Abstract

The problems arising in organizations during the process of investment activities have been considered. The results of financing and implementation of projects and programs have been analysed and interpreted consistently. The issues of expediency of using the profitability indicators of the capital or assets of the company, which requires for the project implementation, have been touched upon. Approaches to the possibility of using the project financing mechanism, have been substantiated. Attention has been paid to the positive and negative aspects of the application of project financing for the company investment activities. It has been noted that project financing is usually used in the implementation of large investment projects, which combine the economic interests of the state and private investors. Particular attention has been paid to the issues of project and program management within the digital environment in order to provide a unified information base. The analysis of possible risks when using the project financing mechanism has been provided and the approaches to influencing the emerging risks have been defined. The result of the study was the formation of factors affecting the effectiveness of the project financing mechanism in the implementation of investment projects and programs.

Abstract

The article discusses the main problems associated with the development and implementation of investment projects at enterprises of the real sector of the economy for sustainable development. The current situation in the investment and construction sector of the economy has been evaluated and the ways of solving the main problems of investment activity in the construction industry are outlined. The issues of formation mechanism of the investment attractiveness of industries and individual organizations, production and investment activities have been consistently considered. Traditional indicators and criteria of investment efficiency have been analysed. The expediency of using indicators of the return on capital or assets of the organization necessary for the implementation of the project has been substantiated. It has been noted that in the process of developing an organization’s sustainable development strategy, it is necessary to take into account its digital transformation, within the framework of the possibility of functioning in a single digital environment in order to increase competitiveness. The applying a comprehensive assessment of the effectiveness of attracting investments for the implementation of investment programs based on the selected criteria, taking into account their functioning in the digital environment, – has been recommended.

Abstract

The article сonsiders topical issues of managing the economic stability of organizations in a crisis. The authors substantiate the need to study regional trends as an integral part of the anticrisis program at the regional level. Early diagnosis of crisis phenomena in the economy proves that a sufficiently high level of socio-economic development does not yet ensure complete economic security and the ability to withstand the threats posed by the unbalanced structure of the economy. Based on this, the problems in the field of ensuring the stability of organizations in the conditions of crises have been analysed and generalized. The problems faced by enterprises and organizations of the investment and construction sector of the economy have been considered. In methodological terms, the authors propose to use a system of indicative analysis and regulation of production and economic activities of enterprises of the real sector of the economy, as an organizational tool for resolving crisis situations in the economy. The study justifies the necessity of the introduction of digital technologies, on the basis of which it is possible to quickly implement the tasks set and achieve the expected results. The authors emphasize that we need our own anticrisis programs that to help maintain the sustainable functioning of organizations in the conditions of crisis phenomena.

Abstract

The paper investigates modern trends in the development of innovative systems in Russia. These trends are caused, on the one hand, by the lack of a clearly formulated concept of an innovation system, and, consequently, by the inability to determine the ways of its development, and by the active use of undeservedly forgotten program-target planning methods, on the other hand. A detailed analysis of various positions and statistical materials allowed us to prove that, contrary to the generally accepted opinion, the Russian model of public administration and financing of innovative activities has signs of cluster and state-corporate types. The article considers the existing approaches to the management of innovation and investment activities of organizations, which is aimed directly at reproduction processes, in particular, the reproduction of fixed capital. The authors substantiate the necessity of creating territorially grouped innovation systems, which is explained by the dependence of industries and sectors of the economy on each other on the technological principle and the need to create technological links between them to realize their potential advantages within the framework of the implementation and creation of innovative products.

Abstract

The article considers the issues of change management taking into account the Russian and American experience. Change management has been reviewed as an integral part of the management theory. It has been proposed to consider the process of change management as primarily the management of phenomena that accompany the process of change.The concept of “change” has been reflected as a process of transformation and as the transformation of an object into new legal forms through reorganization. The presented experience of managing American companies shows, first of all, the situation in which economic growth is carried out and all transformations are aimed at integration processes.The analysis of modern experience of change management has been made in the context of crisis situations that require flexibility in managing all processes in the organization. The article presents a division of changes into groups, allowing you to identify areas that require timely management influence when developing an organization’s development program. The analysis examines the most popular change management models and the actions of companies to manage changes during the coronavirus pandemic.In conclusion, it has been concluded that for modern companies, change management requires not only constant analysis of internal organizational processes, but also the application of specific knowledge to solve problematic management tasks.

