The article deals with the lithology and geochemistry of organic matter of Upper Devonian-Lower Carboniferous deposits (Carbonate Devonian) of the eastern part of the South Tatar arch. Rocks of this age have a high generative potential due to increased concentrations of organic matter (OM) and its phytogenic and zoophytogenic composition. They refer to non-traditional sources of hydrocarbons. The generative potential depends on the facial environment and the transformation of OM. The highest potential have carbonate-siliceous and siliceous-carbonate rhythmites domannic horizon.

Berezovsky depression, located on the northeast of the Pre-Patom foredeep, is a promising target for hydrocarbon prospecting, as evidenced by oil and gas shows established while drilling, the presence of structures and natural reservoirs. However, this area has not yet been adequately explored due to the low drilling density. Geochemical study of organic matter (OM) of the Riphean, Vendian and Lower Cambrian sediments showed that the main oil and gas source rock is represented by terrigenous-carbonate Vendian deposits. Rocks of the Seralakh Formation possess highest initial and residual potential, as well as interlayers of rocks with heightened content of algal OM in the Vendian and Lower Cambrian sections. The oxidation of OM during sedimentation and the presence of bottom algae could have a negative impact on the generation potential of these deposits, which led to a decrease in OM concentrations and deterioration of kerogen; most likely, these factors determine the smaller resources of the Berezovsky depression. Probably, the study of the Riphean complex deserves attention, because there are oil-source rocks highly enriched in OM in the Middle and Upper Riphean of the south-west of the Baikal-Patom fold zone, the transformation of which is corresponded to peak of «oil window» – «gas window».

Unconventional oil and gas resource development and exploration is the one of the most prospective concept in petroleum geology. High carbon Khadum facies are investigated as unconventional resources in the Precaucas basin and contain gas, gas condensate and oil accumulations. Oligocene and Lower Miocene structure, prospectivity and fields distribution are the one of the most relevant subjects nowadays.This study is focused on the main stages Late Eocene and Early Oligocene deposits formation and the paleogeography of the Khadum formation in the Precaucas oil and gas basin. The new prospective zones are developed in Khadum formation based on the complex analysis of seismic, paleogeography and lithofacies analysis.

The paper considers palaeoincisions in the Turnean limestones of the Volga-Ural basin, made by alluvial-deluvial sediments of the Visean age and containing interlayers of coals. Processes occurring in these sediments (catagenic reduction of thickness of coal beds and coal-bearing mudstones, aggressive influence of products of defluidization of coal organic matter on the host rocks, etc.) strengthen the fluid dynamic heterogeneity of intracrustal deposits and contribute to emigration of hydrocarbons (HC). The main oil-and-gas-generating strata in the Carboniferous section include rocks of the Tournaisian stage and the Bobrikovsky horizon of the Viseian stage. Palaeoincisions, along with the area distribution of Viseian coals, can be considered as centers of fluid generation, including liquid and gas HC.

