Objetivo: Describir la frecuencia de las enfermedades reumatológicas inflamatorias de inicio juvenil (RI J) en hospitales de nivel de complejidad III y IV en un quinquenio. Método: Estudio multicéntrico, descriptivo, retrospectivo utilizando códigos CI E-10 de los archivos de hospitales de Asunción y del departamento central. Resultados: Cuatro archivos hospitalarios fueron incluidos. En total 382 pacientes cumplían los criterios de inclusión. El sex ratio (M: F) de la población fue de 1:1.6, edad promedio a la captación de 11.6 años (DE±4.5). El grupo de pacientes con Enfermedad de Kawasaki tenían edad promedio de 5.4 años (± 3.7 DE) y los pacientes con Lupus Eritematoso Sistémico (LES) se presentaron con 13.1 años (± 3.9 DE). El 68,6% de los pacientes fueron captados por consulta especializada de seguimiento. La artritis idiopática juvenil fue la enfermedad RIJ más frecuentemente diagnosticada (n=167 casos, 43,7% del total), seguida por el LES (n=130, 34,0%). La mayoría de los casos (75.9%, n=290) eran pacientes procedentes (al nacimiento) de Asunción o del departamento central. Conclusión: El espectro de las enfermedades RI J es amplio con distribución variable según los hospitales incluidos. Este estudio orienta a conocer la “carga de enfermedad” por enfermedades RI J en Paraguay.

We present the detection of a Warm Neptune orbiting the M-dwarf Gl378, using radial velocity measurements obtained with the SOPHIE spectrograph at the Observatoire de Haute-Provence. The star was observed in the context of the SOPHIE exoplanets consortium's subprogramme dedicated to finding planets around M-dwarfs. Gl378 is an M1 star, of solar metallicity, at a distance of 14.96 pc. The single planet detected, Gl378 b, has a minimum mass of 13.02 $\rm M_{Earth}$ and an orbital period of 3.82 days, which place it at the lower boundary of the Hot Neptune desert. As one of only a few such planets around M-dwarfs, Gl378 b provides important clues to the evolutionary history of these close-in planets. In particular, the eccentricity of 0.1 may point to a high-eccentricity migration. - Abstract
The growth of the world’s cities gives rise to new forms of urban mobility for their inhabitants. In Mexico City, unchecked urban growth in the city’s central areas has created new needs for transportation systems. The solution to this demand was the implementation of two cable car (Cablebus) lines in the city’s most densely populated precincts. The transportation mode called “Cablebus” is a system of gondolas suspended in the air, which transports passengers over short distances from station to station. The rugged topography found in the Gustavo A. Madero and Iztapalapa precincts, located in the north and east of the city, respectively, was the reason behind the decision to implement this mode of urban mobility. The aim of this article is to illustrate the process of active participation among residents, government agencies, and non-profit organizations in Iztapalapa Precinct that unfolded with the implementation of programs like Iztapalapa Mural, in relation to the community’s perception of the urban environment in the context of the implementation of the Cablebus. By making changes in their environment, the community comes to identify with their cultural rights and builds positive expectations for the urban environment. Thus, with the collaboration and support of local homeowners, their efforts led to the creation of 927 artistic murals, enlivening rooftops, perimeter walls, and façades. Local residents express part of their cultural identity in the murals and share it with a wider public along the route of Cablebus Line 2. The perception among residents changes positively with regard to the safety of their living space and urban image. It opened new opportunities to improve and rescue public spaces that have been repurposed to achieve better interaction and communication in the community and create urban-architectural symbols that give the area a unique meaning and promote the integration of urban mobility by incorporating artistic expressions that reflect the l

ObjectivesInduction chemotherapy (ICT) followed by definitive treatment is an accepted non-surgical approach for locoregionally advanced head and neck squamous cell carcinoma (LA-HNSCC). However, ICT remains a challenge for cisplatin-unfit patients. We evaluated paclitaxel and cetuximab (P-C) as ICT in a cohort of LA-HNSCC patients unfit for cisplatin.Materials and MethodsThis is a retrospective analysis of patients with newly diagnosed LA-HNSCC considered unfit for cisplatin-based chemotherapy (age >70 and/or ECOG≥2 and/or comorbidities) treated with weekly P-C followed by definitive radiotherapy and cetuximab (RT-C) between 2010 and 2017. Toxicity and objective response rate (ORR) to ICT and RT-C were collected. Median overall survival (OS) and progression-free survival (PFS) were estimated using the Kaplan–Meier method. Cox regression analysis was performed to determine baseline predictors of OS and PFS.ResultsA total of 57 patients were included. Grade 3–4 toxicity rate to ICT was 54.4%, and there was a death deemed treatment-related (G5). P-C achieved an ORR of 66.7%, including 12.3% of complete responses (CR). After P-C, 45 patients (78.9%) continued with concomitant RT-C. Twenty-six patients (45.6%) achieved a CR after definitive treatment. With a median follow-up of 21.7 months (range 1.2–94.6), median OS and PFS were 22.9 months and 10.7 months, respectively. The estimated 2-year OS and PFS rates were 48.9% and 33.7%, respectively. Disease stage had a negative impact on OS (stage IVb vs. III–IVa: HR = 2.55 [1.08–6.04], p = 0.03), with a trend towards worse PFS (HR = 1.92 [0.91–4.05], p = 0.09). Primary tumor in the larynx was associated with improved PFS but not OS (HR = 0.45 [0.22–0.92], p = 0.03, and HR = 0.69 [0.32–1.54], p = 0.37, respectively).ConclusionP-C was a well-tolerated and active ICT regimen in this cohort of LA-HNSCC patients unfit for cisplatin-based chemotherapy. P-C might represent a valid ICT option for unfit patients and may aid patient selection for definitive treatment.

