Myofibroblastomas are benign mesenchymal tumors that frequently occur in the groin. They show variable morphology, and the differential histopathological diagnoses are broad, including lipomatous to myxoid tumors. In addition, both pathological and imaging findings may be complex, which makes diagnosis challenging. We herein present a case of a mixed-variant myofibroblastoma of the wrist in a 73-year-old woman. Considering the long clinical course of more than 20 years and the imaging findings, a benign myxoid tumor including a schwannoma was suspected; however, the histopathological findings from resected specimens suggested a diagnosis of myxofibrosarcoma. Additional histopathological findings led to a diagnosis of mixed-variant myofibroblastoma. The differential diagnosis of myofibroblastoma extends beyond imaging to pathological findings because of the number of possible variants. This case reinforces the notion that the gold standard treatment for soft tissue tumors is to perform surgery only after determining the correct diagnosis by biopsy. [ABSTRACT FROM AUTHOR]

The entity commonly referred to as chondrolipoma is a rare and enigmatic breast lesion with unclear histogenesis and a complete lack of molecular characterization. It is uncertain whether it represents a hamartoma, choristoma, or a distinct neoplasm, including possibly a variant of mammary-type myofibroblastoma. We report two additional chondrolipomatous lesions of the breast. The lesions had varying histologic and immunohistochemical features similar to myofibroblastoma, including the loss of retinoblastoma (Rb) protein expression in one lesion. Molecular analysis by chromosomal microarray analysis performed on a second lesion did not demonstrate a loss of 13q14 or 16q typical of myofibroblastoma. Our findings further support the concept that at least a subset of breast lesions that historically have been classified as chondrolipoma are related to myofibroblastoma. However, the lack of myofibroblastoma-specific molecular alterations in one lesion suggests chondrolipomas may also have varying origins. [ABSTRACT FROM AUTHOR]

Vulvo-vaginal stromal tumours are a rare and diverse group of mesenchymal neoplasms unique to hormone-responsive stroma of the vulva and vagina. These tumours are mostly benign, except for the locally aggressive deep angiomyxomas. Often these tumours pose diagnostic challenges, resembling certain malignant vulvo vaginal tumours.This case series highlights clinicopathological features of four angiomyxomas; a single angiomyofibroblastoma, and another superficial myofibroblastoma, including their clinical outcomes. All patients were in their 4th or 5th decade of life. Only 1/4 angiomyxomas was correctly diagnosed at the referring hospitals. Three out of four patients harbouring angiomyxomas achieved clinical remission post-surgery, while one patient was lost to follow-up. By immunohistochemistry, tumor cells showed variable positivity for desmin, SMA, ER, and PR, and negativity for S100P and CD34. The angiomyofibroblastoma was initially misdiagnosed as a liposarcoma, and the patient was lost to follow-up after diagnosis. Immunohistochemically, the tumor cells were diffusely positive for SMA and ER; weakly and focally positive for desmin, and negative for AE1/AE3, CD34, and S100P. The patient with superficial myofibroblastoma is in clinical remission post-excision with an 18-month follow-up. Immunohistochemically, the tumor cells showed CD34 positivity. Therapeutically, none of the patient received adjuvant treatment, except for a single patient with angiomyxoma, who underwent chemoradiation for a synchronous cancer cervix post-surgery.This case series provides valuable insights into the clinical heterogeneity, diagnostic intricacies, and outcomes of vulvo-vaginal stromal tumours from an oncology centre in India, further contributing to a better understanding of these rare tumours.

