1. Endoscopıc Anatomy of the Lacrımal Sac: A Cadaverıc Study

Abstract

Objective. To describe the anatomy of the lacrimal sac in relation to the lateral nasal wall by cadaver dissection, and to measure the distances of surgically important landmarks from relevant structures for safer and more efficient surgery.Method. A total of 12 endoscopic dacryocystorhinostomy (DCR) were performed on both sides (right and left) of 6 fresh-frozen cadavers. The distances of the lacrimal sac to the posterior edge of the uncinate process, the frontal process of the maxilla, the maxillary ostium, the nasal vestibule, the middle turbinate attachment and the inferior turbinate were measured. In addition, the width and length of the lacrimal sac were measured.Results. The mean width and length of the lacrimal sac were 5.6 mm and 11.1 mm, respectively. The lacrimal sac was located at 15.2 mm from the posterior edge of the uncinate process, at 35.5 mm from the nasal vestibule, at 13.5 mm from the maxillary ostium, at 12.2 mm from the frontal process of the maxilla, at 8.7 mm from the middle turbinate attachment, and at 7.3 mm from the inferior turbinate.Conclusion. This study provides additional measurements regarding the lacrimal sac and its relationships with nearby landmarks for use in endoscopic dacryocystorhinostomy. The distances of the lacrimal sac to the nasal vestibule, the uncinate process and the frontal process of the maxilla are not as reliable as the middle turbinate attachment for predicting the anatomic localization of the lacrimal sac during DCR.

Abstract

The effects of chronic morphine administration on the rat cerebellum and silver stained nucleolus organizer regions in Purkinje cells were investigated by means of histological, histochemical and histometrical techniques. Thirty-two young (30-32 days of age) Wistar rats (equal numbers of both genders) were randomly divided into 2 groups, as control and morphine-treated, each having equal numbers of both genders, a total of 16 animals. The control animals were injected subcutaneously with 1mL/kg physiological saline, and the morphine-treated rats received 5 mg/kg morphine hydrochloride subcutaneously at daily intervals for 30 days. The thickness of the molecular and granular layers of the cerebellum, the diameters of the nuclei and nucleoli of cerebellar Purkinje neurones, and the number and size of the silver staining nucleolus organizer regions of the Purkinje cell nuclei were determined histomorphometrically. The morphine administration caused slight histological changes in the cerebellum. The molecular layer thickness of the cerebellum was significantly (P, Učinci kronične primjene morfina na mali mozak štakora i na srebrom obojene regije nukleolusne organizacije Purkinjeovih neurona istraživani su histološkim, histokemijskim i histometrijskim tehnikama. Trideset i dva mlada Wistar štakora (dob 30 - 32 dana), u jednakom omjeru spolova, metodom slučajnog izbora podijeljena su u dvije skupine s po 16 jedinki u svakoj skupini. Životinjama u kontrolnoj skupini potkožno je primijenjen 1 mL/kg fiziološke otopine, dok su životinje u pokusnoj skupini dobile 5 mg/kg morfin hidroklorida, potkožno, svaki dan tijekom 30 dana. Za određivanje zadebljanja molekularnih i granulacijskih slojeva maloga mozga, promjera jezgre i nukleolusa Purkinjeovih neurona u malom mozgu te broja i veličine srebrom obojenih regija nukleolusne organizacije Purkinjeovih neurona primijenjena je histomorfometrijska metoda. Abstract

To investigate the morphologic structures and developmental anomalies of the thyroid gland in human fetuses.This study performed in the Department of Anatomy, Meram Faculty of Medicine between February and April in 2002. Fetuses were obtained from the Gynecology Department of the Meram Faculty of Medicine, Selcuk University, and Dr. Faruk Sukan Maternity Hospital (Konya, Turkey). Sixty spontaneously aborted fetuses (30 boys and 30 girls) between the ages of 13.5th and 32.5th weeks, which had no detectable anomalies, were evaluated. The gland was dissected under the microscope. The location of the gland was determined according to the tracheal ring levels and laryngeal cartilage levels. The length, width and thickness of both lobes and isthmus of the gland were measured and then, the developmental anomalies were noted. The obtained data were statistically analyzed by Least Squares in variant analysis.Although there was no significant difference regarding to the gender for all parameters except in width of the right lobe (p0.05), difference in the length and width of lobes and the length of isthmus between trimesters was significant (p0.05). All measured parameters were increasing with advancing gestational age. The pyramidal lobe was observed in 18.3% of the cases.Normal dimensions and developmental anomalies of the fetal thyroid, which was shown in this study, may provide useful information for the prenatal diagnosis and in-utero treatment of thyroid dysfunctions.

In this study we aimed to investigate the normal developmental pattern of the human fetal heart in second and third trimester, and to evaluate the correlation of the values with the gestational age (GA).Seventy spontaneously aborted human fetuses with no external malformations were investigated and 11 parameters from each were assessed. The relationship of these parameters with the GA was evaluated by linear regression and correlation analyses using statistical methods. In addition, the values were compared to the gender and sides of the heart using Student t-test. This study was performed at the Department of Anatomy, Meram Medical Faculty, Selcuk University, Konya, Turkey, during the period from 2000 to 2004.There was a statistically significant linear relationship between the heart wall thicknesses, the development of the valves of the heart and GA. The mean values of the right side of the heart were found to be higher than those of the left side, and differences were statistically significant (p0.05). Although, most of the parameters were found to be higher in females, only 2 of them were statistically significant. All parameters of the cardiac valves measured were significantly higher in the second trimester. There was no difference regarding the correlation ratio of the trimesters between the pulmonary valve diameter and GA. The measurement of the cardiac valves did not show any differences regarding the gender.This study presents the normal values of the development of the fetal heart in second and third trimester that may provide useful background information for clinical approaches.

