Bimetallic nanostructures can exhibit significant broadening of Mie resonances compared to monometallic nanoparticles. Here, we study these materials using Electron Energy Loss Spectroscopy at a single particle level. This technique is immensely effective to probe direct structure-property correlation of nanoparticles. We are thus able to confirm the broadening of Mie resonances at a single particle level. This effect is analyzed in the context of emergence of a new dielectric constant that originates from metallic interfaces. We employ Coronado-Schatz corrections to the usual dielectric constants of the constituent metals to quantitatively simulate these materials. The resultant materials thus exhibit optical cross-sections that are 38.5 % and 4.2 % of the gold and silver nanoparticle optical cross-section at 2.5 eV and 3.45 eV, respectively. A strong agreement between theory and experiment is observed. We also confirmed the effectiveness of our approach for nanoparticles with different morphologies as well as different compositional ratios. Our study suggests an effective strategy to regulate the dielectric properties of bimetallic nanostructures through interface engineering.

Background: Heart failure (HF) is a chronic and progressive condition that significantly impacts patients’ lives, necessitating a comprehensive understanding of the factors influencing their health outcomes. Among these factors, symptom perception, health-related quality of life (HRQoL), and predicted survival are paramount. This study aimed to assess symptom perception, HRQoL and predictive survival among HF patients. Methods: A baseline analysis was carried out in a randomized controlled trial involving patients with HF. Heart Failure Somatic Perception Scale to assess symptom perception, Kansas City Cardiomyopathy Questionnaire-12 to assess HRQoL and Seattle Heart Failure Model to assess predicted survival were used. Results: Among the 160 patients, 62.5 % were between 36 and 59 years of age and were married (88 %). Hypertension and diabetes were reported in 104 and 94 participants respectively, and among them, 65 participants had both hypertension and diabetes. The mean left ventricular ejection fraction was 33(%), and 56.5 % of the participants were in New York Heart Association functional class III and IV. The average symptom perception score was 27.4 ± 18.3, while the mean HRQoL score was 37.2 ± 14.2. The predicted one-year survival score was 80 ± 11.5, and the predicted five-year survival score was 38.5 ± 21.3. HRQoL showed a significant negative correlation with symptom perception (r = −0.75; p = 0.001) and a positive correlation with one-year survival (r = 0.53; p = 0.001), and five-year survival (r = 0.56; p = 0.001). Conclusion: Patients with HF experience persistent symptoms and poor HRQoL. Symptom severity decreases HRQoL. Understanding HRQoL and symptom perception is crucial for developing effective self-care interventions, which are vital for improving patient outcomes and managing HF effectively.

OBJECTIVE: Hypersensitivity pneumonitis (HP) is an inflammatory and/or fibrotic lung disease. The restrictive lung function with low diffusing capacity of carbon monoxide (DLCO) is common in interstitial lung diseases (ILD). There are limited data on pulmonary function test (PFT) in HP and its role for diagnosis is questionable. We analyzed the data of 152 HP patients for type of defect, lung volume, and DLCO. MATERIAL AND METHODS: The present study is a retrospective analysis of 152 patients at one of the tertiary chest institutes in India. All diagnosed cases with at least spirometry were included. PFT findings were classified and graded as obstructive, restrictive, and mixed patterns. The correlation of PFT was calculated with disease duration and 6MWT distance. RESULTS: The majority were female [106 (70%)], with a mean age of 47.8 ± 12.3 years. Spirometry with lung volume data were available for 97% patients. PFT was abnormal in 118 (80%) cases. Among the patterns of abnormality, the most common type was restrictive (74%) followed by mixed (15%) and obstructive (11%) with the majority in the severe to very severe grade. The mean total lung capacity (TLC) and residual volume (RV) were reduced, with the grade more severe when the pattern of abnormality was restrictive while the RV/ TLC was higher suggestive of air tapping. DLCO data were available for 132 (87%) cases, with levels decreased in 67%. The severity of DLCO was highest when the pattern of abnormality was restrictive. One of the PFT parameters was abnormal in 137 (90%) cases, with isolated decreased DLCO levels seen in 16 (10%) cases. Forced vital capacity (FVC), TLC, and DLCO showed positive correlation with 6MWT distance (FVC r = .22, P = .02; TLC r = .28, P = .003; DLCO r = .30, P = .002). CONCLUSION: The PFT abnormality is seen in >80% of HP patients. All types of abnormality were seen, with the restrictive pattern being the most common. Isolated decreased DLCO levels were seen in 10% of cases. We advised to do full PFT for diagnosis and followup of HP