Primitive myxoid mesenchymal tumour of infancy (PMMTI) is a rare mesenchymal tumour that typically appears in those under 6 months of age and preferentially affects the deep soft tissues of the trunk and paravertebral spinal regions. PMMTI has only recently been described, and there is scarce literature reporting cases regarding the management paradigm of the tumour. We report the case of an 11-week-old male who presented with bilaterally reduced movement and brisk reflexes in his lower limbs, and irritability. Despite numerous radiological investigations, including MRI, PMMTI was only diagnosed upon biopsy and histopathology. Although PMMTI is known to be relatively unresponsive to chemotherapy, we observed a notable decrease in tumour size after a series of chemotherapy sessions. After two-staged surgical resection of the tumour, the patient is currently stable and under close follow-up. In this article, we aim to report on the patient's clinical presentation, investigations, diagnosis, and treatment, while also discussing the findings from a review of the literature pertaining to future approaches in managing PMMTI. Overall, this case highlights the importance of considering PMMTI in the differential diagnosis of deep soft tissue tumours in young infants and the potential for a combination of chemotherapy and surgical resection to be effective in treating this rare tumour. Myofibroblastoma (MFB) of the breast is a rare benign neoplasm of the mammary stroma. Several morphologic variants have been described in the literature, which can create diagnostic challenges for pathologists, in particular the epithelioid variant of MFB, which can mimic invasive lobular carcinoma. We report a case of a 72-year-old female who presented for a painless breast lump and was later found to have 2 lesions on imaging, with 1 lesion corresponding to the palpable lump and the other lying in a different quadrant. Core-needle biopsies demonstrated ductal carcinoma in-situ at both lesional sites with what was originally felt to be an invasive lobular carcinoma at the lesional site which did not correspond to the palpable lump. After mastectomy, with more complete visualization microscopically of the lesional area originally felt to be an invasive lobular carcinoma, the final pathology was consistent with a MFB, predominantly epithelioid variant, in addition to ductal carcinoma in-situ and lobular carcinoma in-situ. In this paper we describe the imaging findings of an epithelioid MFB and how its nonspecific nature necessitates close communication between the radiologist and pathologist to make the correct diagnosis.

Superficial myofibroblastoma is a rare benign mesenchymal tumor that presents a challenge in accurate preoperative diagnosis because of its overlapping radiological and histological features. A 27-year-old woman presented with a history of increasing abdominal girth over the prior year and pelvic mass for 1 month. Imaging confirmed the presence of a giant well-circumscribed cystic-solid tumor involving both the extraperitoneal pelvis and vagina. After exploration and excision, superficial vaginal myofibroblastoma was diagnosed pathologically. The patient underwent surgical excision and had no postoperative complications at the 1-month follow-up. Imaging features and clinical reasoning can aid in differentiating superficial myofibroblastoma from more aggressive entities or malignant tumors and guide suitable and appropriate surgical approaches.

Mammary myofibroblastoma is a rare mesenchymal neoplasm that typically presents in older men and women. Less commonly, these benign tumors may also occur in soft tissues located outside of the breast, in which case they are referred to as mammary-type myofibroblastomas. The histologic composition of this benign spindle cell tumor can be markedly varied. We present one such case of myofibroblastoma of the male breast, describing its sonographic appearance and its diagnosis using ultrasound-guided core biopsy.

This letter to the editor discusses the diagnostic challenge in distinguishing between myofibroblastoma and spindle cell lipoma (SPL), two types of orbital tumors that share similar imaging features. The authors emphasize the importance of relying on histopathological findings, including immunohistochemistry (IHC), for accurate diagnosis. However, they caution that both tumor types can display overlapping morphological and genetic features, making it difficult to differentiate between them solely based on IHC. Myofibroblastoma is a rare benign breast stromal tumor comprised of fibroblasts and myofibroblasts, most commonly presenting in postmenopausal women and older men. This report discusses the case of a 69-year-old male diagnosed with myofibroblastoma after mammography, targeted ultrasound, and core needle biopsy of a retroareolar mass. The features of myofibroblastoma are presented.

Introduction Extramammary myofibroblastomas are extremely rare. Case presentation The patient was an 88‐year‐old male. He presented for evaluation of frequent urination and a “pushing up” sensation from the groin during defecation. Thorough physical and radiographic examinations revealed a retroperitoneal tumor on the right side of the rectum. The pathologic examination of the biopsy tissue showed that the tumor was unlikely to be malignant. Nevertheless, the patient was symptomatic and thus underwent a laparoscopic tumor resection through a transperitoneal approach. The tumor was circumscribed with a solid capsule. Based on the pathologic findings, which included immunostaining, the tumor was diagnosed as a myofibroblastoma. There was no evidence of a recurrence 6 months postoperatively. Conclusion We present this case with the clinical course and surgical findings, and discuss the possibility of establishing a preoperative pathologic diagnosis of a myofibroblastoma.

