Abstract

To improve the understanding of chemo-refractory high-grade serous ovarian cancers (HGSOCs), we characterized the proteogenomic landscape of 242 (refractory and sensitive) HGSOCs, representing one discovery and two validation cohorts across two biospecimen types (formalin-fixed paraffin-embedded and frozen). We identified a 64-protein signature that predicts with high specificity a subset of HGSOCs refractory to initial platinum-based therapy and is validated in two independent patient cohorts. We detected significant association between lack of Ch17 loss of heterozygosity (LOH) and chemo-refractoriness. Based on pathway protein expression, we identified 5 clusters of HGSOC, which validated across two independent patient cohorts and patient-derived xenograft (PDX) models. These clusters may represent different mechanisms of refractoriness and implicate putative therapeutic vulnerabilities.

Abstract

The core symptoms of many neurological disorders have traditionally been thought to be caused by genetic variants affecting brain development and function. However, the gut microbiome, another important source of variation, can also influence specific behaviors. Thus, it is critical to unravel the contributions of host genetic variation, the microbiome, and their interactions to complex behaviors. Unexpectedly, we discovered that different maladaptive behaviors are interdependently regulated by the microbiome and host genes in the Cntnap2-/- model for neurodevelopmental disorders. The hyperactivity phenotype of Cntnap2-/- mice is caused by host genetics, whereas the social-behavior phenotype is mediated by the gut microbiome. Interestingly, specific microbial intervention selectively rescued the social deficits in Cntnap2-/- mice through upregulation of metabolites in the tetrahydrobiopterin synthesis pathway. Our findings that behavioral abnormalities could have distinct origins (host genetic versus microbial) may change the way we think about neurological disorders and how to treat them.

Abstract

Glioblastoma (GBM) is characterized by aberrant vascularization and a complex tumor microenvironment. The failure of anti-angiogenic therapies suggests pathways of GBM neovascularization, possibly attributable to glioblastoma stem cells (GSCs) and their interplay with the tumor microenvironment. It has been established that GSC-derived extracellular vesicles (GSC-EVs) and their cargoes are proangiogenic in vitro. To further elucidate EV-mediated mechanisms of neovascularization in vitro, we perform RNA-seq and DNA methylation profiling of human brain endothelial cells exposed to GSC-EVs. To correlate these results to tumors in vivo, we perform histoepigenetic analysis of GBM molecular profiles in the TCGA collection. Remarkably, GSC-EVs and normal vascular growth factors stimulate highly distinct gene regulatory responses that converge on angiogenesis. The response to GSC-EVs shows a footprint of post-transcriptional gene silencing by EV-derived miRNAs. Our results provide insights into targetable angiogenesis pathways in GBM and miRNA candidates for liquid biopsy biomarkers.

Abstract

Hsp104 is an AAA+ protein disaggregase, which can be potentiated via diverse mutations in its autoregulatory middle domain (MD) to mitigate toxic misfolding of TDP-43, FUS, and α-synuclein implicated in fatal neurodegenerative disorders. Problematically, potentiated MD variants can exhibit off-target toxicity. Here, we mine disaggregase sequence space to safely enhance Hsp104 activity via single mutations in nucleotide-binding domain 1 (NBD1) or NBD2. Like MD variants, NBD variants counter TDP-43, FUS, and α-synuclein toxicity and exhibit elevated ATPase and disaggregase activity. Unlike MD variants, non-toxic NBD1 and NBD2 variants emerge that rescue TDP-43, FUS, and α-synuclein toxicity. Potentiating substitutions alter NBD1 residues that contact ATP, ATP-binding residues, or the MD. Mutating the NBD2 protomer interface can also safely ameliorate Hsp104. Thus, we disambiguate allosteric regulation of Hsp104 by several tunable structural contacts, which can be engineered to spawn enhanced therapeutic disaggregases with minimal off-target toxicity.