To get properties and characteristics of oil and gas source rocks and for other geological aims, geologists analyzes core samples from wells and outcrop samples. But we should probably somehow correct geochemical source rock properties data we get on outcrop samples, because organic matter could change significantly due to oxidizing and weathering. This problem is very important but poorly developed in modern publications, so it attracted our interest and led to this investigation. Published information on the organic matter weathering which change it’s content and quality in sedimentary rocks is collected and summarized in this article. The changes of the kerogen (isolated from the mineral matrix) elemental composition during its long-term storage are presented.We compared the results we get in laboratory and results of other authors who dealt with natural changes of organic matter in outcrops. An attempt to interpret the results obtained from the point of view of hypergene changes in OM was made. Uncertainties that require further study and development are indicated. This work is the first step to better understanding of weathering effect on organic matter content and properties – question of great importance for making adequate oil and gas prospects estimations.This study continues the work of our foregoers and Teachers – geochemists of the Petroleum Geology Department of Lomonosov Moscow State University: N.B. The Advisory Board was held on December 24, 2021. The molecular genetic research lead specialists and national lead oncologists discussed issues of diagnosis of NTRK gene translocations in patients with non-small cell lung cancer (NSCLC), as well as current opportunities for the treatment of patients with NSCLC caused by NTRK gene fusions. The experts reaffirmed the necessity to identify timely patients with NSCLC caused by NTRK gene fusions, as the correct diagnosis of the disease, including the use of modern diagnostic methods of NTRK gene fusion (NGS is the most sensitive and specific method) determines the success of patient treatment. In this regard, it is critical that physicians know the advantages and disadvantages of each molecular diagnostic method used to have the opportunity to choose the best approach in each clinical case. In order to have a clear, well-functioning strategy for managing patients with suspected NSCLC caused by NTRK gene fusion, it is necessary to use molecular genetic tests, as well as include TRK inhibitors (in particular, the drug larotrectinib; at the time publication of the Resolution, the drug larotrectinib is not registered in the territory of the Russian Federation) in the clinical guidelines for the treatment of lung cancer. Larotrectinib is a highly selective tropomyosin receptor kinase (TRK) inhibitor. The clinical studies on larotrectinib have demonstrated high response rates and durable responses in adults and children with tumours associated with NTRK gene fusions, including primary CNS tumours and brain metastases. The objective response rate observed with larotrectinib was 79%, with 16% achieving a complete response and 64% achieving a partial response. At the same time, the median progression-free survival on larotrectinib was 28.3 months, and the median overall survival was 44.4 months.

Preparations of glycyrrhizic acid (GA) and ursodeoxycholic acid (UDCA) are effective in the treatment of various forms of non-alcoholic fatty liver disease (NAFLD) and gastroesophageal reflux disease (GERD) occurring against the background of duodeno-gastroesophageal reflux. However, their effectiveness in the treatment of hologenic diarrhea and reflux gastritis has not been sufficiently studied. Currently, there are no clinical guidelines for the diagnosis and treatment of patients with hologenic diarrhea and reflux gastritis, including those with obesity and NAFLD. It is assumed that the combined administration of GA and UDCA can be accompanied by a mutual potentiation of the effect of the drugs. The article presents a clinical case of a patient with NAFLD, obesity, dyslipidemia, GERD, refractory to treatment with proton pump inhibitors, associated with duodenogastroesophageal reflux, reflux gastritis and hologenic diarrhea. It was shown that GERD, reflux gastritis, and hologenic diarrhea developed in the long-term period after cholecystectomy. At the same time, the appointment of a complex preparation containing GA (35 mg) and UDCA (250 mg) made it possible to stop the manifestations of a typical reflux syndrome in patients with a form of GERD refractory to proton pump inhibitors, reflux gastritis, and diarrheal syndrome associated with an excess of bile acids, improve lipid profile parameters. It has been shown that the administration of GA (35 mg) and UDCA (250 mg) can improve the parameters of the intestinal microbiome and lipid metabolism, which can be used in the treatment of patients with atherogenic dyslipidemia. At the same time, patients with comorbid pathology, including NAFLD, GERD, reflux esophagitis, hologenic diarrhea, and dyslipidemia require long-term maintenance therapy with GA (35 mg) and UDCA (250 mg).

