Abstract Various histopathological, clinical and imaging parameters have been evaluated to identify a subset of women diagnosed with lesions with uncertain malignant potential (B3 or BIRADS 3/4A lesions) who could safely be observed rather than being treated with surgical excision, with little impact on clinical practice. The primary reason for surgery is to rule out an upgrade to either ductal carcinoma in situ or invasive breast cancer, which occurs in up to 30% of patients. We hypothesised that the stromal immune microenvironment could indicate the presence of carcinoma associated with a ductal B3 lesion and that this could be detected in biopsies by counting lymphocytes as a predictive biomarker for upgrade. Abstract

The aim of this study is to evaluate factors contributing to poor academic performance of students in higher education. Data was collected in 2017 from 216 students enrolled in different programmes at a higher education provider in New Zealand. The effect of factors relating to family obligations, work and social commitments, and financial concerns were analysed. Results showed significant relationship between academic performance of students and their attendance. Other significant findings included the relationship between attendance and the level of study; work shift; ethnicity; whether participants had any dependents; and how studies were funded. Implications and limitations of this study are presented.

Abstract

Various histopathological, clinical and imaging parameters have been evaluated to identify a subset of women diagnosed with lesions with uncertain malignant potential (B3 or BIRADS 3/4A lesions) who could safely be observed rather than being treated with surgical excision, with little impact on clinical practice. The primary reason for surgery is to rule out an upgrade to either ductal carcinoma in situ or invasive breast cancer, which occurs in up to 30% of patients. We hypothesised that the stromal immune microenvironment could indicate the presence of carcinoma associated with a ductal B3 lesion and that this could be detected in biopsies by counting lymphocytes as a predictive biomarker for upgrade. A higher number of lymphocytes in the surrounding specialised stroma was observed in upgraded ductal and papillary B3 lesions than non-upgraded (p < 0.01, negative binomial model, n = 307). We developed a model using lymphocytes combined with age and the type of lesion, which was predictive of upgrade with an area under the curve of 0.82 [95% confidence interval 0.77-0.87]. The model can identify some patients at risk of upgrade with high sensitivity, but with limited specificity. Assessing the tumour microenvironment including stromal lymphocytes may contribute to reducing unnecessary surgeries in the clinic, but additional predictive features are needed.

7. DEMOCRATIZING LOCAL GOVERNMENT: WHAT MALAYSIA CAN LEARN FROM MEXICO AND INDIA

Abstract

The Pakatan Harapan 2018 election manifesto had established commitments to strengthen the authority of local government, and to make local councils more accountable. This suggested that the local government elections would resume, after an absence of more than 60 years. While the Pakatan Harapan’s tenure as Malaysia’s ruling government was short-lived, their rise to power did put the local government back into the spotlight, as well as elevated the prospects of the resumption of local government elections in the country. There are a number of reasons that supports a more democratic and participatory approach to local government which has been highly hoped for in Malaysia. These include the view that it discourages public malfeasance and produces local officials that are more responsive to the needs of the citizens they serve. However, there are challenges that follow with the introduction of a more democratic form of local government, especially for countries that are attempting to transition from authoritarianism to a democracy at the national level, and contain a diverse multi-ethnic population of differing socio-economic backgrounds. In order to better understand what these challenges are and how they can be overcome, this study examines two case studies, Mexico and India. These two countries have shown some similarities to Malaysia, particularly with regards to regime-type and demographics. This study considers the evolving approach taken by these two countries on their local government, in terms of their structure and practices. It is proposed that the case studies could provide potential lessons for Malaysia. This study begins with a brief historical overview of the system of local government in Malaysia to present day. This is followed by a discussion of the uneven experiences of Mexico and India on the efforts to democratize their local governments. The findings from the examination of both the case studies reveal the manner for which the countries with an authoritarian past often undergo during a transitional period, whereby although the structures of local governance have become more democratic, local officials and citizens are remain trapped in the old authoritarian modes of behavior. Unless interventionist steps are taken, democratizing local government structures alone do not necessarily lead to greater citizen empowerment, especially for those from the more marginalized sections of society.