The issues of cost management from the point of view of substantiating the reduction in the cost of investment products in order to create reasoned conditions for the interest of investors in long-term investments have been considered. The main emphasis on the real economic sector has been made, since it is in this sector that budget revenues at all levels are generated and overall economic growth is ensured. The existing mechanism of cost management in the implementation of investment programs has been assessed. Attention to the main elements of the cost management mechanism in the investment and construction sector of the economy has been paid. Factors that determine the level of production expenses taking into account the specifics of construction products have been analysed. Abstract

The main unresolved issues of choosing the most effective investment projects aimed at introducing innovative technologies have been considered. The traditional indicators of investment efficiency have been analysed. The expediency of using the indicator of the total reduced costs necessary for the implementation of the project has been substantiated. Attention has been paid to the possibilities of public-private partnership in innovation projects. The issues of shares distribution between participants in innovation and investment projects and the use of such objects have been reviewed. The current situation in the investment and construction sector of the economy has been assessed and ways to solve the main problems of investment activity in the construction industry have been outlined. The expediency of creating a mechanism for economic justification of the investment project implementation process, including the creation of reserve funds, has been substantiated. Recommendations for using the indicator of the of the investor’s total stated costs have been given.

Abstract

All business entities, regardless of their form of ownership, must obey established rules, when paying for the right to use licenses for inventions, renting films, publishing books, products, patents, innovations. The calculation of the amount of payment is carried out in accordance with the bilateral agreement. Calculating royalties, based on profits earned by an enterprise, has certain disadvantages. Many of the most diverse factors affect the volume of profit. The author’s approach and the method of determining the value of royalties for the development of mineral resources have been explained in the article. Their basis consists of: determining the value of economic rent as a result of the extraction and sale of mineral resources; ensuring, that mining companies receive income on their production capital at a level not lower than the average for the country’s economy as a whole.

The article consideres the existing practice of the comparative assessment of effectiveness of the economies of constituent entities of the Russian Federation, reveals its shortcomings, and the factors causing them, factors causing them, including the shortcomings of the system and indicators of state statistical reporting and methods for their determination, as well as the consequences of these shortcomings in the system of internal economic and social-labor relations. The authors ’proposals are based on the following: adopting, as a generalized indicator of the economic efficiency of the functioning of the economies of administrative-territorial entities of the country, the indicator of gross domestic income generated in their territories, regardless of which of them are registered; changes in the basic provisions for determining in statistical accounting the volumes of the gross domestic product and gross income, excluding from their volumes the cost of services, that do not represent in their content the newly created value.

Abstract

BACKGROUND. There is enough evidence of the negative impact of excess weight on the formation and progression of res piratory pathology. Given the continuing SARS-CoV-2 pandemic, it is relevant to determine the relationship between body mass index (BMI) and the clinical features of the novel coronavirus infection (NCI).AIM. To study the effect of BMI on the course of the acute SARS-COV-2 infection and the post-covid period.MATERIALS AND METHODS. AKTIV and AKTIV 2 are multicenter non-interventional real-world registers. The АКТИВ registry (n=6396) includes non-overlapping outpatient and inpatient arms with 6 visits in each. The АКТИВ 2 registry (n=2968) collected the data of hospitalized patients and included 3 visits. All subjects were divided into 3 groups: not overweight (n=2139), overweight (n=2931) and obese (n=2666).RESULTS. A higher BMI was significantly associated with a more severe course of the infection in the form of acute kidney injury (p=0.018), cytokine storm (pCONCLUSION. Overweight and/or obesity is a significant risk factor for severe course of the new coronavirus infection and the associated cardiovascular and kidney damage Overweight people and patients with the 1st and 2nd degree obesity tend to have a high risk of death of SARS-CoV-2 infection in both acute and post-covid periods. On top of that, in case of morbid obesity patients this tendency is statistically significant. Normalization of body weight is a strategic objective of modern medicine and can contribute to prevention of respiratory conditions, severe course and complications of the new coronavirus infection.