Myofibroblastoma is a rare, benign mesenchymal tumor first described as a neoplasm of the breast. Extramammary myofibroblastoma is a histopathologically and genetically identical lesion occurring outside the breast. Herein is presented a case of extramammary myofibroblastoma arising in the oral cavity. A 59‐year‐old woman presented with a 1.5 cm nodule on the buccal surface of the lower lip. Wide local excision was performed. Histopathologic examination revealed haphazard fascicles of monomorphic spindle cells with hyalinized collagen bundles without fat. The spindled cells were diffusely positive for CD34, and focally for progesterone receptor. Desmin, smooth muscle actin, estrogen receptor, androgen receptor, S100, and STAT6 were negative. Rb1 expression was lost in tumor cells. Thus, the diagnosis of extramammary myofibroblastoma was made. Differential diagnoses include spindle‐cell lipoma and angiofibroma. All three tumors are members of the 13q14 deletion/RB1 loss family. Indolent but locally aggressive (solitary fibrous tumor, desmoid fibromatosis) and frankly malignant (low‐grade peripheral nerve sheath tumor, dermatofibrosarcoma protuberans) entities can be excluded by immunohistochemistry and careful microscopic examination. Extensive sampling extramammary myofibroblastoma is important to exclude the possibility of malignancy. Introduction Mammary‐type myofibroblastoma is a rare benign tumor, mainly arising along the embryonal mammary ridge. We report a rare case of mammary‐type myofibroblastoma of the perineum. Case presentation A 37‐year‐old Japanese man presented with a 20 mm, progressively‐growing painless mass in the right perineum. Computed tomography showed a subcutaneous tumor with a strong contrast effect. Upon total resection, pathology showed a spindle‐cell tumor positive for desmin but negative for CD34. Further immunohistochemistry showed loss of Rb expression, leading to differential diagnosis. We could not evaluate the exact rarity of the perineal location due to categorization in past reports. Conclusion Due to the similarities between mammary and anogenital tissue, we suggest that tallying perineal and vulvar areas separately from the embryonic mammary ridge sites may be beneficial in gaining insight into the pathophysiology of this tumor.

Mammary myofibroblastoma (MFB) is a rare benign spindle cell tumour predominantly affecting males, but also observed in postmenopausal females. Its diagnosis remains challenging owing to overlapping histological features with malignant lesions and limited tissue sampling in core biopsies. We present a case of incidentally discovered mammary MFB in a 63-year-old man and review its clinical, radiological and histopathological characteristics. The patient, who had a history of distal pancreatectomy and splenectomy, presented with an incidental left anterior chest wall nodule discovered on computed tomography scan. Clinical examination revealed a benign left retroareolar lump, confirmed by breast ultrasound and mammography. Ultrasound-guided core biopsy demonstrated characteristic spindle cells, prompting immunohistochemical staining confirming the diagnosis of MFB. The lesion was surgically excised with clear margins. Mammary MFB is commonly seen in postmenopausal women and older men, presenting as painless, mobile breast lumps. Imaging findings are nonspecific, resembling fibroadenomas or fat necrosis. Histologically, MFB lacks mammary ducts or lobules and displays characteristic spindle cells with collagenous stroma. Immunohistochemistry aids in differentiating it from other spindle cell tumours. Surgical excision is curative, with no reported cases with recurrence. Mammary MFB should be considered in the differential diagnosis of breast masses in males and postmenopausal women. Despite the challenges in diagnosis, its benign nature and favourable prognosis warrant timely recognition and appropriate management through surgical excision. Further research is needed to establish clear management guidelines and explore its underlying pathogenesis.

