The authors discuss about epidemiology, etiopathogenesis, risk factors, prophoilaxis of deep venous thrombosis and pulmonary embolism in patients operated. They report an analisys of their experience, confirming that the incidence of deep venous thrombosis and pulmonary embolism is larger in patients under 60 years old, while over 60, the incidence is similar between neoplastic and non neoplastic subjects.

The purpose of this study was to assess the value of sentinel node (SN) biopsy in oral cancer by means of a lymphoscintigraphic technique and intraoperative detection by blue-dye combined with gamma-ray probe to facilitate identification of the SN. Forty-one T1-T2N0 patients underwent lymphoscintigraphy, SN biopsy, and modified radical neck dissection. An SN was identified in 39 of 41 patients by the combined use of intraoperative blue dye and the probe and was removed. Complete neck dissections were performed and the histological evaluation compared. Thirty-eight SNs in 35 patients were negative at final pathology and correctly predicted the pathological status of the specimens from the full-neck dissections. Five SNs in four patients had micrometastases and were the only metastatic nodes identified. The results of this study on a homogenous series of patients show that SN biopsy is a valuable staging technique in T1 and T2 oral cancer with uninvolved neck, provided that no previous surgery or radiotherapy has altered lymphatic drainage in the oral cavity or in the neck. In a large number of patients, SN biopsy can avoid unnecessary neck dissection and its relevant morphofunctional sequelae.

To investigate the role of Helical CT and the usefulness of three-dimensional (3D) imaging for preoperative planning and follow-up of reconstructive maxillofacial surgery with alloplastic material in neoplastic disease involving this region.From 1996 to 1999 eleven patients were examined with Helical CT and 3D images for planning of maxillofacial plastic and reconstructive surgery for advanced cancer of this anatomically complex region. A 3D-modulated titanium mesh (100%) or micronets was used to rebuild the anterior surface of maxillary bone and the orbital floor. The mesh was cut to the appropriate size and shape and curved where necessary. Within the residual sinusal cavity a siliconed filling was used surmounting an acrylic prosthesis with dental arch to rebuild the palate. A rehydrated bovine pericardium was affixed and moduled on the borders in two cases only. Three-dimensionally reconstructed CT images were obtained preoperatively and at least 6 months postoperatively in all patients. The images were generated on a computer workstation using the shaded surface display (SSD) software with threshold values ranging 425 to 630 HU, and a more closed window for the imaging of titanium mesh/bone interface in the postsurgical follow-up.We always obtained an excellent complete spatial depiction of maxillofacial region both before and after surgery, with no artefacts so important as to affect the 3D reconstruction process and the image quality. Together with the head-neck surgical team we could work for preoperative planning through CT scans by different 3D points of view. The 3D reconstructed follow-up scans showed good filling of the defect in the area where the titanium mesh had been used. Then efficacious bone modelling and good biocompatibility of the alloplastic material were seen in all patients, with no inflammatory reactions.Titanium is a well-known material, which is widely used for cranioplasty. It is a radiolucent, nonferrous metal of low atomic number that allows very clear CT and MR images to be obtained. Further Ti features are strength, biocompatibility and easy handling. 3D Helical CT scan has proved to be the most complete and accurate imaging technique for reconstructive plastic surgery with alloplastic material in advanced maxillofacial cancer, also considering the anatomic and functional complexity of this area. The prospect is provided to identify virtual 3D presurgical ablation planes. These may allow the surgeon to improve plastic reconstruction and shorten intervention time.

Interferon (IFN) is widely considered the most effective agent in the adjuvant therapy of patients with cutaneous melanoma (CM). However, little is known about the effect of IFN on pretreated CM patients who experience disease recurrence. The authors conducted a Phase II study to determine whether intermediate doses of IFN could be beneficial for these patients.A series of 24 consecutive CM patients who had undergone surgery for local, in-transit, or lymph node disease recurrence during adjuvant therapy with low dose IFN (IFNalpha-2b, 3 million units [MU] per day, three times per week) were enrolled for second-line therapy with intermediate dose IFN (IFNalpha-2b, 10 MU per day) for one year.IFN was discontinued in 7 patients (29.2%) because of toxicity. Several patients complained of impairment in their daily activities. Progression of disease was registered in 17 patients (70. 8%), with a median disease free survival of 5.5 months (95% confidence interval, 3.4-14.2). The median follow-up for the 7 patients who did not experience disease recurrence was 15 months (range, 13-22 months).An increased dose of IFN as second-line adjuvant treatment was poorly tolerated and produced negative clinical outcomes in patients with CM. However, these patients probably were unresponsive to IFN regardless of the dosage level. In fact, the first adjuvant IFN treatment was ineffective in all patients. Thus, the key factor in the treatment of CM seems to be patient responsiveness to IFN rather than the total dosage achieved.

To evaluate the role of epiluminescence microscopy (ELM) in the differential diagnosis of cutaneous pigmented lesions, and to improve the early diagnosis of cutaneous malignant melanoma (CMM), 15,719 pigmented lesions from 8782 consecutive patients were evaluated using ELM with a hand-held video microscope imaging system (MS 500B Micro-Scopeman, Moritex). Comparison between risk levels as inferred from ELM screening and histology was performed on 2731 surgically excised lesions. ELM sensitivity, specificity, positive and negative predictive values, as well as agreement with histological results for the different subgroups of lesions, were determined. Overall agreement was 87.3% (ranging from 85.1% to 92.2% for melanocytic and non-melanocytic lesions, respectively); sensitivity and specificity were high (values ranging from 87.3% to 96.3% among different subsets of ELM-analysed lesions) and statistically significant (P0.0001). ELM screening identified 165 new cases of CMM with a high proportion of lesions (115; 70%) in an early phase of tumour growth (Breslow thickness/= 1.5 mm). Thus, ELM is helpful to the clinician in deciding which pigmented lesions need surgical excision, as well as in diagnosing early CMM lesions.