21. Screening of West Nile Virus, Herpesvirus, and Parvovirus in Rattus spp. in Klang Valley, Malaysia

Abstract

Free-roaming and scavenging lifestyles of Rattus spp. in densely populated urban areas expose them to multiple pathogens that facilitate the transmission of infection to the human population more rapidly, raising public health concerns. There is limited information on the status of rat susceptibility to virus infection, particularly West Nile virus (WNV), herpesvirus, and parvovirus, to prepare for emerging zoonosis. A total of 23 (n = 23) blood samples collected from Rattus spp. in the wet market areas of Klang Valley, Malaysia, were subjected to molecular assay using a one-step reverse transcription-polymerase chain reaction (RT-PCR) to detect the highly conserved region of the WNV capsid and pre-membrane protein via nested polymerase chain reaction (PCR) assay targeting highly conserved amino acid motifs within the herpesviral DNA-directed DNA polymerase gene (DPOL) and polymerase chain reaction (PCR) assay targeting the parvovirus non-structural (NS) protein. As a result, 4 out of 23 (17.39%) rats were positive for herpesvirus DNA, but none were positive for WNV RNA and parvovirus DNA. The positive PCR amplicons of herpesvirus DNA were subjected to partial DNA sequencing analysis, 100% identical to Acomys herpesvirus SVMS 226,222 from Betaherpesvirinae, which is highly suggestive of rat cytomegalovirus (RCMV). This study has successfully demonstrated the presence of RCMV from Rattus spp. in the Klang Valley. The RCMV potentially crosses species barriers and establishes infection, raising public health concerns. The non-viraemic state of WNV or parvovirus infection, low sample size, and limited niche distribution emphasise the need for the expansion of this study in the future.

28. Intra-parotid facial nerve neuroma, an overlooked differential for a parotid lesion

Abstract

Facial nerve neuromas rarely occur in the intra-parotid segment of the facial nerve and when they do, masquerade as a parotid lump. The imaging and clinical features of the intra-parotid facial neuroma overlaps with the more commonly encountered salivary gland neoplasms and is thus overlooked. However, if not recognized, may result in serious and avoidable adverse events for the patient if biopsied or surgically removed. These include pain, facial nerve palsy and cosmetic deformity. In this report, we present the case of a 47-year-old male patient with an intra-parotid facial nerve neuroma with images including computer tomography, magnetic resonance imaging and intra-operative photos. The lesion was eventually left in situ to avoid facial nerve palsy and preserve cosmesis. A discussion of the imaging characteristics and differential diagnoses is subsequently elaborated.

29. Measuring long-run housing affordability for Malaysian millennial households: a geospatial and income distribution analysis

Abstract

Purpose The purpose of this study is to use a lifetime income measure to evaluate the long-run housing affordability for an understudied cohort of households in the literature – the millennials. The authors do this in the context of Malaysia, measuring long-run affordability for four housing types across geographic locations and income distributions. Design/methodology/approach This study calculates a long-run housing affordability index (HAI) using data on house prices and household incomes. Essentially a ratio of predicted lifetime incomes to house prices, the HAI is computed for four common housing types in Malaysia from 2005 to 2016 and for six states in the country. The HAI is also compared across four income percentiles. Findings The analysis reveals varying patterns of housing affordability among different states in Malaysia. Housing affordability has declined since 2010, with most housing types being unaffordable for millennial-led households with the lowest income. Housing is most affordable for those in the highest income bracket, although even here, there are pockets of unaffordable housing as well. Practical implications Based on the findings, this study proposes three targeted interventions to improve housing affordability for Malaysian millennials. Originality/value This study fills a gap in the literature by examining the long-run housing affordability of Malaysian millennial-led households based on both geographic location and income distribution. The millennial population is understudied in the housing affordability literature, making this study a valuable contribution to the field.