Abstract Seventh pandemic Vibrio choleare O1 El Tor strain is responsible for the on-going pandemic outbreak of cholera globally. This strain evolved from non-pathogenic V. cholerae by acquiring seventh pandemic gene (VC 2346), pandemic Islands (VSP1 and VSP2), pathogenicity islands (VP1 and VP2) and CTX prophage region. The cholera toxin production is mainly attributed to the presence of ctx gene in these strains. However, several variants of this strain emerged as hybrid strains or atypical strains. The present study aimed to assess the aquatic environment of Cochin, India, over a period of 5 years for the emergence of multidrug resistant V. cholerae and its similarity with seventh pandemic strain. The continuous surveillance and monitoring resulted in the isolation of ctx negative, O1 positive V. cholerae isolate (VC6) from coastal water, Cochin, Kerala. The isolate possessed the biotype specific O1 El Tor tcpA gene and lacked other biotype specific ctx, zot, ace and rst genes. Whole genome analysis revealed the isolate belongs to pandemic sequence type (ST) 69 with the possession of pandemic VC2346 gene, pathogenic island VPI1, VPI2, and pandemic island VSP1 and VSP2. The isolate possessed several insertion sequences and the SXT/R391 family related Integrative Conjugative Elements (ICEs). In addition to this, the isolate genome carried virulence genes such as VgrG, mshA, ompT, toxR, ompU, rtxA, als, VasX, makA, and hlyA and antimicrobial resistance genes such as gyrA, dfrA1, strB, parE, sul2, parC, strA, VC1786ICE9-floR, and catB9. Moreover, the phylogenetic analysis suggests that the isolate genome is more closely related to seventh pandemic V. cholerae O1 N16961 strain. This study reports the first incidence of environmental ctx negative seventh pandemic V. choleare O1 El Tor isolate, globally and its presence in the aquatic system likely to induce toxicity in terms of public health point of view. The presence of this isolate in the aquatic environment warns the strict implementation of the epidemiological surveillance on the occurrence of emerging strains and the execution of flagship program for the judicious use of antibiotics in the aquatic ecosystem.

Background: Staff nurses face frequent emotional situations in their work environment. The constant contact with suffering patients, and the busy work environment, pose tremendous stress on nurses’ physical and emotional health. The Emotional Intelligence skills of empathy, self-awareness, motivation, self-control, and keeping relationships, can help handle difficult emotions and allow nurses to work in an organized, calm, and professional way. This study aims to implement and assess the effectiveness of a training program developed by the investigator, tailored to the mental and emotional needs of staff nurses who are working in an organization. The study also aims to observe any significant change, correlation, and association in the staff nurses’ level of emotional intelligence, intrinsic motivation, self-compassion, emotional labor, and nurse-in-charges’ and patients’ perception of nursing care after the program. Methods: A quasi-experimental (one-group) study design was used in this study. The study will involve 80 staff nurses working in a selected hospital in India. The staff nurses will be selected from the hospital’s general wards using convenience sampling. For the current study, a quasi-experimental design will be used. The investigator will deliver a training program, divided into four sessions of two hours each. Data will be collected from the participants at baseline and 3-months pre-intervention; and post-test data will be collected immediately after the intervention, at 3-month, and 6-month follow-up, to observe any significant change in the study variables before and after the intervention. Results: The current study primarily focuses on the vital aspect of developing emotional needs, for promoting a better work-life balance. Research findings from the study will significantly contribute to the evidence based Emotional Intelligence programs for staff nurses, and if proven effective, could be delivered extensively in the hospitals. Trial registration: The study is registered in June 2019 under the Central Trial Registry of India ( CTRI/2019/08/020592).