Abstract

To develop a map of cell-cell communication mediated by extracellular RNA (exRNA), the NIH Extracellular RNA Communication Consortium created the exRNA Atlas resource (https://exrna-atlas.org). The Atlas version 4P1 hosts 5,309 exRNA-seq and exRNA qPCR profiles from 19 studies and a suite of analysis and visualization tools. To analyze variation between profiles, we apply computational deconvolution. The analysis leads to a model with six exRNA cargo types (CT1, CT2, CT3A, CT3B, CT3C, CT4), each detectable in multiple biofluids (serum, plasma, CSF, saliva, urine). Five of the cargo types associate with known vesicular and non-vesicular (lipoprotein and ribonucleoprotein) exRNA carriers. To validate utility of this model, we re-analyze an exercise response study by deconvolution to identify physiologically relevant response pathways that were not detected previously. To enable wide application of this model, as part of the exRNA Atlas resource, we provide tools for deconvolution and analysis of user-provided case-control studies.

Abstract

The Extracellular RNA Communication Consortium (ERCC) was launched to accelerate progress in the new field of extracellular RNA (exRNA) biology and to establish whether exRNAs and their carriers, including extracellular vesicles (EVs), can mediate intercellular communication and be utilized for clinical applications. Phase 1 of the ERCC focused on exRNA/EV biogenesis and function, discovery of exRNA biomarkers, development of exRNA/EV-based therapeutics, and construction of a robust set of reference exRNA profiles for a variety of biofluids. Here, we present progress by ERCC investigators in these areas, and we discuss collaborative projects directed at development of robust methods for EV/exRNA isolation and analysis and tools for sharing and computational analysis of exRNA profiling data.

16. Association of the humoral immune response with the inflammatory profile in Plasmodium vivax infections in pregnant women.

Abstract

Background: Plasmodium vivax infection, when it occurs during pregnancy, has often been associated with serious adverse pregnancy outcomes. However, immunological alterations in pregnancy and their consequences have been little explored. We characterized the humoral immune response in pregnant women exposed to malaria by P. vivax antigens and its association with the maternal inflammatory profile and poor pregnancy outcomes. Methods: An observational cohort study in the Brazilian Amazon was conducted between 2013 and 2015. After applying exclusion criteria, 242 mother-child pairs were included in the analysis. Data on maternal infection, gestational outcomes, and inflammatory factors were evaluated in the maternal peripheral plasma. In samples from the first infection, the presence of total IgG and its subclasses in plasma against PvMSP119 protein were also quantified. Results: Previous exposure to malaria, observed by anti-total IgG antibodies to the PvMSP119 antigen, increased the inflammatory response to infection when the pregnant woman had malaria during pregnancy. IL-6 and IL-10 levels were positively correlated with parasitemia and with total IgG levels; but they were negatively correlated with the gestational age at delivery from Pv-infected woman. In multivariate linear regression analyses, IgG 1, 2 and 4 was negatively and positively associated with cytokines IL-6 and IL-10, respectively, in P. vivax-infection. Conclusions: An association between the humoral immune response and the peripheral inflammatory cytokine profile with the adverse outcomes in malaria in pregnancy by P. vivax was observed. Previous exposure to the parasite can influence the IL-6 and IL-10 response, which is associated with increased parasitemia, reduced maternal weight gain and premature delivery. Author summary: Plasmodium vivax malaria in pregnancy is also associated with several adverse outcomes, even in low-transmission regions such as South America. However, to our knowledge, no study has previously investigated the link between the humoral immune response due to previous exposure to P. vivax and the immunological profile during pregnancy infection and its association with poor pregnancy outcomes. In this study, after applying exclusion criteria, 242 mother-child pairs were evaluated for different immunological markers of malaria exposure in an endemic area in the Brazilian Amazon. We showed that previous exposure to P. vivax, through the humoral immune response to the PvMSP119 antigen, can exacerbate the inflammatory response during gestational malaria and is associated with increased parasitemia, reduced maternal weight gain and premature delivery. These findings help to understand the association between anti-PvMSP119 antibodies, considered exposure marker, and the maternal peripheral inflammatory profile throughout pregnancy, bringing important implications for understanding the immunobiology of the disease. Abstract

We present the rare case of a 61-year-old female with Mycobacterium fortuitum osteomyelitis of the cuboid bone following penetrating plantar trauma. The patient underwent a single-stage surgery for the condition, including lesion debridement and bone defect filling with absorbable, gentamicin-/vancomycin-loaded, calcium sulfate–hydroxyapatite biocomposites, that resolved favorably 5 months after intervention.

