1. Progress towards edge-localized mode suppression via magnetic perturbations in hydrogen plasmas

Abstract: The suppression of edge-localized modes (ELMs) by applying resonant magnetic perturbations (RMPs) is well studied in low collisionality deuterium plasmas as a measure to reduce transient divertor heat loads. However, ELM suppression has yet to be demonstrated in non-nuclear fuels such as hydrogen and hydrogen + helium mixtures which are the main ion species to be used in the ITER pre-fusion power operation (PFPO) phase. For the first time, attempts have been made to access ELM suppression with RMPs in ITER-like low collisionality hydrogen plasmas at DIII-D and ASDEX Upgrade. The DIII-D experiments focused on operation with injected power slightly above the L–H power threshold similar to the expected conditions in the ITER PFPO phase with limited external heating power. The RMPs were found to trigger H–L backtransitions, which is shown to be avoided by reducing the L–H power threshold by diluting the plasma with helium. The additional helium combined with a larger measured neutral density of hydrogen inside the separatrix compared to ELM suppressed deuterium plasmas precluded access to a pedestal top density below the known RMP-ELM suppression threshold. At ASDEX Upgrade, RMP-ELM suppression has been achieved when the concentration of ^1 H in the hydrogen isotope mix is below $40\%$ . While all known access criteria for RMP-ELM suppression were met above this threshold, full ELM suppression was replaced by strong mitigation. The most prominent difference between the hydrogen and deuterium plasmas was a change of turbulence characteristics in the pedestal where Doppler reflectometry measurements suggest a significant reduction of turbulence even at small hydrogen concentrations. In conclusion, these experiments not only identify issues that may prevent access to RMP-ELM suppression in the ITER PFPO phase, but also highlight missing physics in our current understanding of RMP-ELM suppression such as potentially the role of turbulence in the pedestal gradient region.

Abstract: Experiments on ASDEX Upgrade (AUG) in 2021 and 2022 have addressed a number of critical issues for ITER and EU DEMO. A major objective of the AUG programme is to shed light on the underlying physics of confinement, stability, and plasma exhaust in order to allow reliable extrapolation of results obtained on present day machines to these reactor-grade devices. Concerning pedestal physics, the mitigation of edge localised modes (ELMs) using resonant magnetic perturbations (RMPs) was found to be consistent with a reduction of the linear peeling-ballooning stability threshold due to the helical deformation of the plasma. Conversely, ELM suppression by RMPs is ascribed to an increased pedestal transport that keeps the plasma away from this boundary. Candidates for this increased transport are locally enhanced turbulence and a locked magnetic island in the pedestal. The enhanced D-alpha (EDA) and quasi-continuous exhaust (QCE) regimes have been established as promising ELM-free scenarios. Here, the pressure gradient at the foot of the H-mode pedestal is reduced by a quasi-coherent mode, consistent with violation of the high-n ballooning mode stability limit there. This is suggestive that the EDA and QCE regimes have a common underlying physics origin. In the area of transport physics, full radius models for both L- and H-modes have been developed. These models predict energy confinement in AUG better than the commonly used global scaling laws, representing a large step towards the goal of predictive capability. A new momentum transport analysis framework has been developed that provides access to the intrinsic torque in the plasma core. In the field of exhaust, the X-Point Radiator (XPR), a cold and dense plasma region on closed flux surfaces close to the X-point, was described by an analytical model that provides an understanding of its formation as well as its stability, i.e., the conditions under which it transitions into a deleterious MARFE with the potential to result in a disruptive termination. With the XPR close to the divertor target, a new detached divertor concept, the compact radiative divertor, was developed. Here, the exhaust power is radiated before reaching the target, allowing close proximity of the X-point to the target. No limitations by the shallow field line angle due to the large flux expansion were observed, and sufficient compression of neutral density was demonstrated. With respect to the pumping of non-recycling impurities, the divertor enrichment was found to mainly depend on the ionisation energy of the impurity under consideration. In the area of MHD physics, analysis of the hot plasma core motion in sawtooth crashes showed good agreement with nonlinear 2-fluid simulations. This indicates that the fast reconnection observed in these events is adequately described including the pressure gradient and the electron inertia in the parallel Ohm’s law. Concerning disruption physics, a shattered pellet injection system was installed in collaboration with the ITER International Organisation. Thanks to the ability to vary the shard size distribution independently of the injection velocity, as well as its impurity admixture, it was possible to tailor the current quench rate, which is an important requirement for future large devices such as ITER. Progress was also made modelling the force reduction of VDEs induced by massive gas injection on AUG. The H-mode density limit was characterised in terms of safe operational space with a newly developed active feedback control method that allowed the stability boundary to be probed several times within a single discharge without inducing a disruptive termination. Regarding integrated operation scenarios, the role of density peaking in the confinement of the ITER baseline scenario (high plasma current) was clarified. The usual energy confinement scaling ITER98( p,y ) does not capture this effect, but the more recent H20 scaling does, highlighting again the importance of developing adequate physics based models. Advanced tokamak scenarios, aiming at large non-inductive current fraction due to non-standard profiles of the safety factor in combination with high normalised plasma pressure were studied with a focus on their access conditions. A method to guide the approach of the targeted safety factor profiles was developed, and the conditions for achieving good confinement were clarified. Based on this, two types of advanced scenarios (‘hybrid’ and ‘elevated’ q -profile) were established on AUG and characterised concerning their plasma performance.

