1. Hybrid performance assessment of sand mitigation measures for coastal and desert applications

Abstract

Windblown sand hazard affects both building environment and human activities (such as transport infrastructures, buildings, urban areas) and ecological system (such as coastal dunes and desert oasis) in sandy coastal and desert environments. On the one hand, ongoing climatic changes have increased the frequency and magnitude of wind storms along coastal regions in extra tropical regions, Europe included. On the other hand, desert regions are increasingly hosting human activities and built structures. In light of this, windblown sand interacts with ground-mounted obstacles of any kind inducing sand erosion and sedimentation around them and detrimental effects, such as transport infrastructure loss of capacity, but also destructive failures . Several Sand Mitigation Measures (SMMs) design solutions to mitigate windblown sand effects have been proposed so far. They aim to prevent sand from reaching the protected system. Most of them are located between the sand source and the protected system, and they are intended to trap incoming sand by promoting wind speed lowering and sand sedimentation (Path SMMs). SMMs usually translates into nature-based solutions, such as earthen berms, ditches, porous vegetation belts, or artificial obstacles, such as man-made porous and solid barriers. However, with some remarkable exceptions, the rigorous design and performance assessment of SMMs remain at their early stage in the engineering literature, while they are mostly based on trial- and-error approaches in the technical practice. According to the authors, this is due to the multidisciplinary and multiphysics/multiscale nature of the phenomenon coupling fluid dynamics and aeolian processes. As a result, on one hand, research should benefit from disciplines adjacent and partially overlapping, e.g. fluid dynamics, wind engineering and aeolian geomorphology. On the other hand, experimental and numerical approaches should be mutually supporting to model multiphase windblown sand processes. Physical experiments usually translate into full-scale or wind tunnel scale tests. In-situ full scale tests reflects real world environmental conditions but they are expensive, time-consuming, and subject to environmental setup conditions difficult to control. WST testing is almost entirely carried out by scaling the characteristic lengthof the surface-mounted obstacle for both economic and practicality reasons. Conversely, sand grain diameter can hardly be scaled to avoid switching from sand to dust particles and underlying physics. This opens the door to physical similitude theory based on dimensionless numbers (e.g. Re or Fr numbers) referred to the whole multiphase/multiscale flow . The numerical simulation of windblown sand flow, herein called Erosion-Transport-Deposition (ETD) simulation, is mainly carried out through the resolution of fully Eulerian models coupling wind flow aerodynamics and aeolian processes resulting in in-air sand concentration and the morphodynamic evolution of the sand bed. Among them, Eulerian ETD simulations adapt well to the engineering needs of modelling large-scale processes and cutting costs with respect to WST and full- scale tests. However, ETD simulations have to be always calibrated and validated on physical measurements. In this study, the authors take advantage of both WST tests and ETD simulations to assess the performance of a sinusoidal berm SMM. WST tests are carried out in the Wind Tunnel L-1B of von Karman Institute to characterize the sand flux in open-field conditions and around the SMM. The SMM performance is assessed by taking into account the progressive loss of performance of the SMM caused by the gradual accumulation of sand around it. WST measurements are adopted to properly tune and validate ETD simulations carried out with the same scaling of the WST tests. Finally, a full- scale ETD simulation is performed in order to quantify the performance under real-world conditions and quantify the experimental distortion resulting from non-compliance of physical similitude. The complementary combination of WST and ETD provides deep insight into the scaling effects to SMM performance, lowering the costs with respect to in-situ full scale testing. Furthermore, it constitutes a promising approach to design SMMs and shorten the time required to evaluate in-field efficiency. 2. High-rate entanglement between a semiconductor spin and indistinguishable photons

Abstract

Photonic graph states, quantum light states where multiple photons are mutually entangled, are key resources for optical quantum technologies. They are notably at the core of error-corrected measurement-based optical quantum computing and all-optical quantum networks. In the discrete variable framework, these applications require high efficiency generation of cluster-states whose nodes are indistinguishable photons. Such photonic cluster states can be generated with heralded single photon sources and probabilistic quantum gates, yet with challenging efficiency and scalability. Spin-photon entanglement has been proposed to deterministically generate linear cluster states. First demonstrations have been obtained with semiconductor spins achieving high photon indistinguishablity, and most recently with atomic systems at high collection efficiency and record length. Here we report on the efficient generation of three partite cluster states made of one semiconductor spin and two indistinguishable photons. We harness a semiconductor quantum dot inserted in an optical cavity for efficient photon collection and electrically controlled for high indistinguishability. We demonstrate two and three particle entanglement with fidelities of 80 % and 63 % respectively, with photon indistinguishability of 88%. The spin-photon and spin-photon-photon entanglement rates exceed by three and two orders of magnitude respectively the previous state of the art. Our system and experimental scheme, a monolithic solid-state device controlled with a resource efficient simple experimental configuration, are very promising for future scalable applications.