12. Mineral composition and geochemical features of organomineral deposits of the tract of Lake Peschanoe in the southern part of the Amur-Zeya interfluve

Abstract

Research subject. The article presents the results of a comprehensive study of a column of organomineral deposits (750 cm) in the tract of Lake Peschanoe (Amur Region, between the Amur and Zeya rivers).Material and methods. In the studied section, the composition of organomineral deposits were examined based on radiocarbon dating and palynological analysis.Results. According to radiocarbon dating, the sedimentation began over 4 thousand calendar years. Specific features of the accumulation and distribution of rock-forming oxides and microelements along the organomineral sedimentary profile were established. The mineral composition is dominated by quartz, feldspars, amphiboles, pyrite, garnet and biotite. The highest values of the scattering coefficient in the thickness of the deposits are typical of Hg (8.0), Tl (6.7), La (5.3), below – Zr, Rb and Li (4.7, 4.4 and 4.1, respectively).Conclusions. The study of the geochemical characteristics of organomineral deposits allowed us to identify the industrial period of sedimentation, which was characterized by an increase in the concentrations of Hg, Cd, Pb and Sn. The background contents of elements in the organomineral deposits of the Lake Peschanoe tract can be used as a regional background for deposits in the transboundary territory of the Upper and Middle Amur and its tributaries. The calculated geochemical coefficients indicate the freshwater content of the basin and the alternation of stagnant and oxidizing environments during sedimentation. The increased Ca/Sr values are associated with a change in the climatic regime towards cooling, which occurred during the last 4 thousand years in this area: 4300–4100, 2400–2100, 500–230 years ago.

Abstract

Aim. To analyze newly diagnosed diseases and features of the post-COVID course in patients after a coronavirus disease 2019 (COVID-19) within 12-month follow-up.Material and methods. A total of 9364 consecutively hospitalized patients were included in the ACTIV registry. Enrollment of patients began on June 29, 2020, and was completed on March 30, 2021, corresponding to the first and second waves of the pandemic. Demographic, clinical, and laboratory data, computed tomography (CT) results, information about inhospital clinical course and complications of COVID-19 during hospitalization were extracted from electronic health records using a standardized data collection form. The design included follow-up telephone interviews with a standard questionnaire at 3, 6, and 12 months to examine the course of post-COVID period.Results. According to the ACTIV registry, 18,1% of patients after COVID-19 had newly diagnosed diseases (NDDs) over the next 12 months. Hypertension (HTN), type 2 diabetes and coronary artery disease (CAD) prevailed in the NDD structure. Comparison of the age-standardized incidence of NDDs (HTN, CAD, diabetes) in the post-COVID period in the ACTIV registry with NDD incidence in 2019 according to Rosstat and the expected incidence of NDDs according to the EPOHA study revealed that HTN, diabetes, CAD in patients after COVID-19 were registered more often as follows: HTN by 7,0 and 4,4 times, diabetes by 7,3 and 8,8 times, CAD by 2,3 and 2,9 times, respectively. NDDs most often developed in patients aged 47 to 70 years. Comparison of the actual and expected number of cases of newly diagnosed HTN, CAD and diabetes depending on age showed that the actual number of cases in the population of patients in the ACTIV register is significantly higher than expected for patients aged 45-69 years and for patients with hypertension or diabetes and aged Conclusion. Available evidence suggests that a strategy for managing COVID-19 survivors should include mandatory screening for early detection of cardiovascular disease and diabetes, which will be key to reducing the risk of further COVID-19 consequences.

Abstract

The paper presents a case of pregnant woman with polycystic ovary syndrome, which occurred after hemorrhagic stroke and detected dissecting aneurysm of the ascending aorta at the sinus level. Removal of non-viable fetus was performed, coronaroangiography, resection of aneurysm of ascending aorta and replacement of valve-containing conduit was successfully implemented.