Abstract: The DIII-D tokamak has elucidated crucial physics and developed projectable solutions for ITER and fusion power plants in the key areas of core performance, boundary heat and particle transport, and integrated scenario operation, with closing the core-edge integration knowledge gap being the overarching mission. New experimental validation of high-fidelity, multi-channel, non-linear gyrokinetic turbulent transport models for ITER provides strong confidence it will achieve Q ⩾ 10 operation. Experiments identify options for easing H-mode access in hydrogen, and give new insight into the isotopic dependence of transport and confinement. Analysis of 2,1 islands in unoptimized low-torque IBS demonstration discharges suggests their onset time occurs randomly in the constant β phase, most often triggered by non-linear 3-wave coupling, thus identifying an NTM seeding mechanism to avoid. Pure deuterium SPI for disruption mitigation is shown to provide favorable slow cooling, but poor core assimilation, suggesting paths for improved SPI on ITER. At the boundary, measured neutral density and ionization source fluxes are strongly poloidally asymmetric, implying a 2D treatment is needed to model pedestal fuelling. Detailed measurements of pedestal and SOL quantities and impurity charge state radiation in detached divertors has validated edge fluid modelling and new self-consistent ‘pedestal-to-divertor’ integrated modeling that can be used to optimize reactors. New feedback adaptive ELM control minimizes confinement reduction, and RMP ELM suppression with sustained high core performance was obtained for the first time with the outer strike point in a W-coated, compact and unpumped small-angle slot divertor. Advances have been made in integrated operational scenarios for ITER and power plants. Wide pedestal intrinsically ELM-free QH-modes are produced with more reactor-relevant conditions, Low torque IBS with W-equivalent radiators can exhibit predator-prey oscillations in T _e and radiation which need control. High- β _P scenarios with q _min > 2, q _95 –7.9, β _N > 4, β _T –3.3% and H _98y2 > 1.5 are sustained with high density ( $\bar n$ = 7E19 m ^−3 , f _G –1) for 6 τ _E , improving confidence in steady-state tokamak reactors. Diverted NT plasmas achieve high core performance with a non-ELMing edge, offering a possible highly attractive core-edge integration solution for reactors.

Abstract: Strongly-shaped diverted negative triangularity (NT) plasmas in the DIII-D tokamak demonstrate simultaneous access to high normalized density, current, pressure, and confinement. NT plasmas are shown to exist across an expansive parameter space compatible with high fusion power production, revealing surprisingly good core stability properties that compare favorably to conventional positive triangularity plasmas in DIII-D. Non-dimensionalizing the key parameters, expanded operating spaces featuring edge safety factors below 3, normalized betas above 3, Greenwald density fractions above 1, and high-confinement mode (H-mode) confinement qualities above 1 are observed, even simultaneously, and all with a robustly stable edge free from deleterious edge-localized mode instabilities. Scaling of the confinement time with engineering parameters reveals at least a linear dependence on plasma current although with significant power degradation, both in excess of expected H-mode scalings. These results increase confidence that NT plasmas are a viable approach to realize fusion power and open directions for future detailed study.