Abstract

The use of antibiotics for the treatment of diabetic foot infections (DFIs) over an extended period of time has been shown to be associated with adverse events (AEs), whereas interactions with concomitant patient medications must also be considered. The objective of this narrative review was to summarize the most frequent and most severe AEs reported in prospective trials and observational studies at the global level in DFI. Gastrointestinal intolerances were the most frequent AEs, from 5% to 22% among all therapies; this was more common when prolonged antibiotic administration was combined with oral beta-lactam or clindamycin or a higher dose of tetracyclines. The proportion of symptomatic colitis due to Clostridium difficile was variable depending on the antibiotic used (0.5% to 8%). Noteworthy serious AEs included hepatotoxicity due to beta-lactams (5% to 17%) or quinolones (3%); cytopenia's related to linezolid (5%) and beta-lactams (6%); nausea under rifampicin, and renal failure under cotrimoxazole. Skin rash was found to rarely occur and was commonly associated with the use of penicillins or cotrimoxazole. AEs from prolonged antibiotic use in patients with DFI are costly in terms of longer hospitalization or additional monitoring care and can trigger additional investigations. The best way to prevent AEs is to keep the duration of antibiotic treatment short and with the lowest dose clinically necessary.

Abstract

Sugoi Packing SAS, nace como empresa de acondicionamiento secundario de productos farmaceuticos en la ciudad de Bogotá hace 15 años, para realizar dicha labor, desde un principio tuvo que contar con el certificado BPM. Esta certificación tiene una vigencia por 5 años según el Articulo 7 del Decreto 549 de 2001 del Ministerio de Salud. El acondicionamiento secundario de medicamentos, se encuentra establecido y regulado en Colombia por las Buenas Prácticas de Manufactura (BPM). Teniendo en cuenta que este proceso hace parte de la comercializacion del producto y que los envases, rótulos, etiquetas y empaques, son parte integral del medicamento, por cuanto éstos garantizan su calidad, estabilidad y uso adecuado (INVIMA 2021). Teniendo en cuenta el crecimiento y las nuevas necesidades del mercado, Sugoi Packing expandió el alcance de sus servicios de acondicionamiento para otros productos, tales como como dispositivos médicos, los cuales son regulados por el Decreto 4725 de 2005 del INVIMA, que estableció el Certificado de Capacidad de Almacenamiento y Acondicionamiento (CCAA) y para productos cosméticos regulados por la Resolución 3773 de 2004 del INVIMA, que estableció el Certificado de Capacidad de Producción de Cosméticos (CCPC). Estos dos últimos servicios, han constituido durante los últimos años, la mayor fuente de ingresos de la compañía. Teniendo en cuento lo anterior, Sugoi Packing no vio la necesidad de renovar la certificación BPM, obtenida en sus inicios como empresa una vez culminó su vigencia y dedico sus servicios en los ultimos años al acondicionamiento secundarios de dispositivos medicos y productos cosmeticos. Actualmente, se presenta la dificultad de acceso a clientes que requieran el acondicionamiento secundario de medicamentos limitando el crecimiento de la organizacion, debido a esto nace la necesidad de volver a implementar un sistema basado en las Buenas Practicas de Manufactura (BPM), teniendo en cuenta que este certificado se requiere como requisito legal por el Ministerio de Salud y Protección Social, para funcionar como empresa que presta el servicio de acondicionamiento secundario de medicamentos en Colombia. These last two services have been the company's main source of income in recent years. Taking into account the above, Sugoi Packing did not see the need to renew the BPM certification, obtained in its beginnings as a company once its term ended and dedicated its services in recent years to the secondary packaging of medical devices and cosmetic products.