Abstract

Semiconductor quantum dots in cavities are promising single-photon sources. Here, we present a path to deterministic operation, by harnessing the intrinsic linear dipole in a neutral quantum dot via phonon-assisted excitation. This enables emission of fully polarized single photons, with a measured degree of linear polarization up to 0.994 $\pm$ 0.007, and high population inversion -- 85\% as high as resonant excitation. We demonstrate a single-photon source with a polarized first lens brightness of 0.50 $\pm $ 0.01, a single-photon purity of 0.954 $\pm$ 0.001 and single-photon indistinguishability of 0.909 $\pm$ 0.004., 5 pages, 4 figures, plus supplementary material

Abstract

International audience; Background: The physical activity (PA) level of individuals with knee osteoarthritis is lower than in the general population. International recommendations recommend a non-pharmacological intervention including a self-management education program, weight loss and an adapted exercise program. However, we have no scale assessing the perceived barriers to and facilitators of PA in this population.Objective: We constructed and validated a self-administered questionnaire assessing perceived barriers to and facilitators of regular practice of PA in people with knee osteoarthritis.Methods: Semi-structured interviews identified 24 barriers and facilitators. We developed a 24-item questionnaire, Evaluation of the Perception of Physical Activity (EPPA) that was completed by 548 individuals with knee osteoarthritis, to assess acceptability, construct validity, internal consistency and convergent validity. Participants also completed the Knee Osteoarthritis Fears and Beliefs Questionnaire (KOFBeQ), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and International Physical Activity Questionnaire (IPAQ). Reliability and sensitivity to change were evaluated in a second group of 168 people with knee osteoarthritis at a 3-week spa therapy resort.Results: Factorial analysis identified 17 items grouped into 4 subscales (Barriers, Facilitators, Motivation and Beliefs). The internal consistency was good for Barriers, Facilitators and Motivation subscales (Cronbach alpha > 0.70) and intermediate for the Beliefs subscale (Cronbach alpha = 0.64). The EPPA subscale scores were significantly correlated with KOFBeQ and WOMAC scores but not associated with IPAQ physical activity level. Reliability was good for all subscales, with intraclass correlation coefficients > 0.60. A sensitivity to change was found for only the Beliefs subscale, with a moderate effect size.Conclusions: The EPPA questionnaire has good psychometric properties and can help guide the management of knee osteoarthritis. It can be used in research for evaluating the perception of physical activity.

Abstract

Aeolian sediment transport in desert and sandy coastal environments affects civil structures and infrastructures, such as pipelines, industrial facilities, towns, single buildings, farms, roads, and railways [1]. The wind flow interacts with surface-mounted obstacles of any kind inducing sand erosion, transport, and sedimentation around them. This can lead to detrimental effects such as the loss of functionality of the endangered structure or infrastructure, or even danger for users when structural failure is involved [2]. In order to cope with the effects above, the demand for the characterization of aeolian sand transport and the design of Sand Mitigation Measures (SMMs) has grown in the last decade and is expected to further increase in the next years [1]. The multiphase and multiscale nature of the aeolian flow ranging from the sand grain diameters to the obstacle characteristic lengths make the problem only tractable by means of physical experiments and computational simulations. On the one hand, in-situ full scale field tests are expensive, time-consuming, and subject to environmental setup conditions difficult to control. On the other hand, numerical models shall be carefully validated against physical experiments. Hence, experimental Wind-Sand Tunnel Tests (WSTTs) are often carried out.In this study, windblown sand transport on flat ground is reproduced by means of WSTTs carried out in the wind tunnel L-1B of von Karman Institute for Fluid Dynamics. The aim of WSTTs is twofold. On one hand, they are intended to characterize the incoming sand flux in open field conditions. On the other hand, they allow to properly tune cheaper Wind-Sand Computational Simulations [3], so as to assess the performance of SMMs in full-scale. The wind tunnel setup implements a uniform 5-meter-long sand fetch as sand source. The wind speed boundary layer and sand flux saltation layer are characterized through 2D Particle Image Velocimetry (PIV) and Particle Tracking Velocimetry (PTV) techniques, respectively. Wind flow and sand transport state variables are assessed along the sand fetch by setting the wind speed equal to 1.3, 1.5, 2 times the threshold one, and by assessing the influence of a monoplane grid installed at the inlet of the wind tunnel testing sections. Results from WSTTs are critically discussed by investigating the effects induced by the sand fetch length, wind speed, and turbulence intensity on the sand transport. Abstract

The transport of particulate by wind constitutes a relevant phenomenon in environmental sciences and civil engineering, because erosion, transport and deposition of particulate can cause serious problems to human infrastructures. From a mathematical point of view, modeling procedure for this phenomenon requires handling the interaction between different constituents, the transfer of a constituent from the air to the ground and viceversa, and consequently the ground-surface interaction and evolution. Several approaches have been proposed in the literature, according to the specific particulate or application. We here review these contributions focusing in particular on the behavior of sand and snow, which almost share the same mathematical modeling issues, and point out existing links and analogies with wind driven rain. The final aim is then to classify and analyze the different mathematical and computational models in order to facilitate a comparison among them. A first classification of the proposed models can be done distinguishing whether the dispersed phase is treated using a continuous or a particle-based approach, a second one on the basis of the type of equations solved to obtain particulate density and velocity, a third one on the basis of the interaction model between the suspended particles and the transporting fluid.

10. Relationships between serotonin availability and frontolimbic response to fearful and threatening faces

Abstract

Abstract Serotonin is a critical neurotransmitter in the regulation of emotional behavior. Although emotion processing is known to engage a corticolimbic circuit, including the amygdala and prefrontal cortex, exactly how this brain system is modulated by serotonin remains unclear. Here, we hypothesized that serotonin modulates variability in excitability and functional connectivity within this circuit. We tested whether this modulation contributes to inter-individual differences in emotion processing. Using a multimodal neuroimaging approach with a simultaneous PET-3T fMRI scanner, we simultaneously acquired BOLD signal while participants viewed emotional faces depicting fear and anger, while also measuring serotonin transporter (SERT) levels, regulating serotonin functions. Individuals with higher activity of the medial amygdala BOLD in response to fearful or angry facial expressions, who were temperamentally more anxious, also exhibited lower SERT availability in the dorsal raphe nucleus (DRN). Moreover, higher connectivity of the medial amygdala with the left dorsolateral prefrontal and the anterior cingulate cortex was associated with lower levels of SERT availability in the DRN. These results demonstrate the association between the serotonin transporter level and emotion processing through changes in functional interactions between the amygdala and the prefrontal areas in healthy humans.