5. Plasma performance and operational space with an RMP-ELM suppressed edge

Abstract: The operational space and global performance of plasmas with edge-localized modes (ELMs) suppressed by resonant magnetic perturbations (RMPs) are surveyed by comparing AUG, DIII-D, EAST, and KSTAR stationary operating points. RMP-ELM suppression is achieved over a range of plasma currents, toroidal fields, and RMP toroidal mode numbers. Consistent operational windows in edge safety factor are found across devices, while windows in plasma shaping parameters are distinct. Accessed pedestal parameters reveal a quantitatively similar pedestal-top density limit for RMP-ELM suppression in all devices of just over $3\times 10^{19}$ m ^−3 . This is surprising given the wide variance of many engineering parameters and edge collisionalities, and poses a challenge to extrapolation of the regime. Wide ranges in input power, confinement time, and stored energy are observed, with the achieved triple product found to scale like the product of current, field, and radius. Observed energy confinement scaling with engineering parameters for RMP-ELM suppressed plasmas are presented and compared with expectations from established H and L-mode scalings, including treatment of uncertainty analysis. Different scaling exponents for individual engineering parameters are found as compared to the established scalings. However, extrapolation to next-step tokamaks ITER and SPARC find overall consistency within uncertainties with the established scalings, finding no obvious performance penalty when extrapolating from the assembled multi-device RMP-ELM suppressed database. Overall this work identifies common physics for RMP-ELM suppression and highlights the need to pursue this no-ELM regime at higher magnetic field and different plasma physical size.

Abstract: We study localized edge turbulence in the ASDEX Upgrade tokamak that appears if resonant magnetic perturbations (RMP) are applied to suppress edge localized modes (ELMs) in the high confinement mode. The concomitant density fluctuations are detected by microwave reflectometry at the outboard midplane. Two modes can be distinguished, (a) a broadband fluctuation below a threshold of the RMP field amplitude, and (b) a narrow-band quasi-coherent mode (QCM) above the threshold. The broadband fluctuation is toroidally spread out but disappears at the toroidal position of maximum E × B shear in the gradient region. Temporal and spatial correlation along field lines of the midplane density fluctuation and the divertor particle flux suggests that this mode is producing significant particle transport across the gradient region and into the divertor, hence contributing to the plasma density reduction that is often observed when applying RMP fields (the so-called ‘pump-out’ effect). The QCM is also toroidally localized, its radial extent grows with increasing RMP field amplitude, and leads to further increased divertor particle flux compared to the broadband mode. Our observations suggest that both modes not only play an important role in keeping the plasma density stationary in the absence of ELMs but also to reduce the plasma pressure such that the plasma edge becomes stable against ELMs.

Publisher Copyright: © 2022 IAEA, Vienna. DIII-D physics research addresses critical challenges for the operation of ITER and the next generation of fusion energy devices. This is done through a focus on innovations to provide solutions for high performance long pulse operation, coupled with fundamental plasma physics understanding and model validation, to drive scenario development by integrating high performance core and boundary plasmas. Substantial increases in off-axis current drive efficiency from an innovative top launch system for EC power, and in pressure broadening for Alfven eigenmode control from a co-/counter-I p steerable off-axis neutral beam, all improve the prospects for optimization of future long pulse/steady state high performance tokamak operation. Fundamental studies into the modes that drive the evolution of the pedestal pressure profile and electron vs ion heat flux validate predictive models of pedestal recovery after ELMs. Understanding the physics mechanisms of ELM control and density pumpout by 3D magnetic perturbation fields leads to confident predictions for ITER and future devices. Validated modeling of high-Z shattered pellet injection for disruption mitigation, runaway electron dissipation, and techniques for disruption prediction and avoidance including machine learning, give confidence in handling disruptivity for future devices. For the non-nuclear phase of ITER, two actuators are identified to lower the L-H threshold power in hydrogen plasmas. With this physics understanding and suite of capabilities, a high poloidal beta optimized-core scenario with an internal transport barrier that projects nearly to Q = 10 in ITER at ∼8 MA was coupled to a detached divertor, and a near super H-mode optimized-pedestal scenario with co-I p beam injection was coupled to a radiative divertor. The hybrid core scenario was achieved directly, without the need for anomalous current diffusion, using off-axis current drive actuators. Also, a controller to assess proximity to stability limits and regulate β N in the ITER baseline scenario, based on plasma response to probing 3D fields, was demonstrated. Finally, innovative tokamak operation using a negative triangularity shape showed many attractive features for future pilot plant operation.