Abstract

Aim To study the effect of regular drug therapy for cardiovascular and other diseases preceding the COVID-19 infection on severity and outcome of COVID-19 based on data of the ACTIVE (Analysis of dynamics of Comorbidities in paTIents who surVived SARS-CoV-2 infEction) registry.Material and methods The ACTIVE registry was created at the initiative of the Eurasian Association of Therapists. The registry includes 5 808 male and female patients diagnosed with COVID-19 treated in a hospital or at home with a due protection of patients’ privacy (data of nasal and throat smears; antibody titer; typical CT imaging features). The register territory included 7 countries: the Russian Federation, the Republic of Armenia, the Republic of Belarus, the Republic of Kazakhstan, the Kyrgyz Republic, the Republic of Moldova, and the Republic of Uzbekistan. The registry design: a closed, multicenter registry with two nonoverlapping arms (outpatient arm and in-patient arm). The registry scheduled 6 visits, 3 in-person visits during the acute period and 3 virtual visits (telephone calls) at 3, 6, and 12 mos. Patient enrollment started on June 29, 2020 and was completed on October 29, 2020. The registry completion is scheduled for October 29, 2022. The registry ID: ClinicalTrials.gov: NCT04492384. In this fragment of the study of registry data, the work group analyzed the effect of therapy for comorbidities at baseline on severity and outcomes of the novel coronavirus infection. The study population included only the patients who took their medicines on a regular basis while the comparison population consisted of noncompliant patients (irregular drug intake or not taking drugs at all despite indications for the treatment).ResultsThe analysis of the ACTIVE registry database included 5808 patients. The vast majority of patients with COVID-19 had comorbidities with prevalence of cardiovascular diseases. Medicines used for the treatment of COVID-19 comorbidities influenced the course of the infectious disease in different ways. A lower risk of fatal outcome was associated with the statin treatment in patients with ischemic heart disease (IHD); with angiotensin-converting enzyme inhibitors (ACEI)/angiotensin receptor antagonists and with beta-blockers in patients with IHD, arterial hypertension, chronic heart failure (CHF), and atrial fibrillation; with oral anticoagulants (OAC), primarily direct OAC, clopidogrel/prasugrel/ticagrelor in patients with IHD; with oral antihyperglycemic therapy in patients with type 2 diabetes mellitus (DM); and with long-acting insulins in patients with type 1 DM. A higher risk of fatal outcome was associated with the spironolactone treatment in patients with CHF and with inhaled corticosteroids (iCS) in patients with chronic obstructive pulmonary disease (COPD).Conclusion In the epoch of COVID-19 pandemic, a lower risk of severe course of the coronavirus infection was observed for patients with chronic noninfectious comorbidities highly compliant with the base treatment of the comorbidity.

Abstract

The organizer of the registers “Dynamics analysis of comorbidities in SARSCoV-2 survivors” (AKTIV) and “Analysis of hospitalizations of comorbid patients infected during the second wave of SARS-CoV-2 outbreak” (AKTIV 2) is the Eurasian Association of Therapists (EAT). Currently, there are no clinical registries in the Eurasian region designed to collect and analyze information on long-term outcomes of COVID-19 survivors with comorbid conditions. The aim of the register is to assess the impact of a novel coronavirus infection on long-term course of chronic non-communicable diseases 3, 6, 12 months after recovery, as well as to obtain information on the effect of comorbidity on the severity of COVID-19. Analysis of hospitalized patients of a possible second wave is planned for register “AKTIV 2”. To achieve this goal, the register will include men and women over 18 years of age diagnosed with COVID-19 who are treated in a hospital or in outpatient basis. The register includes 25 centers in 5 federal districts of the Russian Federation, centers in the Republic of Armenia, the Republic of Kazakhstan, the Republic of Kyrgyzstan, the Republic of Belarus, the Republic of Moldova, and the Republic of Uzbekistan. The estimated capacity of the register is 5400 patients.

