A methodology is proposed in this paper allowing the classification of anomalies and subsequently their possible localization in nuclear reactor cores during operation. The method relies on the monitoring of the neutron noise recorded by in-core neutron detectors located at very few discrete locations throughout the core. In order to unfold from the detectors readings the necessary information, a 3-dimensional Convolutional Neural Network is used, with the training and validation of the network based on simulated data. In the reported work, the approach was also tested on simulated data. The simulations were carried out in the frequency domain using the CORE SIM+ diffusion-based two-group core simulator. The different scenarios correspond to the following cases: a generic “absorber of variable strength”, axially travelling perturbations at the velocity of the coolant flow (due to e.g. fluctuations of the coolant temperature at the inlet of the core), fuel assembly vibrations, control rod vibrations, and core barrel vibrations. In all those cases, various frequencies were considered and, when relevant, different locations of the perturbations and different vibration modes were taken into account. The machine learning approach was able to correctly identify the different scenarios with a maximum error of 0.11%. Moreover, the error in localizing anomalies had a mean squared error of 0.3072 in mesh size, corresponding to less than 4 cm. The proposed methodology was also demonstrated to be insensitive to parasitic noise and will be tested on actual plant data in the near future.

At EPFL, the CROCUS reactor has been used to carry out experiments with vibrating fuel rods. The paper presents a first attempt to employ the measured data to validate CORE SIM+, a neutron noise solver developed at Chalmers University of Technology. For this purpose, the original experimental data are processed in order to extract the necessary information. In particular, detector recordings are scrutinized and detrended, and used to estimate CPSDs of detector pairs. These values are then compared with the ones derived from the CORE SIM+ simulations of the experiments. The main trend of the experimental data along with the values for some detectors are successfully reproduced by CORE SIM+. Further work is necessary on both the experimental and computational sides in order to improve the validation process.

An updated Hellenic database of strong ground motion recordings together with the earthquake source properties and the site characterization of the recording stations are described here. The strong motion database consists of 2751 corrected recordings from 497earthquakes of focal depths h4.5and a number of finite fault plane solutions corresponding to earthquakes with M>6.0. The data is compiled from existing catalogs published by Hellenic and international seismological centers. The properties for each recording station include surface geology, a measured or estimated time-averaged shear wave velocity in the upper 30m (VS30), and information on instrument housing. Out of the 338sites, 75 have in-situ shear wave velocity measurements while for the rest VS30 values are derived from proxy-based relationships based on terrain or geology combined with surface gradient.

During earthquake ground shaking earth pressures on retaining structures can cyclically increase and decrease as a result of inertial forces applied to the walls and kinematic interactions between the stiff wall elements and surrounding soil. Limit equilibrium analysis imposes a pseudo-static inertial force to a soil wedge behind the wall (the mechanism behind the widely-used Mononobe-Okabe method), which is a poor analogy for either inertial or kinematic wall-soil interaction. Many basement walls and retaining structures are dominated by kinematic soil-structure interaction (SSI) effects arising from differences in displacement between the wall and the free-field soil. Kinematic SSI solutions are often formulated for uniform soil conditions, but the shear modulus of most soils is known to increase with mean effective stress, and therefore with depth. We examine the influence of vertical heterogeneity of shear modulus on kinematic SSI for rigid walls. An existing free-field displacement solution is presented first, followed by analysis of earth pressure increments using a Winkler assumption. Vertical heterogeneity is shown to reduce seismic earth pressures compared with a uniform soil case (for a given frequency and peak ground surface displacement) because free-field displacements are largest near the surface, where the soil is softest and Winkler stiffness is lowest. The proposed Winkler solution is then compared with an exact analytical solution for vertically heterogeneous soil over a rigid base and retained between two opposing rigid walls. The agreement is imperfect, but reasonable, with differences likely due to assumptions regarding the dynamic Winkler stiffness intensity.

