Abstract

Introduction: Aggressive non-Hodgkin's lymphoma (NHL) is the most common malignant neoplasm of lymphoid tissue , which often affects the elderly [1]. In 2019, aggressive NHL in patients aged 65 years and older accounted for about a quarter of the estimated 74,000 new cases of NHL in the US [1,2]. Up to two-thirds of patients have peripheral lymphadenopathy. Skin rash, hypersensitivity reactions to insect bites, general fatigue, itching, malaise, fever of unknown origin, ascites, and effusions are uncommon [1]. The standard treatments for aggressive NHL are R-CHOP chemotherapy and other chemotherapy drug combinations. This is one of the standard treatments used in the Republic of Moldova. The purpose of the study. To identify and assess the epidemiological, medico-social peculiarities and treatment approaches in aggressive lymphomas. Methods and Material. The thesis research was carried out on a group of 178 patients, diagnosed with aggressive non-Hodgkin's Lymphoma and treated within the Haematology Departments of the IMPH Oncological Institute of the Republic of Moldova. Results. The treatment of aggressive non-Hodgkin's lymphomas carried out in the Republic of Moldova is the same as in other countries, the treatment of choice being the RCHOP chemo-immunotherapy, which was found as the most effective, with a 3-year complete remission maintenance after the diagnosis of 10 .55%. The 3-year survival rate in aggressive non-Hodgkin lymphomas made up 93.3% for T-cell lymphomas, 100% for Burkitt lymphomas, and a 91.6% rate for large B-cell non-Hodgkin lymphomas .

Abstract

What is not yet known on the issue addressed in the submitted Manuscript Despite the development of new methods of diagnosis and treatment with antineoplastic agents, the results of aggressive NHL treatment remain modest, with frequent relapses and primary refractory forms. Research hypothesis It was carried out a qualitative, analytical study, with a narrative synthesis of the literature, as well as studying the group of patients included in the study to detect the challenges that lead to the delay in establishing the diagnosis of aggressive extranodal NHL. The novelty added by manuscript to the already published scientific literature Conducting a broad literature synthesis to demonstrate the importance of continuing research in the field of extranodal aggressive NHL, which remains a current problem of contemporary hemato-oncology and public health. Introduction: Non-Hodgkin's lymphomas form a group of malignant tumors, which develop from extramedullary hematopoietic cells. They are one of the most common forms of hemoblastosis. Diseases can develop in people of any age. The morbidity of NHL increases with age reaching the highest level in people over 50 years of age. The aim of the study was to identify the causes that lead to difficulties in establishing the diagnosis of aggressive extranodal non-Hodgkin's lymphoma. Material and methods: It was carried out a qualitative, analytical study. The article assumed both the research of the specialized literature and the processing of the information within the research group in order to present the most conclusive data related to the current problems in establishing the diagnosis of aggressive extranodal NHL. Results: Aggressive extranodal NHL remains a major problem, with a clear increase in the annual number of cases globally. This trend is observed in several countries of the world, thus morbidity increases by 3% annually in women and by 4% in men. Worldwide, aggressive NHL continues to predominantly affect the working population. Although patients with primary extranodal NHL tend to present to the medical specialist at a lower stage than those with primary nodal disease, the number of those presenting at advanced stages continues to remain high. Conclusions: Although the diagnosis of NHL does not involve great impediments, patients are often detected in the late stages of the disease either because of late referral to a doctor or because of incorrect diagnosis by primary care physicians. Despite the development of new methods of diagnosis and treatment, extranodal aggressive NHL continues to be a current problem of clinical medicine and public health, requiring increased managerial and financial efforts. 47. Management of elderly patients with chronic myeloproliferative hemopathies

Abstract

Background: Chronic myeloproliferative hemopathies (CMPH) as a whole are the most common chronic leukemias in the elderly in the structure of morbidity by hematological malignancies with primary bone marrow involvement, being characterized in the advanced stages by a severe, recurrent evolution and unfavorable prognosis, with negative socio-economic impact. Material and methods: A clinical, analytical, and descriptive study was carried out along with the narrative review of the international literature on the subject. The study enrolled 91 elderly patients with different phases of chronic myeloid leukemia (CML), primary myelofibrosis (PMF) and polycythemia vera (PV), who were followed up and treated at the Institute of Oncology in the period of 1995–2020. According to the impact score, 25 relevant primary sources were identified and selected having a scientific, reproducible and transparent approach to the relevant subject, followed by data extraction and analysis. Results: The overall one- and 5-year survival in patients aged greater than or equal to 60 years old treated with tyrosine kinase inhibitors (TKIs) was 97.6 and 79%, being lower as compared with the same indices in the totality of CML. In elderly PV patients the overall 5-and 10-year survival made up 93.5% and 76.4%, being lesser than registered in all patients with PV. As reported in the recent references, a significant rate of patients with CMPH underwent reduced working hours, discontinued employment, and medical disability: PMF – 38%, 35%, 33%, and PV – 33%, 28%, and 15%, respectively. Conclusions: The long-term treatment results in elderly patients with CMPH fail compared to those in the CMPH totality, due to the development of age-related diseases and vascular accidents caused by leuko- and thrombocytosis.