11. Development and validation of a questionnaire assessing the perception of physical activity (EPAP) in knee osteoarthritis people

Our purpose is to develop and validate a self-questionnaire assessing barriers and facilitators to practice regular PA in patients with knee osteoarthritis. Material and method Semi-structured interviews identified 22 barriers and facilitators. We developed a 24 items questionnaire (evaluated by a 5-point Likert scale) completed by 548 patients with knee osteoarthritis. In addition to EPAP, patients completed 3 other questionnaires: KOFBeQ (assessing the fears and beliefs of knee osteoarthritis patients), WOMAC (assessing the severity of osteoarthritis) and IPAQ (assessing the level of physical activity). Reproductibility and sensitivity to change were evaluated by a second group of 142 patients included in a three weeks spa therapy resort. Results Seven items were removed after preliminary statistical analysis. Seventeen items were grouped into four dimensions (Barriers, Facilitators, Motivations, Beliefs). The internal consistency is good for Barriers, Facilitators and Motivations dimensions (Croncach's α > 0.7); intermediate for Beliefs dimension (Croncach's α = 0.64). EPAP was significantly correlated with KOFBEQ and WOMAC but not with IPAQ. Reproducibility was good for all dimensions with intraclass correlation coefficients higher than 0.60. Only “Beliefs” dimension showed a significant improvement between day 1 and month 3 after spa therapy resort. Conclusion EPAP questionnaire has good psychometric properties and can help to guide the management of patients with knee ostoarthritis. It can be used in research for the evaluation of the perception of physical activity.

12. Intérêt du suivi des concentrations sanguines d’anti-TNF α et de l’apparition d’anticorps anti-médicament chez des patients présentant une uvéite chronique non infectieuse : étude d’une cohorte rétrospective de 21 patients

Abstract

Introduction Le traitement des uveites chroniques non-infectieuses peut inclure l’utilisation d’anticorps monoclonaux anti-TNFα, dont l’adalimumab (ADA) et l’infliximab (IFX). Grâce a l’utilisation de tests commerciaux ELISA, la detection d’anticorps anti-adalimumab (AAA) ou anti-infliximab (AAI) et le dosage de la concentration sanguine d’anti-TNFα peuvent etre realises en pratique clinique courante. L’objectif de notre etude est d’evaluer l’interet de ces dosages, dans la prise en charge des patients presentant une uveite chronique non infectieuse. Patients et methodes Une etude observationnelle monocentrique retrospective a ete realisee en 2017 au Centre Hospitalier Universitaire Grenoble Alpes. Les patients presentant une uveite chronique non infectieuse, traites par adalimumab ou par infliximab, et ayant eu un dosage du taux plasmatique du medicament et une recherche d’anticorps anti-traitement (AAA ou AAI), ont ete inclus. Le dosage du medicament et la recherche d’anticorps induits ont ete realises par technique ELISA (Theradiag®). La reponse ophtalmologique de l’uveite (totale, partielle ou absente) etait determinee a chaque dosage. Cette reponse etait definie, par rapport a l’etat clinique a l’instauration du traitement, selon un critere clinique composite ophtalmologique base sur la presence d’inflammation dans la chambre anterieure ou le vitree, d’un œdeme maculaire cystoide ou d’anomalies a l’angiographie. Resultats Vingt et un patients suivis pour une uveite chronique non infectieuse sous anti-TNFα, dont 12 traites par adalimumab et 9 par infliximab, ont beneficie d’au moins une recherche d’anticorps anti-medicament et d’un dosage sanguin d’anti-TNFα. Trois dosages ont ete realises chez 7 patients, 2 dosages chez 3 patients et enfin, 1 dosage chez 11 patients. Trois patients ont developpe des anticorps. Ils etaient transitoires pour un patient traite par adalimumab. Ce patient poursuit actuellement le traitement sans perte d’efficacite. Ils etaient stables pour deux patients (un traite par IFX et un par ADA) qui ont arrete le traitement pour inefficacite clinique. Alors qu’une reponse clinique totale et partielle etait retrouvee chez 60,5 % des patients en absence d’anticorps, une absence de reponse clinique etait systematiquement retrouvee en presence d’AAA ou d’AAI permanents. En presence d’AAA ou d’AAI, la concentration sanguine d’anti-TNF α etait plus faible : 3,2 ± 5,4 μg/mL [0,1–9,4] vs 9,8 ± 5,0 μg/mL [0,1–16] (p = 0,032). Neanmoins, aucune association entre concentration sanguine et efficacite clinique n’a pu etre mise en evidence. Conclusion Aucune relation, entre l’apparition d’anticorps anti-medicament et l’efficacite clinique sur l’uveite chronique non infectieuse, n’a pu etre mise en evidence. Neanmoins, tous les patients ayant developpes des AAA ou AAI stables ont arrete le traitement pour absence d’efficacite clinique. L’apparition d’anticorps anti-medicament est associee a une diminution de la concentration sanguine de l’anti-TNFα. Une etude prospective semble interessante a mener, pour montrer l’interet des dosages sanguin d’anti-TNFα et d’anticorps anti-medicament dans les uveites chroniques non-infectieuses.

