ФТ умеренной интенсивности проводились на тредмиле не менее 3 раз в неделю; во 2-й - тренировочная ходьба в заданном темпе не менее 2 ч ежедневно; в 3-й - ходьба с утяжелением в произвольном темпе не менее 1 ч ежедневно (патент RU № 2440085 C1 от 20.01.2012); 4-я группа - контроля, не выполняющая ФТ. Продолжительность ФТ составила 12 мес. При оценке клинического статуса учитывали функциональный класс (ФК) стенокардии по Канадской классификации, ФК хронической сердечной недостаточности (ХСН) по NYHA, повторные госпитализации по поводу прогрессирования ИБС и ХСН и развития осложнений, случаи повторной коронароангиографии (КАГ) и проведения ЧКВ, а также приверженность медикаментозной терапии и проводимым длительным КФТ (дневники самоконтроля пациентов).Результаты. При анализе клинического статуса через 12 мес наблюдения в 1-й группе признаков стенокардии напряжения не имели 90,9% пациентов, в 3-й - 92,2%. ХСН I-II ФК имели только 6,3 и 6,5% пациентов соответственно. Во 2-й группе через 12 мес число пациентов со стенокардией напряжения I-II ФК составило 25,9%, в контрольной группе - 31,9% Признаки ХСН I-II ФК во 2-й группе имели 13,5% пациентов, в группе контроля - 33,4%. Через 12 мес наблюдения в контрольной группе 2 человека были госпитализированы по поводу острого коронарного синдрома (ОКС), выполнено 5 КАГ, потребовалось 3 ЧКВ, из которых 2 были выполнены по поводу рестеноза стентированного сегмента. В основной группе в течение 6 мес развитие рестенозов не определялось, через 12 мес наблюдения госпитализаций по поводу ОКС, декомпенсации ХСН, повторных ЧКВ не было. При анализе приверженности наиболее высокий уровень приверженности КФТ через 12 мес зарегистрирован в 3-й группе (82,4%) и 1-й (75,3%). Улучшение приверженности ФТ ассоциировалось с лучшей приверженностью медикаментозной терапии (81,2 и 80,9% соответственно).Выводы. Таким образом, наиболее эффективно улучшению клинического статуса в виде снижения ФК стенокардии напряжения, ФК ХСН, улучшению прогноза засчет сокращения числа госпитализаций по поводу ИБС и ОКС способствовали ФТ умеренной интенсивности на тредмиле не реже 3 раз в неделю и ходьба с утяжелением не менее 1 ч ежедневно.