Abstract: An overview of recent results obtained at the tokamak ASDEX Upgrade (AUG) is given. A work flow for predictive profile modelling of AUG discharges was established which is able to reproduce experimental H-mode plasma profiles based on engineering parameters only. In the plasma center, theoretical predictions on plasma current redistribution by a dynamo effect were confirmed experimentally. For core transport, the stabilizing effect of fast ion distributions on turbulent transport is shown to be important to explain the core isotope effect and improves the description of hollow low-Z impurity profiles. The L–H power threshold of hydrogen plasmas is not affected by small helium admixtures and it increases continuously from the deuterium to the hydrogen level when the hydrogen concentration is raised from 0 to 100%. One focus of recent campaigns was the search for a fusion relevant integrated plasma scenario without large edge localised modes (ELMs). Results from six different ELM-free confinement regimes are compared with respect to reactor relevance: ELM suppression by magnetic perturbation coils could be attributed to toroidally asymmetric turbulent fluctuations in the vicinity of the separatrix. Stable improved confinement mode plasma phases with a detached inner divertor were obtained using a feedback control of the plasma β. The enhanced Dα H-mode regime was extended to higher heating power by feedback controlled radiative cooling with argon. The quasi-coherent exhaust regime was developed into an integrated scenario at high heating power and energy confinement, with a detached divertor and without large ELMs. Small ELMs close to the separatrix lead to peeling-ballooning stability and quasi continuous power exhaust. Helium beam density fluctuation measurements confirm that transport close to the separatrix is important to achieve the different ELM-free regimes. Based on separatrix plasma parameters and interchange-drift-Alfvén turbulence, an analytic model was derived that reproduces the experimentally found important operational boundaries of the density limit and between L- and H-mode confinement. Feedback control for the X-point radiator (XPR) position was established as an important element for divertor detachment control. Stable and detached ELM-free phases with H-mode confinement quality were obtained when the XPR was moved 10 cm above the X-point. Investigations of the plasma in the future flexible snow-flake divertor of AUG by means of first SOLPS-ITER simulations with drifts activated predict beneficial detachment properties and the activation of an additional strike point by the drifts. 9. Influence of triangularity on the plasma response to resonant magnetic perturbations

Abstract: The effect of the axisymmetric plasma shape on the non-axisymmetric plasma response to resonant magnetic perturbations is investigated in experiment and modeling for the DIII-D, EAST and ASDEX Upgrade tokamaks. Systematically modeling the effect of the triangularity, whilst keeping other equilibrium quantities largely unchanged, reveals that the plasma response is strongly suppressed at high triangularity compared to that at low triangularity. This is validated through targeted comparison with experiments at DIII-D, EAST, and ASDEX Upgrade. DIII-D and EAST magnetic measurements are used to validate simulations, while at ASDEX Upgrade the plasma edge displacement is measured. Both experiments and modeling find a reduced magnetic plasma response on the high-field side at high triangularity across devices. Multi-modal analysis of the simulation results extracts the mode structure and applied spectrum dependence of each mode. The amplitude of the dominant mode reveals similar trends with the edge resonance and radial displacement near the X-point, which suggests that the multi-mode plasma response provides another way to understand the edge localized mode (ELM) control physics. The plasma response is strongly reduced at high triangularity compared to that at low triangularity, which implies different ELM control effects as shaping is varied. These findings indicate that the plasma shape should be taken into consideration when designing an RMP-ELM control strategy in experiment, and that predictive plasma response calculations can be used to maximize access to RMP-ELM control in future devices by maximizing the coupling between coils and the plasma.