Abstract

The aim of this paper is to provide a contribution to the analysis of the 3D, high Reynolds number, turbulent, separated and reattached flow around a fixed sharp-edged rectangular cylinder with a chord-to-depth ratio equal to 5. The work is developed in the perspective of the benchmark on the aerodynamics of a rectangular cylinder (BARC), in terms of an exploratory computational study. First, the adopted flow modelling and computational approach are shortly described. Second, the obtained main aerodynamic integral parameters are compared with other results proposed in the literature. Hence, the 3D flow features around the nominally 2D cylinder are investigated by means of both proper orthogonal decomposition and coherence function of the side-surface fluctuating pressure field. Once the main 2D nature of the flow has been pointed out, some of the 2D mechanisms that are responsible for the variation of the fluctuating aerodynamic forces are scrutinised: the computational approach post-processing facilities are employed to look for significant relationships between the flow structures, the pressure field and the aerodynamic force components.

Abstract

In the present contribution, a systematic parametrical study on the spanwise length of the computational domain and on the spanwise grid density is proposed in the framework of the Benchmark on the Aerodynamics of a Rectangular 5:1 Cylinder (BARC). The study aims at evaluating the effects of these computational features on the simulated flow field in terms of bulk parameters, spanwise-averaged distributions, correlation coefficient and correlation length. The effectiveness of the adopted computational approach to provide the correlation measures is discussed. In particular, dense spanwise grids seem to be a key element to simulate some emerging and unexpected features of the cylinder aerodynamic behaviour, such as the asymmetry of the time-averaged flow field.

Abstract

The MAGALI chip is a 65nm digital baseband dedicated to advanced telecommunication applications. Based on a 15-router mesh Network-on-Chip - NoC, it embeds 22 Processing Elements - PE performing the different functions of a complex baseband in a programmable manner. The NoC framework is based on asynchronous routers which provide a complete Globally Asynchronous Locally Synchronous -- GALS framework. Thanks to this structure, each PE is a synchronous island with a programmable frequency. On the architectural side, the NoC supports fast reconfiguration thanks to a distributed scheme. In this demonstration, we propose to show the reconfiguration capabilities of the MAGALI chip on three modes of a 3GPP-LTE receiver part, by switching on-the-fly between these three modes. The resulting transmission quality with different levels of upcoming signal noises is shown.

Abstract

We evaluate the contribution made by multivariate curve resolution–alternating least squares (MCR-ALS) for resolving gel permeation chromatography–Fourier transform infrared (GPC-FT-IR) data collected on butadiene rubber (BR) and styrene butadiene rubber (SBR) blends in order to access in-depth knowledge of polymers along the molecular weight distribution (MWD). In the BR–SBR case, individual polymers differ in chemical composition but share almost the same MWD. Principal component analysis (PCA) gives a general overview of the data structure and attests to the feasibility of modeling blends as a binary system. MCR-ALS is then performed. It allows resolving the chromatographic coelution and validates the chosen methodology. For SBR–SBR blends, the problem is more challenging since the individual elastomers present the same chemical composition. Rank deficiency is detected from the PCA data structure analysis. MCR-ALS is thus performed on column-wise augmented matrices. It brings very useful insight into the composition of the analyzed blends. In particular, a weak change in the composition of individual SBR in the MWD's lowest mass region is revealed.

Abstract

Haemorrhagic erysipelas (cellulitis) is a recently described clinical condition. We report 3 new cases of this rare bacterial dermatosis requiring both antibiotics and systemic corticosteroids for complete resolution.Case 1 was a 75 year-old male patient, with a past history of hypertension and diabetes mellitus. Case 2 was a 69 year-old female patient with a past history of hepatic cirrhosis. Case 3 was a 56 year-old female patient without medical past history, except for obesity. All patients presented with a haemorrhagic and bullous erysipelas of the leg without clinical improvement under parenteral antibiotics. In all cases, adjuvant systemic corticosteroids (prednisone: 0.5 mg/kg/d) for 5-8 days in addition to antibiotics provided dramatic and complete resolution of skin lesions and disappearance of pain and fever.Haemorrhagic cellulitis may be misdiagnosed as necrotizing fasciitis because of ecchymotic areas with frequent bullae resulting in haemorrhagic crusts and partial inefficacy of antibiotics given alone. Adjuvant anticoagulant therapy or coagulation abnormalities may have facilitated the clinical condition in 2 of our 3 cases. The efficacy of adjuvant systemic corticosteroids during 5-8 days, in addition to antibiotics is the hallmark of this syndrome.

