2. Decadal attribution of historic temperature and ocean heat content change to anthropogenic emissions

Abstract

We present an alternative method of calculating the historical effective radiative forcing using the observed temperature record and a kernel based on the CMIP5 temperature response. This estimate is the effective radiative forcing time series that the average climate model would need to simulate the observed global mean surface temperature anomalies. We further infer the anthropogenic aerosols radiative forcing as a residual using the better-known greenhouse gas radiative forcing. This allows an independent estimate of anthropogenic aerosol radiative forcing, which suggests a cooling influence due to aerosols in the early part of the 20th century. The temporal kernels are also used to calculate decadal contributions from the dominant forcing agents to present day temperature, ocean heat content, and thermosteric sea level rise. 19. Teacher Education for a Multi-Cultural Society: Overview of a Program.

Abstract

A joint Teacher Crops/Career Opportunities Program/State of Alaska project in its fifth year, Cross-Cultural Education Development/Alaska Rural Teacher Training Corps (X-CED/ARTTC) focuses on exploration of the ends desirable to meet the educational needs of the culturally different and development of the processes necessary to achieve them. A microcosm of the broader society, it provides a cohesive environment dealing not only with the delivery of instruction but with cross-cultural dynamics and the politics of educational change. X-CED/ARTTC departs from the historical teacher education program in that: (1) the students are field-based; (2) "team leaders" are university instructors with full faculty status, salaried exclusively from State funds and located permanently in the field; and (3) a new and alternative curriculum (in content, process, means, and locale of delivery) is being created by the faculty who combine their knowledge and talents with their experience from living and working in the clientele's cultural environments. This paper presents an overview of the: (1) program's efforts and (2) plan to develop a curriculum which has unique features and will co-exist with the more conventional university offering. The proposed undergraduate and graduate programs are outlined by areas of study and degree emphasis. (NQ)

20. Field Centered Teacher Preparation. Final Report, Cycle 5, 1970-1972.

Abstract

The final report for Cycle 5 of the Alaska Rural Teacher Training Corps (ARTTC) consists of retrospective observations on the program over the period May 1970 through August 1972. The preparation of the report consists of the observations and assessments of the project director. Contents of the report include a project description, program overview, project objectives, discrepancy evaluation, the school system, and the community. Discrepancy evaluation emphasizes the modification of a regular teacher training program, the effects on educational personnel, and the teacher corps as a viable model. Benefits accured, enhanced educational opportunities for children, significant changes in schools, administrators' and teachers' attitudes toward ARTTC, in-service training, and graduating corpsmen are discussed in relation to the school system. The final section presents the school-community-program relationship. (MJM)

Abstract

Aims: Assessment of a supported discharge service for a cohort of patients admitted to Cork University Hospital with COVID-19 that were identified as being appropriate for remote patient monitoring. Methods: Patients uploaded SpO2, subjective breathlessness scores, and temperature readings onto the PatientMpower application, and received a daily phone call from the physiotherapist. Readmission was triggered where appropriate. Patient satisfaction questionnaires were completed following service discharge. Results: Over 12 weeks, 15 patients had a supported discharge. Readmission was triggered for 3 patients (20%). Compared to non-readmitted patient, readmitted patients had more abnormal SpO2 readings (9 (5.5-22.5) vs 1 (0-1), p= 0.022) and all 6 temperature spikes that occurred, but lower subjective breathlessness scores (3 (1-6) vs 4.25 (2-8), p = 0.003). Differences in mean abnormal SpO2% readings were not statistically significant. Conclusion: A supported discharge service including remote monitoring and regular contact with healthcare professionals can facilitate safe, and timely discharges of select patient groups.

Abstract

Common variable immunodeficiency is characterized by impaired B-cell differentiation and defective immunoglobulin production manifesting as recurrent respiratory tract infections. While the condition can masquerade as asthma, late diagnosis of CVID in known asthmatic is rarely reported. We present the case of a 43-year-old lady with recurrent episodes of wheeze, cough, sinusitis and multiple lower respiratory tract infections. Transiently responsive to antibiotics and steroids. These episodes had been occurring for many years and she had a longstanding clinical diagnosis of asthma. As part of her work up for recurrent respiratory tract infections a CT thorax was performed and demonstrated bronchiectasis. Further tests including Immunoglobulin levels revealed critically low IgG, IgM, and IgA levels. Immunoglobulin replacement therapy was commenced with a reduction in exacerbation frequency and severity, and objective improvement of asthma control. Subsequent lung function tests demonstrated reversible airflow limitation (obstructive lung function with 13% reversibility in FEV1 post-bronchodilator) consistent with asthma. Our case illustrates the importance of searching for alternate and co-existent diagnoses in patients diagnosed with asthma who are unresponsive to conventional therapy.