10. Reducing the L-H transition power threshold in ITER-similar-shape DIII-D hydrogen plasmas

Abstract: Recent dedicated DIII-D experiments in low-torque, ITER-similar-shape (ISS) hydrogen plasmas (at a plasma current I p ∼ 1.5 MA and ITER-similar edge safety factor q 95 ∼ 3.6) show that the L-H transition power threshold P LH can be reduced substantially (∼30%) with L-mode helium admixtures n He/n e ⩽ 25%. In the ensuing H-mode, helium ion fractions n He/n H remain below 25%. H-mode normalized pressure and confinement quality are only slightly affected by helium seeding, and Z eff ⩽ 2.15 (including helium and carbon content). The plasmas investigated here are electron-heat dominated, with temperatures T e(0)/T i(0) ⩾ 1 and edge heat flux ratio Q e/Q i(ρ = 0.95) ∼ 1.2–1.5. Without mitigation, P LH is higher by a factor of 2–3 in comparison to similar ISS deuterium plasmas. ISS hydrogen plasmas with lower plasma current I p ∼ 1 MA (increased edge safety factor q 95 ∼ 5.1) exhibit a substantially lower power threshold. This plasma current dependence, also observed previously on ASDEX-U and in JET, is not accounted for by the commonly used 2008 ITPA multi-machine threshold scaling, but could potentially allow H-mode access at marginal heating power during the initial plasma current ramp-up. Attempts to reduce P LH with low-field- and high-field-side hydrogen pellet injection, using 1.7 mm diameter pellets, have not demonstrated a robust threshold reduction, in contrast to successful earlier experiments with larger 2.7 mm pellets. Techniques for reducing P LH are very important for ITER, in particular for accessing H-mode in hydrogen plasmas during the Pre-Fusion Power Operation-1 (PFPO-1) campaign with marginal auxiliary heating power (20–30 MW of ECH).

Abstract: The impact of three-dimensional (3D) tokamak geometry from external magnetic perturbations on edge instabilities has been examined in high confinement mode plasmas with edge localised modes (ELMs) in ASDEX Upgrade. The 3D geometry has been probed using rigidly rotating MP fields. The measured distortions of the plasma boundary are compared to single-fluid ideal magnetohydrodynamic (MHD) equilibria using VMEC and MARS-F applying ideal and resistive MHD, whereas VMEC uses only ideal MHD. Both codes accurately reproduce the measured radial displacements of the edge density and temperature profiles in amplitude, toroidal phase and their dependence on the applied poloidal mode spectrum. The induced 3D geometry distorts the local magnetic shear, which locally reduces the stabilising effect from field-line bending at certain most unstable field lines. Around these field lines, we observe additional stable ideal MHD modes with clear ballooning structure in-between ELMs. After their immediate appearance, they saturate and then grow on timescales of the pedestal pressure recovery. The subsequent ELMs show strongly localised magnetic perturbations of the initial crash and accompanied energetic electrons around the same most unstable field lines. These are strong signatures that filaments at the ELM onset preferentially erupt on these most unstable ('bad') field lines with their unfavourable 3D geometry where preceding ballooning modes are observed.

Abstract: This paper describes the extension of the previous experimental database on density control by pellets and ELM control by RMPs in the ASDEX Upgrade tokamak. A stationary low triangularity plasma is sustained by simultaneous pellet fuelling and RMP ELM control at ion pedestal collisionality of ν ∗i,ped ∼ 0.5. Refuelling of the RMP induced density pump-out requires a pellet rate of Φpel ∼ 0.073 P aux/T t,ped. The duration of refuelling of density pump-out can be minimised to about 3 energy confinement times. Pellet fuelling increases or preserves ion pedestal pressure with respect to unfuelled ELM suppressed/mitigated phase. Pellets cause transition from ELM suppression to ELMy regime: At low triangularity, the first pellet triggers a transition to an ELMy phase but subsequent ELMs are not modulated by pellets. At elevated triangularity, initial pellets trigger ELM-like events but in-between pellets ELM suppression is preserved. In later phase, as density increases the plasma transitions to an ELMy phase similar to low triangularity case.