Abstract

Purpose: We have previously shown that an imaging marker, increased periventricular [11C]flumazenil ([11C]FMZ) binding, is associated with failure to become seizure free (SF) after surgery for temporal lobe epilepsy (TLE) with hippocampal sclerosis (HS). Here, we investigated whether increased preoperative periventricular white matter (WM) signal can be detected on clinical [18F]FDG-PET images. We then explored the potential of periventricular FDG WM increases, as well as whole-brain [11C]FMZ and [18F]FDG images analysed with random forest classifiers, for predicting surgery outcome. Methods: Sixteen patients with MRI-defined HS had preoperative [18F]FDG and [11C]FMZ-PET. Fifty controls had [18F]FDG-PET (30), [11C]FMZ-PET (41), or both (21). Periventricular WM signal was analysed using Statistical Parametric Mapping (SPM8), and whole-brain image classification was performed using random forests implemented in R (http://www.r-project.org). Surgery outcome was predicted at the group and individual levels. Results: At the group level, non-seizure free (NSF) versus SF patients had periventricular increases with both tracers. Against controls, NSF patients showed more prominent periventricular [11C]FMZ and [18F]FDG signal increases than SF patients. All differences were more marked for [11C]FMZ. For individuals, periventricular WM signal increases were seen at optimized thresholds in 5/8 NSF patients for both tracers. For SF patients, 1/8 showed periventricular signal increases for [11C]FMZ, and 4/8 for [18F]FDG. Hence, [18F]FDG had relatively poor sensitivity and specificity. Random forest classification accurately identified 7/8 SF and 7/8 NSF patients using [11C]FMZ images, but only 4/8 SF and 6/8 NSF patients with [18F]FDG. Conclusion: This study extends the association between periventricular WM increases and NSF outcome to clinical [18F]FDG-PET, but only at the group level. Whole-brain random forest classification increases [11C]FMZ-PET's performance for predicting surgery outcome.

Abstract

The European X-ray Observatory, EXOSAT, to be launched in 1981 will carry two Wolter I x-ray telescopes, each having a geometric area of ~100 cm(2). A qualification model of the Wolter I optics has been manufactured and extensively tested in optical and x-ray beams. The influence of manufacturing tolerances on the resolution of the optics is discussed, and mechanical and optical measurements of those deviations are presented. Finally, the x-ray imaging quality of the optics is presented, and the correlation with the achieved tolerances is shown.

Abstract

Spins in semiconductor quantum dots (QDs) are promising local quantum memories to generate polarization-encoded photonic cluster states, as proposed in the pioneering Lindner and Rudolph scheme (2009 Phys. Rev. Lett. 103 113602). However, harnessing the polarization degree of freedom of the optical transitions is hindered by resonant excitation schemes that are widely used to obtain high photon indistinguishability. Here we show that acoustic phonon-assisted excitation, a scheme that preserves high indistinguishability, also allows to fully exploit the polarization selective optical transitions to initialise and measure single spin states. We access the coherence of hole spin systems in a low transverse magnetic field and directly monitor the spin Larmor precession both during the radiative emission process of an excited state or in the QD ground state. We report a spin state detection fidelity of 94.7 ± 0.2 % granted by the optical selection rules and a 25 ± 5 ns hole spin coherence time, demonstrating the potential of this scheme and system to generate linear cluster states with a dozen of photons.

Abstract

We evaluate the contribution made by multivariate curve resolution–alternating least squares (MCR-ALS) for resolving gel permeation chromatography–Fourier transform infrared (GPC-FT-IR) data collected on butadiene rubber (BR) and styrene butadiene rubber (SBR) blends in order to access in-depth knowledge of polymers along the molecular weight distribution (MWD). In the BR–SBR case, individual polymers differ in chemical composition but share almost the same MWD. Principal component analysis (PCA) gives a general overview of the data structure and attests to the feasibility of modeling blends as a binary system. MCR-ALS is then performed. It allows resolving the chromatographic coelution and validates the chosen methodology. For SBR–SBR blends, the problem is more challenging since the individual elastomers present the same chemical composition. Rank deficiency is detected from the PCA data structure analysis. MCR-ALS is thus performed on column-wise augmented matrices. It brings very useful insight into the composition of the analyzed blends. 30. Does Breast Surgery Type Alter Incidental Axillary Irradiation? A Dosimetric Analysis of the "Sentinel Envahi et Randomisation du Curage" SERC Trial.

Abstract

Background: An incidental axillary dose of adjuvant radiotherapy using tangential beams is usually given after breast-conserving surgery for breast cancer. The goal of this sub-study was to evaluate this incidental dose in the setting of post-mastectomy radiotherapy (PMRT) according to two different radiotherapy techniques., Methods: Patients participating in a randomized SERC trial who received PMRT in a single center were included. We collected the incidental axillary dose delivered to the Berg level 1 using different dosimetric parameters and compared two techniques using Student's t -test: three-dimensional conformal radiotherapy (3D-CRT) and volumetric arc therapy (VMAT)., Results: We analyzed radiotherapy plans from 52 patients who received PMRT from 2012 to 2021. The mean dose delivered to the Berg level 1 was 37.2 Gy. It was significantly higher with VMAT than with 3D-CRT-43.6 Gy (SD = 3.1 Gy) versus 34.8 Gy (SD = 8.6 Gy) p < 0.001. Eighty-four percent of the Berg level 1 was covered by 40 Gy isodose in the VMAT group versus 55.5% in the 3D-CRT group p < 0.001., Conclusions: On the Berg level 1, PMRT gives a dose at least equivalent to the one given by post-breast-conserving surgery radiotherapy, making it possible to limit completion axillary lymph node dissections in select pN1a patients treated with a mastectomy. Modern radiotherapy techniques like VMAT tend to increase this incidental dose.