Myofibroblastoma (MFB) of the breast is an uncommon entity of benign spindle neoplasms of the breast. This tumour possesses a broad spectrum of histomorphological patterns. Distinguishing of myofibroblastoma variants from malignant mimics of this benign neoplasm is essential for pathologists to avoid further invasive surgical procedures. In this article, we report the clinical, morphological, and immunohistochemical features of three cases, including two females and one male patient with mammary myofibroblastoma with emphasis on the histomorphological findings. As there is not yet enough information about MFB, more reports of MFB are still required to more clarify the pathogenesis and potential predisposing factors of this rare type of breast tumours.

Myofibroblastomas are rare benign spindle cell tumors that occur within both male and female breasts. They are composed of fibroblasts and myofibroblasts and are not associated with malignant potential. On mammographic and sonographic imaging, these tumors may present as oval circumscribed masses that overlap with the appearance of many benign entities, including fibroadenomas. Occasionally, the tumors may demonstrate interval growth or mimic imaging features of malignancy and require biopsy. Correct pathologic diagnosis is important because many morphologic variants exist, which complicates pathologic interpretation. Inflammatory myofibroblastic tumor (IMT) is a rare benign neoplasm with variable clinical presentation. We hereby present a case of a 27-year-old female who presented to us with an unsuspecting gingival overgrowth in her anterior gingiva. This article aims to describe IMT as a rare intraoral entity which may involve the anterior maxillary gingiva, involving or sparing the underlying bone, as only a handful of such cases have been reported so far. Surgical excision is the mainstay of treatment. Biopsy and histopathological examination of even the smallest lesions is a must. Timely diagnosis and intervention can prevent severe morbidity that can arise if these lesions are left undiagnosed as aggressive management of lesions that become extensive is mandatory. Abstract Background Myofibroblastoma (MFB) and low-grade adenosquamous carcinoma (LGASC) are rare tumours in the breast, respectively. However, a collision tumour of the two types has never been reported. Case presentation A 42-year-old female presented with a palpable mass in diameter of about 2.5 cm in the left breast. Morphologically, the lesion was predominately composed of bland spindle cells admixed with some islands of mature adipocytes and a few epithelial elements dispersing in infiltrating way which formed both tubule and solid structures. The mass showed low positive index of Ki-67. The spindle cells were strongly and diffusely positive for CD34, SMA, desmin, ER and PR. The epithelial elements were positive for CK and EMA, and negative for ER and PR completely. CK5/6 and P63 were positive in the outer-layer of the tubules and nearly all the cells of the solid nests. Conclusions A collision tumour of MFB and LGASC in breast is extremely rare and either component is supposed to be not overlooked. Excision and close follow-up are advised.

Introduction. Myofibroblastomas of the breast are rare mesenchymal lesions thought to be derived from a CD34-positive mammary stromal cell that shows a great capacity for multipotency. Cases presentation. We performed a retrospective analysis of 4 cases with confirmed mammary myofibroblastomas. Hematoxylin and eosin slides and immunohistochemical slides were reviewed. For next-generation sequencing studies we used a TruSight Tumor 15 panel which ran in a MiSeq sequencer and was analyzed with illumina VariantStudio 3.0 and IGV (Integrative Genomic Variation). The case series consisted of 4 patients (3 men, 1 woman). The anatomic location distribution was equal, with a 1:1 ratio between the left and right breast and a gross median size of the lesion of 34 mm. Gross features revealed a well circumscribed lesion with a vaguely lobular cut surface and the microscopical features were that of a classic-type MFB composed of benign, spindle-shaped cells interrupted by thick collagen fibers that show extensive hyalinization. CD34 and desmin immunohistochemical markers were diffusely positive in tumor cells, thus confirming the diagnosis. Next generation sequencing used a TruSight Panel 15 and revealed mutations in TP53, p.P72R gene in two cases and in ERBB2 p.1655V, KIT c.2484C>T, GUSB p.V270M genes in one case. No pathogenic mutations were identified. Conclusions. The diagnosis of these tumors is challenging based on the rareness of these lesions. Molecular analyses are important to confirm the diagnosis based on morphology and immunohistochemistry. Given the fact that these tumors are rare and benign, there is little data about the molecular signature of this tumor.