Abstract: The ASDEX Upgrade (AUG) programme, jointly run with the EUROfusion MST1 task force, continues to significantly enhance the physics base of ITER and DEMO. Here, the full tungsten wall is a key asset for extrapolating to future devices. The high overall heating power, flexible heating mix and comprehensive diagnostic set allows studies ranging from mimicking the scrape-off-layer and divertor conditions of ITER and DEMO at high density to fully non-inductive operation (q95 = 5.5, ) at low density. Higher installed electron cyclotron resonance heating power 6 MW, new diagnostics and improved analysis techniques have further enhanced the capabilities of AUG. Stable high-density H-modes with MW m−1 with fully detached strike-points have been demonstrated. The ballooning instability close to the separatrix has been identified as a potential cause leading to the H-mode density limit and is also found to play an important role for the access to small edge-localized modes (ELMs). Density limit disruptions have been successfully avoided using a path-oriented approach to disruption handling and progress has been made in understanding the dissipation and avoidance of runaway electron beams. ELM suppression with resonant magnetic perturbations is now routinely achieved reaching transiently . This gives new insight into the field penetration physics, in particular with respect to plasma flows. Modelling agrees well with plasma response measurements and a helically localised ballooning structure observed prior to the ELM is evidence for the changed edge stability due to the magnetic perturbations. The impact of 3D perturbations on heat load patterns and fast-ion losses have been further elaborated. Progress has also been made in understanding the ELM cycle itself. Here, new fast measurements of and Er allow for inter ELM transport analysis confirming that Er is dominated by the diamagnetic term even for fast timescales. New analysis techniques allow detailed comparison of the ELM crash and are in good agreement with nonlinear MHD modelling. The observation of accelerated ions during the ELM crash can be seen as evidence for the reconnection during the ELM. As type-I ELMs (even mitigated) are likely not a viable operational regime in DEMO studies of 'natural' no ELM regimes have been extended. Stable I-modes up to have been characterised using -feedback. Core physics has been advanced by more detailed characterisation of the turbulence with new measurements such as the eddy tilt angle—measured for the first time—or the cross-phase angle of and fluctuations. These new data put strong constraints on gyro-kinetic turbulence modelling. In addition, carefully executed studies in different main species (H, D and He) and with different heating mixes highlight the importance of the collisional energy exchange for interpreting energy confinement. A new regime with a hollow profile now gives access to regimes mimicking aspects of burning plasma conditions and lead to nonlinear interactions of energetic particle modes despite the sub-Alfvénic beam energy. This will help to validate the fast-ion codes for predicting ITER and DEMO. Abstract: ELM mitigation by magnetic perturbations is studied at low pedestal collisionalities down to ITER-like values (V∗ e PED = 0.1) in ASDEX Upgrade. A comprehensive database of ELM energy losses for varying plasma density, heating power, edge safety factor and magnetic perturbation structure has been assembled to investigate parameter dependencies of ELM mitigation. It is found that magnetic perturbations with a toroidal mode number n = 2 can reduce the ELM energy loss normalized to the energy stored in the plasma pedestal from about 30% to less than 5%, i.e. by a factor of six, below an electron pedestal collisionality of V∗ e PED = 0.4. At this level of ELM mitigation a significant reduction of the pedestal pressure and, therefore, global plasma confinement occurs. This pedestal pressure reduction is mostly due to a reduction of plasma density, the so-called pump-out effect. Refueling by neutral beams and in particular by pellet injection is possible and can re-establish confinement, however, the ELM energy loss increases as well with increasing density. © 2017 Published by Elsevier B.V.