Engineering assessments of ground failure potential commonly express seismic demands in the form of shear stresses or normalizations thereof. In soil materials underlying foundations, referred to as ‘foundation soils’, dynamic stresses result from wave propagation associated with site response in combination with demands associated with soil-structure interaction (SSI). Engineers presently lack a simplified procedure for analysis of SSI-related stress demands. To provide insight into the physics of the SSI-induced stresses, and to lay the ground work for an eventual simplified procedure, this paper presents preliminary solutions for the problem of a dynamic vertical point load on the surface of an elastic half-space (Lamb’s problem) and the resulting stresses within foundation soils. Results are presented in dimensionless graphical form and indicate the amplitude and phase of dynamic stresses.

Foundation damping incorporates the combined effects of energy loss due to waves propagating away from the vibrating foundation (radiation damping), as well as hysteretic action in the soil (material damping). Two closed-form solutions for foundation damping of a flexible-based single degree-of-freedom oscillator were derived from first principles where the inherent (structural) damping ratio, radiation damping and the soil hysteretic damping ratio appear as variables. Since both formulations are theoretically defensible, yet provide numerically distinct results, we look to case histories for validation. In addition to previously established case histories we evaluated data derived from forced vibration testing of a field test structure installed at two sites in California. Parametric system identification was employed to evaluate the fixed-base and flexible-base first mode period and damping ratio during tests performed across a wide frequency range and multiple load amplitudes. Foundation damping derived from these results is shown to favor one of the theoretical models over the other.

Abstract Background The field of long COVID research is rapidly evolving, however, tools to assess and monitor symptoms and recovery of the disease are limited. The objective of the present study was to develop a new patient-reported outcomes instrument, the Symptoms Evolution of Long COVID‑19 (SE-LC19), and establish its content validity. Methods The 40-item SE-LC19 instrument was developed based on patient-relevant empirical evidence from scientific literature and clinical guidelines that reported symptoms specific to long COVID. A 2-part mixed-method approach was employed. Part 1: Qualitative interviews with a purposive sample of 41 patients with confirmed long COVID were conducted for the content validation of SE-LC19. During cognitive debriefing interviews, patients were asked to describe their understanding of the instrument’s instructions, specific symptoms, response options, and recall period to ensure its relevance and comprehensiveness. Five clinicians of different medical specialties who regularly treated patients with long COVID were also interviewed to obtain their clinical expert opinions on SE-LC19. Part 2: Exploratory Rasch Measurement Theory (RMT) analysis was conducted to evaluate the psychometric properties of the SE-LC19 data collected during the interviews. Results Overall, patients reported that the instructions, questions, recall period, and response options for SE-LC19 were comprehensive and relevant. Minor conceptual gaps reported by patients captured nuances in the experience of some symptoms that could be considered in future studies. Some patients suggested a revision of the recall period from 24 h to 7 days to be able to capture more symptoms given the waxing and waning nature of some symptoms. Clinicians found the instrument comprehensive with minimal suggestions regarding its content. Exploratory RMT analyses provided evidence that the SE-LC19 questionnaire performed as intended. Conclusion The present mixed-methods study in patients with confirmed long COVID supports the content validity and applicability of the SE-LC19 instrument to evaluate the symptoms of patients with long COVID. Further research is warranted to explore the psychometric properties of the instrument and refine a meaningful and robust patient-relevant endpoint for use in different settings such as clinical trials and clinical practice to track the onset, severity, and recovery of long COVID.

We report on results from triaxial drained monotonic and cyclic compression tests for four different aggregates (gabbro, limestone, demolished concrete, steel slag), and discuss the applicability of UBCSAND model for simulating the relevant non-linear stress-strain curves. First, UBCSAND is integrated analytically for the elastic and plastic volumetric and deviatoric strains, leading to a pair of novel closed-form solutions expressed in terms of hypergeometric functions. These solutions are inherently free of numerical errors and can be used for the evaluation of results from numerical integration. Second, three calibration techniques for the model parameters are presented and compared: (1) a graphical “chart” solution based on results from forward numerical simulations; (2) a numerical approach based on minimization of residuals; and (3) an informed trial-and-error approach based on the analytical solution. Errors in numerical integration of the model are explored by comparing the analytical solution against results from an Euler integration. Variations in the calibrated parameters are discussed for drained monotonic and cyclic triaxial loading considering different types of aggregates.