Phytic acid and phytate anions are among the principal reserves of phosphorus. A few methods have been used for their detection, but these methods require harsh conditions. In this work, graphene quantum dots are successfully synthesized by Opuntia sp. extract through a pyrolysis microwave treatment. This green synthesis method allows us to controllably obtain nanostructures with excellent dispersion and a narrow size distribution to use for detecting phosphates and phytic acid and amplifying fluorescence quenching at pH = 8.8. Their application allows for a one-step turn-off system. The quenching percentage for phytic acid (PA) detection is 53%, and the quenching percentages are 26 and 21% for sodium phosphate monobasic (SPMB) and sodium phosphate dibasic (SPDB), respectively. The Stern-Volmer analysis shows that static and dynamic quenching mechanics contribute to phytic acid quenching.

15. Seasonal demography of the threatened Montevideo Redbelly Toad (Melanophryniscus montevidensis) in a protected area of Uruguay

Estimates of demographic parameters are scarce for Neotropical amphibians, a concerning fact because this region has the highest proportion of threatened amphibians in the world. We conducted a 3-year study where we applied a robust capture--mark--recapture design to assess the importance of breeding and non-breeding activity patterns over the survival rates, detection probabilities, and abundances of the Montevideo Redbelly Toad (Melanophryniscus montevidensis (Philippi, 1902)), a threatened anuran from Uruguay. The best models grouped seasons into hot and cold periods cyclically, were state-dependent in transition probabilities, and were time-dependent in detection probabilities for adults, but had constant detection probabilities for juveniles. Averaged estimates suggest a high survivorship rate during cold seasons (above 80%), but lower probabilities (below 60%) during hot seasons, especially for males. Analogously, the non-breeding activity had a seasonal pattern, with higher activity during spring and higher sheltering rates during autumn. These activity rates negatively influenced the averaged survivorship rates of adult males and females. Long-term (matrix) projections of seasonal survivorships, along with assessments of the causes of these patterns, should be carried out to determine extinction probabilities and possible threats for the conservation of the genus Melanophryniscus. Les estimations de parametres demographiques pour des amphibiens des Neotropiques sont rares, ce qui est inquietant etant donne que cette region compte la plus grande proportion d'amphibiens menaces du monde. Nous avons mene une etude triennale dans laquelle nous avons utilise un schema de capture--marquage--recapture robuste pour evaluer l'incidence des motifs d'activite de reproduction et d'activite internuptiale sur le taux de survie, la probabilite de detection et l'abondance du crapaud a ventre rouge de Montevideo (Melanophryniscus montevidensis (Philippi, 1902)), un anoure menace de l'Uruguay. Les meilleurs modeles regroupent les saisons cycliquement en periodes chaudes et froides, sont dependants de l'etat en ce qui concerne la probabilite de transition et sont dependants du temps en ce qui concerne la probabilite de detection pour les adultes, mais presentent des probabilites de detection constantes pour les juveniles. Les estimations moyennes indiqueraient un taux de survie eleve durant les saisons froides (plus de 80%), mais des probabilites plus faibles (moins de 60%) durant les saisons chaudes, particulierement pour les males. De meme, l'activite internuptiale presente un motif saisonnier, etant plus forte au printemps, alors que la frequence de sejour sous abri est plus elevee a l'automne. Objectives To describe and quantify the dengue-related knowledge, attitudes and practices of residents in an urban shantytown in Lima, Peru.Design/setting A cross-sectional survey of adults between 18 and 80 years living in approximately 120 blocks in Oasis, an urban shantytown situated in the low-to-middle income district of Villa El Salvador in Southern Lima. The survey was adapted from an existing survey previously used in Iquitos, Peru, and included questions relating to knowledge of dengue symptoms, transmission, prevention and current mosquito control practices.Participants A total of 240 surveys were completed with 80% of respondents being female and approximately 50% of all respondents describing themselves as housewives.Results Although 97.9% of respondents had heard of dengue, only 6.2% of people knew someone who had experienced the disease. Approximately half (54.2%) of the respondents knew dengue was transmitted by mosquitoes and 51.7% were able to identify fever and one other correct symptom of dengue. Female sex was significantly associated with greater symptom knowledge (OR 2.22, 95% CI 1.08 to 4.72) and prevention knowledge (OR 2.12, 95% CI 1.06 to 4.21). Past or current higher education attendance was significantly associated with symptom knowledge (OR 2.56, 95% CI 1.25 to 5.44) and transmission knowledge (OR 3.46, 95% CI 1.69 to 7.57). Knowledge of dengue was not significantly associated with carrying out practices to control mosquitoes (OR 1.76, 95% CI 0.87 to 3.54).Conclusions This population demonstrated baseline dengue knowledge. However, this was incomplete and substantially less when compared with endemic areas. Given the sporadic nature of dengue transmission in Lima, it is not surprising that knowledge of the disease was not associated with carrying out practices to reduce mosquitoes. However, as dengue transmission in Lima is likely to increase, understanding how best to improve public knowledge of the disease and how to translate this into appropriate community action will be a key public health consideration.