One hundred and ninety-one patients, all candidates for major clean-contaminated surgery, were classified according to the risk of developing postoperative wound infection, using clinical and nutritional factors. A widely validated nutritional assessment model, the Prognostic Nutritional Index (PNI), was used preoperatively to evaluate the risk and probability of post-operative septic complications. Based on the predicted outcome, patients were assigned to either a poor or a good risk group for statistical comparison. A prospective, double-blind, randomized trial was performed to compare the effectiveness of the monocyclic beta-lactam, aztreonam, with gentamicin for short-term perioperative prophylaxis. In the poor risk group (PNI greater than 40), aztreonam prophylaxis significantly reduced postoperative septic complications (p less than 0.05). The use of an objective scoring system to assess the risk of postoperative sepsis is recommended for future studies on antibiotic prophylaxis as it offers a realistic assessment basis.

The preliminary results of four multicenter, comparative protocols on the efficacy and safety of aztreonam are discussed. This monobactam antibiotic was used as short-term prophylaxis in patients undergoing colorectal, gynecologic or urologic surgery. When compared with gentamicin in a group of 295 patients undergoing colorectal surgery, aztreonam significantly reduced the incidence of abdominal wound infection (p less than 0.025); it also reduced the incidence of perineal wound and intra-abdominal infections, although these differences were not statistically significant. In patients undergoing abdominal and vaginal hysterectomy, aztreonam was compared with cefotaxime (both combined with clindamycin) in 170 and 142 patients, respectively. No difference in the incidence of infection was found between the two treatment regimens. In the 175 patients undergoing transurethral resection of the prostate, aztreonam was compared with placebo. The prophylactic regimen significantly reduced the number of urinary tract infections (p less than or equal to 0.0001). Its efficacy was particularly evident during the first 10 postoperative days. Side effects experienced by patients taking aztreonam were minor and similar in all three treatment groups studied. Aztreonam (alone or combined with clindamycin) offers a new and efficacious alternative to the standard antibiotic regimens currently used prophylactically in the high-risk postoperative patient.

Polyps are the most frequent benign tumours, being found in 3% of symptomatic patients subjected to upper gastrointestinal endoscopy. Hyperplastic polyps are the most common form (85%). Adenomatous polyps account for less than 5% and mesenchymal ones constitute 10% of the total. Multiple or diffuse polyps are unusual.

The criteria applicable to the selection of the approach route in the surgical management of essential gross obesity by means of a jejunal-ileal by-pass are discussed. Median vertical and transverse incision are compared in the light of early and late hypoxaemia in the immediate postoperative period; postoperative acidosis and alkalosis; rapidity; exposure; extendibility; tightness; postoperative pain; cosmetic results. A preference for the vertical approach is both stated and justified.

The case of a surgical patient with a full blown syndrome of zinc deficiency is presented and the various implications related to a deficiency of this trace mineral are discussed. The symptomatology involved mental depression, visual disturbances, glucose intolerance, decreased serum alkaline phosphatase levels, wound healing impairment, eczematoid dermatitis and reduced humoral and cellular immune defences. Oral supplementation with zinc sulphate dramatically reversed the signs and symptoms of the syndrome and resulted in rapid wound healing. This case seems to have exhibited most of the different signs and symptoms reported from time to time in the literature. Awareness of zinc deficiency is being brought to light in many medical and surgical conditions. Total parenteral nutrition (T.P.N.) can exacerbate the lack of this trace metal and supplementation with fresh blood derivate are, as demonstrated, useless in these cases. Zinc, as component of various metalloenzymes, accounts for the clinical manifestations of its deficiency.

The criteria underlying the choice of operation in a given case and the personal technique adopted are described. The technique in question is a personal modification of end-to-side jejuno-ileal bypass codified by Payne. To minimize reflux into the blind loop, a number of technical steps are considered indispensable: 1) fluted section of the jejunal stump; 2) isoperistaltic construction of the anastomosis and, particularly, 3) the creation of a single or double valve system upstream from the anastomosis by spiral introflexion of the mucosa using seroserous stitches parallel and perpendicular to the ileal lumen. Other techniques in use are reported. These are held to be extremely interesting but are not yet supported by a long enough follow-up.

The usefulness of antibiotic prophylaxis in reducing the incidence of postoperative infections following colorectal surgery has been defined in many clinical studies. Since a single antibiotic agent covering all the potential pathogens (i.e. Gram-negative aerobes and anaerobes) is not currently available, a combination therapy is usually administered. The present study reports the preliminary results on comparative efficacy and safety of aztreonam and gentamicin (both combined with clindamycin) as short-term prophylaxis in colorectal surgery. Four hundred and fifty-four patients, out of 495 enrolled, were considered evaluable. Incidence of abdominal wound infections was significantly lower in the aztreonam-treated group (5.8% vs 12.5% - p less than or equal to .025). The incidence of perineal wound and abdominal infections, of fever, of need for postoperative antibiotics and the length of postoperative stay were lower in the monobactam group, even if none of these differences were statistically significant. Gram-negative aerobic microorganisms were more often isolated in infected patients from the gentamicin group. Side effects were minor and similar in the two groups. From these preliminary data, the aztreonam-clindamycin combination can be considered a major candidate to replace many of the presently available oral and parenteral regimens.