This article aims to constitute a noteworthy contribution to the domain of direction-of-arrival (DoA) estimation through the application of deep learning algorithms. We approach the DoA estimation challenge as a binary classification task, employing a novel grid in the output layer and a deep convolutional neural network (DCNN) as the classifier. The input of the DCNN is the correlation matrix of signals received by a $4 \times 4$ uniformly spaced patch antenna array. The proposed model's performance is evaluated based on its capacity to predict angles of arrival from any direction in a three-dimensional space, encompassing azimuth angles within the interval $[0^\circ, 360^\circ)$ and polar angles within $[0^\circ, 60^\circ ]$. We aim to optimize the utilization of spatial information and create a robust, precise, and efficient DoA estimator. To address this, we conduct comprehensive testing in diverse scenarios, encompassing the simultaneous reception of multiple signals across a wide range of signal-to-noise ratio values. Both mean absolute error and root mean squared error are calculated to assess the performance of the DCNN. Rigorous comparison with conventional and state-of-the-art endeavors emphasizes the proposed model's efficacy.

Abstract An adult domestic short‐haired feline leukemia virus‐infected cat was referred for kidney failure and worsening anemia requiring transfusions. ABC blood typing was performed with an immunochromatographic strip assay at different occasions. Gel column systems were used for the major and minor crossmatching tests, and anti‐A and anti‐B titers were determined. No discrete A or B bands appeared on the immunochromatographic strips at any time point for the recipient cat. The recipient's plasma agglutinated RBCs from tested type A and B cats. The recipient's RBCs appeared compatible with plasma from 1 type A and 2 B donors, and incompatible with plasma from another type A cat. Genotyping of recipient blood revealed a single homozygous c.179G>T CMAH variant predicting a blood type B. These studies suggest an unusual weak type B or missing all ABC antigens. The latter resembles the exceedingly rare Bombay phenotype in the human ABO blood group system.

On October 30, 2020 14:51 (UTC), a moment magnitude (Mw) of 7.0 (USGS, EMSC) earthquake occurred in the Aegean Sea north of the island of Samos, Greece. Turkish and Hellenic geotechnical reconnaissance teams were deployed immediately after the event and their findings are documented herein. The predominantly observed failure mechanism was that of earthquake-induced liquefaction and its associated impacts. Such failures are presented and discussed together with a preliminary assessment of the performance of building foundations, slopes and deep excavations, retaining structures and quay walls. On the Anatolian side (Turkey), and with the exception of the Izmir-Bayrakli region where significant site effects were observed, no major geotechnical effects were observed in the form of foundation failures, surface manifestation of liquefaction and lateral soil spreading, rock falls/landslides, failures of deep excavations, retaining structures, quay walls, and subway tunnels. In Samos (Greece), evidence of liquefaction, lateral spreading and damage to quay walls in ports were observed on the northern side of the island. Despite the proximity to the fault (about 10 km), the amplitude and the duration of shaking, the associated liquefaction phenomena were not pervasive. It is further unclear whether the damage to quay walls was due to liquefaction of the underlying soil, or merely due to the inertia of those structures, in conjunction with the presence of soft (yet not necessarily liquefied) foundation soil. A number of rockfalls/landslides were observed but the relevant phenomena were not particularly severe. Similar to the Anatolian side, no failures of engineered retaining structures and major infrastructure such as dams, bridges, viaducts, tunnels were observed in the island of Samos which can be mostly attributed to the lack of such infrastructure.