Aim: to identify the risk factors for gestational diabetes mellitus (GDМ) and predictors of perinatal lesions of central nervous system (CNS) combined with GDМ and maternal obesity.Materials and Methods. А retrospective observational case-control non-combined study was conducted to determine GDМ risk factors and their effect on perinatal pathology in 250 women divided into 2 groups. The main group included 150 pregnant women diagnosed with GDМ, the control group – 100 pregnant women without carbohydrate metabolism disorders. An assessment of hereditary, obstetric and gynecological history, as well as somatic health was carried out. Patients from the main group were subdivided into smaller groups: 1А (n = 77) – mothers whose newborns postnatally exerted adverse perinatal outcomes associated with impacting maternal hyperglycemia, and 1В (n = 73) – mothers whose newborns were born healthy. CHAID method (Chi Squared Automatic Interaction Detection) was used to create an algorithm for predicting adverse perinatal outcomes in GDМ. Аt the second stage, a single-center prospective observational non-combined cohort study was conducted to assess an effect of maternal hyperglycemia on formation of perinatal CNS lesions. Pre-labor concentration of neuron-specific enolase (NSE) was measured in the amniotic fluid of full-term fetuses in the group of pregnant women with GDM (n = 33) and in the group of pregnant women lacking carbohydrate metabolism disorders (n = 42).Results. Obesity, late reproductive age, family history of type 2 diabetes mellitus, abortions, early reproductive losses, macrosomic delivery in history are the main risk factors for GDM development. An algorithm was developed that allowed to predict a risk of newborn perinatal pathology in mother with GDM with an overall percentage of correct predictions of 68.7 ± 3.8 %. Pre-labor concentration of NSE in the amniotic fluid of full-term fetuses was elevated by 1.68 times (p = 0.006) in women with combined GDM and obesity (5.56 [3.37–6.24] ng/ml) compared to pregnant women with normal weight lacking carbohydrate metabolism disorders (3.29 [1.49–4.89] ng/ml).Conclusion. Pregnant women with obesity and type 2 diabetes mellitus familial history were featured with most prominent potential of developing perinatal complications. Rise in amniotic fluid NSE level in patients with GDМ corroborates damage of fetal CNS during antenatal period. The maximum NSE level was found in women comorbid with GDM and obesity.

Background. The implementation of tyrosine kinase inhibitors into clinical practice improved treatment outcomes in patients with radioiodine-refractory differentiated thyroid cancer (RR-DTC). Lenvatinib is recommended as a first-line drug for these patients. The study objective is to analyze clinical experience with lenvatinib in patients with RR-DTC in the Russian Federation. Materials and methods. The data from 18 clinical sites in Russia was analyzed for the period December 2015 and September 2019. Seventyseven patients with histologically verified DTC, proven resistance to radioactive iodine therapy, and tumor progression (according to the Response Evaluation Criteria In Solid Tumors 1.1 criteria) were included in the study. Results.Median progression-free survival in patients included into analysis (n = 72) was 26.1 months. In patients who responded to therapy (including those with partial and complete response), median progression-free survival reached 36.2 months, which is higher than that reported in the updated results of the SELECT study (33.1 months). Lenvatinib-associated adverse events (AEs) were observed in 87 % of patients. Severe AEs were registered in 18.2 % of participants. In 6.5 % of cases, AEs lead to lenvatinib cessation; in 74 % of cases, AEs required dose reduction.Conclusion. Our findings suggest high efficacy and good tolerability of lenvatinib in patients with RR-DTC in routine clinical practice in the Russian Federation.

The review presents data on calprotectin, lactoferrin, leukocytes labeled with isotope indium 111In, calgranulin C and pyruvate kinase type M2 - highly sensitive biomarkers to assess the severity of intestinal inflammation. Their importance in diagnostics, determination of treatment efficiency, including as predictors of recurrence of ulcerative colitis and Crohn's disease is shown.

Radioiodine therapy of metastatic differentiated thyroid cancer is a highly effective method of treatment which makes it possible to achieve the complete cure of the majority of patients, however in some cases radioiodine refractory type develops, it leads to significant deterioration in overall and relapse-free survival. The article presents own experience of the therapy of radioiodine refractory thyroid cancer by the nexavar (sorafenib).