A man in his 30s with intellectual disability presented with 1month of diarrhoea, weight loss and dyspnoea. Investigations were hampered due to significant anxiety. Laboratory tests detected microcytic anaemia and hypoalbuminaemia. CT demonstrated a fat-containing infiltrate in the mediastinum, mesentery and axillae, and pulmonary ground-glass infiltrates. Biopsy of the axilla showed cystic lymphatic malformations involving adipose tissue and lymph nodes, leading to a provisional diagnosis of generalised lymphatic anomaly. Over the subsequent 4months, the patient’s respiratory status deteriorated, leading to type 1 respiratory failure necessitating intubation. After multidisciplinary discussion, a decision was made to trial bevacizumab, an anti-VEGF agent, with subsequent improvement in respiratory status. While intubated, gastroscopy was performed; duodenal biopsies revealed pathognomonic changes of Whipple’s disease, confirmed on PCR of duodenal and axillae biopsies. This was deemed the most likely unifying diagnosis; antibiotic treatment was commenced, bevacizumab was ceased, and the patient has remained well after 18 months. [ABSTRACT FROM AUTHOR]

Abstract

For more than two decades attempts have been made to identify a subset of women diagnosed with lesions with uncertain malignant potential (B3 lesions) who could safely be observed rather than being treated with surgical excision and/or chemoprevention. Various histopathological, clinical and imaging parameters for risk recommendation have been evaluated, with little impact on clinical practice. The primary reason for surgery is to rule out an upgrade lesion to either ductal carcinoma in situ (DCIS) or invasive breast cancer (IBC). While on average 30% of these patients are upgraded after diagnostic biopsy, a large number are over treated,making this an important harm of screening.Here we evaluated stromal lymphocytes from B3 biopsies (n=264) as a predictive biomarker for upgrade. A higher number of stromal lymphocytes were observed in upgraded B3 lesions than non-upgraded (p< 0.01, zero inflated binomial model) for both ductal and papillary lesions (n=174). This observation was validated in an independent cohort (pIn conclusion, we can identify a subset of the patients at risk of upgrade with high specificity. Assessing the tumour microenvironment including stromal lymphocytes may contribute to reducing unnecessary surgeries in the clinic.

Abstract

Background: Recent reports raise the possibility of cerebral amyloid angiopathy (CAA) leading to intracerebral hemorrhage in young adults following childhood neurosurgery, suggesting transmission of amyloid-β (Aβ) through neurosurgical procedures including dura mater grafting. Parenchymal Aβ deposition, and to a lesser extent tau aggregation, similar to that seen in Alzheimer disease, have also been described. Methods: We conducted a database review of 634 consecutive intracerebral hemorrhage patients aged Results: We identified 3 patients aged in their thirties who presented with spontaneous lobar intracerebral hemorrhage, with imaging or neuropathology consistent with CAA, and a history of childhood neurosurgery. Two of these patients had undergone a dural repair using cadaveric dura mater (Lyodura). In addition to CAA, both patients had neuropathologically confirmed parenchymal Aβ and tau deposits, characteristic of Alzheimer disease. Conclusions: Our findings support the concept of neurosurgical Aβ transmission but suggest that such cases are rare in standard clinical practice.

Abstract

This study seeks to present the relationship between executive compensation, dividend payout policy and ownership structure of listed firms in Malaysia. We examine a panel data on a sample of 300 of the largest Malaysian public listed companies (PLCs) on Bursa Malaysia for the years 2008 to 2014. Based on 2,009 firm-year observations, our results show consistent empirical positive evidence on the association between dividend payout and executive compensation across all models. However, the results are inconsistent with Bhattacharyya model of dividend payout. Further, there is evidence that government and family ownerships are positively associated with dividend payout. Our results show that the positive relationship between executive compensation and dividend payout is more evident in politically connected firms (PCON firms) which is consistent with the clientele (catering) theory.