On October 30, 2020 14:51 (UTC), a moment magnitude (M) 7.0 (USGS, EMSC) earthquake occurred in the Aegean Sea. This paper presents the reconnaissance findings regarding the site effects on recorded strong ground motion intensities and duration, along with the resulting induced-structural damage in Izmir Bay and Samos Island, respectively. In all rock records, relatively high intensity long period rock spectral accelerations were observed in the mid to long period range of 0.5–1.5 s, which are attributed to the source, more specifically, to the slower rupture-mechanism of the event. These rich spectral intensities were further amplified by soil site effects and soil-superstructure resonance, leading to two to six times amplified overall responses and prolonged seismic shaking durations, more pronounced in Bayrakli and other Izmir Bay sites in Turkey. However, these amplified and prolonged excitations are still below design basis earthquake levels, which addresses the lack of proper structural design and construction deficiencies, as the underlying causes for the collapse to heavy damage performance of 795 buildings. On the other hand, although located only about 10 km from the rupture (22 km from the epicenter) and within the near fault zone, the town of Vathy on Samos Island (Greece) was rather lightly affected by the earthquake, with relatively few collapsed or heavily damaged buildings, partially attributed to the low height/low weight of structures in the area. However, a concentration of damage in low-rise buildings in Ano Vathy hill is considered indicative of a combination of coupled valley and topography effects on the strong motion. This event once again addressed the need to develop region-specific zonation and provisions, when more general code practices are proven to be inadequate to assess these extreme site effects.

BackgroundLevels of plasma SARS-CoV-2 nucleocapsid (N) antigen may be an important biomarker in patients with COVID-19 and enhance our understanding of the pathogenesis of COVID-19.ObjectiveTo evaluate whether levels of plasma antigen can predict short-term clinical outcomes and identify clinical and viral factors associated with plasma antigen levels in hospitalized patients with SARS-CoV-2.DesignCross-sectional study of baseline plasma antigen level from 2540 participants enrolled in the TICO (Therapeutics for Inpatients With COVID-19) platform trial from August 2020 to November 2021, with additional data on day 5 outcome and time to discharge.Setting114 centers in 10 countries.ParticipantsAdults hospitalized for acute SARS-CoV-2 infection with 12 days or less of symptoms.MeasurementsBaseline plasma viral N antigen level was measured at a central laboratory. Delta variant status was determined from baseline nasal swabs using reverse transcriptase polymerase chain reaction. Associations between baseline patient characteristics and viral factors and baseline plasma antigen levels were assessed using both unadjusted and multivariable modeling. Association between elevated baseline antigen level of 1000 ng/L or greater and outcomes, including worsening of ordinal pulmonary scale at day 5 and time to hospital discharge, were evaluated using logistic regression and Fine-Gray regression models, respectively.ResultsPlasma antigen was below the level of quantification in 5% of participants at enrollment, and 1000 ng/L or greater in 57%. Baseline pulmonary severity of illness was strongly associated with plasma antigen level, with mean plasma antigen level 3.10-fold higher among those requiring noninvasive ventilation or high-flow nasal cannula compared with room air (95% CI, 2.22 to 4.34). Plasma antigen level was higher in those who lacked antispike antibodies (6.42 fold; CI, 5.37 to 7.66) and in those with the Delta variant (1.73 fold; CI, 1.41 to 2.13). Additional factors associated with higher baseline antigen level included male sex, shorter time since hospital admission, decreased days of remdesivir, and renal impairment. In contrast, race, ethnicity, body mass index, and immunocompromising conditions were not associated with plasma antigen levels. Plasma antigen level of 1000 ng/L or greater was associated with a markedly higher odds of worsened pulmonary status at day 5 (odds ratio, 5.06 [CI, 3.41 to 7.50]) and longer time to hospital discharge (median, 7 vs. 4 days; subhazard ratio, 0.51 [CI, 0.45 to 0.57]), with subhazard ratios similar across all levels of baseline pulmonary severity.LimitationsPlasma samples were drawn at enrollment, not hospital presentation. No point-of-care test to measure plasma antigen is currently available.ConclusionElevated plasma antigen is highly associated with both severity of pulmonary illness and clinically important patient outcomes. Multiple clinical and viral factors are associated with plasma antigen level at presentation. BackgroundEnsovibep (MP0420) is a designed ankyrin repeat protein, a novel class of engineered proteins, under investigation as a treatment of SARS-CoV-2 infection.ObjectiveTo investigate if ensovibep, in addition to remdesivir and other standard care, improves clinical outcomes among patients hospitalized with COVID-19 compared with standard care alone.DesignDouble-blind, randomized, placebo-controlled, clinical trial. A composite safety outcome that included death, serious adverse events, end-organ disease, and serious infections was assessed through day 90.ResultsAn independent data and safety monitoring board recommended that enrollment be halted for early futility after 485 patients were randomly assigned and received an infusion of ensovibep (n = 247) or placebo (n = 238). The odds ratio (OR) for a more favorable pulmonary outcome in the ensovibep (vs. placebo) group at day 5 was 0.93 (95% CI, 0.67 to 1.30; P = 0.68; OR > 1 would favor ensovibep). The 90-day cumulative incidence of sustained recovery was 82% for ensovibep and 80% for placebo (subhazard ratio [sHR], 1.06 [CI, 0.88 to 1.28]; sHR > 1 would favor ensovibep). The protection provided by natural versus hybrid immunity from COVID-19 is unclear. We reflect on the challenges from trying to conduct a randomized post-SARS-CoV-2 infection vaccination trial study with rapidly evolving scientific data, vaccination guidelines, varying international policies, difficulties with vaccine availability, vaccine hesitancy, and a constantly evolving virus.