Abstract: The interaction of externally applied small non-axisymmetric magnetic perturbations (MP) with tokamak high-confinement mode (H-mode) plasmas is reviewed and illustrated by recent experiments in ASDEX Upgrade. The plasma response to the vacuum MP field is amplified by stable ideal kink modes with low toroidal mode number n driven by the H-mode edge pressure gradient (and associated bootstrap current) which is experimentally evidenced by an observable shift of the poloidal mode number m away from field alignment (m = qn, with q being the safety factor) at the response maximum. A torque scan experiment demonstrates the importance of the perpendicular electron flow for shielding of the resonant magnetic perturbation, as expected from a two-fluid MHD picture. Two significant effects of MP occur in H-mode plasmas at low pedestal collisionality, : (a) a reduction of the global plasma density by up to and (b) a reduction of the energy loss associated with edge localised modes (ELMs) by a factor of up to 9. A comprehensive database of ELM mitigation pulses at low in ASDEX Upgrade shows that the degree of ELM mitigation correlates with the reduction of pedestal pressure which in turn is limited and defined by the onset of ELMs, i. e. a modification of the ELM stability limit by the magnetic perturbation.

Abstract: The impact of three-dimensional (3D) tokamak geometry from external magnetic perturbations on edge instabilities has been examined in high confinement mode plasmas with edge localised modes (ELMs) in ASDEX Upgrade. The 3D geometry has been probed using rigidly rotating MP fields. The measured distortions of the plasma boundary are compared to single-fluid ideal magnetohydrodynamic (MHD) equilibria using VMEC and MARS-F applying ideal and resistive MHD, whereas VMEC uses only ideal MHD. Both codes accurately reproduce the measured radial displacements of the edge density and temperature profiles in amplitude, toroidal phase and their dependence on the applied poloidal mode spectrum. The induced 3D geometry distorts the local magnetic shear, which locally reduces the stabilising effect from field-line bending at certain most unstable field lines. Around these field lines, we observe additional stable ideal MHD modes with clear ballooning structure in-between ELMs. After their immediate appearance, they saturate and then grow on timescales of the pedestal pressure recovery. The subsequent ELMs show strongly localised magnetic perturbations of the initial crash and accompanied energetic electrons around the same most unstable field lines. These are strong signatures that filaments at the ELM onset preferentially erupt on these most unstable (‘bad’) field lines with their unfavourable 3D geometry where preceding ballooning modes are observed. Abstract: This paper describes the extension of the previous experimental database on density control by pellets and ELM control by RMPs in the ASDEX Upgrade tokamak. A stationary low triangularity plasma is sustained by simultaneous pellet fuelling and RMP ELM control at ion pedestal collisionality of ν*i,ped ∼ 0.5. Refuelling of the RMP induced density pump-out requires a pellet rate of Φpel ∼ 0.073 Paux/Tt,ped. The duration of refuelling of density pump-out can be minimised to about 3 energy confinement times. Pellet fuelling increases or preserves ion pedestal pressure with respect to unfuelled ELM suppressed/mitigated phase. Pellets cause transition from ELM suppression to ELMy regime: At low triangularity, the first pellet triggers a transition to an ELMy phase but subsequent ELMs are not modulated by pellets. At elevated triangularity, initial pellets trigger ELM-like events but in-between pellets ELM suppression is preserved. 21. Robust Avoidance of Edge-Localized Modes alongside Gradient Formation in the Negative Triangularity Tokamak Edge.

Abstract: In a series of high performance diverted discharges on DIII-D, we demonstrate that strong negative triangularity (NT) shaping robustly suppresses all edge-localized mode (ELM) activity over a wide range of plasma conditions: ⟨n⟩=0.1-1.5×10^{20} m^{-3}, P_{aux}=0-15 MW, and |B_{t}|=1-2.2 T, corresponding to P_{loss}/P_{LH08}∼8. The full dataset is consistent with the theoretical prediction that magnetic shear in the NT edge inhibits access to ELMing H-mode regimes; all experimental pressure profiles are found to be at or below the infinite-n ballooning stability limit. Our present dataset also features edge pressure gradients in strong NT that are closer to an H-mode than a typical L-mode plasma, supporting the consideration of NT for reactor design.