Aim. To compare the effectiveness of the effect of combination therapy (local and systemic administration) with bone marrow mesenchymal stromal cells (MSC), anticlitokine therapy with infliximab (IFX), and antibiotic (AB)/immunosuppressive (IS) therapy on the frequency of healing of simple perianal fistulas in Crohn's disease. Materials and methods. In our study, the 1-st group of patients aged 19 to 58 years (Me-29) (n = 12) received MSCs culture systemically according to the scheme and locally. The 2-nd group of patients with CD (n = 10) from 20 to 68 years old (Me-36) received anticytokine therapy with infliximab (IFX) according to the scheme. The 3-d group of patients with CD (n=14) from 20 to 62 years old (Me-28) received antibiotics (AB) and immunosuppressors (IS). Efficacy was assessed by the index of perianal activity of Crohn's disease (PCDAI) and the frequency of relapses. Results. After 12 weeks among patients of the 1-st group, healing of simple fistulas was noted in 8/12 patients (66.6%), in the 2-nd group in 6/10 (60.0%) In the 3-d group, in 1/14 patients (7.14%). After 6 months in the 1-st group of patients, healing of simple fistulas was preserved in 8/12 (66.6%), in the 2-nd group - in 6/10 (60.0%). In the 3-d group - in 1/14 patients (7.14%). After 12 months in the 1-st group, healing of simple fistulas was preserved in 7/12 (58.3%), in the second group - in 6/10 (60.0%). In the 3-d group - in 2/14 patients (14.3%). After 24 months, among the patients of the 1-st group, fistula closure was maintained in 5/12 patients (41.6%), in the 2-nd group - in 4/10 (40.0%). In the 3-d group - in 0/14 patients (0.0%). Conclusion. Combined cellular and anticytokine therapy of CD with perianal lesions significantly contributes to the more frequent and prolonged closure of simple fistulas, as compared to antibiotics/immunosuppressors, and to a decrease in the frequency of recurrence of the disease.

Personalized medicine (personalized medicine, individualized medicine) represents the totality of methods of prevention of a pathological condition, diagnosis and treatment in the event of its occurrence, based on individual patient characteristics. Such individual characteristics include genetic, epigenetic, and transcript, proteome, metabolomic and metagenomic markers, as well as a set of variable phenotypic traits - both of the patient's body and its separate tissues or cells. For example, treatment of inflammatory bowel diseases (IBD) can most clearly show the importance of applying personalized approaches. Currently in the treatment of patients with IBD paid great attention to genetic studies, monitoring of the concentration of the biological drugs and the level of antibodies to them, the role of microbiota as a predictor of effectiveness of therapy of IBD. Used clinical, laboratory, instrumental methods, as well as new biomarkers to assess the forecasting efficiency of conservative therapy in IBD patient. In the future treatment of patients with IBD will include a number of personalized data in order to better predict outcomes of the disease in each patient and more accurately select the appropriate treatment regimen.

Crohn's disease (CD) is a chronic, recurring disease of the gastrointestinal tract of unclear etiology. One of the new approaches to CD therapy is the use of the possibilities of stem cells, in particular, mesenchymal stromal cells (MSCs). Currently, the use of MSC in clinical practice for the treatment of chronic inflammatory and autoimmune diseases is being studied in patients who receive concomitant therapy with other immunomodulatory medications. Aim. To evaluate the effectiveness of MSCs therapy in patients with CD receiving azathioprine (AZA). Materials and methods. The study included 34 patients with inflammatory (luminal) form of CD. The 1st group of patients (n=15) received anti-inflammatory therapy using MSCs culture in combination with AZA. The 2nd group (n=19) received MSCs without AZA. The severity of the attack was assessed in points in accordance with the of Crohn's disease activity index (CDAI). Immunoglobulins (IgA, IgG, IgM), interleukins (IL) 1β, 4, 10, tumor necrosis factor-α (TNF-α), interferon-γ (INF-γ), transforming growth factor-1β (TGF-1β), C-reactive protein (CRP), platelets and erythrocyte sedimentation rate (ESR) at 2, 6 and 12 months from the beginning of MSCs therapy. Results. The initial mean CDAI in the 1st group was 337.6±17.1 points, in the 2nd group - 332.7±11.0 points (p=0.3). In both groups of patients there was a significant decrease in CDAI after 2 months. From the beginning of therapy MSCs: in the 1st group to 118.9±12.4 points, in the 2nd - 120.3±14.1 points (p=0.7), after 6 months - 110.3±11.1 and 114.3±11.8 points (p=0.8), respectively. After 12 months CDAI in the 1st group was 99.9±10.8 points, in the 2nd group it was 100.6±12.1 points (p=0.8). The level of IgA, IgG, IgM was significantly lower in the group of patients with a longer history of the disease and long-term ASA. After the introduction of MSC in both groups of patients with BC, there was a tendency for the growth of pro - and anti-inflammatory cytokines, with a significantly lower level of pro-inflammatory cytokines - INF-γ, TNF-α, IL-1β - in the 1st group, indicating potentiation of the immunosuppressive effect of MSCs and AZA, which provides a more pronounced anti-inflammatory effect. Conclusion. Transplantation of MSCs promotes an increase in the serum of patients with CD initially reduced concentration of IG, cytokines and restoring their balance as the onset of clinical remission. The combination with AZA has a more pronounced anti-inflammatory effect.