Abstract

Aim. To study the lipid profile in hospitalized patients with coronavirus disease 2019 (COVID-19) depending on the outcome of its acute phase according to the AKTIV international registry.Material and methods. The AKTIV registry included men and women over 18 years of age with a diagnosis of COVID-19, who were treated in a hospital. A total of 9364 patients were included in the registry, of which 623 patients were analyzed for levels of total cholesterol, low-density lipoprotein cholesterol (LDL-C) and triglycerides on days 1-2 of hospitalization. The level of high-density lipoprotein cholesterol (HDL-C) was calculated using the Friedewald equation.Results. We found that a decrease in LDL-C level was significantly associated with an unfavorable prognosis for hospitalized patients with COVID-19. This pattern persisted in both univariate and multivariate analyses. LDL-C levels in the final multivariate model had a significant relationship with the prognosis (an increase in the death risk by 1,7 times with a decrease per 1 mmol/l). In addition, we found that the survival of patients with an indicator level of Conclusion. A decrease in LDL-C in the acute period is significantly associated with an unfavorable prognosis for hospitalized patients with COVID-19. Determination of LDL-C can be included in the examination program for patients with COVID-19. However, the predictive value of this parameter requires further study in prospective clinical studies.

Abstract

The article investigates problems of evaluating the economic efficiency of investment projects for reconstruction of existing facilities and ways to justify investments in the process of reproduction of fixed assets. Special attention is paid to the selection and justification of criteria that ensure selective selection of an array of investment projects for inclusion in the investment program in the conditions of the economy digitalization. The classification of criteria is analyzed depending on the requirements of various participants in the investment and construction process by the implementation of reconstruction and technical re-equipment projects. The authors focus on the in-depth examination of investment projects for the reconstruction of existing enterprises.

Abstract

The article presents a case report of spontaneous coronary artery dissection (SCAD) in the young woman with signs of undifferentiated connective tissue dysplasia (UCTD). The patient was admitted urgently with a clinical and electrocardiographic signs of acute ST-segment elevation coronary syndrome. The patient underwent a coronary angiography, which revealed a type D linear intimal dissection. The decision was made to perform balloon vasodilation with the placement of a drug-eluting stent. With multicomponent therapy, the patient’s condition improved. Physical examination revealed external markers of UCTD. Therefore, it was quantified by systems and organs. The patient had no other cardiovascular diseases, previous infection, or other trigger factors. She was discharged with recommendations to continue the prescribed therapy. This case report demonstrates SCAD as a rare cause of acute myocardial infarction in a young patient. The background for SCAD development in this case was UCTD. Algorithms for managing patients with SCAD against the background of UCTD have not been defined and require further study.

Abstract

Aim.To study the clinical course specifics of coronavirus disease 2019 (COVID-19) and comorbid conditions in COVID-19 survivors 3, 6, 12 months after recovery in the Eurasian region according to the AKTIV register.Material and methods.The AKTIV register was created at the initiative of the Eurasian Association of Therapists. The AKTIV register is divided into 2 parts: AKTIV 1 and AKTIV 2. The AKTIV 1 register currently includes 6300 patients, while in AKTIV 2 — 2770. Patients diagnosed with COVID-19 receiving in- and outpatient treatment have been anonymously included on the registry. The following 7 countries participated in the register: Russian Federation, Republic of Armenia, Republic of Belarus, Republic of Kazakhstan, Kyrgyz Republic, Republic of Moldova, Republic of Uzbekistan. This closed multicenter register with two nonoverlapping branches (in- and outpatient branch) provides 6 visits: 3 in-person visits during the acute period and 3 telephone calls after 3, 6, 12 months. Subject recruitment lasted from June 29, 2020 to October 29, 2020. Register will end on October 29, 2022. A total of 9 fragmentary analyzes of the registry data are planned. This fragment of the study presents the results of the post-hospitalization period in COVID-19 survivors after 3 and 6 months.Results.According to the AKTIV register, patients after COVID-19 are characterized by long-term persistent symptoms and frequent seeking for unscheduled medical care, including rehospitalizations. The most common causes of unplanned medical care are uncontrolled hypertension (HTN) and chronic coronary artery disease (CAD) and/or decompensated type 2 diabetes (T2D). During 3- and 6-month follow-up after hospitalization, 5,6% and 6,4% of patients were diagnosed with other diseases, which were more often presented by HTN, T2D, and CAD. The mortality rate of patients in the post-hospitalization period was 1,9% in the first 3 months and 0,2% for 4-6 months. The highest mortality rate was observed in the first 3 months in the group of patients with class II-IV heart failure, as well as in patients with cardiovascular diseases and cancer. In the pattern of death causes in the post-hospitalization period, following cardiovascular causes prevailed (31,8%): acute coronary syndrome, stroke, acute heart failure.Conclusion.According to the AKTIV register, the health status of patients after COVID-19 in a serious challenge for healthcare system, which requires planning adequate health system capacity to provide care to patients with COVID-19 in both acute and post-hospitalization period.