India's west coast region is unique due to the co-existence of abundant natural resources and continuous agricultural production. In this region, arecanut is a dominant crop, but due to many factors such as unstable markets, price fluctuations, diseases, etc., the risk faced by farmers has increased. The integrated farming system (IFS) is a viable option for reducing risks and improving productivity and income while also offering multiple benefits to the farm family. Considering these, we collected data from ten farmers' fields in coastal Karnataka, India for the period 2013–2022, to compare arecanut farming with IFS systems where coconut, nutmeg and banana were intercrops with livestock component. Results indicated that IFS had diverse plant species such as plantation crops, fruits, vegetables, flowers, spices, and forest trees offering multiple benefits to the farm families. Various plant species found on the IFS farms were primarily utilized for culinary, ornamental, and religious purposes (benefits worth US$ 421 year−1). The arecanut equivalent yield in IFS farms was consistently and significantly higher during the last ten years. The mean net return derived from IFS farms (US$ 7857) was also significantly higher than the arecanut farming (US$ 3990). Energy budgeting indicated that, in IFS, significantly higher direct and renewable energy and lower indirect and non-renewable energy was utilized. Energy output (192,202 ​MJ ​ha−1), net energy (120,477 ​MJ ​ha−1), energy use efficiency (2.68), and energy profitability (1.68) were significantly higher under the IFS farm compared to the arecanut farming. Employment generation was also significantly higher in IFS (1236.7 mandays year−1) than in control farms (755.2 mandays year−1), especially for female laborers (2.05 times higher). This study concludes that the adoption of IFS helps in enhancing farm profitability and generating employment opportunities while improving energy use efficiency. Considering these benefits, promoting IFS through subsidy and policy support would help in sustainable development of agriculture sector in this region.

AIM: The aim of the study was to analyze orbitofacial anthropometric parameters such as inner and outer canthal distances (ICD and OCD), palpebral fissure length (PFL), interpupillary distance (IPD), and canthal index (CI) in children with pseudostrabismus and to compare the measured IPD (mIPD) with calculated IPD (cIPD). MATERIALS AND METHODS: This was a prospective study of sixty children (6 months–18 years) with pseudostrabismus. ICD, OCD, PFL, and IPD were measured by digital Vernier caliper. The formula used was cIPD: 0.21+0.24 ICD+0.58°CD for males and 1.4+0.31 ICD+0.41°CD for females. Values measured by caliper were compared with that calculated by the formula. The formula used was CI: ICD × 100/OCD. Data were analyzed statistically. RESULTS: The mean age was 6.66 ± 3.57 years. Telecanthus was the most common finding (55%). The mean ICD and OCD in males were 30.89 ± 3.33 mm and 87.96 ± 8.09 mm and in females were 30.91 ± 3.05 and 86.22 ± 6.81 mm, respectively. The mean right eye PFL in males was 28.53 ± 2.63 mm and in females was 27.66 ± 2.22 mm and left eye PFL in males was 28.53 mm ± 2.63 and in females was 27.66 ± 2.22 mm. CI in males was 35.10 ± 1.65 and in females was 35.84 ± 1.71. Mean mIPD and cIPD: male – 55.37 ± 4.75 mm and 58.56 ± 5.34 mm, female – 53.32 ± 4.74 mm and 46.26 ± 3.71 mm. A good agreement was found between mIPD and cIPD. CONCLUSION: This study helps in documenting the anthropometric pattern of the orbitofacial parameters in children with pseudostrabismus which can act as reference data. This helps in the management of orbitofacial, craniofacial syndromes/deformities and lid reconstructive surgeries in retaining ethnical features and obtaining better function. In children's spectacle frame 1 and lens making, where measuring IPD is difficult, cIPD can be a simple alternative.