- Abstract
La foliculitis pseudolinfomatosa, descripta por McNutt en 1986, es una afección de etiología desconocida y poco frecuente, que simula un linfoma cutáneo tanto por su clínica como por su histología. Se presenta como una lesión nodular solitaria, eritematosa, de 0,5 hasta 3 cm, de crecimiento rápido, sobre todo en la cara, en personas de 40 a 60 años, con una histopatología caracterizada por un infiltrado linfocitario B y T perifolicular, y células dendríticas positivas en la inmunohistoquímica para S100 y CD1a. Su curso es benigno, muchas veces autolimitado. Se expone el caso de una paciente con una particular forma clínica de pseudolinfoma.

23. The Adolescent Problem Gambling Prevalence Associated with Leisure-Time Activities and Risky Behaviors in Southern Spain

Gambling addiction is increasing and is becoming a public health concern due to the rise of gambling-related harms affecting the youth. Previous studies suggest a strong link between problem gambling (PG) and substance use and psychosocial and familial factors. Our main objective was to analyze the association between PG and factors like sport, leisure-time activities, and risk-taking behaviors in adolescents. A survey on substance use and addictive behaviors was performed in 2019 on a representative sample of 2240 subjects (14-18 years) from the Southern Spain Region. Data variables like socio-demographic characteristics, sport, leisure and free time activities, family environment, PG (Lie-Bet Scale), compulsive internet use (CIUS Scale), and consumption of alcohol and cannabis were collected using a standardized questionnaire. Weighted PG prevalence was estimated in either sex, as well as the differences between various levels of PG using chi-square tests. Crude and adjusted weighted logistic regression models were used to identify predictors associated with PG. The prevalence of PG was associated with shopping frequency, compulsive internet use, cannabis use in the previous month, higher family economic status, and having a homemaker father which increased the likelihood of PG. On the contrary, cultural hobbies such as playing an instrument, painting, singing, and writing, and having a working mother were inversely associated with PG. Our results suggest that encouraging participation in creative activities along with supervised shopping and monitoring compulsive internet use and cannabis consumption may contribute some protection against adolescent PG.The online version contains supplementary material available at 10.1007/s11469-022-00950-7.

Background: Dementia, vascular disease, and cancer increase with age, enabling complex comorbid interactions. Understanding vascular and cancer contributions to dementia risk and neuropathology in oldest-old may improve risk modification and outcomes. Objective: Investigate the contributions of vascular factors and cancer to dementia and neuropathology. Methods: Longitudinal clinicopathologic study of prospectively followed Mayo Clinic participants dying≥95 years-old who underwent autopsy. Participants were stratified by dementia status and compared according to demographics, vascular risk factors, cancer, and neuropathology. Results: Participants (n = 161; 83% female; 99% non-Hispanic whites)≥95 years (95–106 years-old) with/without dementia did not differ based on demographics. APOE ɛ2 frequency was higher in no dementia (20/72 [28%]) versus dementia (11/88 [12%]; p = 0.03), but APOE ɛ4 frequency did not differ. Coronary artery disease was more frequent in no dementia (31/72 [43%]) versus dementia (23/89 [26%]; p = 0.03) associated with 56% lower dementia odds (odds ratio [OR] = 0.44 [confidence interval (CI) = 0.19–0.98]; p = 0.04) and fewer neuritic/diffuse plaques. Diabetes had an 8-fold increase in dementia odds (OR = 8.42 [CI = 1.39–163]; p = 0.02). Diabetes associated with higher cerebrovascular disease (Dickson score; p = 0.05). - Abstract
Colorectal cancer (CRC) has a high mortality rate and has recently involved human profilin II (Pfn2), an actin-binding protein promoter of its invasiveness and progression. This work evaluated the binding affinity of oleanolic acid (Naturally Occurring Saponin) over Pfn2 and its structural stability. QM and MM techniques were applied to perform geometrical optimization and calculation of the reactive sites from Oleanolic Acid, whereas molecular docking and MD simulations for protein-ligand interaction under physiological conditions. Oleanolic acid saponin showed a high binding affinity to the Pfn2 PLP-binding site. Analysis of the protein-ligand structure suggests saponin as a molecule with high potential for developing new drugs against Pfn2 in colorectal cancer cells.

Fundamento: la eliminación de la tuberculosis como problema de salud pública precisa el perfeccionamiento de las acciones preventivas sobre los casos de tuberculosis pulmonar con baciloscopia positiva.Objetivo: analizar la incidencia de tuberculosis pulmonar con baciloscopia positiva en el municipio Cienfuegos durante 1995-2013 y establecer pronóstico para el cuatrienio 2014- 2017.Métodos: estudio descriptivo retrospectivo que incluyó 133 casos de pacientes con baciloscopia positiva notificados. Se analizó: edad, sexo, área de salud, resultado de la baciloscopia, demora y lugar de diagnóstico. La predicción fue realizada con el modelo suavizado exponencial de Brown.Resultados: la baciloscopia positiva presentó una tendencia decreciente, representó el 56,1 % del total de casos de tuberculosis pulmonar. Predominaron los enfermos masculinos y el grupo de edades de 60 años y más. El 84,2 % de los casos presentó codificaciones altas en los exámenes microscópicos de esputo realizados. Menos del 52 % de los diagnósticos fueron realizados en la atención primaria de salud y solo el 50 % se enmarcó dentro del tiempo establecido por el programa como indicador operacional. Se predice que ocurrirá un discreto descenso de casos nuevos de pacientes con baciloscopia positiva en los próximos cuatro años. Conclusiones: eliminar la tuberculosis como problema sanitario es posible a mediano plazo en Cienfuegos; sin embargo el incumplimiento persistente de los indicadores que evalúan las acciones preventivas realizadas en los casos con baciloscopia positiva es y será un obstáculo serio para reducir la morbilidad y transmisibilidad de la enfermedad en la localidad.