Abstract

The international AKTIV register presents a detailed description of out- and inpatients with COVID-19 in the Eurasian region. It was found that hospitalized patients had more comorbidities. In addition, these patients were older and there were more men than among outpatients. Among the traditional risk factors, obesity and hypertension had a significant negative effect on prognosis, which was more significant for patients 60 years of age and older. Among comorbidities, CVDs had the maximum negative effect on prognosis, and this effect was more significant for patients 60 years of age and older. Among other comorbidities, type 2 and 1 diabetes, chronic kidney disease, chronic obstructive pulmonary disease, cancer and anemia had a negative impact on the prognosis. This effect was also more significant (with the exception of type 1 diabetes) for patients 60 years and older. The death risk in patients with COVID-19 depended on the severity and type of multimorbidity. Clusters of diseases typical for deceased patients were identified and their impact on prognosis was determined. The most unfavorable was a cluster of 4 diseases, including hypertension, coronary artery disease, heart failure, and diabetes mellitus. The data obtained should be taken into account when planning measures for prevention (vaccination priority groups), treatment and rehabilitation of COVID-19 survivors.

Abstract

Results of experimental investigations on radar sensing of inhomogeneous media and objects with the use of both superbroadband (from 0.5 to 17 GHz) multifrequency scanning and supershort nanosecond and subnanosecond radar pulses are considered. It is demonstrated that addition of angular and spatial scanning and subsequent synthesis of a large aperture allow a three-dimensional tomography of low-contrast inhomogeneities to be realized with a spatial resolution of about 1 cm. Examples are presented that confirm a high efficiency of the method for contactless tomography of the forest structure and detection and visualization of infantry mines below a rough sand surface.

Abstract

Swampy soils of the Upper and Middle Amur Region territory occupy an area of 5663.5 km 2 accounting for 13.6% of the whole territory of the Amur Region. Swampy soils are formed under sphagnum mosses and, partly, sedges. In the zone of deciduous forests, these soils are formed from different grasses. The upper peat-bearing horizon of soils consists of vegetation in the stage of semidecay. This horizon is underlain by a mineral glue layer consequently transforming into rock. About 600 deposits of peat with summary potential resources of 1.3 billion tons are located on the investigated territory [1]. The aim of this work was to study the concentration levels of the following noble metal elements (NMs) in peats of the Amur Region: Ru, Rh, Pd, Ag, Os, Ir, Pt, and Au. Mass shares of NMs were detected in the laboratory of chemical analysis of the Institute of Geology and Nature Management, Far East Division, Russian Academy of Sciences, from 10 g samples after stepwise ashing at temperatures below 600 ° C with a heating speed of 100 ° C per hour. The mass shares of Au, Ag, Rh, Pd, and Pt have been detected by the atomic adsorption method [2, 3]. The kinetic method was used for detecting Os and Ir [4, 5], and the catalytimetric method for Ru detection [6] (the average values are shown in Table 1). Analysis was performed according to the III accuracy category with the maximal error of NM determination amounting to ± 30%. Oxidized forms of NMs are well dissolved in mineral acids that are not oxidants. In contrast, metal forms of NMs are practically insoluble in it. This feature was used for extracting the oxidized forms of NMs by an acid extract. Peat samples of 10 g in weight were processed by a definite amount of strong hydrochloric acid (with a density of 1.19 g/cm 3 ) during 1 hour of mixing. The concentration of the solution was further brought to 2‐3 M of HCl, and then it was heated under a cover up to boiling. The hot solution was passed through the double filter “blue band” with a pore diameter of 1‐2 µ m, and then it was twice washed by a hot solution of 3 M HCl. The acid extract consists of nanoparticles of NMs with dimensions less than 1 µ m and ions of oxidized forms of NMs. The filtrate was analyzed due to the methods of [2‐6]. The concentration of oxidized forms of NMs was determined from the acid extracts by their extraction of the following mixture: 0.025 M paraalkilaniline and 0.05 M diethylhexylditiophosphoric acid in toluene. Abstract

