Abstract

Google's operational flood forecasting system was developed to provide accurate real-time flood warnings to agencies and the public with a focus on riverine floods in large, gauged rivers. It became operational in 2018 and has since expanded geographically. This forecasting system consists of four subsystems: data validation, stage forecasting, inundation modeling, and alert distribution. Machine learning is used for two of the subsystems. Stage forecasting is modeled with the long short-term memory (LSTM) networks and the linear models. Flood inundation is computed with the thresholding and the manifold models, where the former computes inundation extent and the latter computes both inundation extent and depth. The manifold model, presented here for the first time, provides a machine-learning alternative to hydraulic modeling of flood inundation. When evaluated on historical data, all models achieve sufficiently high-performance metrics for operational use. The LSTM showed higher skills than the linear model, while the thresholding and manifold models achieved similar performance metrics for modeling inundation extent. During the 2021 monsoon season, the flood warning system was operational in India and Bangladesh, covering flood-prone regions around rivers with a total area close to 470 000 km2, home to more than 350 000 000 people. More than 100 000 000 flood alerts were sent to affected populations, to relevant authorities, and to emergency organizations. Current and future work on the system includes extending coverage to additional flood-prone locations and improving modeling capabilities and accuracy.

Abstract

Objective: To determine the prevalence of hypertension in newly diagnosed type 2 diabetic patients and its association with risk factors for cardiovascular and diabetic complications.Design: Cross-sectional study.Patients: Newly diagnosed type 2 diabetic patients (n = 3648, mean age 52 years, 59% male) recruited for the UK Prospective Diabetes Study (UKPDS).Measurements: Blood pressure, body mass index, waist:hip ratio, ECG signs of ischaemia and of left ventricular hypertrophy (Minnesota code), fasting plasma glucose, urate, creatinine, insulin, triglycerides, high-density lipoprotein-, low-density lipoprotein- and total cholesterol, urinary albumin: creatinine ratio, retinopathy grading.Results: Thirty-nine per cent of the patients (35% of the males, 46% of the females) were hypertensive (mean blood pressure greater-than-or-equal-to 160 systolic and/or greater-than-or-equal-to 90 mmHg diastolic 2 and 9 months after diagnosis of diabetes, or taking antihypertensive therapy). The hypertensive patients had a greater mean body mass index (30.1 versus 28.0 kg/m2, P 5.0 g/mol; 24 versus 14%, P < 0.0001), of ECG signs of probable and possible ischaemia (24 versus 14%, P < 0.0001) and of left ventricular hypertrophy (8.5 versus 3.8%; P < 0.0001). The prevalence of retinopathy was similar in the two groups.Conclusions: Hypertension is common in newly diagnosed type 2 diabetes and is associated with obesity. The association between hypertension and higher triglyceride and insulin levels may be secondary to obesity in this population. An association between hypertension and cardiovascular complications is already apparent at diagnosis of diabetes.

Abstract

Four cases of femoral pseudoaneurysms from substance abuse are presented. One patient, a 32 year-old women needed amputation (by disarticulation of the hip) after failed revascularization. Another patient had symptoms of claudication after triple ligation and resection without bypass grafting. The last two had an uneventful recovery: one after saphenous vein bypass, the other after simple resection and repair with a vein patch.