The objective of this study was to determine the differential mapping of plasma biomarkers to postmortem neuropathology measures. We identified 64 participants in a population-based study with antemortem plasma markers (amyloid-β [Aβ] x-42, Aβx-40, neurofilament light [NfL], and total tau [T-tau]) who also had neuropathologic assessments of Alzheimer's and cerebrovascular pathology. We conducted weighted linear-regression models to evaluate relationships between plasma measures and neuropathology. Higher plasma NfL and Aβ42/40 ratio were associated with cerebrovascular neuropathologic scales (p0.05) but not with Braak stage, neuritic plaque score, or Thal phase. Plasma Aβ42/40 and NfL explained up to 18% of the variability in cerebrovascular neuropathologic scales. In participants predominantly with modest levels of Alzheimer's pathologic change, biomarkers of amyloid and neurodegeneration were associated with cerebrovascular neuropathology. NfL is a non-specific marker of brain injury, therefore its association with cerebrovascular neuropathology was expected. The association between elevated Aβ42/40 and cerebrovascular disease pathology needs further investigation but could be due to the use of less specific amyloid-β assays (x-40, x-42).

Resume Objectif Analyser et decrire l’incidence des effets indesirables des agents biologiques chez des enfants atteints de maladies inflammatoires, dans une cohorte suivie en situation reelle. Methodes Cette etude observationnelle retrospective multicentrique internationale a evalue, par la methode de Kaplan–Meier, la survenue d’effets indesirables chez des enfants recevant un traitement biologique et suivis dans le reseau JIRcohorte de rhumatologie pediatrique (JIR, Juvenile Inflammatory Rheumatism). Un modele de Cox a ete elabore pour identifier des facteurs predictifs independants d’effets indesirables. Resultats Au total, 813 patients totalisant 3439 patients-annee (p-a) d’agents biologiques ont ete inclus. Le principal diagnostic etait l’arthrite juvenile idiopathique (AJI) (84 %). Au total, 222 patients (27,3 %) ont presente 419 effets indesirables, soit un taux d’incidence de 12,2 pour 100 p-a, IC 95 % [11,0 ; 13,4]. L’incidence globale des effets indesirables graves etait de 3,9 pour 100 p-a, IC 95 % [3,2 ; 4,6]. Le tocilizumab et l’infliximab ont ete associes de maniere significative a des effets indesirables et le canakinumab a des effets indesirables graves. L’analyse univariee et multivariee des effets indesirables et des effets indesirables graves a montre qu’ils etaient plus frequents chez les patients prenant des agents biologiques en association avec des immunosuppresseurs (autres que le methotrexate). Conclusion Cette etude suggere que la securite des agents biologiques est globalement acceptable chez les enfants atteints de maladies rhumatismales inflammatoires traites par des agents biologiques. Cependant, la prescription d’immunosuppresseurs associee cree un risque notable d’effets indesirables.

Ocular trauma is one of the leading causes of decreased visual acuity and monocular blindness in the pediatric population. Since its occurrence is potentially preventable, it is essential to understand its demographic characteristics and risk factors to alert and implement prevention and health promotion programs.We conducted a retrospective study in which we reviewed 187 clinical records of patients ≤ 15 years old with the diagnosis of severe ocular trauma admitted in 2017 to the Hospital Civil de Guadalajara. We analyzed demographic variables, circumstances of the event, type of treatment, and the evolution of visual acuity and complications.In total, 187 patients and 188 eyes were included; the average age was 6.99 ± 4.1 years. Children of 2 to 5 years of age (41%) were the most affected, and a higher occurrence was observed in males (73%). Open ocular trauma was the most common type of injury (72.7%) caused by sharp objects (45.4%). Most injuries occurred at home (78.1%), without adult supervision (48.1%), and during leisure time (74.9%). Most events were accidental (80.4%), although 12.3% were direct aggression. Surgery was required in 179 patients, in whom corneal injury repair (43.2%) was the most commonly used procedure. Final visual acuity was quantified in 132 eyes, and visual acuity20/200 was found in 18.1%. Loss of 4.2% of the eyeballs was recorded.The highest proportion of cases was identified in male patients ≤5 years old, unsupervised by an adult. Many eye injuries are preventable, so it is necessary to implement socio-educational programs in alliance with pediatric organizations that alert the severity of the problem and promote safe environments.El trauma ocular es una de las principales causas de disminución de la agudeza visual y de ceguera monocular en la población pediátrica. Su ocurrencia es potencialmente prevenible, por lo que es importante entender sus características demográficas y riesgos para alertar e implementar programas enfocados hacia la prevención y la promoción de la salud.Estudio retrospectivo en el que se analizaron 187 expedientes de pacientes ≤ 15 años con diagnóstico de trauma ocular grave ingresados en 2017 al Hospital Civil de Guadalajara. Se analizaron variables demográficas, circunstancias del evento y tipo de tratamiento, así como la evolución de la agudeza visual y las complicaciones.Se incluyeron 187 pacientes y 188 ojos; el promedio de edad en el momento del trauma fue de 6.99 ± 4.1 años. Los niños de 2-5 años (41%) fueron los más afectados y se observó una mayor ocurrencia en el sexo masculino (73%). La agudeza visual final se cuantificó en 132 ojos y se encontró20/200 en el 18.1%. Se registró la pérdida del 4.2% de los globos oculares.La proporción más alta de casos se identificó en pacientes de sexo masculino ≤ 5 años, que se encontraban sin supervisión. Una gran proporción de las lesiones oculares son prevenibles, por lo que es necesario implementar programas educativos sociales en coordinación con organizaciones pediátricas que alerten de la gravedad del problema y promuevan entornos seguros.