The Amur region, a unique region (in terms of the structural-tectonic setting) is located in eastern Asia within the Mongol‐Okhotsk branch of the Pacific ore belt. It consists of three major structural units: the Stanovoi area of Proterozoic folding in the north, the Bureya median mass of Proterozoic folding in the south, and the Mongol‐Okhotsk area of Paleozoic folding that separates the first two areas. The studied region bounded by the Amur district covers an area of 363700 km 2 [1]. The study region incorporates gold placer and primary deposits confined to ancient faults separating structural elements of the Earth’s crust [1]. Gold deposits, as well as gold- and platinum-bearing areas are concentrated in 46 gold-bearing and 7 potential platinumbearing, ore-placer nodes occupying 45% of the region territory. The main gold placer areas, which are located within orogenic areas and along the periphery of the Amur‐Zeya Plain, embrace landscapes of different altitudes [2]. Direct indicators of platinum mineralization were established incidentally when studying and exploiting gold deposits and placers. The authors of [3] distinguish the Upper Amur platinum province, the boundaries of which overlap the study region. Platinum mineralization is mainly encountered in basic and ultrabasic rocks and is 3‐7 times greater than analogous ore occurrences in granite rocks. It includes the following minerals: sperrylite, native platinum, isoferroplatinum, iridosmines, osmirides, rutheniridosmines, laurite, erlichmanite, and less common sulfides and sulfoarsenides. In general, the study region includes two mineral associations: sperrylite and rutheniridosmine. New sources of platinoid mineralization are mentioned in [4] and reported from black shale sequences and metasomatites of the metamorphic and hydrothermal rock classes. Noble metals (NM) occur in nature mainly as fine-dispersed particles of a metallic (unoxidized) form. They are irregularly distributed in the Earth’s crust. Natural clarke NM values make up 3‐900 mg/t (according to [5]) or 1‐10 mg/t in the Earth’s crust (according to [6]). The purpose of this work was to study the distribution and concentration of elements from the NM group: Ru, Rh, Pd, Ag, Os, Ir, Pt, and Au in coals of the Amur‐ Zeya and Bureya depressions in the middle Amur region. The studied region was drained completely at the end of the Proterozoic. In the Mesozoic, troughs appeared on the Precambrian and Paleozoic basements, where continental sediments had accumulated since the Late Triassic and Jurassic. The region underwent the Mesozoic tectonomagmatic activation. Upwarping of the region took place beginning in the Late Jurassic, and landscapes acquired recent outlines beginning with the Cretaceous [1].

Abstract

Blood smears of 11 and 13-day-old embryos (20) and newborns (10) of golden hamsters were analysed. The smears were stained with brilliant cresyl blue in a moist chamber for 5--7 min. About 2--6 thousand cells were counted in 200 visual fields. By means of an ocular micrometer MOB-1 the diameters of 300 cells were measured and the curves on cell distribution were plotted according to the diameters. It was found that the blood of 11-day-old golden hamster embryos contained about 8% of binuclear and sixnuclear megaloblasts, and that of 13-day-old embryos--about 2.5% of binuclear and sixnuclear erythroblasts and megablasts of the second generation. In the bloood of newborn hamster polynuclear cells are absent.