Abstract

Objective: To identify prognostic factors for return to work (RTW) after stroke., Data Sources: PubMed, MEDLINE, Cochrane, and Embase were systematically searched., Study Selection: Studies had to include people of working age (<65 years old) at the time of stroke (ischemic, hemorrhagic, or subarachnoid hemorrhage). The evaluation of RTW and rate of RTW had to be mentioned. Study selection was done by 2 independent authors. In total, 1241 articles were screened, 39 met all inclusion criteria., Data Extraction: Characteristics of included studies were recorded independently by 2 authors. Differences were resolved through discussion or with a third author. Quality was assessed using the Scottish Intercollegiate Guidelines Network quality assessment tool., Data Synthesis: Among the 39 studies, prognostic factors for RTW were hemorrhagic stroke (odds ratio 0.53 [95% confidence interval 0.45-0.60], n=18 studies), sex (men) (1.26 [1.14-1.40], n=31), aphasia (0.37 [0.20-0.69], n=7), occupation (white collar worker) (1.84 [1.64-2.06], n=17), independence in activities of daily living (3.99 [1.73-9.23], n=7), and stroke severity (NIHSS) (1.23 [1.08-1.39], n=6)., Conclusions: This meta-analysis highlighted positive and negative prognostic factors associated with RTW after stroke. Two categories were distinguished: modifiable and non-modifiable prognostic factors. This study provides information to help understand the issues, set appropriate objectives and implement appropriate strategies to guide people to RTW after stroke. 34. Bright Polarized Single-Photon Source Based on a Linear Dipole.

Abstract

Semiconductor quantum dots in cavities are promising single-photon sources. Here, we present a path to deterministic operation, by harnessing the intrinsic linear dipole in a neutral quantum dot via phonon-assisted excitation. This enables emission of fully polarized single photons, with a measured degree of linear polarization up to 0.994±0.007, and high population inversion-85% as high as resonant excitation. We demonstrate a single-photon source with a polarized first lens brightness of 0.50±0.01, a single-photon purity of 0.954±0.001, and single-photon indistinguishability of 0.909±0.004.

Abstract

Hong-Ou-Mandel interference is a cornerstone of optical quantum technologies. We explore both theoretically and experimentally how unwanted multiphoton components of single-photon sources affect the interference visibility, and find that the overlap between the single photons and the noise photons significantly impacts the interference. We apply our approach to quantum dot single-photon sources to access the mean wave packet overlap of the single-photon component. This study provides a consistent platform with which to diagnose the limitations of current single-photon sources on the route towards the ideal device.

Abstract

Background: The efficacy of spa therapy in osteoarthritis (OA) has ever been demonstrated, with a good level of evidence for pain and disability. The effect of a self-management program with spa therapy on physical activity (PA) level has never been demonstrated., Objective: This study aimed to assess, at 3 months, the effectiveness of 5 sessions of a self-management exercise program in patients with knee OA (KOA) who benefit from 18 days of spa therapy and received an information booklet (on proposed physical exercises) on improvement in at least one PA level., Methods: This was an interventional, multicentre, quasi-randomized controlled trial with a cluster randomized design (1-month period). People 50 to 75 years old with symptomatic knee OA were included in 3 spa therapy centres in France (Bourbon Lancy, Le Mont Dore, Royat). Both groups received conventional spa therapy sessions during 18 days and an information booklet on the benefits of PA practice for KOA. The intervention group additionally received 5 self-management exercise sessions. The main outcome was improvement in at least one PA level according to the International Physical Activity Questionnaire (IPAQ) short-form categorical score (low to moderate or high, or moderate to high) at 3 months. Secondary outcomes were the evolution of PA (MET-min/week), disability, pain, anxiety, depression, self-efficacy, fears and beliefs concerning KOA, barriers to and facilitators of regular PA practice, consumption of painkillers and adherence to physical exercise program at 3 months. Assessors but not participants or caregivers were blinded., Results: In total, 123 patients were randomized, 54 to the intervention group and 69 to the control group. Considering the main outcome, at 3 months, 37% of patients in the intervention group showed improvement in at least one PA level according to the IPAQ categorical score versus 30.4% in the control group (P=0.44). In the intervention group, 13 (24.1%) patients showed improvement from low to moderate PA level (vs. 8 [11.6%] in the control group), 2 (3.7%) from low to high (vs. 2 [2.9%]) and 5 (9.3%) from moderate to highvs. 11 [15.9%]). Both intervention and control groups showed increased IPAQ continuous scores (MET-min/week) at 3 months, although not significantly. HAD anxiety and depression scores were significantly reduced in the intervention group (P=0.001 and P=0.049, respectively) and the perception of PA was better in the intervention than control group for motivation and barriers scores (P=0.019 and P=0.002, respectively)., Conclusions: This study showed the lack of impact of a short self-management program on PA level in addition to 18-day spa therapy for KOA, but both intervention and control groups showed improved PA level., (Copyright © 2019 Elsevier Masson SAS. Abstract

Background: The physical activity (PA) level of individuals with knee osteoarthritis is lower than in the general population. International recommendations recommend a non-pharmacological intervention including a self-management education program, weight loss and an adapted exercise program. However, we have no scale assessing the perceived barriers to and facilitators of PA in this population., Objective: We constructed and validated a self-administered questionnaire assessing perceived barriers to and facilitators of regular practice of PA in people with knee osteoarthritis., Methods: Semi-structured interviews identified 24 barriers and facilitators. We developed a 24-item questionnaire, Evaluation of the Perception of Physical Activity (EPPA) that was completed by 548 individuals with knee osteoarthritis, to assess acceptability, construct validity, internal consistency and convergent validity. Participants also completed the Knee Osteoarthritis Fears and Beliefs Questionnaire (KOFBeQ), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and International Physical Activity Questionnaire (IPAQ). Reliability and sensitivity to change were evaluated in a second group of 168 people with knee osteoarthritis at a 3-week spa therapy resort., Results: Factorial analysis identified 17 items grouped into 4 subscales (Barriers, Facilitators, Motivation and Beliefs). The internal consistency was good for Barriers, Facilitators and Motivation subscales (Cronbach α>0.70) and intermediate for the Beliefs subscale (Cronbach α=0.64). The EPPA subscale scores were significantly correlated with KOFBeQ and WOMAC scores but not associated with IPAQ physical activity level. Reliability was good for all subscales, with intraclass correlation coefficients>0.60. A sensitivity to change was found for only the Beliefs subscale, with a moderate effect size., Conclusions: The EPPA questionnaire has good psychometric properties and can help guide the management of knee osteoarthritis. Abstract