Se eval��a la percepci��n del riesgo de la poblaci��n del municipio cabecera de la provincia. Se aplica un cuestionario en forma de entrevista estructurada con 23 preguntas, de ellas 10 abiertas a una muestra aleatoria y estratificada de 153 personas. La percepci��n del riesgo se valora mediante las escalas de vulnerabilidad total y de vulnerabilidad socioecon��mica. Existe baja percepci��n del riesgo en un escenario de peligro y vulnerabilidad altos. Los rasgos positivos de la percepci��n del riesgo son: reconocimiento de preocupaci��n del gobierno y confianza en su gesti��n y los significados correctos acerca de los sismos y los negativos: no reconocimiento del peligro, desconocimiento de pr��cticas negativas antes, durante y despu��s del sismo y medidas de protecci��n y el bajo reconocimiento de la probabilidad, peligrosidad y afectaciones de los sismos, as�� como una capacidad propositiva casi nula. La percepci��n media a baja de la poblaci��n est�� dada por la carencia de experiencias vividas que le permitan dar respuesta adecuada a un evento s��smico, con diferencias en los Consejos Populares, por lo que se requiere desarrollar la educaci��n ambiental comunitaria para enfrentar estos peligros naturales en el marco la gesti��n comunitaria para la reducci��n del riesgo desastres

36. Leucaena feeding systems in Paraguay

Keynote paper presented at the International Leucaena Conference, 1‒3 November 2018, Brisbane, Queensland, Australia.Leucaena leucocephala became naturalized in Paraguay long ago. However, due to cases of toxicity in horses and cattle, now identified as mimosine toxicity, leucaena was considered a weed until the beginning of this millennium. At this time the mimosine toxicity problem was overcome by the introduction of ruminal fluid from Australia containing the mimosine-degrading and -detoxifying bacterium Synergistes jonesii. As long as an internationally funded technical assistance project was operating (offering technical advice, provision of seed, seed scarification service and transmission of ruminal fluid containing Synergistes), the area sown to leucaena (either in twin rows into grass pastures or as fodder banks) increased rapidly in Paraguay, particularly in the Chaco area. However, the powdery fluvisols of the drier parts of the Chaco were not well suited to growth of leucaena, as persistence was restricted due to the impact of rodents, termites and also leaf-cutting ants, which prosper particularly well in this part of the Chaco. In more humid areas with usually heavier soils, currently leucaena represents an integral part of the feeding systems in hundreds of Paraguayan farms (large-scale as well as smallholders), mainly for steer fattening and dairy cow supplementation. After taking into account the above-mentioned setbacks, the total area of leucaena is currently estimated at about 10,000 ha.

Estimates of demographic parameters are scarce for Neotropical amphibians, a concerning fact because this region has the highest proportion of threatened amphibians in the world. We conducted a 3-year study where we applied a robust capture–mark–recapture design to assess the importance of breeding and non-breeding activity patterns over the survival rates, detection probabilities, and abundances of the Montevideo Redbelly Toad (Melanophryniscus montevidensis (Philippi, 1902)), a threatened anuran from Uruguay. The best models grouped seasons into hot and cold periods cyclically, were state-dependent in transition probabilities, and were time-dependent in detection probabilities for adults, but had constant detection probabilities for juveniles. Averaged estimates suggest a high survivorship rate during cold seasons (above 80%), but lower probabilities (below 60%) during hot seasons, especially for males. Analogously, the non-breeding activity had a seasonal pattern, with higher activity during spring and higher sheltering rates during autumn. These activity rates negatively influenced the averaged survivorship rates of adult males and females. Long-term (matrix) projections of seasonal survivorships, along with assessments of the causes of these patterns, should be carried out to determine extinction probabilities and possible threats for the conservation of the genus Melanophryniscus.

The condensed nearest neighbor (CNN) classifier is one of the techniques used and known to perform recognition tasks. It has also proven to be one of the most interesting algorithms in the field of data mining despite its simplicity. However, CNN suffers from several drawbacks, such as high storage requirements and low noise tolerance. One of the characteristics of CNN is that it focuses on the selection of prototypes, which consists of reducing the set of training data. One of the goals of CNN seeks to achieve the reduction of information in such a way that the reduced information can represent large amounts of data to exercise decision-making on them. This paper mentions some of the most recent contributions to CNN-based unsupervised algorithms in a review that builds on the mathematical principles of condensed methods.