10. An ectopic parathyroid gland in the left axillary region: case report

Abstract

This is a review of ectopic localizations of the parathyroid gland in one case. A hard mobile mass (2 x 2.5 cm) was detected on the right side of the neck of a patient who had previously been given a diagnosis of hyperparathyroidism. Computed tomography showed an adenoma (25 x 35 mm) in the paratracheal region on the right side of the neck. Scintigraphy revealed an activity increase on the right side of the neck and in the left axillary region. Blood measurements were as follows: Ca, 12.7 mEq/l; parathyroid hormone (PTH), 235 U/l; and alkaline phosphatase (ALP), 776 U/l. The patient was operated on to remove the lesions located in the cervical and left axillary regions. Histopathological examination revealed that the lesion in the right cervical region was a parathyroid adenoma, whereas the lesion in the left axillary region was normal parathyroid gland tissue. Parathyroid glands may indicate ectopic locations in very different regions. We think the case presented here is the first case with this axillary location. Scintigraphy, alone or combined with other methods, may be used to determine ectopic localizations.

Abstract

The aim of our study was to identify congenital morphological abnormalities of distal vertebral arteries (CMADVA) and their association with cerebral hypoperfusion leading to vertigo, and the role of MR and MRA in the diagnosis of vertebrobasilar (VB) abnormalities.768 patients who complained of dizziness and/or vertigo were included in the study and evaluated by MR and MRA. CMADVAs were determined in 88 of the cases with no other explanation for vertigo and dizziness. 150 patients without dizziness or vertigo were used as a control group. The 3D TOF (Time- of- flight) MR angiographic technique was used.CMADVAs were detected in 88 of 768 patients (11.5%) with vertigo and/or dizziness. We also detected CMADVAs in 4 (2.7%) of 150 persons in the control group. The patients with abnormal vertebral artery findings were classified into ten categories. Lacunar infarcts in the brain stem were found in 8 patients after long-duration vertigo attacks. There was significant correlation between the control and vertigo groups in terms of CMADVA (p=0.0001). After excluding the control group, there was a significant relationship between CMADVA and vertigo and/or dizziness symptoms (p=0.0001).We believe that vertigo and dizziness associated with CMADVA is a real entity that deserves greater attention. For this reason, the vertebrobasilar system in these patients should be examined for CMADVA with MRA. This would enable us to take preventive measures against brainstem ischemia or at least elucidate the etiology of vertigo in these patients.

Introduction: The type of endoscopic approach chosen for vidian neurectomy can be specified by evaluating the vidian canal and the surrounding sphenoid sinus structures. Objective: The variations and morphometry of the vidian canal were investigated, focusing on the functional correlations between them which are crucial anatomical landmarks for preoperative planning. Methods: This study was performed using paranasal multidetector computed tomography images that were obtained with a section thickening of 0.625 mm of 250 adults. Results: The distributions of 500 vidian canal variants were categorized as follows; Type 1, within the sphenoid corpus (55.6%); Type 2, partially protruding into the sphenoid sinus (34.8%); Type 3, within the sphenoid sinus (9.6%). Abstract

Introduction: It is stated that students cannot carry their knowledge on basic medical sciences that they gained with classical education to clinical classes and clinical practice and problem based learning (PBL) can compensate this drawback. It was aimed to evaluate the effect of PBL scenario written on this topic on level of knowledge about childhood poisoning of third class students who completed the most of theoretical pharmacology education.Materials and Method: A questionnaire form prepared by researchers was applied to randomly chosen PBL groups without writing names and on the basis of voluntariness before and after a case of childhood poisoning which was applied as the second PBL scenario in the second midterm of 2009-2010 academic years. Numbers, percentages, chi-square and student’s t-test were used for evaluation of the questionnaire form comprised of demographic data, open-closed ended questions for measuring attitudes and level of knowledge against case of poisoning and statements as making a priority ranking.Results: In the study group, 89 students took part in pre-test and 96 students took part in post-test. Abstract

Anatomy teaching dates from the time of the earliest Medical schools. Traditional anatomy concept at undergraduate study of medicine is based on cadaveric dissection practice. However, somewhere anatomy education is based on modern technology. Those methods are computer simulation, plastic models etc. In our Department of Anatomy cadaveric dissection is an essential method of anatomy teaching. We have noticed a need for further anatomy education after the undergraduate study, during residency. Learning of anatomy is sometimes included in residency programmes, but not as a practice, only as sort of theoretical reminding. There is a need for official anatomy education governed by anatomists in different fields of surgery and other medical professions such as anesthesiology. Also, there is a need for long-life learning of anatomy through thematic courses in all clinical fields. Such courses should be performed on cadaveric specimens and conducted by professional anatomists. Department of Anatomy, Faculty of Medicine, University of Rijeka has more than 50 years of the teaching experience of practice on cadaveric specimens. Such a professional background along with dissection rooms equipped by adequate technology adjusted to clinicians needs, made us recognizable as successful Department of Anatomy and also unique in Croatia.