Objective:: To validate the Arthritis Self-Efficacy Scale-French version (ASES-F) of the ASES questionnaire designed to evaluate self-efficacy in arthritis population and to confirm its psychometric properties., Design:: The translation/back-translation procedure was done in line with cross-cultural adaptation international guidelines. Prospective multicenter validation was performed on 168 patients with osteoarthritis who filled out the ASES-F. Function subscale of the Western Ontario and McMaster Universities Arthritis Index, Hospital Anxiety and Depression (HAD), Knee Osteoarthritis Fears and Beliefs Questionnaire, and pain on visual analog scale were collected. Acceptability, internal consistency, reproducibility, internal and external structure validity, and sensitivity following a self-management program focused on physical activity were analyzed. The retest was performed 15 days later., Results:: A culturally adapted version was obtained following the validation procedure. The final translated questionnaire is available within the full article. Cronbach's alpha coefficient was at 0.95 for the overall ASES-F. Regarding the test-retest reproducibility, Lin's concordance coefficient was at 0.84. The internal and external validity was also explored with correlations at -0.50 between ASES-F and (WOMAC), and significant correlations between ASES-F and HAD. Sensitivity shown significant improvement at three months for the subscale function only (+2.65, P < 0.01)., Conclusion:: The ASES-F, French version of the ASES, available in supplemental data, was validated in knee osteoarthritis patients and could be used in clinical practice and research for French-speaking patients. Sensitivity needs to be more explored.

Background: Osteoarthritis (OA) is not limited to joint pain and stiffness, which can lead to disability; it is also linked to comorbidities such as overweight, obesity and fears and beliefs related to the pathology. The knee OA population appears more affected by these risk factors and has a lower physical activity (PA) level than the general population. The key challenge for OA treatment is increasing the PA level to decrease the risk factors., Methods: We aim to perform a prospective, multicentric, quasi-randomized controlled trial with an alternate-month design (1-month periods). People aged 50-75 years old with symptomatic knee OA (stage I-IV Kellgren and Lawrence scale) with low and moderate PA level will be included in 3 spa therapy resorts. The experimental arm will receive 5 self-management exercise sessions (1.5 h each; education, aerobics, strength training, range of motion) + an information booklet + 18 sessions (1 h each) of spa therapy treatment (STT). The active comparator arm will receive an information booklet + 18 sessions of STT. The primary outcome will be a change at 3 months in PA level (International Physical Activity Questionnaire short form score). Secondary outcomes will be function (WOMAC) pain (numerical scale), anxiety/depression (HAD), fears and beliefs about OA (KOFBeQ) and arthritis self-efficacy (ASES). The barriers to and facilitators of regular PA practice will be assessed by using specific items specifically designed for the study because of lack of any reference scale., Discussion: The study could demonstrate the impact of a self-management exercise program associated with spa therapy in the medium term by increasing PA level in people with OA. A benefit for ameliorating fears and beliefs and anxiety/depression and improving self-efficacy will also be analysed. The findings could offer new prospects while establishing best clinical practice guidelines for this population., Trial Registration: ClinicalTrials.gov NCT02598804 (November 5, 2015).

41. Cobalt-Iron-Manganese Catalysts for the Conversion of End-of-Life-Tire-Derived Syngas into Light Terminal Olefins.

Abstract

Co-Fe-Mn/γ-Al 2 O 3 Fischer-Tropsch synthesis (FTS) catalysts were synthesized, characterized and tested for CO hydrogenation, mimicking end-of-life-tire (ELT)-derived syngas. It was found that an increase of C 2 -C 4 olefin selectivities to 49 % could be reached for 5 wt % Co, 5 wt % Fe, 2.5 wt % Mn/γ-Al 2 O 3 with Na at ambient pressure. Furthermore, by using a 5 wt % Co, 5 wt % Fe, 2.5 wt % Mn, 1.2 wt % Na, 0.03 wt % S/γ-Al 2 O 3 catalyst the selectivity towards the fractions of C 5+ and CH 4 could be reduced, whereas the selectivity towards the fraction of C 4 olefins could be improved to 12.6 % at 10 bar. Moreover, the Na/S ratio influences the ratio of terminal to internal olefins observed as products, that is, a high Na loading prevents the isomerization of primary olefins, which is unwanted if 1,3-butadiene is the target product. Thus, by fine-tuning the addition of promoter elements the volume of waste streams that need to be recycled, treated or upgraded during ELT syngas processing could be reduced. The most promising catalyst (5 wt % Co, 5 wt % Fe, 2.5 wt % Mn, 1.2 wt % Na, 0.03 wt % S/γ-Al 2 O 3 ) has been investigated using operando transmission X-ray microscopy (TXM) and X-ray diffraction (XRD). Abstract

We evaluate the contribution made by multivariate curve resolution-alternating least squares (MCR-ALS) for resolving gel permeation chromatography-Fourier transform infrared (GPC-FT-IR) data collected on butadiene rubber (BR) and styrene butadiene rubber (SBR) blends in order to access in-depth knowledge of polymers along the molecular weight distribution (MWD). In the BR-SBR case, individual polymers differ in chemical composition but share almost the same MWD. Principal component analysis (PCA) gives a general overview of the data structure and attests to the feasibility of modeling blends as a binary system. MCR-ALS is then performed. It allows resolving the chromatographic coelution and validates the chosen methodology. For SBR-SBR blends, the problem is more challenging since the individual elastomers present the same chemical composition. Rank deficiency is detected from the PCA data structure analysis. MCR-ALS is thus performed on column-wise augmented matrices. It brings very useful insight into the composition of the analyzed blends. In particular, a weak change in the composition of individual SBR in the MWD's lowest mass region is revealed.