We present a clinical case of a 24-year old female patient who was hospitalized with high active ulcerative colitis associated with latent hereditary thrombophilia uncontrolled due to the exacerbation of ulcerative colitis and local steroid treatment. The patient was treated with combination of systemic steroids, adalimumab and anticoagulants. We emphasize the necessity of the multidisciplinary approach to her treatment. The clinical case demonstrates the possibility of safe use of anticoagulants to prevent thromboembolic complications in patients with ulcerative colitis and high risk of thrombus formation, under careful monitoring of coagulation parameters and thromboelastography. We discuss the need in clear guidelines to determine treatment strategy for patients with inflammatory bowel diseases, coagulopathies and thromboembolic complications.

Introduction. High level of ecological culture in modern society is the most important condition of self-preservation and sustainable development of a human civilization. The processes of globalization force to consider environmental problems with a support on polycultural practice, to take into account national and regional peculiarities in their integrity. Thus, there is the need of the international cooperation not only at the government level, but also at the levels of expert communities, separate groups of society and citizens of the country. Moreover, ecological culture is constantly highlighted in numerous studies, materials and documents of the international forums, summits and conferences of the UN and UNESCO. The aim of this publication is to present the authors’ didactic complex of development tools of ecological culture of students, and to show the potential of teaching foreign languages (on the example of French) for students’ ecological culture formation by means of development of cross-cultural communicative competence. Methodology and research methods. Culturological approach has been chosen as a key approach for creation of integrative model of development of ecological culture. The methods involve: the system-based analysis of the content of ecological education; generalization of the theory and practice of implementation of the international strategies of ecological culture development and the analysis of efficiency of the pedagogical technologies intended for this purpose; modeling of the process of formation of ecological culture of students. The diagnostics of components of students’ ecological culture has been performed by means of internal questioning, observation, and comparative analysis of group interactions. Also, pedagogical ascertaining experiments, methods of pedagogical design for forms of the educational environment organization, design of the educational programmes, and methods of graphical representation of the research results have been involved. Results and scientific novelty. New principles for the development of ecological culture are singled out. It is stated that intercultural communication is becoming the main mechanism for ecological culture development. Selection of the most productive innovative pedagogical technologies of ecological competence formation of students is carried out. Based on the obtained students’ questioning data, the factors promoting the increase of the individual ecological culture level are defined. It has become clear that pupils attach the greatest importance to the participation in international ecological projects, ecological tourism and acquaintance with traditions of different nationalities; at the same time, the role of foreign languages knowledge, reading foreign non-translated sources on environmental issues, and, in general, the importance of cross-cultural communication are estimated by students at a very low level. The authors conclude that relatively little attention is drawn to the issues of ecological culture in the process of foreign languages learning in higher education institutions; students practically do not get the experience of intercultural interaction. The integrative didactic model of ecological culture formation among students is offered. Consequently, intercultural communication plays an important part in the development of ecological culture formation. The educational programme of the course «Development of Ecological Culture of Students in the course of Intercultural Communication in French» is designed on the basis of the proposed model; the results of its approbation are described. Practical significance. The materials of the article can be useful when updating the content of the discipline «Foreign Language», and improvement of the educational process at higher school aimed at the development of students’ ecological culture as well.

