6. Measures of postural steadiness: differences between healthy young and elderly adults

Abstract

Measures of postural steadiness are used to characterize the dynamics of the postural control system associated with maintaining balance during quiet standing. The objective of this study was to evaluate the relative sensitivity of center-of-pressure (COP)-based measures to changes in postural steadiness related to age. A variety of time and frequency domain measures of postural steadiness were compared between a group of twenty healthy young adults (21-35 years) and a group of twenty healthy elderly adults (66-70 years) under both eyes-open and eyes-closed conditions. The measures that identified differences between the eyes-open and eyes-closed conditions in the young adult group were different than those that identified differences between the eye conditions in the elderly adult group. Mean velocity of the COP was the only measure that identified age-related changes in both eye conditions, and differences between eye conditions in both age groups. The results of this study will be useful to researchers and clinicians using COP-based measures to evaluate postural steadiness.

Abstract

Recent research has emphasized the importance of the metabolic cluster, which includes glucose intolerance, dyslipidemia, and high blood pressure, as a strong predictor of the obesity-related morbidities and premature mortality. Fundamental to this association, commonly referred to as the metabolic syndrome, is the close interaction between abdominal fat patterning, total body adiposity, and insulin resistance. As the initial step in identifying major genetic loci influencing these phenotypes, we performed a genomewide scan by using a 10-centiMorgan map in 2,209 individuals distributed over 507 nuclear Caucasian families. Pedigree-based analysis using a variance components linkage model demonstrated a quantitative trait locus (QTL) on chromosome 3 (3q27) strongly linked to six traits representing these fundamental phenotypes [logarithm of odds (lod) scores ranged from 2.4 to 3.5]. This QTL exhibited possible epistatic interaction with a second QTL on chromosome 17 (17p12) strongly linked to plasma leptin levels (lod = 5.0). Situated at these epistatic QTLs are candidate genes likely to influence two biologic precursor pathways of the metabolic syndrome.

40. Spinal Cord Evaluation by Cortical Evoked Responses

Abstract

• In ten monkeys, selective segmental lesions of the dorsal columns at the upper thoracic and middle cervical levels resulted in almost total attenuation of the cortical evoked potential responses to peripheral nerve stimulation. Conversely isolated segmental dorsal column preservation showed intact transmission of the evoked responses at rostral spinal cord, nucleus ventralis posterior lateralis, and cortical levels. Responses recorded from the intralaminar thalamic nuclei in the region of nucleus centrum medianum were unaffected by dorsal column ablation, but were markedly attenuated following bilateral ventral column ablation.

Abstract

Injuries produced by compression or tension loads delivered either axially or in association with flexion or extension represent the majority of spinal injuries observed in most centers. The spinal cord routinely is injured by ligament failure, or dislocation, with attendant impact to the spinal cord with bone or disk. While a rich literature exists detailing the retrospective evaluation and injury force vectors associated with spinal injury, few quantitative studies are available. This presentation will provide a review of typical clinical findings observed in our institution with a comparison of studies conducted on 50 unembalmed human male cadaver specimens studied with forces applied in compression and transverse to the cervical or thoracolumbar columns. Briefly all specimens were determined to be within normal limits by medical history and x-ray examinations conducted prior to the test. All tissues were x-rayed following each test. Abstract

The temporal events and injury forces and energies following impact of the helmeted and unhelmeted human have not been fully described. To further advance our understanding in this area, a lumped parameter model with one-dimensional motion along the spinal axis has been developed. The muscles and ligaments and overlying tissue are assumed to offer negligible resistance in compression and the skull stiffness is neglected. Torso mass is reduced appropriately to account for the angle of impact of the person with another human or fixed object.