44. Fast and robust method for the determination of microstructure and composition in butadiene, styrene-butadiene, and isoprene rubber by near-infrared spectroscopy.

Abstract

In the tire industry, synthetic styrene-butadiene rubber (SBR), butadiene rubber (BR), and isoprene rubber (IR) elastomers are essential for conferring to the product its properties of grip and rolling resistance. Their physical properties depend on their chemical composition, i. e., their microstructure and styrene content, which must be accurately controlled. This paper describes a fast, robust, and highly reproducible near-infrared analytical method for the quantitative determination of the microstructure and styrene content. The quantitative models are calculated with the help of pure spectral profiles estimated from a partial least squares (PLS) regression, using (13)C nuclear magnetic resonance (NMR) as the reference method. This versatile approach allows the models to be applied over a large range of compositions, from a single BR to an SBR-IR blend. The resulting quantitative predictions are independent of the sample path length. As a consequence, the sample preparation is solvent free and simplified with a very fast (five minutes) hot filming step of a bulk polymer piece. No precise thickness control is required. Thus, the operator effect becomes negligible and the method is easily transferable. The root mean square error of prediction, depending on the rubber composition, is between 0.7% and 1.3%. The reproducibility standard error is less than 0.2% in every case.

Abstract

The aim of this study was to compare ethanol concentrations in right cardiac blood, left cardiac blood and peripheral blood. Samples were taken from a series of 30 medicolegal autopsies. Ethanol was measured by headspace GC-FID. In each case, the degree of putrefaction, chest or abdominal injury, and/or regurgitation of gastric contents into the airways were noted. Our results show that there exists in certain cases a marked increase in ethanol concentration in left cardiac blood compared with right cardiac blood and peripheral blood. In these cases, we observed (i) a high concentration of ethanol in the gastric contents and (ii) regurgitation of gastric contents into the airways. The authors discuss the post-mortem redistribution mechanisms which could explain these results and stress the value of sampling right cardiac blood at autopsy.

Background: Rare osteoarticular complications occurring after erysipelas have been reported. We describe 9 patients in whom various osteoarticular complications developed during erysipelas., Objective: We sought to analyze osteoarticular complications during erysipelas, paying special attention to clinical, bacteriologic, and radiologic data., Methods: Data were retrospectively recorded from the files of patients seen in 3 dermatologic centers between 1998 and 2000. They included laboratory tests, bacteriologic cultures, radiologic investigations, and treatment modalities and outcome of both erysipelas and osteoarticular complications., Results: We observed 9 patients (7 men and 2 women; mean age 49.6 years) who first presented with typical erysipelas of the lower limb and then osteoarticular complications developed during the course of their disease, always localized to a joint contiguous to the erysipelas plaque. These complications included: relatively benign complications, ie, bursitis (n = 5) or algodystrophy (n = 1), occurring after erysipelas with favorable course; and more severe complications, ie, osteitis (n = 1), arthritis (n = 1), and septic tendinitis (n = 1), occurring after erysipelas characterized by local cutaneous complications (abscess, necrosis)., Conclusions: Osteoarticular complications of erysipelas can be divided into the 2 groups of nonseptic complications (mainly bursitis), which are characterized by a favorable outcome, and septic complications (osteitis, arthritis, tendinitis), which require specific, often prolonged treatment and, sometimes, operation. Their diagnosis is on the basis of clinical and radiologic findings rather than joint aspirations, which are usually not possible through infected skin tissue.

Abstract

Background: Haemorrhagic erysipelas (cellulitis) is a recently described clinical condition. We report 3 new cases of this rare bacterial dermatosis requiring both antibiotics and systemic corticosteroids for complete resolution., Case Reports: Case 1 was a 75 year-old male patient, with a past history of hypertension and diabetes mellitus. Case 2 was a 69 year-old female patient with a past history of hepatic cirrhosis. Case 3 was a 56 year-old female patient without medical past history, except for obesity. All patients presented with a haemorrhagic and bullous erysipelas of the leg without clinical improvement under parenteral antibiotics. In all cases, adjuvant systemic corticosteroids (prednisone: 0.5 mg/kg/d) for 5-8 days in addition to antibiotics provided dramatic and complete resolution of skin lesions and disappearance of pain and fever., Discussion: Haemorrhagic cellulitis may be misdiagnosed as necrotizing fasciitis because of ecchymotic areas with frequent bullae resulting in haemorrhagic crusts and partial inefficacy of antibiotics given alone. Adjuvant anticoagulant therapy or coagulation abnormalities may have facilitated the clinical condition in 2 of our 3 cases. The efficacy of adjuvant systemic corticosteroids during 5-8 days, in addition to antibiotics is the hallmark of this syndrome.