Myofibroblastoma (MFB) of the breast is a rare benign mesenchymal tumour that can show morphological heterogeneity and occur in both sexes over a wide age range. It may present a diagnostic dilemma on radiology and cytomorphology where it can be confused with other benign and malignant breast lesions. Histology and immunohistochemistry are required to make accurate diagnosis of MFB. Surgical excision with long-term follow up is recommended as is being done for index patient. We present a case of MFB of the breast in a 48-year-old woman with a literature review.

Spindle cell tumor is very rare. Herein, we report a case of retroperitoneal spindle cell tumor in a 52-year-old female. The patient first presented with a complaint of persistent pain in the right upper abdomen. In the follow-up, a CT scan was performed and showed a retroperitoneal soft tissue density mass measuring 11 cm in diameter. Then, a subsequent operation was performed, and we completely removed the tumor and partially invaded lesions. The tumor was histologically diagnosed as a spindle cell tumor. Therefore, it is imperative for us to enhance the understanding of this seldom found tumor. Surgery remains the best option for treatment.

Non-neoplastic tumor-like conditions of the liver can appear similar to hepatic neoplasms. In many cases, a biopsy is required to confirm the pathology. However, several tumor-like conditions can be correctly diagnosed or suggested prospectively, thus saving patients from unnecessary anxiety and expense. In this image-focused review, we present the ultrasound, computed tomography, magnetic resonance imaging, and positron emission tomography scan features of eight such entities. Clues that indicate the correct pathology are discussed, and the usual clinical setting is described. Many of these lesions are treated differently from true neoplasms, and the current treatment plan is discussed in many of the cases presented. After reviewing this article, the reader will have a better understanding of these lesions and the situations in which they should be included in the differential diagnosis.

Myofibroblastoma (MFB) is a rare benign mesenchymal tumor usually occurring in the breast parenchyma. This tumor can present as a palpable nodule or can be incidentally detected as a nonpalpable mass on routine screening mammogram. We first report a rare case of histologically proven MFB of the breast revealed by fluoro‐deoxyglucose uptake on PET‐CT examination in a patient with a lung nodule. Tumor exhibited an unusual morphology, being predominantly composed of polygonal, epithelioid, and deciduoid‐like cells set in a myxoid stroma. The most striking feature was the multifocal presence of atypical/bizarre, mono/bi‐nucleated cells that, in addition to diffuse myxoid stromal changes, were a concern of malignancy, especially on core biopsy. The final diagnosis of MFB was achieved on surgically resected specimen and, similarly to other benign soft tissue tumors (especially leiomyoma and schwannoma/neurofibroma), the term "bizarre cell MFB of the breast" is proposed to emphasize the degenerative/reactive nature of the atypia. Atypical spindle cell/pleomorphic lipomatous tumor (ASPLT) is a newly accepted entity that belongs to the group of low-grade adipocytic neoplasms. ASPLT commonly manifests a soft tissue mass in both upper and lower extremities but is extremely rare in the gastrointestinal tract. Here we report a case of a gastric ASPLT in a 59-year-old male, who presented for the evaluation of new onset of dysphagia. To our best knowledge, this is the first case report of ASPLT in the upper gastrointestinal tract.

Myofibrolastoma of the breast (MFB) is a rare and benign stromal tumor. Although MFB is a benign entity, in radiologic evaluation, there is no specific diagnostic feature. Conventional breast imaging findings have been published before. Sonoelastography has been used as an imaging method that allows us to evaluate tissue stiffness in vivo and increases the specificity of B-mode ultrasonography in the discrimination of benign-malignant lesions. In this case report, it was shown that MFB is of high stiffness in shear wave elastography (SWE) evaluation. Abstract We reported a 65‐year‐old male with a benign mammary myofibroblastoma. This report shows that not all masses of male breast are malignant. To differentiate benign masses from malignant neoplasms, careful preoperative diagnosis should be performed. Preoperative diagnosis of the tumor prevents placing a huge financial and mental burdens on patients.