This is a retrospective series of five eyes of four cases with diabetic macular edema (DME) secondary to large capillary aneurysms. Larger capillary aneurysms were identified noninvasively based on the presence of white rim in color photograph. On optical coherence tomography (OCT), the larger capillary aneurysms were seen as vertically oval structures with heterogeneous lumen and hyperreflective margin. Two of the four eyes were treated with intravitreal therapy with poor response before considering laser photocoagulation. In one eye, laser photocoagulation was considered as primary therapy in view of the poor response to intravitreal therapy in the fellow eye. In one eye, intravitreal steroid with prompt laser was considered. In one eye, laser was considered as primary therapy. Complete obliteration of the capillary lumen was noted on OCT in all the five eyes after laser photocoagulation. Complete resolution of macular edema was noted in all the five eyes with no recurrence over a follow-up period of 4–18 months. DME secondary to larger capillary aneurysms responds well to targeted laser photocoagulation. These larger capillary aneurysms can be identified on clinical examination and color photograph by the presence of white rim and can be confirmed on OCT.

Introduction: The co-existence of obstructive sleep apnea (OSA) and interstitial lung diseases (ILD) results in significant morbidity and mortality. So screening for OSA is important for its early diagnosis among ILD patients. The commonly used questionnaires for screening of OSA are Epworth sleep score (ESS) and STOP-BANG. However, the validity of these questionnaires among ILD patients is not well studied. The aim of this study was to assess the utility of these sleep questionnaires in detection of OSA among ILD patients. Methods: It was a prospective observational study of one year in a tertiary chest centre in India. We enrolled 41 stable cases of ILD who were subjected to self-reported questionnaires (ESS, STOP-BANG, and Berlin questionnaire). The diagnosis of OSA was done by Level 1 polysomnography. The correlation analysis was done between the sleep questionnaires and AHI. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were calculated for all the questionnaires. The cutoff values of STOPBANG and ESS questionnaire were calculated from the ROC analyses. P value of 90%. The sensitivity also increased with the increasing severity of OSA. AHI showed positive correlation with ESS (r = 0.618, P < 0.001) and STOPBANG (r = 0.770, P < 0.001). Conclusion: The ESS and STOPBANG showed high sensitivity with positive correlation for prediction of OSA in ILD patients. These questionnaires can be used to prioritize the patients for polysomnography (PSG) among ILD patients with suspicion of OSA.

Purpose: To study the presence and development of strabismus in children with telecanthus, epicanthus, and hypertelorism. Methods: This is a prospective, longitudinal, and observational study. Sixty children aged between 6 months and 18 years with telecanthus, epicanthus, and hypertelorism in isolation or in combination were recruited. A detailed analysis of the history, determination of best corrected visual acuity, complete evaluation of strabismus, and ocular examination were carried out. The presence of telecanthus, epicanthus, and hypertelorism and associated strabismus, if any, was noted. All children were followed up for a minimum and maximum period of 12 and 18 months, respectively, to analyze the strabismus (previously present) and for detection of strabismus in those who did not have. The data were analyzed descriptively with mean and standard deviation. Chi square test and Fishers exact test were used to analyze the data between the groups. A P value less than 0.05 was considered to be statistically significant. Results: Telecanthus was the most common lid feature (55%). At baseline, ten (16.66%) children had strabismus (six: esotropia; four: exotropia). Two (3.33%) children underwent surgery. One child developed exotropia at the third follow-up (18 months). At the end of the study, 11 (18.33%) children had strabismus. No significant association was seen between lid characteristics and the type of strabismus. Conclusion: Children with telecanthus, epicanthus, and hypertelorism in isolation or in combination may or may not have associated strabismus. These features can pose difficulty in strabismus diagnosis, which mandates a careful examination, especially in younger age groups and small-angle strabismus. On the other hand, children without strabismus need longer follow-up to detect the development of strabismus and to initiate further management at the earliest.