The paper considers the main provisions of the 2009 RECIST guidelines (version 1.1.) criteria for a standard approach to measuring lung tumor lesions and assessing the dynamics of the tumor process during treatment. This paper contains a list of conditions for carrying out multislice spiral computed tomography, under which the latter becomes maximally reproducible for the same patient, as well as an assessment of the course of the disease does optimal.

Aim. To evaluate the efficacy and safety of adalimumab (ADA) in patients with Crohn’s disease (CD) treated at the Department of Inflammatory Bowel Diseases, Moscow Clinical Research and Practical Center, and to determine the predictors of a therapy response. Subjects and methods. All the patients with CD treated with ADA were followed up for at least 6 months or until the drug was discontinued. Therapeutic effectiveness was evaluated at 4 weeks and 6 months after the initiation of treatment and at the end of a follow-up. Complete intestinal mucosal healing was assessed at 3 and 12 months following treatment initiation. Univariate and multivariate analyses were used to determine the predictors of treatment response. Results. A clinical analysis covered 70 patients (57.1% male); the follow-up period averaged 112 weeks. Perianal fistulas were at baseline established in 22 (31.4%) patients with CD. 12 (17.4%) patients had been previously treated with infliximab (INF), 7 of them discontinued the drug for secondary loss of response and 5 for adverse reactions. 68 (97.1%) patients responded to an induction course of ADA. At 4 weeks, 6 months, and at the end of the follow-up, clinical remission occurred in 66.7, 80.4 and 67.4 % of patients with luminal CD and in 45.4, 36.5, and 36.4% of those with perianal CD, respectively. At 3 and 12 months and at the end of the follow-up, there was complete healing of the intestinal mucosa in 23.5, and 41.2 and 29.5% of cases, respectively. Six (8.8%) patients responding to the induction course needed to be optimized with ADA to 40 mg weekly. The time interval between treatment initiation and dose optimization averaged 30 weeks (range 12-120 months). There were 15 (21,4%) adverse events that were responsible for ADA discontinuation in 3 (4,2%) patients. Conclusion. The findings demonstrate the efficacy and safety of ADA used in clinical practice.

Aim. To compare the safety of therapy in patients with ulcerative colitis (UC) and Crohn's disease (CD) who have received combination anti-inflammatory therapy using bone marrow mesenchymal stromal cells (MSC) and standard therapy with 5-aminosalicylic acid, glucocorticosteroids, and immunosuppressive agents. Subjects and methods. Unfavorable consequences were analyzed in 103 patients (56 with UC and 47 with CD) with inflammatory bowel disease (IBD) after MSC administration. The findings were compared with data obtained in 208 patients with UC and CD on standard anti-inflammatory therapy. All the patients were similar in demographic parameters, the duration of disease, the extent of intestinal injury, the nature of a course, the type and degree of disease. The analyzed groups did not include patients who had received therapy with anti-TNF-α drugs. The safety of therapy was evaluated from the presence of complications occurring during the follow-up. Results. By analyzing the unfavorable consequences in 103 patients with IBD and comparing them with treatment results in 208 patients with UC and CD on standard anti-inflammatory therapy, the authors revealed no differences in the development of acute posttransfusion reactions, infectious complications, exacerbations of chronic inflammatory diseases, severe infectious complications, malignant transformation, and fatal cases in patients with UC and CD, except for those with transient fever. Conclusion. The results of this study demonstrate that the innovative method of cell therapy is clinically safe.