Abstract Introduction The basophil activation test (BAT) is a flow cytometry laboratory technique that assesses the level of activation indicators expressed on the surface of basophils. We conducted a real-life study in a prospective cohort of patients with reported drug hypersensitivity reactions to determine the true relevance of BAT as a diagnostic tool for assessing immediate hypersensitivity reactions to medicines. Methods We prospectively assessed individuals with clinical suspicion of immediate hypersensitivity reactions to drugs over a 2-year period. The allergological evaluation was carried out in accordance with European Academy of Allergy and Clinical Immunology (EAACI) guidance. All patients underwent BAT using the activation marker CD63. Results In total 13 patients with 54 reported immediate drug hypersensitivity reactions to medications were included in this study. Twelve were female (92.3%) and one was male (7.70%). The mean ± SD age of the patients was 47.31 ± 19.94 years. Antibiotics were tested in 35.2% (19/54) of patients, corticosteroids in 24.1% (13/54), iodinated contrast medium in 14.8% (8/54), and NSAIDs in 5.6% (3/54). There was no correlation between the BAT results and the age of patients, gender, type of medication, or time interval between the allergic reaction and BAT procedure. The sensitivity of BAT 5% CD63+ basophils to drugs was 97.6%, specificity was 96% for drug allergies, positive predictive value (PPV) was 94.3%, and negative predictive value (NPV) was 95.2%. Conclusions The sensitivity of BAT for drug allergies is limited, but it can nevertheless be very helpful before contemplating provocation testing in cases of life-threatening drug allergies where patients cannot be rechallenged or in cases of medications for which no other tests are available or their results are ambiguous.

Invasive fungal infections continue to increase as at-risk populations expand. The high associated morbidity and mortality with fungal diseases mandate the continued investigation of novel antifungal agents and diagnostic strategies that include surrogate biomarkers. Biologic markers of disease are useful prognostic indicators during clinical care, and their use in place of traditional survival end points may allow for more rapid conduct of clinical trials requiring fewer participants, decreased trial expense, and limited need for long-term follow-up. A number of fungal biomarkers have been developed and extensively evaluated in prospective clinical trials and small series. We examine the evidence for these surrogate biomarkers in this review and provide recommendations for clinicians and regulatory authorities.