Myofibroblastoma of the breast is a rare mesenchymal tumor, probably originating from stromal fibroblasts. In this case report, cyto-histopathological findings of myofibroblastoma of the breast are discussed. A 68-years-old male patient was admitted to our clinic with a rapidly growing, painless mass in the breast. Radiological examination revealed a mobile, solid mass resembling to fibroadenoma. Fine needle aspiration revealed spindle cell clusters with fibrous stroma and a clear background. Hypocellular aspirates demonstrated neoplastic cells with cyanophilic cytoplasm and mild/moderate pleomorphism, and naked elongated nuclei including fine granular chromatin, nuclear groove and inclusion. The lesion was reported as “benign spindle cell neoplasm”. Tru-cut biopsy revealed short fascicules composed of uniform benign spindle cells with hyalinized collagen bands accompanied by mast cells and patchy lymphoid infiltration. Immunoreactivity was positive with CD34, smooth muscle actin (SMA), desmin and vimentin, but negative with CD117, CD10, Epithelial Membrane Antigen, S-100, pan-cytokeratin and p53. This case was reported as “classic variant of myofibroblastoma”, and the patient was operated in another clinic. There was no recurrence during follow-up. Myofibroblastoma is a rare, benign mesenchymal tumor, usually seen in the breast parenchyma. While the tumor is diagnosed based on the fibro-myofibroblastic differentiation characteristics, other tumors and tumor-like spindle cell lesions should also be kept in mind. The cytological features of 22 cases in the literature were similar. Aspirates are generally hypercellular with mild-moderate pleopmorphism, nuclear groove and inclusions. Histopathology shows positivity for CD34, vimentin, desmin and SMA. Clinical/radiological characteristics and atypia, growth pattern, mitosis and immunohistochemistry should all be evaluated together. Cytomorphology may be the first step in diagnosis; however, immunohistochemical studies should not be ignored in differential diagnosis.

Mammary-type myofibroblastoma (MFB) is a benign spindle cell tumor of the breast and soft tissue characterized by 13q14 alterations leading to loss of Rb-1 protein expression, a feature shared among spindle cell lipoma and cellular angiofibroma. In this article, we present a novel case of MFB arising in the left breast of a 70-year old man that microscopically showed an abrupt transition from classic MFB morphology to an area with cytologic atypia and mitotic activity, akin to sarcomatous transformation described in cellular angiofibromas. A thorough workup of the molecular underpinnings of both components using chromosomal microarray and next-generation sequencing platforms supported a clonal relationship. Nearly identical copy number changes, including a single copy loss of 13q14, were found in both components; in addition, the sarcomatous component harbored biallelic TP53 alterations. Epithelioid myofibroblastoma of the breast is a rare benign stromal tumor occurring in the postmenopausal females and elderly males. We report one such case in a 65-year-old female who underwent trucut biopsy from a well-defined right breast lump, which was misinterpreted as invasive lobular carcinoma on histology. Tumor showed mild to moderately pleomorphic tumor cells arranged in single file pattern, loosely dispersed and focally in sheets with inconspicuous nucleoli embedded in a fibrotic and hyalinized stroma. Immunohistochemistry for E-cadherin was negative further substantiating the morphological diagnosis. However, mastectomy specimen revealed it to be a case of epithelioid myofibroblastoma with the help of a large panel of immunohistochemical stains. Extra-mammary myofibroblastomas are rare benign mesenchymal neoplasms, histologically and immune-phenotypically identical to mammary myofibroblastomas. Histologically, they are characterized by the abundance of spindle cells in hyalinized collagenous stroma with positive expression of CD 34 and desmin. We present an extremely rare case of extra-mammary myofibroblastoma of the orbit in a 29-year – old male who presented with painless proptosis of the right eye, with a description of clinical, radiological, and histological findings. Mammary myofibroblastoma is a rare soft-tissue tumor. Extramammary myofibroblastomas are particularly rare. We here report the case of a 78- year-old man presenting with pelvic pain relieved by defecation or urination. Rectal examination showed a mass in front of the anterior rectal wall. The magnetic resonance imaging (MRI) showed a wellcircumscribed and heterogeneous mass measuring 10 x 6 x 8cm located behind the bladder which was pushed forward in front of the rectosigmoid. Immunohistochemical analyses showed diffuse coexpression on CD34 cells and desmin, Rb expression on most cells, oestrogen receptor expression, intense and diffuse P16 expression and a ki67 proliferation index of 25%. The patient had no recurrence 8 months after radiotherapy followed by surgery. Breast myofibroblastoma is a rare and benign tumor. Recurrence is hardly observed after local treatment. We present a case of low-grade endometrial stromal sarcoma (ESS) that metastasized to the breast 25 years after the original diagnosis, with clinical, radiographic and pathologic follow-up data. The diagnosis of metastatic ESS was supported by the morphologic features of the breast tumor and its immunohistochemical profile, which included positive staining for CD10, WT1, estrogen receptor (ER) and progesterone receptor (PR). The diagnosis of metastatic ESS was confirmed by fluorescence in situ hybridization (FISH) testing, which revealed a characteristic JAZF1 gene rearrangement. The unusual clinical presentation raised a differential diagnosis that included mammary myofibroblastoma of the breast. We compared the morphologic characteristics and immunohistochemical profile of 15 low-grade ESS cases (8 primary and 7 metastatic) to eight cases of mammary myofibroblastoma. Immunostains that differentiated between these entities included CD34, retinoblastoma protein (Rb) and FOXL2. The pathologist can play a key role in the recognition of uncommon metastases to breast, and although rare, metastatic low-grade ESS should be included in the differential diagnosis of spindle cell tumors of the breast.

