2. Fracture healing in osteopenic bone and the influence of simvastatin

Abstract

In part one of the study 20, 3-month-old female, Wistar rats underwent ovariectomy (Ovx) while a further 20 had a sham procedure to act as controls. Seven weeks later a transverse fracture was created in the proximal tibia of each animal by three-point bending with the resulting fractures supported by an intramedullary wire. Half of the animals in each group were euthanased at two weeks and the remainder at four weeks post fracture with tibiae removed post mortem. All tibiae were then x-rayed. The mechanical properties of half of the healing fractures were ascertained by four-point bending to failure while the remaining specimens were prepared for histological analysis and immunohistochemistry. There were no mechanical differences in the fracture calluses from the ovx animals compared with control at two weeks but by four weeks post fracture the ultimate load at failure of the fractures from the ovx animals were rescued to 71% of that from controls. Stiffness (54%) and stress at yield (74%) were also reduced while the strain at yield was increased by 40% in fractures from the ovx group. In the second part of the study the same animal model was used with the groups once again divided into ovx and sham controls. Half of each group received placebo while the other half received simvastatin 20mg/kg daily for 14 days post fatigue. The same time points and outcome measures were used as in the first part of the study. The dose and method of delivery of simvastatin had no apparent effect on the fracture healing in normal bone. However simvastatin appeared to have a deleterious effect on fracture healing in the osteopenic model causing a reduction in callus size and maturity and reducing the healing fractures’ ability to withstand load. This study does not support a role for simvastatin in the enhancement of fracture healing in osteopenia.

Abstract

Aims: This international study aimed to assess: 1) the prevalence of preoperative and postoperative COVID-19 among patients with hip fracture, 2) the effect on 30-day mortality, and 3) clinical factors associated with the infection and with mortality in COVID-19-positive patients. Methods: A multicentre collaboration among 112 centres in 14 countries collected data on all patients presenting with a hip fracture between 1st March-31st May 2020. Demographics, residence, place of injury, presentation blood tests, Nottingham Hip Fracture Score, time to surgery, management, ASA grade, length of stay, COVID-19 and 30-day mortality status were recorded. Results: A total of 7090 patients were included, with a mean age of 82.2 (range 50–104) years and 4959 (69.9%) being female. Of 651 (9.2%) patients diagnosed with COVID-19, 225 (34.6%) were positive at presentation and 426 (65.4%) were positive postoperatively. Positive COVID-19 status was independently associated with male sex (odds ratio (OR) 1.38, p = 0.001), residential care (OR 2.15, p < 0.001), inpatient fall (OR 2.23, p = 0.003), cancer (OR 0.63, p = 0.009), ASA grades 4 (OR 1.59, p = 0.008) or 5 (OR 8.28, p < 0.001), and longer admission (OR 1.06 for each increasing day, p < 0.001). Patients with COVID-19 at any time had a significantly lower chance of 30-day survival versus those without COVID-19 (72.7% versus 92.6%, p < 0.001). COVID-19 was independently associated with an increased 30-day mortality risk (hazard ratio (HR) 2.83, p < 0.001). Increasing age (HR 1.03, p = 0.028), male sex (HR 2.35, p < 0.001), renal disease (HR 1.53, p = 0.017), and pulmonary disease (HR 1.45, p = 0.039) were independently associated with a higher 30-day mortality risk in patients with COVID-19 when adjusting for confounders. Conclusion: The prevalence of COVID-19 in hip fracture patients during the first wave of the pandemic was 9%, and was independently associated with a three-fold increased 30-day mortality risk. Among COVID-19-positive patients, those who were older, male, with renal or pulmonary disease had a significantly higher 30-day mortality risk.