Abstract Infections caused by Staphylococcus aureus, notably methicillin‐resistant S. aureus (MRSA), pose treatment challenges due to its ability to tolerate antibiotics and develop antibiotic resistance. The former, a mechanism independent of genetic changes, allows bacteria to withstand antibiotics by altering metabolic processes. Here, a potent methylazanediyl bisacetamide derivative, MB6, is described, which selectively targets MRSA membranes over mammalian membranes without observable resistance development. Although MB6 is effective against growing MRSA cells, its antimicrobial activity against MRSA persisters is limited. Nevertheless, MB6 significantly potentiates the bactericidal activity of gentamicin against MRSA persisters by facilitating gentamicin uptake. In addition, MB6 in combination with daptomycin exhibits enhanced anti‐persister activity through mutual reinforcement of their membrane‐disrupting activities. Crucially, the “triple” combination of MB6, gentamicin, and daptomycin exhibits a marked enhancement in the killing of MRSA persisters compared to individual components or any double combinations. These findings underscore the potential of MB6 to function as a potent and selective membrane‐active antimicrobial adjuvant to enhance the efficacy of existing antibiotics against persister cells. The molecular mechanisms of MB6 elucidated in this study provide valuable insights for designing anti‐persister adjuvants and for developing new antimicrobial combination strategies to overcome the current limitations of antibiotic treatments.

Patients with rheumatoid arthritis (RA) who receive immunosuppressive medications have a heightened risk of infection. The goal of our study was to calculate the pooled cumulative incidence and risk of infection in patients with RA treated with Janus kinase inhibitors (JAKi). The PubMed and EMBASE databases were queried for randomized controlled trials comparing patients with RA treated with JAKi (upadacitinib, baricitinib, tofacitinib, peficitinib, or filgotinib), defined as the treatment group, compared with control subjects, defined as participants receiving placebo or treatment regimen that was similar to that of participants in the treatment group, with the exception of JAKi. The primary study endpoint was the relative risk (RR) of any-grade and severe infection. The secondary endpoints were RR and cumulative incidence of opportunistic infections, herpes zoster, and pneumonia. The Stata v17 software was used for all data analysis. Results showed that treatment with baricitinib was associated with an increased risk of any-grade (RR 1.34; 95% CI: 1.19-1.52) and opportunistic (RR 2.69; 95% CI: 1.22-5.94) infection, whereas treatment with filgotinib (RR 1.21; 95% CI: 1.05-1.39), peficitinib (RR 1.40; 95% CI: 1.05-1.86) and upadacitinib (RR 1.30; 95% CI: 1.09-1.56) was associated with increased risk of any-grade infection only. Analysis based on type of infection showed a pooled cumulative incidence of 32.44% for any-grade infections, 2.02% for severe infections, 1.74% for opportunistic infections, 1.56% for herpes zoster, and 0.49% for pneumonia in patients treated with any JAKi during the follow-up period. Treatment with specific JAKi in patients with RA is associated with an increased risk of any-grade and opportunistic infections but not severe infection. Close clinical monitoring of patients with RA treated with JAKi is required to establish the long-term infection risk profile of these agents.

Objectives There is limited qualitative research on patients’ experiences with long COVID-19, and how specific symptoms impact their daily lives. The study aimed to understand patients’ lived experiences of long COVID-19 and to develop a conceptual model representing the symptoms and their impact on overall quality of life.Setting Qualitative study consisting of a comprehensive literature review, and in-depth clinician and patient semistructured interviews.Participants Forty-one adult patients with long COVID-19, of whom 18 (44%) were recruited through Regeneron Pharmaceuticals’s clinical trials and 23 (56%) through recruitment agencies; 85.4% were female and 73.2% were White. Five independent clinicians treating patients with long COVID-19 were interviewed. Concept saturation was also assessed.Primary and secondary outcomes Interview transcripts were analysed thematically to identify concepts of interest spontaneously mentioned by patients, including symptoms and their impacts on daily life, to guide the development of the conceptual model.Results Findings from the literature review and clinician and patient interviews resulted in the development of a conceptual model comprising two overarching domains: symptoms (upper respiratory tract, lower respiratory tract, smell and taste, systemic, gastrointestinal, neurocognitive and other) and impacts (activities of daily living, instrumental activities of daily living, physical impacts, emotional, social/leisure activities and professional impacts). Saturation was achieved for the reported impacts. The symptoms reported were heterogenic; neurocognitive symptoms, such as numbness, ringing in ears, haziness, confusion, forgetfulness/memory problems, brain fog, concentration, difficulties finding the right word and challenges with fine motor skills, were particularly pertinent for several months.Conclusion The conceptual model, developed based on patient experience data of long COVID-19, highlighted numerous symptoms that impact patients’ physical and mental well-being, and suggests humanistic unmet needs. Prospective real-world studies are warranted to understand the pattern of long COVID-19 experienced in larger samples over longer periods of time.