Introduction: Yoga is a type of practice with numerous benefits for health. This systematic review aimed to synthesize evidence on yoga therapy and determine its effects on caregiver stress and mental health among those who provide care for people with dementia. Methods: The Cochrane methodological guidelines were adopted and reported using the PRISMA statement. MEDLINE, Cochrane Central, CINAHL, EMBASE, PROQUEST, Scopus, and Web of Science were among the seven online databases searched between January 2010 and October 2021 for randomized controlled trials. The risk of bias in the trials was evaluated using the Cochrane risk of bias tool. Meta-analysis was done using Revman software 5.4 version. Results: Thirteen randomized controlled trials consisting of 522 research participants assessed the impact of yoga on caregiver stress, burden, mental health, and depression. A random effects model on the effect of yoga revealed that yoga is beneficial in caregiver stress reduction and enhancing the psychological well-being of caregivers of people with dementia with statistical significance (95%CI: 0.64–0.89, p

We present a case of prepapillary venous loop complicated by central retinal vein occlusion (CRVO). A 56-year-old female reported to us with blurring of vision in the right eye, noticed 8 days ago. Fundus examination in the right eye showed multiple superficial retinal hemorrhages in all the four quadrants with dilated veins and cystoid macular edema, suggestive of CRVO. Prepapillary venous loops were seen in both the eyes. In our case, both the eyes had an abnormal venous anatomy at the disc. On fundus fluorescein angiography, only the looped segment showed laminar flow in late phase in both the eyes. Hence, patients with an abnormal retinal venous anatomy should be treated with caution as they are considered to be at a high risk of retinal vein occlusion.

Indian fisheries sector is receiving increasing attention and policy support owing to the livelihood and foreign exchange potential of the sector apart from its nutritional significance. The Government of India has initiated ‘Blue Revolution’ to augment the fish production and to strengthen the fish based value chain. But, unequipped manpower is often highlighted as one of the major deterrents for leveraging the full potential in harvest and post-harvest fisheries sector, which is highly skill intensive in nature. The issue would emerge more serious in case of marine fisheries. In this paper, we attempted to deliberate on the issue of skill-deficit that would emerge in harvest and post-harvest sectors of marine fisheries sector in India, and suggest measures to address them.

Purpose: To evaluate the agreement in the macular vascular density (MVD) measured by two protocols: 4.5 mm × 4.5 mm and 6 mm × 6 mm. Design: Cross‑sectional observational study. Methods: Healthy volunteers between the age group of 19 and 39 years were recruited. Topcon DRI OCT Triton plus (Topcon Corporation, Tokyo, Japan) was used to acquire the fovea‑centered optical coherence tomography angiography (OCTA) image using two protocols: 4.5 mm × 4.5 mm and 6 mm × 6 mm. MVD was measured by the manufacturer software in the superficial capillary plexus slab in five regions: central, superior, nasal, inferior, and temporal subfields of early treatment diabetic retinopathy study grid in each protocol. Results: The study included 79 eyes of 40 healthy volunteers. The difference in the mean MVD between two protocols was 3.84% in right eye and 4.2% in left eye in central subfield, 0.93% in right eye and 1.13% in left eye in superior subfield, 0.06% in right eye and 1.45% in left eye in nasal subfield, 1.65% in right eye and 0.7% in left eye in inferior subfield, 0.4% in right eye and 0.54% in left eye in temporal subfield. The measurements were significantly higher in 6 mm × 6 mm in central subfield in both the eyes and in nasal field in the left eye. Whereas, the measurements were significantly higher in 4.5 mm × 4.5 mm in superior and inferior subfield in both the eyes and in temporal subfield in the left eye. Conclusion: The protocols should not be used interchangeably and it is necessary to include recommendation of the field of view to measure MVD while standardizing OCTA reporting.