Aim. To establish the rate of lactase deficiency (LD) in patients with post-infectious irritable bowel syndrome (PI-IBS), to define a role of enteric bacteria in the pathogenesis of hypolactasia, and to evaluate the efficiency of probiotic therapy. Subjects and methods. Examinations were made in 386 patients with PI-IBS, including 112 (79.4%) women; mean age 33.9±9.1 years; disease duration 2.6±1.4 years. Rapid tests of small intestinal mucosa (SIM) biopsy specimens obtained from the duodenal retrobulbar segment were used to diagnose LD. Bacterial growth was estimated by a hydrogen breath test using a Н2 MICRO gas analyzer. Results. The patients with PI-IBS were revealed to have moderate and severe LD in 25.6 and 10.9%, respectively. Calculating pattern of horn irradiator and mirror antenna which were met the requirements was received. The construction choice was limited by the preproduction possibilities, mass and dimensions. Mirror antenna consists of skeleton framing with mirroring elements which are fixing on it. Mirroring plane is multiplex and consists off rectangular planes made by hydroforming method. Antenna was tested and adjusted at the antenna darkroom after fabricating. The paper gives a case history of a patient with common variable hypogammaglobulinemia (СVHGG). Its clinical picture was characterized by frequent inflammatory airway diseases, pansinusitis, and periodic rises in temperature up to febrile ones. The diagnosis was established on the basis of the decreased serum concentrations of immunoglobulins (IG) A, M, and G, the unusual small intestinal (SI) wall relief created by large lymphoid follicles, as well as the characteristic sign of CVHGG, namely, the lack of plasma cells in the lamina propria of the SI mucosa. Antibacterial therapy was noted to have a good effect that confirmed the leading role of excessive bacterial growth in the pathogenesis of chronic diarrhea and fever, which are the major clinical manifestations of CVHGG in this patient.

AIM: To estimate the incidence of secondary lactase deficiency (SLD) in patients with postinfectious irritable bowel syndrome (PIBS) and the value of the small bowel microflora in its development and to elaborate treatment options for SLD/MATERIAL AND METHODS: One hundred and thirty-eight patients with PIBS, including 112 (81.2%) women and 26 (18.8%) men, were examined. The patients' mean age was 33.9±9.1 years. The duration of the disease was 2.6±1.4 years. Lactase deficiency (LD) was diagnosed using the color scale to test biopsy specimens from the duodenal retrobulbar region. The bacterial overgrowth syndrome (BOS) was identified by a 2-hour lactulose (20 ml) hydrogen breath test. Sixty patients with moderate SLD were randomized to 2 groups: 1) 41 patients received basic therapy (mesim forte as one tablet t.i.d., no-spa, 40 mg, t.i.d.) and combined probiotic bifiform (Ferrosan) containing Bifidobacterium longum 107, Enterococcus faecium 107 as one capsule t.i.d. for 14 days. Group 2 patients (n=19) had basic therapy in combination with placebo/RESULTS: SLD was detected in 59.4% of the patients with PIBS, including 43.5 and 15.9% with moderate and severe forms, respectively. The associations of the most frequent carriages of allelic thrombogenic mutations and polymorphisms in women with chorion presentation in the first trimester of pregnancy living in the Altai region were determined in this research. 300 women were investigated and divided into three groups. It depends on the chorion presentation: the main group consisted of 50 patients with complete chorion presentation in the first trimester of pregnancy and the lack of migration in the second trimester; the comparison group included 150 pregnant women with chorion presentation in the first trimester and further migration in the second half of pregnancy; control group had 100 pregnant women with physiological placentation. The frequency of thrombogenic mutations and polymorphisms in women from the main group was in 82,0% of cases, whereas in the comparison group these genetic defects were revealed in 36,8% of cases and there were 3% in the control group. Intergenic associations were detected in 58% of cases, whereas in the comparison group in 7,3% of cases and they were absent in the control group. The condition of reproductive and somatic health of women with chorion presentation was also evaluated.