Introducci?n: Los Odont?logos tienen una serie de trastornos m?sculo esquel?ticos (TME) asociados a la ergonom?a, este estudio tiene como objetivo analizar la evidencia disponible acerca de la frecuencia y factores asociados a TME de mu?eca y mano. Materiales y M?todos: La b?squeda bibliogr?fica se realiz? en PUBMED, SCIELO, LILACS; mediante 5 ?rboles de b?squeda con palabras clave indexadas en MESH y DECS. Los art?culos seleccionados fueron revisados por la investigadora y posteriormente se incorporaron los que cumplieron criterios de inclusi?n. Resultados: Se seleccionaron 26 estudios, el promedio hallado entre los estos estudios fue 29% con IC 95% (28.5%-30.3%). El valor m?s bajo de TME fue 1%, el m?s alto fue 97%. Conclusiones: La frecuencia de TMEdMM en odont?logos es alta. Los factores de riesgo como, sexo femenino, trabajar de pie, movimientos repetitivos, m?s de 40 a?os parecen estar comprobados. Otros como; carga laboral, actividad f?sica, tabaquismo, deben estudiarse con mejores dise?os o muestras m?s grandes. Mag?ster en Salud Ocupacional y Seguridad en el Trabajo

To review the clinical characteristics and complications associated with cholelithiasis in pediatric patients by identifying risk factors and assessing surgical management and results.Retrospective study in patients under 18 years of age undergoing cholelithiasis surgery. The following data were analyzed: age, sex, body mass index (BMI), associated comorbidities, clinical presentation, symptom duration, surgical treatment, pathological report, postoperative complications, and hospital stay.135 cholelithiasis patients underwent surgery from 2013 to 2018, with an increasing trend in the annual number of cholecystectomies. Most patients were adolescents (86.7%) and female (72.6%), and they had cholesterol gallstones (86.6%) and85 BMI (33%). Mean symptom duration was 85 days (SD: 148). Symptomatic cholelithiasis was present in 131 cases (97%). CL associated complications were recorded in 64.4% of patients, with cholecystitis, pancreatitis, and choledocholithiasis being the most frequent ones. All patients underwent laparoscopic cholecystectomy; 4 (2.9%) required conversion to open surgery, and 6 (4.4%) had postoperative complications. Mean hospital stay and postoperative follow-up were 5.7 days (SD: 4) and 2.3 months (SD: 1.9), respectively.Non-hemolytic cholelithiasis in the pediatric population is more frequent in female overweight and obese adolescents. Symptomatic cholelithiasis is associated with higher risk of complications such as cholecystitis, pancreatitis, or choledocholithiasis, which supports early surgical management.Revisión de las características clínicas y complicaciones asociadas de los pacientes pediátricos con colelitiasis, mediante la identificación de factores de riesgo, la evaluación del manejo quirúrgico y sus resultados.Estudio retrospectivo en pacientes menores de 18 años, intervenidos quirúrgicamente por colelitiasis. Se analizaron datos como edad, género, índice de masa corporal (IMC), comorbilidades asociadas, presentación clínica, tiempo de los síntomas, tratamiento quirúrgico, reporte de patología, complicaciones posoperatorias y estancia hospitalaria.Se intervinieron quirúrgicamente 135 pacientes con colelitiasis entre los años 2013 a 2018, con una tendencia ascendente en el número de colecistectomías realizadas anualmente. La mayoría fueron pacientes adolescentes (86,7%), con cálculos de colesterol (86,6%), de género femenino (72,6%) y con IMC 85 (33%). El tiempo de los síntomas en promedio fue de 85 días (DE 148). Colelitiasis sintomática se presentó en 131 casos (97%). Complicaciones asociadas a CL se registraron en 64,4% pacientes, siendo las más frecuentes colecistitis, pancreatitis y coledocolitiasis. En todos los pacientes se realizó colecistectomía laparoscópica, 4 (2,9%) requirieron conversión a cirugía abierta y 6 (4,4%) presentaron complicaciones postoperatorias. La estancia hospitalaria y el seguimiento posoperatorio fueron en promedio 5,7 días (DE 4) y 2,3 meses (DE 1,9), respectivamentea.La colelitiasis no hemolítica en la población pediátrica, es más frecuente en pacientes adolescentes de género femenino con sobrepeso u obesidad. La colelitiasis sintomática está asociada con mayor riesgo de complicaciones como colecistitis, pancreatitis o coledocolitiasis, en consecuencia, se justifica una intervención quirúrgica temprana.