The Samos Island (Aegean Sea) Earthquake occurred on 30 October 2020. It produced a tsunami that impacted coastal communities, ground shaking that was locally amplified in some areas and that led to collapse of structures with 118 fatalities in both Greece and Turkey, and wide-ranging geotechnical effects including rockfalls, landsliding, and liquefaction. As a result of the global COVID-19 pandemic, the reconnaissance of this event did not involve the deployment of international teams, as would be typical for an event of this size. Instead, following initial deployments of separate Greek and Turkish teams, the reconnaissance and documentation efforts were managed in a coordinated manner with the assistance of international partners. This coordination ultimately produced a multi-agency joint report published on the 2-month anniversary of the earthquake, and this special issue. This paper provides an overview of the reconnaissance activities undertaken to document the effects of this important event and summarizes key lessons spanning topic areas from seismology to emergency response.

This prospective, randomized study aimed to assess the anxiolytic efficacy of gabapentin or alprazolam in cats during short-term postoperative hospitalization. Sixty cats were randomly assigned to three groups (gabapentin-treated [100 mg per cat], alprazolam-treated [0.125 mg per cat], or placebo-treated), with treatments administered twice daily for two days. Stress levels were evaluated using Cat Stress Scores, serum cortisol, and glucose concentrations. Pain scores, food consumption, and adverse effects such as sedation were also monitored. Fifty-five cats completed the study. Both medications demonstrated similar reductions in stress levels. Cats receiving gabapentin had lower pain scores, while those receiving alprazolam exhibited significantly increased food intake on the first postoperative day. However, both medications resulted in comparable levels of sedation. In the context of postoperative hospitalization, pharmacological intervention with anxiolytics could be effective in reducing stress levels. Despite potential side effects, gabapentin and alprazolam may contribute to an improved quality of short-term hospitalization for cats.

Defining predictive biomarkers for targeted therapies and optimizing anti-tumor immune response is a main challenge in ongoing investigations. Progastrin has been studied as a potential biomarker for detecting and diagnosing various malignancies, and its secretion has been associated with cell proliferation in the gastrointestinal tract that may promote tumorigenesis. Progastrin is a precursor molecule of gastrin, synthesized as pre-progastrin, converted to progastrin after cleavage, and transformed into amidated gastrin via biosynthetic intermediates. In cancer, progastrin does not maturate in gastrin and becomes a circulating and detectable protein (hPG80). The development of cancer is thought to be dependent on the progressive dysregulation of normal signaling pathways involved in cell proliferation, thus conferring a growth advantage to the cells. Understanding the interaction between progastrin and the immune system is essential for developing future cancer strategies. To that end, the present review will approach the interlink between gastrointestinal cancers and progastrin by exploring the underlying molecular steps involved in the initiation, evolution, and progression of gastrointestinal cancers. Finally, this review will focus on the clinical applications of progastrin and investigate its possible use as a diagnostic and prognostic tumor circulating biomarker for disease progression and treatment effectiveness, as well as its potential role as an innovative cancer target.

An updated Hellenic database of strong ground motion recordings together with the earthquake source properties and the site characterization of the recording stations are described here. The strong motion database consists of 2751 corrected recordings from 497earthquakes of focal depths h4.5and a number of finite fault plane solutions corresponding to earthquakes with M>6.0. The data is compiled from existing catalogs published by Hellenic and international seismological centers. The properties for each recording station include surface geology, a measured or estimated time-averaged shear wave velocity in the upper 30m (VS30), and information on instrument housing. Out of the 338sites, 75 have in-situ shear wave velocity measurements while for the rest VS30 values are derived from proxy-based relationships based on terrain or geology combined with surface gradient.