Abstract

This work was carried out in real clinical practice. Five clinical observations of anemia caused by vitamin B12 deficiency are presented, which demonstrate the difficulties of differential diagnosis. Differential diagnosis of B12‑deficiency anemia is carried out with folate deficiency anemia, hemolytic anemia, myelodysplastic syndrome, acute leukemia – erythromyelosis, aplastic anemia, myelocarcinomatosis, anemia due to copper deficiency. The relevance of the clinical problem is determined by the fact that B12‑deficiency anemia is one of the common anemias, the frequency of its detection varies in different age groups: after 60 years, vitamin B12 deficiency is found in one in 50 people, after 70 years – in every 15th. Currently, an increasingly common cause of megaloblastic anemia is vitamin B12 deficiency, which occurs while taking certain medications. Vitamin deficiency leads to a wide range of various disorders, primarily the occurrence of neurological manifestations, while there is insufficient awareness of clinicians about causes of its occurrence, nonspecific manifestations, diagnostic methods and effective therapy.

Abstract

Aim. To analyze the lipid profile changes during the acute phase of coronavirus disease 2019 (COVID-19) and within 12 months after discharge from the hospital (post-Covid period) in comparison with the lipid profile of patients before COVID-19.Material and methods. The subanalysis of the registry was carried out as follows: clinical investigators in each of the centers included patients in the AKTIV registry selected from the database patients who met three following criteria: (1) availability of lipid profile test no more than 60 days before COVID-19; (2) availability of lipid profile test during the acute period; (3) availability of lipid profile test within 6-12 months after discharge from the hospital.Results. In patients after COVID-19, a decrease in the levels of all lipid profile parameters in the acute COVID-19 phase and their subsequent increase were found.In the post-COVID-19 period, 25,7% of patients experienced an increase in lipid profile parameters (increased levels of total cholesterol (TC) and/or cholesterol, low-density lipoproteins and/or triglycerides and/or cholesterol excluding high-density lipoproteins by ≥0,5 mmol/l) despite the fact that lipid-lowering therapy in these patients was at least no worse than in patients without lipid profile changes.Multivariate analysis found that such variables as age (direct relationship), body mass index (direct relationship), estimated glomerular filtration rate in the acute phase (inverse relationship) and cholesterol level excluding high-density lipoproteins in the acute period (inverse relationship), as well as heart failure, obesity and cytokine storm in the acute COVID-19 phase are independent predictors of an increase in one or more lipid parameters by ≥0,5 mmol/l over 6-12 months of post-COVID-19 period.Conclusion. COVID-19 likely contributes to the onset and/or progression of lipid metabolism disorders in COVID-19 survivors (Eurasian population).

Abstract

Aim. To investigate on post-COVID period in patients of the Eurasian region.Material and methods. A total of 9364 consecutively hospitalized patients were included in ACTIV registry. Enrollment of patients began on June 29, 2020, and was completed on March 30, 2021, corresponding to the first and second waves of the pandemic. Demographic, clinical, and laboratory data, computed tomography (CT) results, information about inhospital clinical course and complications of COVID-19 during hospitalization were extracted from electronic health records using a standardized data collection form. The design included follow-up telephone interviews with a standard questionnaire at 3, 6, and 12 months to examine the course of post-COVID period.Results. According to ACTIV register, 63% of patients after COVID-19 had new adverse symptoms or exacerbations of the existing symptoms lasting for up to 1 year. After hospital discharge, 79,8% of patients sought unscheduled medical attention in the first 3 months, 79,1% at 4-6 months, and 64,8% at 7-12 months. Readmission rate was 11,8% in the first 3 months, 10,9% at 4-6 months, and 10,1% at 7-12 months. The most common reasons for unscheduled treatment in the first 3 months were uncontrolled hypertension, decompensated type 2 diabetes, destabilization of coronary artery disease, gastrointestinal disease, AF episodes, exacerbation of asthma and chronic obstructive pulmonary disease, decompensated heart failure (HF). The 12-month mortality of COVID-19 survivors after the discharge was 3,08%. Multivariate analysis showed that independent risk factors for fatal outcome were age (direct correlation), the levels of hemoglobin (inverse correlation), oxygen saturation (inverse correlation), and aspartate aminotransferase (direct correlation), as well as class III-IV HF, prior stroke, cancer, inhospital acute kidney injury. Based on these identified risk factors, a nomogram was constructed to determine the 3-month mortality risk after discharge.Conclusion. Analysis of ACTIV register showed that end of the acute phase of COVID-19 does not imply a complete recovery.