22. The Effect of Collaborative Reviews of Electronic Patient-Reported Outcomes on the Congruence of Patient- and Clinician-Reported Toxicity in Cancer Patients Receiving Systemic Therapy: Prospective, Multicenter, Observational Clinical Trial.

Abstract: Background: Electronic patient-reported outcomes (ePRO) are a relatively novel form of data and have the potential to improve clinical practice for cancer patients. In this prospective, multicenter, observational clinical trial, efforts were made to demonstrate the reliability of patient-reported symptoms., Objective: The primary objective of this study was to assess the level of agreement κ between symptom ratings by physicians and patients via a shared review process in order to determine the future reliability and utility of self-reported electronic symptom monitoring., Methods: Patients receiving systemic therapy in a (neo-)adjuvant or noncurative intention setting captured ePRO for 52 symptoms over an observational period of 90 days. At 3-week intervals, randomly selected symptoms were reviewed between the patient and physician for congruency on severity of the grading of adverse events according to the Common Terminology Criteria of Adverse Events (CTCAE). The patient-physician agreement for the symptom review was assessed via Cohen kappa (κ), through which the interrater reliability was calculated. Chi-square tests were used to determine whether the patient-reported outcome was different among symptoms, types of cancer, demographics, and physicians' experience., Results: Among the 181 patients (158 women and 23 men; median age 54.4 years), there was a fair scoring agreement (κ=0.24; 95% CI 0.16-0.33) for symptoms that were entered 2 to 4 weeks before the intended review (first rating) and a moderate agreement (κ=0.41; 95% CI 0.34-0.48) for symptoms that were entered within 1 week of the intended review (second rating). However, the level of agreement increased from moderate (first rating, κ=0.43) to substantial (second rating, κ=0.68) for common symptoms of pain, fever, diarrhea, obstipation, nausea, vomiting, and stomatitis. Similar congruency levels of ratings were found for the most frequently entered symptoms (first rating: κ=0.42; second rating: κ=0.65). The symptom with the lowest agreement was hair loss (κ=-0.05). With regard to the latency of symptom entry into the review, hardly any difference was demonstrated between symptoms that were entered from days 1 to 3 and from days 4 to 7 before the intended review (κ=0.40 vs κ=0.39, respectively). In contrast, for symptoms that were entered 15 to 21 days before the intended review, no congruency was demonstrated (κ=-0.15). Congruency levels seemed to be unrelated to the type of cancer, demographics, and physicians' review experience., Conclusions: The shared monitoring and review of symptoms between patients and clinicians has the potential to improve the understanding of patient self-reporting. 23. Patient preferences for allogeneic haematopoietic stem cell transplantation: how much benefit is worthwhile from the patient's perspective?

Abstract: Oncological studies have shown that patients consider small benefits sufficient to make adjuvant chemotherapy worthwhile. We sought to determine the minimal survival benefits that patients considered enough to legitimate allogeneic haematopoietic stem cell transplantation (HCT) and the factors associated with patient preferences. One hundred eighty-four patients having previously received allogeneic HCT at our centre were included and completed a questionnaire exploring patient expectations elicited by time trade-off scenarios as well as quality of life (QoL), symptoms of graft-versus host disease (GvHD) and sociodemographic characteristics. The majority of patients considered a minimal survival benefit of at least 5 (38.6%) or 10 years (41.9%) sufficient to justify HCT, with less than 5% considering survival < 1 year sufficient to warrant HCT. In terms of minimal cure rate, a cumulative 14.8% of patients accepted cure rates below 30% and 30.6% rates below 50%. Likelihood-ratio tests were significant for the effect of age at transplant on expected minimal survival (p = 0.007) and cure rates (p = 0.0001); that is, younger patients at HCT were more likely to accept smaller survival and cure rates. Pre-transplant risk score, QoL, GvHD score and sociological factors did not seem to influence patients' expectations. In conclusion, patient expectations of treatment were much higher than what had been reported in oncological studies. Patients who experienced HCT considered a survival superior to 1 year and cure rates above 50% sufficient to make it worthwhile.