The clinical diagnosis of von Willebrand disease (VWD), particularly type 1, can be complex because several genetic and environmental factors affect von Willebrand factor (VWF) plasma levels. An estimated 60% of the phenotypic variation is attributable to hereditary factors, with the ABO blood group locus being the most influential. However, recent studies provide strong evidence that nonsynonymous single nucleotide variants (SNVs) contribute to VWF and factor VIII phenotypic variability in healthy individuals. This study aims to investigate the role of common VWF SNVs on VWD phenotype by analyzing data from 219 unrelated patients included in the “Molecular and Clinical Profile of von Willebrand Disease in Spain project.” To that end, generalized linear mixed-effects regression models were fitted, and additive and epistatic analyses, and haplotype studies were performed, considering five VWD-related measures (bleeding score, VWF:Ag, VWF:RCo, factor VIII:C, and VWF:CB). According to these analyses, homozygotes: for p.Thr789Ala(C) would be expected to show 39% higher VWF:Ag levels; p.Thr1381Ala(C), 27% lower VWF:Ag levels; and p.Gln852Arg(C), 52% lower VWF:RCo levels. Homozygotes for both p.Thr789Ala(C) and p.Gln852Arg(T) were predicted to show 185% higher VWF:CB activity, and carriers of two copies of the p.Thr1381Ala(T)/p.Gln852Arg(T) haplotype would present a 100% increase in VWF:RCo activity. These results indicate a substantial effect of common VWF variation on VWD phenotype. Although additional studies are needed to determine the true magnitude of the effects of SNVs on VWF, these findings provide new evidence regarding the contribution of common variants to VWD, which should be taken into account to enhance the accuracy of the diagnosis and classification of this condition. Os autores descrevem o caso de um paciente do sexo masculino, de 66 anos de idade, com lesão neoplásica gástrica Borrmann IV, desde 2 cm do cárdia até a região pré-pilórica, cujo exame anatomopatológico revelou adenocarcinoma tubulopapilífero pouco diferenciado. O mesmo paciente era portador de glioblastoma multiforme do lobo temporal esquerdo. Neoplasias malignas primárias múltiplas que incluam neoplasias cerebrais primárias são raras, havendo apenas 17 casos descritos na literatura. Os autores discutem a etiopatogenia dessa infreqüente patologia.