Background: The category of the “stromal tumors of the lower female genital tract” encompasses a wide spectrum of lesions with variable heterogeneity, which can be nosologically classified on the basis of their morphologic and immunohistochemical profiles as deep (aggressive) angiomyxoma (DAM), cellular angiofibroma (CAF), angiomyofibroblastoma (AMFB) or myofibroblastoma (MFB). Despite the differential diagnosis between these entities being usually straightforward, their increasingly recognized unusual morphological variants, along with the overlapping morphological and immunohistochemical features among these tumours, may raise serious differential diagnostic problems. Methods and Results: The data presented in the present paper have been retrieved from the entire published literature on the PubMed website about DAM, CAF, AFMB and MFB from 1984 to 2021. The selected articles are mainly represented by small-series, and, more rarely, single-case reports with unusual clinicopathologic features. The present review focuses on the diagnostic clues of the stromal tumours of the lower female genital tract to achieve a correct classification. The main clinicopathologic features of each single entity, emphasizing their differential diagnostic clues, are discussed and summarized in tables. Representative illustrations, including the unusual morphological variants, of each single tumour are also provided. Conclusion: Awareness by pathologists of the wide morphological and immunohistochemical spectrum exhibited by these tumours is crucial to achieve correct diagnoses and to avoid confusion with reactive conditions or other benign or malignant entities.

We describe a case of a 37-year-old man with painless left inguinal bulging due to a 5-cm solid mass adjacent to spermatic cord. Ultrasound-guided core needle biopsy was performed and histopathological examination revealed a mammary-type myofibroblastoma. Despite being rare, this is an emblematic case of a spermatic cord tumor which could be managed with marginal surgical excision, preserving spermatic cord, because of preoperative diagnosis. Background: Myofibroblastoma (MFB) and low-grade adenosquamous carcinoma (LGASC) are rare tumours in the breast, respectively. However, a collision tumour of the two types has never been reported. Case presentation: A 42-year-old female presented with a palpable mass in diameter of about 2.5 cm in the left breast. Morphologically, the lesion was predominately composed of bland spindle cells admixed with some islands of mature adipocytes and a few epithelial elements dispersing in infiltrating way which formed both tubule and solid structures. The mass showed low positive index of Ki-67. The spindle cells were strongly and diffusely positive for CD34, SMA, desmin, ER and PR. The epithelial elements were positive for CK and EMA, and negative for ER and PR completely. CK5/6 and P63 were positive in the outer-layer of the tubules and nearly all the cells of the solid nests. Conclusions: A collision tumour of MFB and LGASC in breast is extremely rare and either component is supposed to be not overlooked. Excision and close follow-up are advised. [ABSTRACT FROM AUTHOR]