A simplified analytical solution is derived for the dynamic response of a flexible vertical retaining wall supported on a rotationally compliant footing, subjected to vertically-propagating harmonic S-waves under plane-strain conditions. The wall retains a semi-infinite, homogeneous viscoelastic soil layer of constant thickness and material properties. The proposed solution is based on the Veletsos-Younan simplifying assumption of zero vertical normal stresses in the soil, and negligible variation of vertical displacements with horizontal distance from the wall. A modified integration technique is employed, inspired by the seminal work of Vlasov and Leontiev, which simplifies the analysis by suppressing the vertical coordinate and transforming the governing partial differentialequation into an ordinary one that admits an elementary solution. Both cantilever and top-hinged walls are studied. Closed-form solutions are derived for lateral soil displacements, dynamic soil pressures, and equivalent Winkler springs connecting the wall to the far-field soil. It is shown that for cantilever conditions even a small amount of wall flexibility leads to a strong reduction in soil thrust, while the rotation at the wall base causes an additional decrease in thrust. The predictions of the method are in good agreement with available solutions, while new resultsfor combined wall flexibility and rotational compliance are presented. The proposed approach offers a simpler alternative to the complex elastodynamic solutions of Veletsos and Younan.

Foundation damping incorporates the combined effects of energy loss due to waves propagating away from the vibrating foundation (radiation damping), as well as hysteretic action in the soil (material damping). Two closed-form solutions for foundation damping of a flexible-based single degree-of-freedom oscillator were derived from first principles where the inherent (structural) damping ratio, radiation damping and the soil hysteretic damping ratio appear as variables. Since both formulations are theoretically defensible, yet provide numerically distinct results, we look to case histories for validation. In addition to previously established case histories we evaluated data derived from forced vibration testing of a field test structure installed at two sites in California. Parametric system identification was employed to evaluate the fixed-base and flexible-base first mode period and damping ratio during tests performed across a wide frequency range and multiple load amplitudes. Foundation damping derived from these results is shown to favor one of the theoretical models over the other.

Engineering assessments of ground failure potential commonly express seismic demands in the form of shear stresses or normalizations thereof. In soil materials underlying foundations, referred to as ‘foundation soils’, dynamic stresses result from wave propagation associated with site response in combination with demands associated with soil-structure interaction (SSI). Engineers presently lack a simplified procedure for analysis of SSI-related stress demands. To provide insight into the physics of the SSI-induced stresses, and to lay the ground work for an eventual simplified procedure, this paper presents preliminary solutions for the problem of a dynamic vertical point load on the surface of an elastic half-space (Lamb’s problem) and the resulting stresses within foundation soils. Results are presented in dimensionless graphical form and indicate the amplitude and phase of dynamic stresses.

During earthquake ground shaking earth pressures on retaining structures can cyclically increase and decrease as a result of inertial forces applied to the walls and kinematic interactions between the stiff wall elements and surrounding soil. Limit equilibrium analysis imposes a pseudo-static inertial force to a soil wedge behind the wall (the mechanism behind the widely-used Mononobe-Okabe method), which is a poor analogy for either inertial or kinematic wall-soil interaction. Many basement walls and retaining structures are dominated by kinematic soil-structure interaction (SSI) effects arising from differences in displacement between the wall and the free-field soil. Kinematic SSI solutions are often formulated for uniform soil conditions, but the shear modulus of most soils is known to increase with mean effective stress, and therefore with depth. We examine the influence of vertical heterogeneity of shear modulus on kinematic SSI for rigid walls. An existing free-field displacement solution is presented first, followed by analysis of earth pressure increments using a Winkler assumption. Vertical heterogeneity is shown to reduce seismic earth pressures compared with a uniform soil case (for a given frequency and peak ground surface displacement) because free-field displacements are largest near the surface, where the soil is softest and Winkler stiffness is lowest. The proposed Winkler solution is then compared with an exact analytical solution for vertically heterogeneous soil over a rigid base and retained between two opposing rigid walls. The agreement is imperfect, but reasonable, with differences likely due to assumptions regarding the dynamic Winkler stiffness intensity.