Background: The paediatric SLE (pSLE) are estimated 10 to 17% of SLE (1). Studies comparing pSLE with adult-onset SLE reported high prevalence of pre-existing organ damage at diagnosis due to the diagnosis delay, more severe disease clinical course with high disease activity resulting in worse prognosis in pSLE (1,2). Recently, whole exome sequencing and next-gene sequencing have facilitated the identification of rare monogenic variants associated with SLE and lupus-like phenotypes with high penetrance (3,4) Objectives: To reporting the whole spectrum of monogenic pSLE according to pathway abnormalities, using a meta-analysis (MA) approach. Methods: A systematic review of studies focused on monogenic pSLE were analysed. To be included in the systematic review, studies had to meet simultaneously all the following criteria: (i) reported at least 1 case of SLE (ii) started before the age of 16 (iii) with clearly description of the diagnosis criteria and (iv) provided genetic data of the case. We pooled the genes into three main groups of signalling pathways: (i) genes that affect adaptive immunity pathways including the lymphocyte apoptosis (FAS, KRAS, PTPN11, SHOC2) and the B and T cell development checkpoints (PRKCD, RAG2, IKZF1,DOCK8, WASP), (ii) genes containing type I interferon (IFN) pathways (ACP5, TREX1, SAMHD1, RNASEH2A, RNASEH2B, RNASEH2C, IFIH1, DNASE1L3, DNASE1, TMEM173, Tetrasomy 9), and (iii) metabolism pathways including both amino acid metabolism (PEPD, SLC7A7, MAN2B1) and oxidative metabolism (NCF2). Clinical and laboratory data were analysed according to the SLICC classification. Results: We included 42 articles encompassing 88 patients, plus 2 additional unpublished cases of one author (AB). The MA process were carried out according to the PRISMA statement. According to the SLICC classification, the blood disorders were significantly higher in the group of the metabolism pathway as well as the positivity of direct Coombs test. The positivity of antibodies anti-Sm was also a distinguishing element for this group, while the positivity of ALP antibodies was the lonely manifestation to distinguish the adaptive immunity pathway from the others. Fever, recurrent infections, hepatosplenomegaly, and pulmonary diseases were significantly predominant in the metabolism pathways. In the group of the IFN pathway, the most distinctive criteria were the neurological manifestations and bone changes. There were no statistically significant differences in all groups concerning all other clinical or immunological criteria. Conclusion: This is the first quantitative analysis of multiples cases reports, comparing the whole spectrum of monogenic pSLE according to three different pathways. Our results suggest that abnormalities in signaling pathway delimitate different SLE juvenile subset. The clinical and biological differing between the three signaling pathway were not specific, but our results may help to detect phenotypes requiring targeted genetic investigations. We present the detection of a Warm Neptune orbiting the M-dwarf Gl378, using radial velocity measurements obtained with the SOPHIE spectrograph at the Observatoire de Haute-Provence. The star was observed in the context of the SOPHIE exoplanets consortium's subprogramme dedicated to finding planets around M-dwarfs. Gl378 is an M1 star, of solar metallicity, at a distance of 14.96 pc. The single planet detected, Gl378 b, has a minimum mass of 13.02 $\rm M_{Earth}$ and an orbital period of 3.82 days, which place it at the lower boundary of the Hot Neptune desert. As one of only a few such planets around M-dwarfs, Gl378 b provides important clues to the evolutionary history of these close-in planets. In particular, the eccentricity of 0.1 may point to a high-eccentricity migration. A largely orange species with forcipate cercus armed terminally with a decumbent tooth and with a small unilobate paraproct. Description of holotype. Head (Fig. 1). Labrum, genae, clypeus and antefrons orange, epicranium black with small orange spot anterolateral to lateral ocellus, large orange postocular spots confluent laterally with margin of compound eye; scape orange, pedicel black; rear of head black above, pale orange below with an ill-defined isolated black spot above gena. Thorax. Prothorax orange except for anterior margin of anterior lobe black and dorsal portion of middle and hind lobes black with narrow middorsal stripe of orange on middle and posterior lobes. Mesothorax (Fig. 1) with mesepisternum orange, paler orange laterally, black parallel middorsal stripe as wide as pale antehumeral stripe; black humeral stripe divided at upper 0.60, the anterior branch a narrow line along mesopleural suture with mesopleural fossa black, the posterior branch gradually narrowing and ending before subalar carina; metapleural stripe black, slightly narrower than humeral stripe, metapleural carina edged with black; anterior half of mesepisternum and most of metepimeron black; venter of thorax pale orange. Wings. With a long stiff flagellum, and a shorter medial trilobed process, the two lateral lobes of which are triangular in ventral (ectal) view (as in Fig. 9), and the decumbent median lobe is quadrate apically (as in Fig. 8); the latter process with an evenly curved arcuate ridge; more basally at base of flagellum a pair of decumbent rounded lobes. Caudal appendages. Torifer orange, torus large, planar, roundly triangular (as in Fig. 4), occupying entire ventral margin of torifer and not overlapping bilobed epiproct; epiproct black, small, slightly bilobed (as in Fig. 6); cercus (as in Figs. 4���6) slightly longer than paraproct, about three times longer than wide, slightly swollen basally with medial basal margin slightly convex and slightly narrowing distally with a strongly acuminate mediolaterally directed tooth, dorsal surface of cercus with long cluster of thick hairs; mediodorsal surface of cercus slightly depressed along medial margin; paraproct unilobate, almost quadrate in lateral view with a narrow tuberculate tip (as in Fig. 5). Female. unknown. Measurements. Hw 19.0, abdomen 26.5, total length 33.0. Variation in paratype. Similar to holotype but with black humeral stripe divided at upper 0.70 (e. g. Fig. 10), wings slightly more flavescent and with small middorsal orange stripe confined to hind lobe of prothorax and S9 entirely black. Pterostigma surmounting one cell in all wings; postnodals: Fw 12���12, Hw 11���10; postquadrangular cells Fw 3���3, Hw 3���3; RP 2 at Fw 6���5, Hw 4���4. Dimensions. Hw 18.0, abdomen 24.5, total length 31.0. Diagnosis. This species is unique by overall orange coloration, mostly black S9 and 10, and genital ligula and caudal appendage morphology. The only other South American species of Argia with a largely orange body is A. infumata Selys but it differs greatly in genital and appendage morphology (Garrison & von Ellenrieder 2015). Genital ligula and appendage morphology of A. mauffrayi resemble those of A. cuspidata Garrison & von Ellenrieder, 2018 and A. acridens, although appendage morphology is most similar to that of A. acridens (Fig. 11���17). However, they differ by: 1) dorsal surface of cercus covered with thick hairs in A. mauffrayi (Figs. 4, 5) compared to being bare in A. acridens (Figs. 13, 14), and 2) the cercus in dorsal view being slightly forcipate, with basomedial margin only slightly convex in A. mauffrayi (Fig. 6) versus greatly expanded into a basomesal round lobe as in A. acridens (Fig. 15). Additionally, the torus in A. mauffrayi (Fig. 4) is roughly triangular but is smaller and more oval in A. acridens (Fig. 13). The apical process of the paraproct of A. mauffrayi in lateral view is small and minutely bilobed (Fig. 5) whereas the apical process in A. acridens is broader and roughly quadrate (Fig. 14). The basal trilobate process and paired lateral lobes in the genital ligula in A. mauffrayi (Figs. 7���9) are lacking in A. acridens (Figs. 16, 17). Remarks: Argia mauffrayi is unusual in its overall orange coloration and lack of any pale color on the terminal abdominal segments. Specimens are noticeable in the field as Sanchez, one of the collectors of the paratype, noted its orange color when reporting its capture to Garrison. Mauffray and Tennessen noted the following: "[The holotype] was collected at approx 9:30am along forested trail on a very steep hill with a few small sun-lit spots. It was collected as it flew upward and perched on a leaf in one of these sunny spots along with Argia variegata [F��rster], Argia oculata [Hagen in Selys] and Polythore gigantea [(Selys)]. There was no seepage nor stream at that location; however, there was a small seepage stream about 50 meters from the location. Ken and I continued to look for other specimens and even returned to the site about an hour later, but the weather had turned cloudy" and "..when I first saw the specimen flying���it appeared mostly orange���[and thought] it's a small Heteragrion !" Although the female of A. mauffrayi is unknown, we suspect that the mesostigmal plates will likely be poorly developed similar to the female sex of A. acridens, A. cuspidata (Garrison & von Ellenrieder 2018, Figs. 69 & 72 respectively) and A. mishuyaca Fraser, 1946 (Garrison & von Ellenrieder 2015, Fig. 7c). The curvilinear cercus in the males of these three species are longer than the paraproct and are armed terminally with a apical decumbent tooth���a condition similar to that illustrated here for A. mauffrayi.