Aims: This international study aimed to assess: 1) the prevalence of preoperative and postoperative COVID-19 among patients with hip fracture, 2) the effect on 30-day mortality, and 3) clinical factors associated with the infection and with mortality in COVID-19-positive patients. Methods: A multicentre collaboration among 112 centres in 14 countries collected data on all patients presenting with a hip fracture between 1st March-31st May 2020. Demographics, residence, place of injury, presentation blood tests, Nottingham Hip Fracture Score, time to surgery, management, ASA grade, length of stay, COVID-19 and 30-day mortality status were recorded. Results: A total of 7090 patients were included, with a mean age of 82.2 (range 50–104) years and 4959 (69.9%) being female. Of 651 (9.2%) patients diagnosed with COVID-19, 225 (34.6%) were positive at presentation and 426 (65.4%) were positive postoperatively. Positive COVID-19 status was independently associated with male sex (odds ratio (OR) 1.38, p = 0.001), residential care (OR 2.15, p < 0.001), inpatient fall (OR 2.23, p = 0.003), cancer (OR 0.63, p = 0.009), ASA grades 4 (OR 1.59, p = 0.008) or 5 (OR 8.28, p < 0.001), and longer admission (OR 1.06 for each increasing day, p < 0.001). Patients with COVID-19 at any time had a significantly lower chance of 30-day survival versus those without COVID-19 (72.7% versus 92.6%, p < 0.001). COVID-19 was independently associated with an increased 30-day mortality risk (hazard ratio (HR) 2.83, p < 0.001). Increasing age (HR 1.03, p = 0.028), male sex (HR 2.35, p < 0.001), renal disease (HR 1.53, p = 0.017), and pulmonary disease (HR 1.45, p = 0.039) were independently associated with a higher 30-day mortality risk in patients with COVID-19 when adjusting for confounders. Conclusion: The prevalence of COVID-19 in hip fracture patients during the first wave of the pandemic was 9%, and was independently associated with a three-fold increased 30-day mortality risk.

15. Tolerance of Antarctic cyanobacterial mats to enhanced UV radiation

Abstract

To assess the biological implications of ozone depletion over the Antarctic Peninsula, the ultraviolet (UV) regime of two Antarctic cyanobacterial communities (composed of Leptolyngbya sp. and Phormidium sp.) was manipulated using screens that cut out UV radiation and a lamp which enhanced the dose of UV-B radiation (280–315 nm). The biological response of the cyanobacterial mats was monitored by measurement of chlorophyll fluorescence and pigment concentrations. The Leptolyngbya mat showed significant photochemical inhibition due to increased UV-B relative to photosynthetically active radiation (400–700 nm). The effect of UV on the Phormidium mat was less pronounced and dependent on the method of analysis: significantly lower photochemical yields were observed in UV-enhanced Phormidium mats compared to UV-excluded treatment, but if the yield data relative to the time zero control were considered then no effect of UV enhancement was observed. The Phormidium mat contained over 25 times the absolute concentration of UV-protecting mycosporine-like amino acid (MAA) and double the concentration of carotenoids compared to the Leptolyngbya mat, but the latter contained a higher ratio of carotenoids+MAAs to chlorophyll. There were no significant treatment-related changes in the concentrations of MAA, carotenoids and chlorophyll a in the Phormidium mat. The Leptolyngbya mat showed significantly lower chlorophyll a concentrations under UV enhancement, which could account for the lower photochemical yield in this sample. Our results show that different cyanobacterial species have differing photochemical sensitivity to UV-B radiation, which may confer a subtle advantage to the UV-B tolerant species over the less tolerant type during a period of high UV-B irradiance.

17. Paediatric intrasubstance posterior cruciate ligament rupture.

Abstract

The authors present the case of a 4-year-old boy who sustained an intrasubstance posterior cruciate ligament (PCL) tear whist trampolining. He was managed non-operatively with return to full function by 8 months. A high index of suspicion is required when assessing paediatric hyperflexion/extension injuries at the knee as ligamentous injury may occur without osteochondral fracture and may be missed on routine radiographs. Early MRI can identify such injuries in addition to osteochondral avulsions which are often amenable to acute internal fixation. In the case of paediatric intrasubstance PCL tears, it appears that non-operative management yields a good functional outcome in the short term in the skeletally immature. [ABSTRACT FROM AUTHOR]

Abstract

The objective of this study was to investigate the outcome of in-toeing referrals to a paediatric orthopaedic department. Two hundred and two patients referred to the Royal Hospital for Sick Children, Edinburgh between July 2005 and March 2008 were retrospectively reviewed. Increased femoral anteversion and internal tibial torsion formed the majority of diagnoses. The median age of referral was 4 years. No patient in the audit period required surgery. Eighty-six percent of children were discharged after their first visit. No significant pathology was identified in the 14% reviewed. Management and outcome for these children were not affected by referral to the orthopaedic clinic.

Abstract

Pretibial swellings in children usually represent erythema nodosum which exhibits characteristic skin changes. Three cases of pretibial subcutaneous lesions are presented which had no skin involvement or bony abnormalities on plain radiographs. At initial presentation, concerns of malignancy were raised but these lesions were ultimately diagnosed as granuloma annulare, fat necrosis and subcutaneous infection. In combination with clinical assessment and plain radiographs, MRI proved invaluable in reaching diagnosis and excluding neoplasia. Biopsy was only required in one case in this series after MRI. A tissue diagnosis, however, remains mandatory if there is any doubt over the nature of such lesions.