Цель. Изучение влияния никотинамида и его комбинаций с витаминами Е и С на систему перекисного окисления липидов (ПОЛ) и антиоксидантной защиты у больных СД 2 типа. Материалы и методы. Под наблюдением находились 78 больных (26 мужчин и 52 женщины) в возрасте от 38 до 70 лет. Все больные разделены на 4 группы методом случайной выборки. В 1-й группе (20) больные получали никотинамид в дозе 1800 мг в сутки. Во 2-й группе (18) больным назначали никотинамид в дозе 900 мг/сут в сочетании с ?-токоферолом в дозе 400 мг/сут. В 3-й группе (20 чел.) больные получали лечение сочетанием препаратов: никотинамид по 900 мг/сут, ?-токоферол по 400 мг/сут и аскорбиновая кислота по 600 мг/сут. Больные 4-й группы (20) получали ?-токоферол по 800 мг/сут. Эффективность оценивали по показателям до и после завершения лечения: HbAlc, концентрация малонового диальдегида; активность супероксиддисмутазы и глутатионпероксидазы в эритроцитах, содержание общего холестерина, триглицеридов, липопротеидов высокой и низкой плотности, показатели коагулограммы, тромбоэластограммы, фибриногена, фибринолитической активности. Результаты. У больных СД 2 типа, принимавших никотинамид, витамины Е, С в течение 2 недель, выявлено снижение активности процессов ПОЛ, а также усиление системы антиоксидантной защиты, максимальный антиоксидантный эффект отмечается при сочетании никотинамида с витаминами. На фоне приема препаратов выявлена положительная тенденция к нормализации показателей углеводного, липидного обмена и коагулограммы, улучшение состояния сосудов глазного дна и дистальной нейропатии. Выводы. Применение никотинамида и его комбинаций с витаминами Е и С может входить в комплексную терапию больных СД 2 типа.

Актуальность. Установлена важная роль неферментативного свободнорадикального окисления липидов (ПОЛ) в патогенезе сахарного диабета (СД) и ангиопатий. Представляет интерес высокоэффективный противовирусный препарат [арбидол 1-метил-2-фенилтиометил-3-карбоэтокси-4-(диметилам и нометил)-5-окси-6-броминдол гидрохлорид моногидрат], обладающий также иммуностимулирующим действием. Цель. Изучение влияния препарата арбидола на систему ПОЛ и антиоксидантной защиты у больных СД 2. Материалы и методы. 31 человек в возрасте от 39 до 70 лет. Все больные получали терапию сахароснижающими препаратами. Исследовались следующие показатели: индекс массы тела (ИМТ), гликированный гемоглобин (HbAlс), вибрационная чувствительность, уровни общего холестерина (ХС), триглицеридов (ТГ), липопротеидов высокой (ЛПВП) и низкой (ЛПНП) плотности, показатели системы гемостаза (активированное время рекальцификации - АВР, активированное парциальное тромбопластиновое время (АЧТВ), протромбиновый индекс (ПТИ), тромбиновое время (ТВ); тромбоэластограмма; уровень фибриногена; фибринолитическая активность. Больные получали арбидол в дозе 0,1 г (1 таблетка) в день в течение 6 нед. Результаты. У больных СД, получавших препарат арбидол, отмечалось значительное высокодостоверное снижение уровня МДА по сравнению с исходными данными, высокодостоверне повышение уровня СОД. Выявлено положительное влияние арбидола на показатели липидного обмена: увеличение уровня ЛПВП, снижение содержания агрессивных ЛПНП и ТГ и гликированного гемоглобина. Обращает на себя внимание снижение протромбинового индекса на 4%. Выводы. У больных СД 2 типа, принимавших арбидол по 0,1 г в сутки в течение 6 нед, выявлено снижение активации процессов ПОЛ и усиление системы ан?тиоксидантной защиты. На фоне приема арбидола выявлена тенденция к нормализации показателей липидного обмена и системы гемостаза.
